Low-dose amitriptyline combined with proton pump inhibitor for functional chest pain

AIM:To investigate the efficacy of amitriptyline with proton pump inhibitor(PPI)for the treatment of functional chest pain(FCP).METHODS:This was a randomized,open-label trial investigating the addition of low dose amitriptyline(10 mg at bedtime)to a conventional dose of rabeprazole(20 mg/d)(group A,n = 20)vs a double-dose of rabeprazole(20 mg twice daily)(group B,n = 20)for patients with FCP whose symptoms were refractory to *** primary efficacy endpoints were assessed by global symptom score assessment and the total number of individuals with > 50% improvement in their symptom ***:The between-group difference in global symptom scores was statistically significant during the last week of treatment(overall mean difference;3.75 ± 0.31 vs 4.35 ± 0.29,the between-group difference;P 50%,whereas only 26.3% of patients in group B had a similar treatment response(70.6% vs 26.3%,P = 0.008).Specifically,patients in group A had a significantly greater improvement in the domains of body pain and general health perception than did patients in group B(52.37 ± 17.00 vs 41.32 ± 12.34,P = 0.031 and 47.95 ± 18.58 vs 31.84 ± 16.84,P = 0.01,respectively).CONCLUSION:Adding amitriptyline to a PPI was more effective than a double-dose of PPI in patients with FCP refractory to a conventional dose of PPI.

辣椒(Capsicum annuum)是美洲最古老的驯化作物之一,在全世界被广泛种植。本研究对CM334(墨西哥地方品种)进行了全基因组测序和组装(覆盖度为186.6×),同时还对另外两个栽培种和一个野生种分别进行了重测序和从头(de novo)测序。结果表明辣椒基因组大小约为番茄的4倍,辣椒基因组上存在大量gypsy转座子和花椰菜病毒家族元件的积累。结合转录组综合分析认为,辣椒素合成酶基因表达模式的改变及新功能化是导致辣椒素生物合成的原因,研究还发现辣椒与番茄在乙烯合成及果实成熟调控方面存在明显不同的分子模式。辣椒参考基因组的发布为辣椒营养品质和药用价值的改善提供了重要平台。

Benefit of neoadjuvant concurrent chemoradiotherapy for locally advanced perihilar cholangiocarcinoma

To clarify the role of neoadjuvant concurrent chemoradiotherapy (NACCRT) followed by surgical resection for localized or locally advanced perihilar cholangiocarcinoma (CCA).METHODSWe retrospectively reviewed 57 patients who underwent surgical resection with or without NACCRT for perihilar CCA; 12 patients received NACCRT and 45 patients did not received NACCRT. Patients with locally advanced perihilar CCA requiring NACCRT were defined as follows: (1) a mass involving unilateral branches of the portal vein or hepatic artery with insufficient volume of the anticipated remnant lobe; or (2) an infiltrating mass in the main portal vein that was too long for reconstruction, identified at preoperative *** median disease-free survival (DFS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 26.0 and 15.1 mo, respectively (P = 0.91). The median overall survival (OS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 32.9 and 27.1 mo, respectively (P = 0.26). The NACCRT group showed a downstaging tendency compared to the non-NACCRT group as compared with the tumor stage confirmed by histological examination after surgery and the tumor stage confirmed by imaging test at the time of diagnosis (P = 0.01).CONCLUSIONNACCRT does not prolong DFS and OS in localized or locally advanced perihilar CCA. However, NACCRT may allow tumor downstaging and improve tumor resectability.

This paper presents a novel digit-serial-in-serial-out systolic array for performing the power-sum operation C + AB in finite fields GF(2) with the standard basis representation. If the appropriate digit-size is selected,the proposed method can meet the throughput requirement of a specific application with minimum *** the digit size of the regular square form,the latency of the array can be reduced by 20%as before in GF(2). The new digit-serial systolic array with the unidirectional data flow is highly regular, nearest-neighbor connected,and thus well suited for VLSI implementation.

随着经济全球化的不断发展,鞋服企业的创新活动呈复杂化趋势转变,在鞋服企业发展过程中,仅依靠自身资源进行创新活动愈发困难,基于此鞋服企业渐渐开始通过与其他鞋服企业、高校、科研机构、政府等创新主体形成合作,建立不同的协同创新模式,促进鞋服企业可持续发展。以往的研究表明不同的协同创新模式对企业创新绩效有着正向的影响。然而,哪种协同创新模式更有利于提高鞋服企业的创新绩效,在鞋服情境下各模式又有哪些具体的成效和不足。这些问题研究缺乏相关理论阐述与实证探讨。基于此,本文进行了文献整理、问卷调查和案例分析以期解决这些问题。首先,本文进行文献研究。基于国内外已有研究,对战略联盟模式、专利转移模式、研发外包模式、要素共享模式4种模式进行区分,从理论的角度整理了它们对企业创新绩效的影响。其次,本文对208家鞋服企业进行问卷调查,探究鞋服企业运用战略联盟模式、专利转移模式、研发外包模式、要素共享模式与创新绩效之间关系与影响强弱。数据分析结果表明:战略联盟模式、专利转移模式、研发外包模式对鞋服企业创新绩效均起到正向显著地促进作用,其中研发外包模式对提高鞋服企业创新绩效效用最大,而要素共享模式对鞋服企业创新绩效影响作用较为不显著。最后,本文进行了案例研究,通过将协同创新成熟理论框架置于鞋服企业中,对典型案例进行剖析,验证问卷调研的结果,同时根据鞋服企业现实情况,具体分析了每种模式的成效和存在的不足,并提出了相应的改进对策。综上所述,本文探究鞋服情境下,各协同创新模式对企业创新绩效的影响,进而更好地指导鞋服企业,选择合适创新路径,提升企业创新绩效。同时也将协同创新模式置于鞋服行业中,探究其对应的特点、不足和改进对策,从理论到实践,赋予理论现实意义。
