目的探讨胃内球囊术治疗不同体质量指数(BMI)肥胖症的短期临床疗效。方法采用回顾性描述性研究方法。收集2017年1月至2021年12月3家医学中心收治的62例(赫斯特维尔私立医院56例、新南威尔士大学圣乔治私立医院4例、东悉尼私立医院2例)不同BMI肥胖症行胃内球囊术患者的临床资料;男15例,女47例;年龄为45.1(18.0~67.0)岁;BMI为35.1(27.8~48.4)kg/m^(2)。62例患者中,27 kg/m^(2)≤BMI<35 kg/m^(2)33例,BMI≥35 kg/m^(2)29例。观察指标:(1)首次手术及术后并发症情况。(2)首次术后随访情况。(3)再次行胃内球囊术及术后并发症情况。(4)再次行胃内球囊术后随访情况。采用门诊方式进行随访。患者于术后1、3、6个月进行随访,了解术后并发症发生情况及减重效果。正态分布的计量资料以x±s表示,偏态分布的计量资料以M(范围)表示。计数资料以绝对数表示。结果(1)首次手术及术后并发症情况。62例肥胖症患者均顺利施行首次胃内球囊术。62例患者中,45例发生术后并发症,包括胃食管反流38例,吞咽困难或打嗝12例,上腹部疼痛9例,恶心呕吐4例,同一例患者可合并多种并发症。33例27 kg/m^(2)≤BMI<35 kg/m^(2)患者和29例BMI≥35 kg/m^(2)患者发生上述并发症分别为21、8、7、2例和17、4、2、2例。(2)首次胃内球囊术后随访情况。62例肥胖症患者中,完成术后1、3、6个月随访分别为52、46、38例,其中27 kg/m^(2)≤BMI<35 kg/m^(2)和BMI≥35 kg/m^(2)分别为30、25、20例和22、21、18例。62例患者中,选择提前取出胃内球囊15例,包括因持续恶心呕吐7例,对短期减重效果不满意6例,急性上腹部疼痛、妊娠各1例。30例27 kg/m^(2)≤BMI<35 kg/m^(2)患者和22例BMI≥35 kg/m^(2)患者术后1个月体质量下降分别为(4.8±3.0)kg和(6.6±2.8)kg;25例27 kg/m^(2)≤BMI<35 kg/m^(2)患者和21例BMI≥35 kg/m^(2)患者术后3个月体质量下降分别为(6.7±4.4)kg和(10.6±4.8)kg;20例27 kg/m^(2)≤BMI<35 kg/m^(2)患者和18例BMI≥35 kg/m^(2)患者术后6个月体质量下降分别为(8.5±4.8)kg和(9.8±3.9)kg,多余体质量减轻百分比分别为53%±26%和29%±15%,总体质量减轻率分别为14%±5%和10%±5%。(3)再次行胃内球囊术及术后并发症情况。完成首次术后6个月随访的38例肥胖症患者中,9例27 kg/m^(2)≤BMI<35 kg/m^(2)和7例BMI≥35 kg/m^(2)患者选择取出首次胃内球囊后立即再次行胃内球囊术,再次手术后发生胃食管反流、吞咽困难或打嗝、上腹部疼痛、恶心呕吐分别为4、3、0、0例和4、0、2、1例。(4)再次行胃内球囊术后随访情况。16例选择再次行胃内球囊术患者均完成术后6个月随访,无患者提前取出胃内球囊。9例27 kg/m^(2)≤BMI<35 kg/m^(2)患者和7例BMI≥35 kg/m^(2)患者术后6个月体质量下降分别为(8.8±8.5)kg和(18.9±9.7)kg,多余体质量减轻百分比分别为44%±38%和41%±15%。结论胃内球囊术应用于BMI≥27 kg/m^(2)肥胖症患者可获得较好短期减重效果,胃食管反流、吞咽困难或打嗝、上腹部疼痛、恶心呕吐是术后常见并发症。

In this paper, we address the control problem of an uncertain robotic manipulator with input saturations, unknown input scalings and disturbances. For this purpose, a model reference adaptive control like （MRAC-like） is used to handle the input saturations. The model reference is input to state stable （ISS） and driven by the errors between the required control signals and input saturations. The uncertain parameters are dealt with by using linear-in-the-parameters property of robotic dynamics, while unknown input scalings and disturbances are handled by non-regressor based approach. Our design ensures that all the signals in the closed-loop system are bounded, and the tracking error converges to the compact set which depends on the predetermined bounds of the control inputs. Simulation on a planar elbow manipulator with two joints is provided to illustrate the effectiveness of the proposed controller.

Nanohairs, which can be found on the epidermis of Tokay gecko＇s toes, contribute to the adhesion by means of van der Waals force, capillary force, etc. This structure has inspired many researchers to fabricate the attachable nano-scale structures. However, the efficiency of artificial nano-scale structures is not reliable sufficiently. Moreover, the mechanical parameters related to the nano-hair attachment are not yet revealed qualitatively. The mechanical parameters which have influence on the ability of adhesive nano-hairs were investigated through numerical simulation in which only van der Waals force was considered. For the numerical analysis, finite element method was utilized and van der Waals force, assumed as 12-6 Lennard-Jones potential, was implemented as the body force term in the finite element formulation.

