Adenosine or Magnesium Sulphate as Adjuvants for Pectoral Nerves Block in Modified Radical Mastectomy: A Randomized Controlled Trial

Introduction: Magnesium sulphate can prolong the effect of local anaesthetics. Adenosine has not been compared to other local anaesthetics adjuvants. In the present study we aimed to compare the effect of adding magnesium sulphate and adenosine to bupivacaine for pectoral nerves block. Patients and Methods: In this randomized controlled trial, 90 females scheduled for breast surgery were included. Patients were divided into three groups. Patients received general anaesthesia with pectoral nerves block. Group C, A, and M had bupivacaine only, bupivacaine + adenosine, and bupivaciane + magnesium sulphate respectively. Post-operative visual analogue score, block duration, post-operative morphine consumption, sedation score, and peri-operative haemodynamic changes were recorded. Results: Visual analog score was lower in Group M compared to other groups. Group A and M showed significant longer duration of the block. Group M showed significant longer duration of action than Group A (p = 0.034). The total peri-operative morphine used was higher in Group C (p 0.05). Conclusions: Local anaesthetic adjuvants such as adenosine or magnesium sulphate can improve pectoral nerves block characteristics. Magnesium sulphate has an advantage over adenosine by increasing the block duration.

Small for size syndrome difficult dilemma: Lessons from 10 years single centre experience in living donor liver transplantation

AIM To analyze the incidence, risk factors, prevention, treatment and outcome of small for size syndrome(SFSS) after living donor liver transplantation(LDLT). METHODS Through-out more than 10 years: During the period from April 2003 to the end of 2013, 174 adult-to-adults LDLT(A-ALDLT) had been performed at National Liver Institute, Menoufiya University, Shibin Elkoom, Egypt. We collected the data of those patients to do this cohort study that is a single-institution retrospective analysis of a prospectively collected database analyzing the incidence, risk factors, prevention, treatment and outcome of SFSS in a period started from the end of 2013 to the end of 2015. The median period of follow-up reached 40.50 m, range(0-144 m). RESULTS SFSS was diagnosed in 20(11.5%) of our recipients. While extra-small graft [small for size graft(SFSG)], portal hypertension, steatosis and left lobe graft were significant predictors of SFSS in univariate analysis(P = 0.00, 0.04, 0.03, and 0.00 respectively); graft size was the only independent predictor of SFSS on multivariate analysis(P = 0.03). On the other hand, there was lower incidence of SFSS in patients with SFSG who underwent splenectomy [4/10(40%) SFSS vs 3/7(42.9%) no SFSS] but without statistical significance, However, there was none significant lower incidence of the syndrome in patients with right lobe(RL) graft when drainage of the right anterior and/or posterior liver sectors by middle hepatic vein, V5, V8, and/or right inferior vein was done [4/10(28.6%) SFSS vs 52/152(34.2%) no SFSS]. The 6-mo, 1-, 3-, 5-, 7-and 10-year survival in patients with SFSS were 30%, 30%, 25%, 25%, 25% and 25% respectively, while, the 6-mo, 1-, 3-, 5-, 7-and 10-year survival in patients without SFSS were 70.1%, 65.6%, 61.7%, 61%, 59.7%, and 59.7% respectively, with statistical significant difference(P = 0.00). Nonlinear restructuring of patterned thin films by residual stress engineering into out-of-plane wavy-shaped electrostatic microactuators for high-performance radio-frequency switches

Electrostatic microelectromechanical (MEMS) switches are the basic building blocks for various radio-frequency (RF) transceivers. However, conventional cantilever-based designs of MEMS switches require a large actuation voltage, exhibit limited RF performance, and suffer from many performance tradeoffs due to their flat geometries restricted in two dimensions (2D). Here, by leveraging the residual stress in thin films, we report a novel development of three-dimensional (3D) wavy microstructures, which offer the potential to serve as high-performance RF switches. Relying on standard IC-compatible metallic materials, we devise a simple fabrication process to repeatedly manufacture out-of-plane wavy beams with controllable bending profiles and yields reaching 100%. We then demonstrate the utility of such metallic wavy beams as RF switches achieving both extremely low actuation voltage and improved RF performance owing to their unique geometry, which is tunable in three dimensions and exceeds the capabilities of current state-of-the-art flat-cantilever switches with 2D-restricted topology. As such, the wavy cantilever switch presented in this work actuates at voltages as low as 24 V while simultaneously exhibiting RF isolation and insertion loss of 20 dB and 0.75 dB, respectively, for frequencies up to 40 GHz. Wavy switch designs with 3D geometries break through the design limits set by traditional flat cantilevers and provide an additional degree of freedom or control knob in the switch design process, which could enable further optimization of switching networks used in current 5G and upcoming 6G communication scenarios.

Effect of Notch Severity on Fatigue Behaviour of ADI Castings

An investigation was carried out to examine the influence of notch sensitivity on fatigue behaviour of austempered ductile iron (ADI). Samples were made of ductile iron with a chemical composition of 3.55% C, 2.18% Si, 0.35% Mn, 0.022 P, 0.008 S and 0.045% Mg. The samples were heat treated by austenitizing at 900℃?for 1 h and then rapidly quenched into two different salt baths at 350 and 400℃?for 1 h each. This work aims at studying the capability of mechanical stress analysis software (ANSYS 12.0), as a nondestructive tool, to characterize and quantify the fatigue strength of the notched ADI samples with different radii (from 1 to 3 mm). In addition, experimental testing was performed using rotary bending fatigue machine on notched samples with radii of 1, 1.5 and 2-mm to verify the theoretical data obtained by ANSYS-12. The results show that the predicted fatigue strength estimated by ANSYS-12 is very close to the experimental one. Therefore, fatigue performance of the rotating parts made of ADI can be estimated theoreticcally using ANSYS 12.0. The fatigue strength of these parts can also be improved by increasing the formed filled radius due to decreasing the notch sensitivity factor.

