This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

AIM:To elucidate etiologic associations between Helicobacter pylori(H pylori),lifestyle,environmental factors and gastric cardiac adenocarcinoma(GCA)among men. METHODS:A hospital-based case-control study was conducted in Taiwan from 2000 to *** cases were newly confirmed as primary *** controls were selected matching with age,sex,and admission date to each *** were informedof potential risk factors with a structured question- naire by trained interviewers during hospitalization and provided a blood sample for the determination of H pylori *** ratio(OR)and 95%confidence interval(95%CI)were used to evaluate risk,and a multivariate conditional logistic regression model was performed. RESULTS:All participants recruited for this study were men,consisting of 41 cases and 205 *** of the univariate analysis showed that significant factors associated with the etiology of GCA included H pylori(OR =2.69,95%CI=1.30-5.53),cigarette smoking(OR= 2.28,95%CI=1.05-4.96),working or exercising after meals(OR=3.26,95%CI=1.31-8.11),salted food (OR=2.51,95%CI=1.08-6.11),fresh vegetables(OR =0.28,95%CI=0.09-0.80),fruits(OR=0.19,95% CI=0.04-0.89),and rice as principal food(OR=0.53, 95%CI=0.30-0.85).Multivariate conditional logistic regression models indicated that a significantly elevated risk of contracting GCA was associated with working or exercising after meals(OR=3.18,95%CI=1.23-9.36) and H pylori infection(OR=2.93,95%CI=1.42-6.01). In contrast,the consumption of fresh vegetables(OR =0.22,95%CI=0.06-0.83),fruits(OR=0.28,95% CI=0.09-0.79)and rice as principal food(OR=0.48, 95%CI=0.24-0.93)remained as significant beneficial factor associated with GCA. CONCLUSION:Working or exercising after meals and H pylori infection increase the risk of GCA,but higher intakes of rice,fresh vegetables and fruits reduce the risk.

The production and utilization of high-energetic explosives often pose a range of safety hazards,with sensitivity being a key factor in evaluating these *** investigate how temperature,particle size,and air humidity affect the responsiveness of commonly used high-energetic explosives,a series of BAM(Bundesanstalt für Materialforschung und-prüfung)impact and friction sensitivity tests were carried out to determine the critical impact energy and critical load pressure of four representative high-energetic explosives(RDX,HMX,PETN and CL-20)under different temperatures,particle sizes,and air humidity *** experimental findings facilitated an examination of temperature and particle size affecting the sensitivity of high-energetic explosives,along with an assessment of the influence of air humidity on sensitivity *** results clearly indicate that high-energetic explosives display a substantial decline in critical reaction energy when subjected to micrometre-sized particles and an air humidity level of 45%at a temperature of 90℃.Furthermore,it was noted that the critical reaction energy of high-energetic explosives diminishes with an increase in temperature within 25℃−90℃.In the same vein,as the particle sizes of high-energetic explosives increase,so does the critical reaction energy for micrometre-sized *** air humidity significantly affects the sensitivity testing of high-energetic explosives,emphasizing the importance of refraining from conducting sensitivity tests in such conditions.

The Pearl River Delta(PRD)region on the southeast coast of China has long been known as a highly productive fishing *** the late 1980s,fishing pressure in the PRD has been intense,which warrants concerns of potential fishery-related impacts on the food resources and foraging ecology of apex marine predators in this region,such as the Indo-Pacific humpback dolphin(Sousa chinensis).In this study,we examined 54 stomachs with food remains,collected from beached carcasses of humpback dolphins recovered during fifteen years between 2003 and *** 6043 identified prey items represent 62 teleost taxa,primarily small estuarine fish,but also larger reef *** dolphins appear to be opportunistic foragers,hunting across the water-column,with preference for shoaling and meaty fishes(*** lucidus IRI%=38.6%,Johnius belangerii IRI%=23.1%,Mugil cephalus IRI%=14.0%).Our findings suggest a dietary shift in recent years,from primarily demersal(as previously reported)to greater intake of neritic and pelagic *** foraging group size has decreased in recent years,which corresponds with declining size and numbers of prey items retrieved from dolphin *** suggest that these are indicators of declining food *** with a shortage of preferred prey,humpback dolphins may have broadened their dietary spectrum to maintain their daily energy intake,while their foraging group size decreased in response to the altered tradeoff between the costs and benefits of group foraging.

