Audiovestibular functioning of post-menopausal females with osteoporosis and osteopenia

Introduction:Osteoporosis and osteopenia are progressive disorders characterized by decreased bone mass,especially in postmenopausal women.These can be associated with body pain,fractures,hearing loss and balance disorders.The present study aims to evaluate audio-vestibular function in postmenopausal patients with osteopenia or osteoporosis.Methods:The study included 48 postmenopausal women(new subjects)diagnosed with osteoporosis(n=23)or osteopenia(n=25)in the age range of 50e66 years,as well as 28 normal women as controls.Audiological testing included pure tone audiometry(conventional and extended high-frequency audiometry),speech audiometry,impedance audiometry and otoacoustic emissions,including both transient evoked otoacoustic emissions(TEOAEs)and distortion product otoacoustic emissions(DPOAEs).All subjects also underwent vestibular evoked myogenic potentials testing(both ocular and cervical VEMPs).Results:In the present study,hearing was worse at all frequencies in the osteoporosis group in comparison with the osteopenia and control groups,with worse speech recognition and discrimination scores and OAEs.Vestibular function was affected in 95.65%of women with osteoporosis and 76%of those with osteopenia.Conclusion:Osteoporosis and osteopenia are risk factors for vestibular dysfunction and hearing deficits in postmenopausal women.Thus,hearing and vestibular function should be monitored by audiological and vestibular testing periodically in these individuals.

The occurrence of diabetes mellitus(DM)in pancreatitis is being increasingly recognized lately.Diabetes can develop not only with chronic pancreatitis but even after the first episode of acute pancreatitis(AP).The incidence of diabetes after AP varies from 18%to 23%in 3 years and reaches up to 40%over 5 years.The exact pathogenesis of diabetes after AP is poorly understood and various mechanisms proposed include loss of islet cell mass,AP-induced autoimmunity,and alterations in the insulin incretin axis.Risk factors associated with increased risk of diabetes includes male sex,recurrent attacks of pancreatitis,presence of pancreatic exocrine insufficiency and level of pancreatitic necrosis.Diagnosis of post-pancreatitis DM(PPDM)is often excluded.Treatment includes a trial of oral antidiabetic drugs in mild diabetes.Often,insulin is required in uncontrolled diabetes.Given the lack of awareness of this metabolic disorder after AP,this review will evaluate current information on epidemiology,risk factors,diagnosis and management of PPDM and identify the knowledge gaps.

TO THE EDITORWe read with great interest the article, 'Non-alcoholic fatty liver disease may not be a severe disease at presentation among Asian Indians' by Madan et al in the recent issue of WJG. Twenty-eight (55%) out of 51 patients with nonalcoholic fatty liver disease (NAFLD) who presented with abnormal transaminases had histological evidence of nonalcoholic steatohepatitis (NASH).

Pregnancy,Pancreatitis,and Hypoglycemia:Multi-modal Management in a Case with Multiple Endocrine Neoplasia-1 Syndrome

A 32-year-old lady presented to us at 6 months of gestation with acute pancreatitis due to parathyroid hormone-dependent hypercalcemia and with insulin-dependent hypoglycemia,owing to parathyroid adenoma and possibly insulinoma,respectively.The parathyroid adenoma was localized on the magnetic resonance imaging of the neck;however,imaging for the insulinoma could not be done due to the gravid state.Due to the simultaneous occurrence of tumours in two endocrine glands,namely,parathyroid gland and pancreatic islet cells,a diagnosis of MEN-1(multiple endocrine neoplasia)was considered,which is very rarely seen in pregnancy.Her hypercalcemia was effectively managed with cinacalcet and alcohol ablation of the parathyroid adenoma while her hypoglycemic episodes were managed with short and long-acting octreotide injections during the antenatal period.She had a full-term cesarean delivery,with no maternal or neonatal complications,except for transient neonatal hypoglycemia.The patient was followed up for 1 year after her delivery with no further episodes of hypercalcemia and hypoglycemia,on medical management.Tc99m sestamibi scan was done after delivery which confirmed the presence of a left inferior parathyroid adenoma.MEN-1 with pregnancy thus poses a diagnostic and therapeutic challenge and our case highlights the role of multimodal medical therapy for successful management.

Dragonfly Interaction Algorithm for Optimization of Queuing Delay in Industrial Wireless Networks

In industrial wireless networks,data transmitted from source to destination are highly repetitive.This often leads to the queuing of the data,and poor management of the queued data results in excessive delays,increased energy consumption,and packet loss.Therefore,a nature-inspired-based Dragonfly Interaction Optimization Algorithm(DMOA)is proposed for optimization of the queue delay in industrial wireless networks.The term“interaction”herein used is the characterization of the“flying movement”of the dragonfly towards damselflies(female dragonflies)for mating.As a result,interaction is represented as the flow of transmitted data packets,or traffic,from the source to the base station.This includes each and every feature of dragonfly movement as well as awareness of the rival dragonflies,predators,and damselflies for the desired optimization of the queue delay.These features are juxtaposed as noise and interference,which are further used in the calculation of industrial wireless metrics:latency,error rate(reliability),throughput,energy efficiency,and fairness for the optimization of the queue delay.Statistical analysis,convergence analysis,the Wilcoxon test,the Friedman test,and the classical as well as the 2014 IEEE Congress of Evolutionary Computation(CEC)on the benchmark functions are also used for the evaluation of DMOA in terms of its robustness and efficiency.The results demonstrate the robustness of the proposed algorithm for both classical and benchmarking functions of the IEEE CEC 2014.Furthermore,the accuracy and efficacy of DMOA were demonstrated by means of the convergence rate,Wilcoxon testing,and ANOVA.Moreover,fairness using Jain’s index in queue delay optimization in terms of throughput and latency,along with computational complexity,is also evaluated and compared with other algorithms.Simulation results show that DMOA exceeds other bio-inspired optimization algorithms in terms of fairness in queue delay management and average packet loss.The proposed algorithm is also evaluated for the conflicting o

