Administration of Nebulised Ketamine for Managing Pain in the Intensive Care Unit of Obstetrics and Gynaecology

Introduction: The use of inhaled ketamine to manage a variety of painful conditions has been endorsed by the American College of Emergency Physicians and the American Academy of Emergency Medicine. Nebulized analgesia has multiple benefits, including rapid, effective and titratable analgesic delivery. The aim of our study is to assess the efficacy and safety of intranasal analgesic-dose ketamine compared to multimodal analgesia in patients presenting with acute postoperative pain or headache after a spinal anaesthetic in the intensive care unit of obstetrics and gynaecology. Materials and Methods: This was a prospective descriptive study, with hospital Ethics Committee approval and written informed consent from study participants. We compared the effect of nebulized ketamine and multimodal analgesia postoperatively in 120 patients belonging to the physical status I - II of the American Society of Anesthesiologists, in the intensive care unit of obstetrics and gynaecology, at the Ibn Rochd University Hospital Center in Casablanca from June 2021 to June 2022. Results: We included 120 patients in our study divided into two groups of 60 patients: the average age was 35 years, with extremes ranging from 18 to 45 years, All patients were hospitalized for postoperative care: all women underwent locoregional anaesthesia with a standard dose according to the service protocol (10 mg of bupivacaine, 25γ of fentanyl, 100γ of morphine), where pain was the common denominator. Among these patients, 59 were admitted for management of postpartum haemorrhage, 43 for postoperative monitoring, 15 for post-spinal anaesthesia headache and 3 for pelviperitonitis. The results of the pain assessment 30 minutes after the ketamine nebulization were marked by a request for analgesia in 12 patients, which is 20% of group A, including 5 patients, whose visual analogue scale (VAS) on admission was between 5 and 7, and 7 patients whose VAS at admission was ≥8;all these patients received a second d

The hydrothermal behavior of air inside a solar channel heat exchanger equipped with various shaped ribs is analyzed *** bottom wall of the exchanger is kept adiabatic,while a constant value of the temperature is set at the upper *** duct is equipped with a flat rectangular fin on the upper wall and an upstream V-shaped baffle on the lower ***,five hot wall-attached rib shapes are considered:trapezoidal,square,triangular pointing upstream(type Ⅰ),triangular pointing downstream(type Ⅱ),and equilateral-triangular(type Ⅲ)cross *** of the flow rates are also inspected for various Reynolds numbers in the turbulent regime(1.2×10^(4)-3.2×10^(4)).The highest performance(η)value is given for the Ⅱ-triangular rib case in all Re values,while the square-shaped ribs show a significant decrease in the η along the achieved Re *** η value at Remax is 2.567 for the Ⅱ-triangular roughness *** with the other simulated cases,this performance is decreased by about 3.768%in the case of Ⅰ-triangular ribs,15.249% in the case of Ⅲ-triangular ribs,20.802% in the case of trapezoidal ribs,while 27.541% in the case of square ribs,at the same ***,a comparison ismade with air-heat exchangers that have non-rough walls and contain cross-shaped VGs presented previously,in order to highlight the effectiveness of the rough surface presence in the baffled and finned *** obtained results indicated that the triangular-shaped rib(type Ⅱ)has the most significant hydrothermal behavior than the other *** indicates the necessity of roughness heat transfer surfaces for finned and baffled channels to improve significantly the performance of the air-heat exchangers they contain.

The magnetic properties of spinel ferrites Cu_（1-x）Zn_xFe_2O_4 are studied using high-temperature series expansions combined with the Padé approximates. The exchange interactions, inter and intra-sublattices J_（AA）, J_（BB） and J_（AB） are obtained using a probability distribution law. The critical exponent associated with the magnetic susceptibility is obtained.