Development in proteome research leads to new demand for protein analysis. The assay of trace proteins in complicated biological fluid such as human urine provides an important non-intrusive method to identify newly discovered pharmaceutical proteins and for diagnosis of kidney related diseases. Due to the extreme low levels of proteins and the complicated sample matrix, time-consuming purification step has to be used, leading to the loss of analyte proteins during sample pretreatment. The recent development of Gas-phase Electrophoretic Mobility Molecular Analyzer（GEMMA） enables separation of protein particle by their different electrophoretic mobility in air prior to subsequent number counting, an inherently sensitive detection technique. It can directly measured proteins at trace levels and obviate the need for sample pretreatment before analysis. Two different types of urinary proteins are studied: early pregnancy indicating proteins（h CG） present in trace levels and pathologic urinary proteins（IgG, HSA and β2-MG） at different levels. The results indicate a short ten-minute assay time without the need for sample pretreatment and an extremely high sensitivity to detect proteins under investigation. The capability and limitation of GEMMA for protein determination will be discussed at the meeting.

長期吸煙者的肺功能研究

目的瞭解長期吸煙對肺功能的影響,並及早發現慢性阻塞性肺病(chronicobstructive pulmonary disease,COPD)。方法2017年5月~2019年12月,對長期吸煙者進行肺通氣功能檢查,對早期COPD和肺功能異常進行篩查。結果吸煙組199例,非吸煙組73例完成肺功能檢查。吸煙組的1秒率(FEV1/FVC)和FEV1%Pred明顯低於非吸煙組。兩組肺功能異常檢出異常率分别為46.2%和15.1%,吸煙組明顯高於非吸煙組(χ^(2)=22.04,P<0.01),其中COPD檢出率36.2%(95%CI:29.5%~42.9%);保留比率受損肺功能(Preserved ratio impaired spirometry,PRISm)檢出率10.1%(95%CI:5.8%~14.2%)。非吸煙組COPD檢出率6.8%(95%CI:1.1%~12.6%);PRISm檢出率8.2%(95%CI:5.2%~14.5%),吸煙組COPD檢出率明顯高於非吸煙組。結論長期吸煙人群中肺功能異常率高、可能COPD檢出率高。肺功能檢查有助於早期發現COPD和肺功能異常人群,為慢性肺病的早期防治提供依據。

以3,4-二甲氧基吡喃酮为吸电子基团、半花菁染料为给电子基团,通过Witting反应合成了一种基于真菌荧光素的近红外荧光探针fung。通过^(1)HNMR、^(13)CNMR和HRMS表征了其结构,测试其对不同溶液体系的黏度、极性的响应性能,考察其在响应过程中的抗干扰能力、p H适用范围。结果表明,探针fung在水中的斯托克斯位移为178 nm,荧光发射波长(λ_(em))为700 nm,能同时对溶液体系的黏度和极性作出响应,符合扭曲的分子内电荷转移机制;在不同体积分数甘油水溶液中,探针fung对溶液黏度的响应表现为荧光强度线性变化,随着溶液的黏度的增加,其在700 nm处荧光强度增加6倍多,荧光寿命也能随之增加;探针fung对溶液极性的响应表现为荧光发射光谱的红移,随着溶剂极性参数的增加,λ_(em)从606 nm红移至698 nm;在甲苯/二甲基亚砜(DMSO)中,随着甲苯体积分数从100%减至10%,λ_(em)从669 nm红移至712 nm;在1,4-二氧六环/DMSO中,随着1,4-二氧六环体积分数从100%减至10%,λ_(em)从631 nm红移至717 nm;探针fung对溶液黏度和极性的响应具有选择性好、抗干扰能力强、pH适用范围广等特点。
