Epidemiology of pelvic fractures in adults: Our experience at a tertiary hospital

Purpose: Pelvic fractures are severe injuries and are often associated with multiple system injuries, exacerbating the overall outcome. In India, the incidence of pelvic fractures is on a rise due to suboptimal roads and traffics but related literature regarding the overall epidemiology of these injuries is scarce and scanty. Our aim was to study the epidemiology of patients admitted with pelvic fractures at a level 1 trauma centre in India. Methods: A 16-month (between September 2015 and December 2016) prospective observational study was carried out on trauma patients with pelvic fractures at a level 1 trauma centre of a tertiary care hospital. Demography of patients, mechanism of injuries and complications were recorded prospectively. Results: We observed 75 patients who presented with pelvic fractures, where 56 were males and 19 were females. Mean age of the study populationwas 37.57 years. Road traffic accidents were the most common mode of injuries. Lateral compression injuries were the most common pattern. Associated injuries frequently encountered were lower extremities and acetabulum fractures, blunt abdominal trauma, urogenital injuries and head injuries. Out of the 75 patients, 52 were treated surgically and 23 were managed by conservative methods. Associated injuries of the extremities, head, abdomen and urogenital system indicated a longer hospital stay. Conclusion: Pelvic fractures, although belong to a relatively rare trauma subset, cause a high morbidity and mortality with considerable burden on the economy. Proper road safety training and driving etiquettes along with its strict implementation in true sense and spirit are the need of the hour.

Introduction: The root cause of TIPS procedure in liver cirrhosis is dueto the complications which cannot be treated by medical therapy andunavailability of liver donors. At Present TIPS has been accepted at globallevel.Objectives: To study the laboratory findings, complications, prognosticindicators and imaging modalities to predict the TIPS status.Materials and Methods: The data of all patients who went throughTIPS from May,2011to May,2013were studied and followed-up. A totalof110patients were analyzed. Their clinical parameters and laboratoryvalues of their first visit or of the earliest visit whenever the findings weredocumented were recorded and analyzed via Graph pad Prism5.0software.Results: Among the110patients, eighty five were males and twentyfive were females. The mean age of presentation was42.03years (Agerange from45to60years). The most common mild complication washepatic encephalopathy (7.5%) followed by shunt stenosis (2%) within sixmonths after TIPS. Recurrent gastrointestinal bleeding was the most common cause of death, accounting for (4.5%) of the deaths, followed byliver failure (2.72%) and Hepatic encephalopathy (0.9%) *** was no significant effect on laboratory parameters from discharge tofollowed-up of one to2years following the TIPS procedure.Conclusion: selecting right client for TIPS procedure and assessingsome significant laboratory parameters post-TIPS surgery indicates TIPS asa valuable procedure in the liver cirrhotic patients.

