Effects of resveratrol and methylprednisolone on biochemical,neurobehavioral and histopathological recovery after experimental spinal cord injury

Aim:To investigate the neuroprotective effect of resveratrol in an experimentalspinal cord injury(SCI)model in ***:Male Wistar albino rats weighing200-250 g were randomized into six ***-drop trauma was performedfor *** 1 underwent laminectomy *** 2 underwent laminectomyfollowed by *** 3,4,5,and 6 underwent laminectomy followed by SCIand received resveratrol(100 mg/kg),methylprednisolone(MP)(30 mg/kg),resveratrol(100 mg/kg)plus MP(30 mg/kg),and ethanol(2%),*** were divided into two subgroups for biochemical analysis(killed at 24 h aftersurgery)and for neurobehavioral and histopathological evaluation(killed at 6weeks after surgery).Posttraumatic neurological recovery after surgery was re-corded ***:Groups 3 and 5 revealed significantly lower malon-dialdehyde,nitric oxide,xanthine oxidase,and higher glutathione levels than group4(P<0.05).Neurological recovery rates were significantly better in groups 3 and5 than group 4(P<0.05).When spinal trauma size ratios were compared,there wasno significant difference between treatment ***:Resveratroltreatment revealed better biochemical recovery in the acute stage of trauma thanMP *** resveratrol and combined treatment revealed betterneurobehavioral recovery than MP treatment;resveratrol,MP,and combined treat-ment modalities improved histopathological recovery at the same level in the finalstage of the *** studies involving different doses of resveratroland different doses combinations with MP could promise better results as eachdrug has a different anti-oxidative mechanism of action.

Resection and primary anastomosis with or without modified blow-hole colostomy for sigmoid volvulus

AIM: To evaluate the efficacy of resection and primary anastomosis (RPA) and RPA with modified blow-hole colostomy for sigmoid volvulus. METHODS: From March 2000 to September 2007, 77 patients with acute sigmoid volvulus were treated. A total of 47 patients underwent RPA or RPA with modified blow-hole colostomy. Twenty-five patients received RPA (Group A), and the remaining 22 patients had RPA with modified blow-hole colostomy (Group B). The clinical course and postoperative complications of the two groups were compared. RESULTS: The mean hospital stay, wound infection and mortality did not differ significantly between the groups. Superficial wound infection rate was higher in group A (32% vs 9.1%). Anastomotic leakage was observed only in group A, with a rate of 6.3%. The difference was numerically impressive but was statistically not significant. CONCLUSION: RPA with modified blow-hole colostomy provides satisfactory results. It is easy to perform and may become a method of choice in patients with sigmoid volvulus. Further studies are required to further establish its role in the treatment of sigmoid volvulus.

Comparison of clinical and histopathological features of patients who underwent incidental or emergency appendectomy

BACKGROUND Incidental appendectomy can be defined as the removal of a clinically normal appendix during another surgical procedure unrelated to appendicitis or other appendicular *** To compare the demographic, biochemical, and histopathological features of the patients who underwent incidental and standard *** The demographic, biochemical, and histopathological data of 72 patients(Incidental App group) who underwent incidental appendectomy during living donor hepatectomy at our Liver Transplant Center between June 2009 and December 2016 were compared with data of 288 patients(Acute App group) who underwent appendectomy for presumed acute appendicitis. The Incidental App group was matched at random in a 1:4 ratio with the Acute App group in the same time frame. Appendectomy specimens of both groups were re-evaluated by two experienced *** Statistically significant differences were found between groups in terms of age(P= 0.044), white blood cell count(P < 0.001), neutrophil(P < 0.001), lymphocyte(P< 0.001), red cell distribution width(P = 0.036), mean corpuscular hemoglobin(P= 0.001), bilirubin(P = 0.002), appendix width(P < 0.001), and presence of acute appendicitis histopathologically(P < 0.001). However, no statistically significant differences were found between groups in terms of gender, platelet, mean platelet volume, mean corpuscular volume, platelet distribution width, appendix length. While the most common histopathological findings in the Incidental App group were normal appendix vermiformis(72.2%), fibrous obliteration(9.7%)and acute appendicitis(6.9%), the most common histopathological findings in the Acute App group were non-perforated acute appendicitis(62.8%), perforated appendicitis(16.7%), lymphoid hyperplasia(8.3%), and appendix vermiformis(6.3%).CONCLUSION Careful inspection of the entire abdominal cavity is useful for patients undergoing major abdominal surgery such as donor hepatectomy. Prediction of salinity intrusion in the sheltered estuary of Terengganu River in Malaysia using 1-D empirical intrusion model