Numerical modelling of post-failure behaviors of coal specimens

A modelling approach consisting of best-fit relations to estimate the post-yield strength parameters is presented for simulating post-peak behavior beyond the point of residual strength of coal pillars having different w/h ratios.The model was developed based on back-analysis of the complete stress-strain behavior of specimens belonging to six different Indian coal seams with different w/h ratios of 0.5 e13.5.It was found that the simultaneous degradation of the cohesion and friction angle of the Mohr-Coulomb rock material characterizes the post-peak strength behavior of the rock.The resulting expressions are simplistic as they require parameters that can be easily determined using uniaxial and triaxial compression results.Eventually,the developed model was validated by simulating the triaxial tests of coal specimens with different sizes under varying confining stresses and comparing its findings with the published test results.The study showed that its implementation in the numerical model could reproduce laboratory-observed mechanical response,deformation behavior,and failure mechanism very closely.

Multidisciplinary approach toward enhanced recovery after surgery for total knee arthroplasty improves outcomes

Knee osteoarthritis is a degenerative disorder of the knee,which leads to joint pain,stiffness,and inactivity and significantly affects the quality of life.With an increased prevalence of obesity and greater life expectancies,total knee arthroplasty(TKA)is now one of the major arthroplasty surgeries performed for knee osteoarthritis.When enhanced recovery after surgery(ERAS)was introduced in TKA,clinical outcomes were enhanced and the economic burden on the healthcare system was reduced.ERAS is an evidence-based scientific protocol aimed at ameliorating the surgical stress response.ERAS aims to enhance the recovery phase,which encompasses multidisciplinary strategies at every step of perioperative care,including the rehabilitation phase.Implementation of ERAS in TKA aids in reducing the length of hospital stay,improving pain management,reducing perioperative complications,and enhancing patient satisfaction.Multidisciplinary collaboration,integrating the expertise of anesthesiologists,orthopedic surgeons,nursing personnel,and other healthcare professionals,is the cornerstone of ERAS in patients undergoing TKA.

Pain management in chronic pancreatitis

Pain in chronic pancreatitis(CP)is difficult to manage.Many patients suffer from inadequate pain relief,completely incapacitating them in their daily activities.Historically,despite their well-known adverse effects,opioids have been the pillar of treatment regimens in painful CP.The management is now gradually evolving with a better understanding of the underlying pathophysiology of CP-related pain.Clinicians should follow a holistic approach to the management of CPassociated pain,which must involve lifestyle changes that are coupled with analgesic medications and other pain-relieving interventions.Furthermore,there is no easy cure for vanquishing CP-associated pain.Each patient must be evaluated on a case-by-case basis by a multidisciplinary team to decide which treatment option is best suited for that individual.

Resistance and resilience of soil biological indicators:A case study with multi-walled carbon nanotube

Soil ecosystem is experiencing stresses due to climate change,and soil inhabitants try to demonstrate their inherent resistance and resilience against those stresses.Application of nanomaterials as agricultural inputs could bring shifts in resistance and resilience patterns of soil microbes and associated enzymes,especially under short-term heat stress.With this background,the impacts of multi-walled carbon nanotube(MWCNT)on the resistance and resilience of soil biological indicators were evaluated.An incubation experiment was conducted with varied MWCNT concentrations(0,50,100,250,and 500 mg kg-1soil)for 90 d after 24-h heat stress at 48±2?C to assess the impacts of MWCNT on soil enzyme activities and microbial populations vis-à-vis their resistance and resilience indices under short-term exposure to heat stress.Enzyme activities were reduced after exposure to heat stress.Resistance indices of enzyme activities were enhanced by MWCNT application on day 1 after heat stress,whereas there was no recovery of enzyme activities after 90-d incubation.Like soil enzyme activities,resistance index values of soil microbial populations followed the similar trend and were improved by MWCNT application.Multi-walled carbon nanotube has the potential to improve resistance indices of soil enzyme activities and microbial populations under heat stress,although they could not recover to their original state during periodical incubation after heat stress.This study helps to understand the relative changes of biological indicators under MWCNT and their ability to withstand heat stress.

Dengue virus infection is endemic in India with seasonal variations.It can lead to lethal consequences with plasma leakage and/or bleeding[1,2].Dengue shock syndrome has high mortality in developing countries like India.Dengue can cause functional proteinuria and even heavy proteinuria[3-6].Though dengue has been implicated to cause glomerulonephritis[7],to the best of our knowledge,dengue as a cause of relapse of minimal change disease(MCD)has not been reported.