目的:这个研究的主要目的是在急性前壁心肌梗死的患者中,根据Ⅱ,Ⅲ,aVF导联ST段是否压低来做出关于犯罪血管狭窄部位的预测,同时结合冠状动脉血管造影和其他指标如肌钙蛋白I和前壁射血分数来验证我们的猜想。背景:心脏是一种搏动性的器官,主要由三个冠状动脉供血,即右冠状动脉,左前降支动脉和左回旋动脉。心脏前壁主要由左前降支供血,因此左前降支闭塞易发生急性前壁心肌梗死。急性前壁心肌梗死多累及心脏前部和室间隔。十二导联心电图显示V1-V6,I和aVL导联的ST段抬高,胸导联V3显著抬高以及肢体导联III显著压低。包括早期诊断和治疗在内的二级预防对于降低心肌梗死的死亡率和发病率非常重要,因为急性前壁心肌梗死的早期24至48小时死亡风险最大。CK-MB是心肌梗死的重要指标,心肌梗死后4至6小时血液中CK-MB开始升高,并在24至48小时内恢复正常。此外,心肌梗死后肌钙蛋白-T在3-5小时内升高,可在血液中持续高达21天。心脏的无创CT扫描称为心脏扫描,或钙评分可以通过量化冠状动脉中的钙化来计算发展冠心病的风险。然而,冠状动脉造影仍然是治疗研究心肌梗死的金标准。急性心肌梗死后存活的患者术后复发率较高,心肌梗死后1年内大约有10%的死亡率。事件发生后存活的患者AWMI后复发率高,MI发生1年内死亡率为10%。在最初的3到4个月内出现的高死亡率和死亡的潜在原因是晚期冠状动脉粥样硬化性疾病。方法:我们收集了2018年1月至2018年12月,在吉林大学第一医院心血管内科收治的151例前壁心肌梗死患者的临床记录,进行了一项回顾性研究。只有151名患者(男性112名和女性39名)符合我们的入选标准,入院前胸痛超过或等于30分钟,肌钙蛋白I水平升高,2例或更多导联ST段压低,冠状动脉造影显示前降支完全闭塞或严重狭窄>70%。排除标准包括心电图导联Ⅱ,Ⅲ,avF ST段抬高的患者,前壁MI的既往史,既往冠状动脉旁路移植术或入院前经皮冠状动脉介入治疗以及LAD和RCA或三支血管疾病的明显狭窄因此无法确定单个梗塞相关动脉。在研究之前获得了机构伦理委员会(IEC)的道德批准。结果:我们评估了151名诊断为前壁心肌梗死的患者。在评估所有人口统计学,临床,过去医学和研究(肌钙蛋白I,EF,ECG,冠状动脉血管造影)记录后,所有151名患者被纳入进一步分析。男性患者112例(74.17%),女性患者39例(25.82%)。主要危险因素是高血压67例(44.37%),糖尿病25例(16.55%)。在纳入的患者中,吸烟者106例(70.19%)。此外,冠状动脉造影中近端狭窄患者的平均年龄为61.47岁,其中冠状动脉造影中间和远端狭窄患者的平均年龄为59.76岁。在11例(52.4%)男性患者中发现近端狭窄,在女性患者中发现10例(47.6%)。在101例(77.7%)男性患者和29例(22.3%)女性患者中发现中远端狭窄。近端狭窄患者肌钙蛋白I的平均值为109.21±84.11,中远端狭窄患者的平均值为96.38±78.77。此外,近端狭窄的平均EF为53,范围为45-56,中间和远端狭窄为51,范围为44-56。在评估的患者中,年龄(P=0.511),肌钙蛋白I(P=0.494),EF(P=0.972),II、III、aVF导联压低(P=0.390)在判定前降支在近端或中、远端狭窄方面均无统计学差异。然而,结果表明性别是有意义的影响因素(P<0.05)。观察到男、女性患者的前壁心肌梗死(P=0.014)有显著差异。结论:总之,每例前壁心肌梗死患者的12导联心电图ST段压低并不明显。尽管心电图正常,但冠状动脉造影是诊断狭窄区域的金标准研究。因此,临床上出现心肌梗死症状且心电图正常的患者必须进一步评估以排除AWMI并降低发病率和死亡率。另外,高血压、糖尿病是发生前壁心肌梗死的危险因,且吸烟人群的AWMI患病率高于非吸烟者。

Finite Element Modeling and Design of Rectangular Patch Antenna with Different Feeding Techniques

The finite element modeling of three dimensional structures is important for researchers especially in the field of antennas and other domains of electromagnetic waves. This paper presents a finite element calculations and numerical analysis for the microstrip patch antennas. In this paper, two different designs have been modelled and analyzed and both designs are based on the rectangular patches. The feeding point of one design is inside the patch while the other design contains feeding point outside the patch is T shaped. The computational analysis showed some interesting results for radiation pattern and far field domain. For these designs, the characteristic impedance taken is 50 Ω and the operating frequency domain is 1.4 to 1.7 GHz. The microstrip patch antennas are encapsulated in the inert spherical atmosphere of 20 mm thickness containing air inside it.