Generally one dimensional （l-D） empirical salinity intrusion model is limited to natural alluvial estuary. However, this study attempts to investigate its ability to model a sheltered alluvial estuary of the Terengganu River in Malaysia. The constructed breakwater at the mouth of the river shelters the estuary from direct influence of the open sea. The salinity density along the estuary was collected during the wet and dry seasons for scenarios before and after the constructed breakwater. Moreover, the freshwater discharges, tidal elevations and bathymetry data were also measured as model inputs. A good fit was demonstrated between simulated and observed variables, namely salinity distribution and intrusion length for both scenarios. Thus, the results show that 1-D empirical salinity model can be utilized for sheltered estuarine condition at the Terengganu Estuary, but with an appropriate determination of an initial point. Furthermore, it was observed that the salinity intrusion in the study area is largely dependent on the freshwater discharge rather than tidal elevation fluctuations. The scale of the salinity intrusion length in the study area is proportional to the river discharge of the -1/2 power. It was appeared that the two lines of the 1-D empirical salinity model and discharge power based equation fitted well to each other, with the average predicted minimum freshwater discharge of 150 m^3/s is going to be required to maintain acceptable salinity levels during high water slack （HWS） near the water intake station, which is located at 10.63 km from river mouth.

Comparison of the effects of intravitreal bevacizumab and dexamethasone in experimental posterior penetrating eye injury

AIM： To compare the effects of intravitreal anti-vascular endothelial growth factor（VEGF） and dexamethasone in an experimental rabbit model of posterior penetrating ocular ***： Thirty white New Zealand rabbits were included in the study. A posterior penetrating ocular injury was performed at the superotemporal quadrant. They were randomly divided into three groups. The rabbits in group 1 received intravitreal dexamethasone, in group 2 they received intravitreal bevacizumab and those in group 3 received intravitreal physiological saline solution in both eyes. All eyes were examined ophthalmologically on the 1^st, 3^rd, 7^th, 14^th and 28^th days following the injury and the clinical findings were scored. On the day 28, the eyes were enucleated, evaluated and scored macroscopically, histopathologically and scanning electron ***： The median clinical score on the 14 th and 28 th days and the median macroscopic score of the dexamethasone group was significantly better than that of control（P=0.004, 0.018）. Dexamethasone group was also better than that of bevacizumab group but the differences did not reach statistical significance. Retinal detachment rate was 8.3%, 16.6% and 12.5% in the dexamethasone group, bevacizumab group and control group, respectively（P=0.476）. More extensive fibrocelluler proliferations were observed in controls compared with dexamethasone and bevacizumab groups. But these differences did not reach the statistical significance（P=0.538）. In scanning electron microscopy all groups showed fibreous stalk and dense collagen fibrils in vitreous. CONCLUSION： This study shows that intravitreal injection of both dexamethasone and bevacizumab may reduce the intraocular fibrous proliferation after an experimental posterior penetrating ocular injury in rabbits.

