Brain-derived neurotrophic factor as a potential biomarker of cognitive recovery in schizophrenia

Brain-derived neurotrophic factor(BDNF) has been proposed as a biomarker of schizophrenia and, more specifically, as a biomarker of cognitive recovery. Evidence collected in this review indicates that BDNF is relevant in the pathophysiology of schizophrenia and could play a role as a marker of clinical response. BDNF has been shown to play a positive role as a marker in antipsychotic treatment, and it has been demonstrated that typical antipsychotics decrease BDNF levels while atypical antipsychotics maintain or increase serum BDNF levels. Furthermore, BDNF levels have been associated with severe cognitive impairments in patients with schizophrenia. Consequently, BDNF has been proposed as a candidate target of strategies to aid the cognitive recovery process. There is some evidence suggesting that BDNF could be mediating neurobiological processes underlying cognitive recovery. Thus, serum BDNF levels seem to be involved in some synaptic plasticity and neurotransmission processes. Additionally, serum BDNF levels significantly increased in schizophrenia subjects after neuroplasticity-based cognitive training. If positive replications of those findings are published in the future then serum BDNF levels could be definitely postulated as a peripheral biomarker for the effects of intensive cognitive training or any sort of cognitive recovery in schizophrenia. All in all, the current consideration of BDNF as a biomarker of cognitive recovery in schizophrenia is promising but still premature.

Results By analyzing six pathological complete response(pCR) patients and 25 patients with non-pCR,300 probes(231 genes) were identified as differentially expressed between pCR and residual disease by the sam program when the fold change was>*** gene functional enrichment analysis revealed 15 prominent gene categories that were different between pCR and non-pCR patients,most notably the genes involved in the PPAR(Peroxisome Proliferator Activated Receptor),DNA repair and ER signal pathways and in the immune-related gene *** qRT-PCR analysis results for the genes in the PPAR pathway(LPL,SORBSl,PLTP,SCD5,mmPl and CSTA) in independent validation set were consistent with the results from the microarray data analysis. Can Intensified Tuberculosis Case Finding Efforts at Nutrition Rehabilitation Centers Lead to Pediatric Case Detection in Bihar, India?

Introduction: Seven district-level Nutritional Rehabilitation Centres (NRCs) in Bihar, India provide clinical and nutritional care for children with severe acute malnutrition (sam). Aim: To assess whether intensified case finding (ICF) strategies at NRCs can lead to pediatric case detection among sam children and link them to TB treatment under the Revised National Tuberculosis Control Programme (RNTCP). materials and methods: A retrospective cohort study was conducted that included medical record reviews of sam children registered for TB screening and RNTCP care during July-December 2012. Results: Among 440 sam children screened, 39 (8.8%) were diagnosed with TB. Among these, 34 (87%) initiated TB treatment and 18 (53%) were registered with the RNTCP. Of 16 children not registered under the RNTCP, nine (56%) weighed below six kilo-grams—the current weight requirement for receiving drugs under RNTCP. Conclusion: ICF approaches are feasible at NRCs;however, screening for TB entails diagnostic challenges, especially among sam children. However, only half of the children diagnosed with TB were treated by the RNTCP. more effort is needed to link this vulnerable population to TB services in addition to introducing child-friendly drug formulations for covering children weighing less than six kilo-grams.

Objective: This study was designed to evaluate the long-term safety and efficacy of 0.1%tacrolimus ointment in adult and pediatric patients with atopic dermatitis (AD). methods: A total of 408 adult and 391 pediatric patients with AD who had participated in a previous clinical trial of tacrolimus ointment were enrolled in this long-term, open-label, noncomparative trial. Tacrolimus ointment 0.1%was applied twice daily either intermittently or continuously to the affected areas. Efficacy and safety assessments included percent body surface area affected, Eczema Area and Severity Index score, individual signs of AD, and the incidence of adverse events. Results: A total of 799 patients were evaluated, of whom 300 (37.5%) were followed for more than 3 years (maximum 49 months). Improvements in efficacy parameters were observed within 1 week of treatment and continued for the duration of the study. Common adverse events included skin burning, pruritus, skin infection, skin erythema, flu-like symptoms, and headache. The incidence of adverse events, including cutaneous infections, did not increase with time on study. Conclusion: Tacrolimus ointment therapy is a rapidly effective and safe treatment for the management of AD in pediatric and adult patients for up to 4 years.