Comparative Study of Inflammatory and Oxidative Stress Biomarkers in Different Metabolically Healthy Obesity Phenotypes

Aims: Obesity is the major contributor of the metabolic syndrome (MetS), but a unique phenotype of obesity known as metabolically healthy obese (MHO) shows healthier metabolic profile; however understanding of their biochemical correlates is poorly understood. Obesity is defined by Body mass index (BMI), but controversy exists regarding ethnic-specific BMI cut-offs. The present study used the Asian Indian BMI cut-offs to assess relationships of MHO phenotypes with oxidative stress (OS) and inflammation. Methods: In this case-control study, 299 metabolically-healthy (MH) respondents were divided into four groups as per Asian criteria for obesity: MH non-obese (MHNO), MH overweight (MHOW), MHO and MH severely obese (MHSO). Their oxidative stress and pro-inflammatory markers were measured. Brachymetatarsia with accessory navicular in right foot： A rare coincidental finding

A 33 years old female patient presented with posttraumatic pain in the right ibot tor which radiographs of the right foot was advised. No fracture was detected on radiographs and patient was managed conservatively on medications and posterior splint immobilization. We found coincidentally a short fourth metatarsal and an accessory navicular bone in the right foot radiographs. After 3 weeks of immobilization, she underwent mobilization of the right foot, weight bearing and intensive physio- therapy for 6 weeks. After two months of injury she was still complaining of pain on the plantar aspect of right foot which was diagnosed as metatarsalgia and operated on by excision of the neuroma present in the 3rd web space of the right foot. After surgery she was completely relieved of pain and could do activities well related to the right foot.

We report an important observation that the surface conductivity of antibody layer immobilized on polylysine-coated glass substrate decreases upon the formation of complex with their specific antigens. This change in conductivity has been observed for both monoclonal and polyclonal antibodies. The conductance of monoclonal mouse 1gG immobilized on polylysine-coated glass substrate changed from 1.02×10^-8 Ω^-1 to 1.41×10^-11 Ω^-1 at 10 V when complex is formed due to the specific biomolecular interactions with rabbit anti-mouse 1gG F（ab′）2. Similar behavior was observed when the same set up was tested in two clinical assays：（1） anti-Leishmania antigen polyclonal antibodies taken from Kala Azar positive patient serum interacting with Leishmania promastigote antigen, and （2） anti-p21 polyclonal antibodies interacting with p21 antigen. The proposed concept can represent a new immunodiagnostic technique and may have wide ranging applications in biosensors and nanobiotechnology too.

Total Antioxidant Status and Other Markers to Distinguish Severely Obese Volunteers with and without Metabolic Syndrome

Obesity (Ob) is an accepted major risk factor for the metabolic syndrome (MetS), a combination of at least three of five risk factors, which predispose to high oxidative stress (OS), but all obese do not show symptoms of MetS. There is dearth of data comparing OS homeostasis of severely obese adults with and without MetS, and need for biomarkers to help in differential diagnosis. Erythrocytic lipid and protein damage markers, malondialdehyde (MDA) and protein carbonyl (PCO), antioxidant enzymes erythrocytic superoxide dismutase(SOD), catalase (CAT), plasma glutathione peroxidase (GPX), and total antioxidant capacity (TAC) as ferric-reducing-ability-of-plasma (FRAP) were compared to understand OS homeostasis among 102 severely Ob (body mass index > 30), 102 Ob with severe (z-score > 2) MetS as per National Cholesterol Education Program-Adult Treatment Panel III guidelines and 100 healthy non-obese Controls. MDA/PCO and all antioxidant enzymes were lowest for ObMetS, followed by Ob, indicating greater damage to protein moieties of the erythrocytic membrane. Multiple regression analysis confirmed z-scores > 2 as significant predictor of lowered enzymes and TAC. Receiver Operator Curve analysis predicted that TAC was the most potential biomarker for the diagnosis and prognosis of MetS with an Odds Ratio of 88.5 indicating the high probability that FRAP would be low for ObMetS (z-score > 2) than for Ob with BMI > 30, but z-scores < 1. TAC is qualified as the most effective biomarker to distinguish between severely obese respondents with and without metabolic syndrome, and as a useful candidate for study of homeostatic breakdown in metabolic syndrome and the importance of z-score in assessment of MetS in obese respondents.