An assessment for the viability of recovering heat from a smoke extract system

Over the course of industrial manufacturing,additional heat within the extract systems is usually released into the atmosphere and its intrinsic energy is *** paper investigated a cold abatement smoke extract system for a fire testing wall furnace to determine the viability in recovering heat from the hot *** scenarios were investigated:1)the extract system was closed and only 300°C smoke was present;2)the system took in ambient air around the furnace and heat recovery occurred at 80°C in smoky air;3)the smoke had been removed from the air and the temperature was 60°*** was found that there was a significant build-up of soot on Scenarios 1&2 with a build-up rate of 0.25𝜇m/s which totalled 2.7 mm of soot after a three-hour *** soot had a low heat transfer rate and therefore acted as an insulator on the heat exchanger which reduced the efficiency significantly of it over *** to this loss in efficiency,it was more viable to recover heat in Scenario 3 at 60°C in clean air than it was to recover heat at 300°C or 80°C in smoky *** results show that having clean air was more important than a higher temperature when it came from recovering heat from a cold abatement system for a fire testing *** paper contributes to reveal the possibilities of harnessing the“waste heat”for use in other applications in the vicinity of the manufacturing processes,such as heating water within a central heating plant,domestic hot water or electricity generation,or re-cycled within the industrial plant itself.

Lithium-ion batteries are considered the substantial electrical storage element for electric vehicles(EVs). The battery model is the basis of battery monitoring, efficient charging, and safety management. Non-linearmodelling is the key to representing the battery and its dynamic internal parameters and performance. This paperproposes a smart scheme to model the lithium-polymer ion battery while monitoring its present charging currentand terminal voltage at various ambient conditions (temperature and relative humidity). Firstly, the suggestedframework investigated the impact of temperature and relative humidity on the charging process using the constantcurrent-constant voltage (CC-CV) charging protocol. This will be followed by monitoring the battery at thesurrounding operating temperature and relative humidity. Hence, efficient non-linear modelling of the EV batterydynamic behaviour using the Hammerstein-Wiener (H-W) model is implemented. The H-W model is considered ablack box model that can represent the battery without any mathematical equivalent circuit model which reducesthe computation complexity. Finally, the model beholds the boundaries of the charging process, not affecting onthe lifetime of the battery. Several dynamic models are applied and tested experimentally to ensure theeffectiveness of the proposed scheme under various ambient conditions where the temperature is fixed at40°C and the relative humidity (RH) at 35%, 52%, and 70%. The best fit using the H-W model reached 91.83% todescribe the dynamic behaviour of the battery with a maximum percentage of error 0.1 V which is in goodagreement with the literature survey. Besides, the model has been scaled up to represent a real EV and expressedthe significance of the proposed H-W model.

Should the Surgical Instrument Parts That Are Broken and Remain in the Distance of the Disc Be Removed? Should It Be Quit? A Case Report

Background and Aim: Lumbar posterior open microdiscectomy is a procedure that is widely used to treat lumbar disc diseases. These operations have a low risk of complications. It is unusual for the tool to break and remain in the intervertebral space during surgery. In this situation, we wanted to provide a set of suggestions based on our research of the literature on pituitary forceps blade fractures and the procedure for removing the fractured portion from the disc space during lumbar posterior open microdiscectomy surgery. Case Presentation: 10 days ago, a 37-year-old female patient presented to our clinic complaining of low back pain, left leg pain, and left foot weakness. A diagnosis of lumbar disc herniation necessitating surgery was obtained following neurological and radiographic examinations. The patient was advised to have surgery. The patient had standard lumbar microdiscectomy surgery. However, the tip of the pituitary forceps was broken during disc removal and remained in the L5-S1 disc space. The scope confirmed that the alien object was in space. After the evaluation, it was decided to remove the piece of instrument that was broken and remained in the disc space. The broken surgical handpiece was removed and documented under fluoroscopy. Additionally, it was forwarded to the technical unit for examination. Conclusion: A few case reports in the literature describe a surgical tool piece fracture that remained in the disc distance of the lumbar microdiscectomy. Complication management may be time-consuming and risky. Such a complication should be addressed and resolved appropriately because this situation might have detrimental terms on surgical risks and the legal procedure.