Flood damage assessment(FDA)is a key component of risk-based method for flood *** the current FDA approaches,generally the uniqueness of the building is disregarded in the ***,they are unfit for detailed applications in which case-by-case analysis of building damage is an essential *** limitation is compounded by the use of incomplete and often low-quality data inputs about the building and the assumptions and approximations made regarding the geometry and materials of its *** shortcomings may result in incomplete and uncertain *** the benefits and increasing use of three-dimensional(3D)urban modeling and Building Information Model in various urban management processes,in this paper,an integrated framework for utilization of detailed 3D building models for the assessment and 3D visualization of flood damage to building according to its distinct behavior against flood is presented.A proof-ofconcept demonstration of the framework in a case study underlined the feasibility of implementation of the framework,which can potentially benefit a variety of users and be used as a complementary approach to the current FDA methods for improving the resilience of the community toward floods and their adverse impacts.

The sliding wear behaviors of ZrO2-22 wt pct MgO （MZ） and ZrO2-8 wt pct Y2O3（YZ） coatings deposited on a cast aluminum alloy with bond layer （NiCrCoAlY） by plasma spray were investigated under dry test conditions at room temperature. Under all load conditions, the wear mechanisms of the MZ and YZ coatings were almost the same. The material transfer and pullout were involved in the wear process of the studied coatings under the test conditions. The wear rate of the MZ coating was less than that of the YZ coating. While increasing the normal load, the wear rates of the MZ and YZ coatings increased. SEM was used to examine the worn surfaces and to elucidate likely wear mechanisms. Energy dispersive X-ray spectroscopy （EDX） analysis of the worn surfaces indicated that material transfer occurred in the direction from the SiC ball to the disk. Fracture toughness had a significant influence on the wear performance of the coatings. It was suggested that the material transfer played an important role in the wear behavior.

The coal processing methods used at the Zarand coal washery plant are the heavy medium bath,jigs,and *** coal-containing materials that are fed to the plant are acquired from different mines,and they have different washability *** this paper,the validity of Mayer curve (M-curve),the conventional method for determining coal washability,was evaluated on blending of raw coals that are fed to the *** curves were prepared for the hand-blended samples and compared with the washability predicted by the Mayer *** samples from different seams and mines were blended by hand in 50:50 ratios,and sink and float tests were performed;the resulting washability curves were in good agreement with the washability curves predicted using the Mayer method for the same blending *** work resulted in the preparation of the optimum plant feed that can be achieved from the blending of different coal samples to produce the best plant yield at a given ash content.

Current study evaluated the effect of a fine and coarsely ground insoluble dietary fibre source on the gastrointestinal development of suckling *** hulls(OH)were selected as a model feedstuff,rich in cellulose,lignin,and insoluble dietary *** experimental supplemental diets were formulated:a finely ground,low fibre and nutrient dense diet served as control(CON).For the 2 high fibre diets,15%heat-treated starch in CON was exchanged with OH,either finely(OH-f)or coarsely ground(OH-c).Litters of 10 primi-and multiparous sows(mean litter size 14.6±0.84)were *** a litter,experimental diets were allotted to triplets of 4 *** approximately 12 d of age,piglets’individual feed intakes were recorded 2 times per day when separated from their dam for 70 *** could suckle with their dam for the remainder of the *** d 24 and 25,from the total pool of 120 piglets,seven healthy well-eating piglets per treatment were selected for post-mortem evaluation,resulting in 14 replicates per *** of OH-c and OH-f did not impede clinical health and production performance of *** full stomach weights tended to be greater for OH-c compared to OH-f whereas CON was intermediate(P=0.083).Supplementing OH significantly increased ileal villus height and caecal dry matter concentration(P<0.05).For the colon,OH increased its length,contents weight,short-chain fatty acid concentration and reduced total bacterial count as well asγ-proteobacteria count and proportion(P<0.05).The OH-c treatment specifically increased full gastrointestinal tract weight and caecum contents weight compared to piglets fed CON and ***,OH-c reduced colonic crypt depth when compared to OH-f(P=0.018).In conclusion,supplementing OH to a diet for suckling piglets exerted subtle developmental effects on gastrointestinal morphology and colonic microbial *** effects were largely independent from the particle size of the OH.

Management of gastroparesis remains challenging,particularly in pediatric *** care and pharmacological therapies for symptoms remain the mainstay *** they are effective for mild and some moderately severe cases,often time they do not work for severe *** are a few prokinetics available,yet the use of these drugs is limited by a lack of persistent efficacy and/or safety ***,the only modality for adult patients with severe intractable gastroparesis is surgery,e.g.,pyloroplasty and partial gastrectomy,however,this option is generally considered too radical for a growing *** therapeutic approaches,particularly those which are less invasive,are *** article explores gastric electrical stimulation(GES),a new therapy for *** others,it neither needs medications nor gastrectomy;rather,it treats through the use of microelectrodes to deliver high-frequency low energy electric stimulation to the pacemaker area of the ***,it is tolerated and safe in *** in adult patients,GES appears to work in releasing symptoms,improving nutrition,and enhancing the quality of life;it also helps wean off medications and eliminate many needs for *** the transient nature of gastroparesis in children in many occasions,GES is considered a“bridging”therapy after failed medical interventions and before surgery.

The effects of chemical spills on aquatic nontarget organisms were evaluated in this *** on a review of three types of current eco-toxicological models of chemicals,i.e.,ACQUATOX model of the US-EPA,Hudson River Model of PCBs,and critical body residual(CBR)model and dynamic energy budget(DEBtox)model,this paper presents an uncoupled numerical ecotoxicological *** transport and transformation of spilled chemicals were simulated by a chemical transport model(including flow and sediment transport),and the mortalities of an organism caused by the chemicals were simulated by the extended threshold damage model,*** to extreme scarcity of data,this model was applied to two hypothetical cases of chemical spills happening upstream of a *** analysis and simulated results indicated that this model is capable of reasonably predicting the acute effects of chemical spills on aquatic ecosystems or organism killings.
