目的探讨影响肥胖症腹腔镜胃袖状切除术（LSG）后脱发的相关危险因素。方法采用回顾性病例对照研究方法。收集2013年11月至2015年6月同济大学附属东方医院收治的54例行LSG肥胖症患者的临床资料。患者均行LSG，术后观察患者脱发情况．分析影响其术后脱发严重程度的因素：性别、年龄、术前BMI、术后多余体质量减少率（EWL）、TBil、Alb、Hb、铁、锌、铜、叶酸、维生素B12、维生素d.观察指标：（1）随访和术后脱发情况：随访例数、随访时间、脱发例数、脱发严重程度、脱发时间、脱发治疗情况。（2）影响LSG术后脱发严重程度的单因素分析。（3）影响LSG术后脱发严重程度的多因素分析。采用门诊和微信方式进行随访。随访的内容为患者体质量变化情况、脱发情况。随访时间截至2016年9月。正态分布的计量资料以2＋s表示，组间比较采用t检验；计数资料比较采用疋。检验：多因素分析采用Logistic回归模型。结果（1）随访和术后脱发情况：54例行LSG患者术后均获得随访，随访时间为术后15个月。术后42例患者发生脱发，男性脱发比例为6／11，女性脱发比例为36／43。42例脱发患者中，轻、中、重度分别为21、10、11例，开始脱发时间为术后（3．4±1．4）个月，脱发结束时间为（9．0±3．6）个月，42例脱发患者中，15例口服药物治疗（硫酸亚铁剂6例、复合维生素5例、葡萄糖酸锌口服液制剂4例）脱发无明显改善。随访期内42例患者脱发均停止并逐渐长出新头发。（2）影响LSG术后脱发严重程度的单因素分析结果显示：性别、术后EWL、叶酸是影响LSG术后脱发严重程度的因素（x2=5．161．t=-5．114，4．266，P〈O．05）。（3）影响LSG术后脱发严重程度的多因素分析结果显示：术后EWL和叶酸是影响LSG术后脱发严重程度的独立因素（OR=1．039，0．499，95％可信区间：1．011．1．068，0．300～0．802，P〈0．05）、该模型对术后脱发严重程度的预测准确率为85．2％。结论术后EWL和叶酸是影响LSG减重手术后脱发的独立因素。

背景与目的我们力图回顾性分析患者在面对整形外科医师时的各种心理异常情况，以及整形外科医师给予其患者的各种心理学影响方式。方法系统性检索荷兰医学文摘数据库（Em-base）和PubMed.据库的相关文献并进行提炼（以我们制定的排除、入选标准为基础），共选出65项研究。此外，我们还挑选了4本书、2篇新闻稿和1份网络数据库作为有价值的信息，加入到本研究中。本研究入选标准为：所选研究须为关于患者寻求不同整形外科治疗的心理学相关的效果、背景及人格类型的研究。另外，还包括整形外科手术对患者心理状况及生活质量改变的研究。排除标准为：病例数小于30例的研究，非特定整形外科手术的研究，以及关于修复（非初次）手术而产生的心理后遗症的研究。结果对于美容外科而言，白恋、做作的人格缺陷及身体畸形恐惧症是患者前来就诊的最常见的3个心理学情况。总的来说，整形外科治疗不仅可对不美观或有缺陷的身体单元进行外观、功能上的修复，还可使患者的不良心理状况得到改善、缓和。结论对于有心理问题的患者，应在外科手术前对其进行适当的心理帮助，无论最后是否施行整形外科手术，都会让其成为更健康的个体。

目的探讨胃三维容积超声造影与上消化道碘水造影在经口内镜下幽门肌切开术(G-POEM)疗效评估中的价值。方法选取确诊为胃轻瘫且需行G-POEM的患者94例,随机分为胃三维容积超声检查组48例(A组)和X线碘水造影检查组46例(B组),A组口服超声造影剂后5 min、15 min、30 min、60 min及90 min分别行超声检查,B组口服碘水造影剂后于上述相同时间点行上腹部平片检查,于G-POEM前后各测量一次,获得三维容积超声造影和X线碘水造影估测的胃容积,分析手术前后胃排空时间和胃排空率的差异,评估G-POEM后的胃排空改善情况。结果①A组胃三维容积超声造影检查的成功率97.9%(47/48),B组上消化道碘水造影检查的成功率97.8%(45/46),两组比较差异无统计学意义;②A、B组分别有31例患者的术后胃排空时间较术前明显减少(均P<0.05);③A组16例患者术前及术后胃排空时间均为90 min,90 min时间点测得胃排空率较术前明显改善者8例,差异均有统计学意义(均P<0.05),胃排空率无明显改善者8例,与术前比较差异均无统计学意义。B组14例患者术前及术后胃排空时间均为90 min,90 min时间点测得胃排空率较术前明显改善者5例,差异均有统计学意义(均P<0.05),胃排空率无明显改善者9例,与术前比较差异均无统计学意义;④A组47例超声造影患者中,31例胃排空情况较术前显著改善,8例有一定改善,余8例患者无明显改善;B组45例碘水造影患者中,31例胃排空情况较术前显著改善,5例有一定改善,余9例无明显改善。两组胃排空改善率比较差异无统计学意义(83%vs.80%)。结论胃三维容积超声造影和上消化道碘水造影均可有效评估G-POEM前后的胃排空情况,临床可首选胃三维容积超声造影评估G-POEM前后的胃排空情况和术后胃排空功能的改善效果,上消化道碘水造影可作为补充方法,为临床提供客观的影像学信息。

d.abetic neuropathies are a family of nerve d.sord.rs caused.by d.abetes. Symptoms of the d.sease includ. nerve palsy, mononeuropathy, mononeuropathy multiplex, d.abetic amyotrophy, painful polyneuropathy, autonomic neu- ropathy, and.thoracoabd.minal neuropathy. In this stud., type 2 d.abetes in rats was ind.ced.with nicotinamid.- streptozotocin. d.ug treatment was initiated.on the d.15, with the combination regimen of metformin, pioglitazone and.glimipirid. or metformin and.sitagliptin or sitagliptin, amitriptyline and.sitagliptin and.led.to significantly im- proved.glycemic control, increased.grip strength and.paw jumping response on d.21, 28 and.35 （P 〈 0.001）. Signif- icant increases in blood.protein levels and.d.creases in urinary protein levels were observed.in the animals treated.with the d.fferent regimens on d.21, 28 and.35 （P 〈 0.001）. Combined.treatment of streptozotocin and.nicotinamid. caused.marked.d.generation of nerve cells, while ad.inistration of metformin and.sitagliptin showed.tissue regen- eration and.no bod. weight gain. In conclusion, treatment with sitagliptin and.sitagliptin combined.with metformin or amitriptyline results in no bod. weight gain, but causes an increase in grip strength and.pain sensitivity, exhibits neural protection, and.reverses the alteration of biochemical parameters in rats with streptozotocin-nicotinamid. ind.ced.type 2 d.abetes.