移动机器人在未知环境，多障碍物环境中的导航问题一直是一个困难的问题。机器人要能够感知其所处的环境，解释感知到的信息，以获得其位置和环境的知识，然后规划一条无障碍路径，从初始位置到目标位置，并能够避开障碍物，控制机器人方向和速度达到目标。行为方法已经显示出其在不确定的未知环境和动态环境中的成功应用。行为控制方法分解导航系统为特殊的特定任务的行为模块，比如避障，目标搜索，路径跟随等，与传感器和执行机构直接连接，并行操作。简单的行为结合在一起以产生复杂的策略，能够达到策略目标，有效的对任何紧急情况作出反应。因此，该结构能够实时发挥作用，有很好的鲁棒性。模糊逻辑方法是许多方法之一，设计行为控制结构，由于对于行为设计的重要问题是确保不确定性出现时的鲁棒操作。模糊系统能利用语法规则处理不确定和不准确的信息，提供了实现人类知识和经验的可能性。本文对机器人自主移动和导航提出模糊行为控制系统，要求机器人自主到达给顶目标，避开其路径上遇到的任何静态和动态障碍物。仿真结果证明了方法的有效性。

Case Report: Osteoblastoma of the Maxillar Sinus

Osteoblastoma is an uncommon benign tumor which commonly affects the vertebrae and the long bones. It occasionally arises in the maxilla. It is important to do differential diagnoses which include osteoid osteoma, giant cell bone tumour, aneurysmal bone cyst and fibrous dysplasia giant cell tumor and osteogenic sarcoma. The current study presents the case of a 14-year-old boy with a tumor in the ethmoid cell and maxillary sinus. Previous literature was reviewed and discussed.

Inappropriateness of prostate cancer screening by PSA in young Algerian farmers

Introduction: It is not yet established, in Algeria, a consensus on the use of prostate specific antigen (PSA) for the screening of prostate cancer (PCA) while some doctors speculate and recommend a serum-PSA assay to the young men. The objective is to assess the prevalence of PSA by age in young farmers. Methods: This study was undergone on 403 farmers, exposed to the risk of pesticides, aged between 30 - 70 years and living in rural areas of saida (Western Algeria), during the period 2006 to 2012. The values of the PSA were collected during PCA screening and compared by statistical tests that revealed significant difference. Results: It was found that 15.6% of patients were under the age of 40 years. The total PSA (TPSA) increases with age and varies from 1 to 2 ng/ml for the respective age groups of 30 - 39 and 60 - 69 years. Approximately, 16.6% of men (or 1/6), who underwent PSA assay were under the age of 40 years. Conclusion: According to the recommendations of Algerian associations of cancer, men younger than 50 years may not be candidates for screening by PSA.

Determination of Antioxidant and Cytotoxic Activities of Red Seaweed, （Gracilaria manilaensis） against Different Cancer Cell Lines

Antioxidants play an important role in inhibiting and scavenging free radicals in human, providing protection against cellular damage in relation to cancer initiation. Seaweeds have been proved to have high antioxidant activity. Thus, this research was carried out to determine the antioxidant and anticancer properties of edible red seaweed, Gracilaria manilaensis （Gracilariales, Rhodophyta）. The extracts were prepared by Soxhlet extraction using organic solvents with different polarities. The antioxidant activities of extracts were determined in terms of their flee radical scavenging activity （RSA IC50） and total phenolic content （TPC）. The cytotoxic activity of extracts were tested against human ovarian cancer cell line （Caov-3）, human breast cancer cell line （MDA-MB-231 and MCF-7）, human cervical cell line （HeLa）, mouse fibroblast cell line （L929） and Madin-Darby canine kidney （MDCK） and the cell viability after 72 h incubation was determined by methylene blue assay. The findings showed that acetone extract has the lowest DPPH IC50 value followed by ethyl acetate extract. Both extracts also showed high values of TPC. Dichloromethane extract had the strongest cytotoxic on MDA-MB-231 （53.90 μg/mL ± 5.59 μg/mL） and HeLa （95.50 μg/mL）. While, acetone and ethyl acetate extracts were cytotoxic on MCF-7 （66.07 μg/mL） and Caov-3 （69.67 μg/mL ± 13.94 μg/mL）. It could be concluded that the antioxidant and cytotoxic activities of G. manilaensis were influenced by the types of solvents used and thus had a potential to develop as a cancer chemoprevention or anticancer agent against selected cancer.