Goals: To determine whether increased pulmonary artery pressure(PAP) following transjugular intrahepatic portosystemic shunting (TIPSS) results in short-term mortality or cardiorespiratory complications. Background: TIPSS is frequently performed for complications of cirrhosis. PAP increases following TIPSS; however consequences of this phenomenon are unknown. Study: Demographics, disease severity and etiology were recorded among patients undergoing TIPSS. PAP before and following TIPSS were measured and the relationship between PAP before and after TIPSS, and subsequent cardiorespiratory complications and mortality was examined. Results:Thirty-one patients were enrolled (mean age 53 years,74% men,55% Child-Pugh class C cirrhosis). TIPSS was performed for variceal bleeding in 84% of cases. Ten patients (32% ) died 5-20 days following TIPSS. PAP increased significantly following TIPSS (mean 20.8 mm Hg pre-TIPSS (95% CI 18.2-23.4) to 26.9 mm Hg post-TIPSS(95% CI 24.2-29.6, P=0.0016). Congestive heart failure developed in 4 patients (13% ), sepsis in 4 (13% ), and ARDS in 8 (26% ). Increased PAP following TIPSS was not associated with early mortality (P=0.13), CHF(P=0.31), or ARDS (P=0.43). ARDS was the only significant predictor of short-term mortality following TIPSS (OR 18.7,P=0.02 (95% CI 1.5-232)). Conclusion: PAP increases after TIPSS and cardiorespiratory complications are common, yet unrelated to increased PAP. ARDS is independently associated with increased risk of mortality after TIPSS.

Objective: Intercellular adhesion molecule (ICAm) expression in liver and bile duct remnant is a feature of the inflammatory component of biliary atresia (BA). Circulating levels of such soluble adhesion molecules (sam) should reflect intrahepatic disease and would prove a useful adjunct in the evaluation of BA. Study Design: Serum ICAm-1 (sICAm-1), serum vascular cell adhesion mol-ecule (sVCAm-1), and serum E selectin (sE-selectin) were measured by enzyme-linked immunosorbent assay in infants with BA at the time of portoenterostomy and stratified by outcome. Results are quoted as medians (range). Binary logistic regression analysis and actuarial survival curves were used to investigate the relationship of sam profile to outcome. Significance was assumed at P values of ≤ .05. Results: Sixty-one infants with BA were treated between 1996 and 2002 and at follow-up; 39 of these were alive and jaundice-free (good outcome, n=39); 21 had been transplanted, and 1 died (poor outcome, n=22). Preoperative values for sICAm-1 were 1233 (400-2000) ng/mL; sVCAm-1, 1204(517-1921) ng/mL; and sE-selectin, 71(26-192) ng/mL. sVCAm-1(Pm-1(P mpared with normal control infants, although sE-selectin did not (P = .17). There was a significant correlation of age at surgery with sICAm-1 (r = 0.33, P = .01) but not with sVCAm-1 (r = 0.16, P = .23) or sE-selectin (r = 0.05, P = .70). Binary logistic regression analysis showed that the variables, sICAm-1, sE-selectin, or age at surgery, were not significant predictors of outcome, although sVCAm-1 approached significance (P = .069). A cutoff value for sVCAm-1 of 1380 ng/mL was defined by receiver operating characteristic curve analysis, and the cohort of patients with sVCAm-1 (>1380 ng/mL) showed a significantly worse actuarial survival (P = .05). Conclusion: Levels of sICAm-1 and sVCAm-1 are grossly elevated in BA, whereas sE-selectin levels are normal. Only sVCAm-1 levels have prognostic signi