褪黑素能调节多项生理功能，但其对妊娠结局的影响所知甚少。本研究旨在探讨补充褪黑素对12～1aN龄的Wistar-Kyoto（WKY）和Sprague-Dawley（SD）大鼠妊娠结局的影响。动情前期确认后，每只雌性大鼠与一只同品种雄性大鼠同笼饲养过夜，第二天阴道涂片确认交配后，雌性大鼠转移至单独的代谢笼，WKY大鼠每天给予含0、25、50或100mg／kg褪黑素的饮水，SD大鼠则每天给予含0或100mg／kg褪黑素的饮水，从怀孕第一天持续至产后21天。记录母鼠体重、孕期、窝产仔数、仔鼠出生体重和出生后7～42d体重及仔鼠死亡率，使用重复测量的方差分析对数据进行分析。结果显示，与对照组相比，褪黑素组母鼠孕期体重增长较小（P〈0．01），窝产仔数减少（尸〈0．01），仔鼠平均出生体重仅在100mg／kg褪黑素组显著降低（P〈0．001），出生后7～42d仔鼠平均体重显著降低（P〈0．01）。25和100mg／kg褪黑素组WKY仔鼠死亡率分别为9．5％和21．6％，且都发生在断奶21天后。以上结果提示，产前和产后补充褪黑素会严重影响WKY和SD大鼠窝产仔数、仔鼠生长和死亡率。

Efficacy and safety of once daily tacrolimus compared to twice daily tacrolimus after liver transplantation

BACKGROUND Once daily tacrolimus regimen was found to exhibits similar bioavailability,safety and efficacy properties compared to twice-daily tacrolimus in kidney transplantation *** To compare the efficacy and safety of once-daily prolonged release tacrolimus compared to twice-daily tacrolimus in liver transplantation *** MEDLINE,EMBASE,CENTRAL databases were searched for clinical trials until December *** outcome measured as the rate of treatment failure indicated by biopsy-proven acute rejection,Serum creatinine,graft loss,or *** reviewers independently selected studies,collected data and assessed risk of *** results are reported as risk ratio with 95%confidence interval(CI)for dichotomous *** Seven studies included with 965 *** the included studies were of moderate quality according to the risk of bias assessment using Cochrane Risk of Bias ***-proven acute rejection was reported in four studies,and pooled analysis of those studies indicated similar rejections in both twice daily and once daily tacrolimus groups(risk ratio:1.06,95%CI:0.84-1.34,n=758,I2=0%)and also we found no significant difference between both groups for renal outcome(serum creatinine;mean difference,0.001 mg/dL,95%CI:-0.042 to 0.043,n=846,I2=18.6%).Similarly,there was similar number of adverse events such as hypertension,headache,back pain,blood related disorders,infections and nausea observed in both *** The analysis findings confirm that both once daily and twice daily tacrolimus formulations are comparable in terms of efficacy and safety outcomes.

Experimental Testing and Validation of the Mathematical Model for a Self-Humidifying PEM Fuel Cell

This paper presents an experimental testing and validation results for a zero-dimensional self-humidifying PEM (Proton Exchange Membrane) fuel cell stack. The model incorporates major electric and thermodynamic variables and parameters involved in the operation of the PEM fuel cell under different operational conditions. The mathematical equations are modelled by using Matlab-Simulink tools in order to simulate the operation of the developed model with a commercially available 1 kW Horizon (H-1000) PEM fuel cell stack, which is used for the purposes of model validation and tuning of the developed model. The model is mathematically modelled and presented in the recent published work of authors. The observations from model simulations provide sufficient evidence and support to the results and observations obtained from testing 1 kW Horizon (H-1000) PEM fuel cell stack used in this research. The developed model can be used as a generic model and simulation platform for a self-humidifying PEM fuel cell with an output power varying from 50 W to 1 kW, with extrapolation to higher powers is also possible.

Environmental Impact of High Altitudes on the Operation of PEM Fuel Cell Based UAS

Fuel cell is a device that converts the chemical energy in the reactants into the electrical energy after steps of sequential electrochemical processes with no significant impact on the environment. For high altitude long endurance (HALE) of unmanned aircraft system (UAS) where fuel cell operates as a prime source of power, the operation and performance of a PEM fuel cell at different level of altitudes is vitally important. In this paper, the impact of direct using extracted air from high altitudes atmosphere in order to feed the stack is investigated, and the governing equations of the supplied air and oxygen to the PEM fuel cell stack are developed. The impact of high altitudes upon the operation and the consumption of air are determined in order to maintain certain level of delivered power to the load. Also the implications associated with operating the PEM fuel cell stack at high altitudes and different technical solutions are proposed. Various modes of Integral, Proportional-Integral, and Proportional-Integral-Derivative controller are introduced and examined for different time setting responses in order to determine the most adequate trade-off choice between fast response and reactants consumption which provides the necessary optimization of the air consumption for the developed model of PEM fuel cell used for UAS operation.
