Cumulative effects of excess high-normal alanine aminotransferase levels in relation to new-onset metabolic dysfunction-associated fatty liver disease in China

BACKGROUND Within the normal range,elevated alanine aminotransferase(ALT)levels are associated with an increased risk of metabolic dysfunction-associated fatty liver disease(MAFLD).AIM To investigate the associations between repeated high-normal ALT measurements and the risk of new-onset MAFLD prospectively.METHODS A cohort of 3553 participants followed for four consecutive health examinations over 4 years was selected.The incidence rate,cumulative times,and equally and unequally weighted cumulative effects of excess high-normal ALT levels(ehALT)were measured.Cox proportional hazards regression was used to analyse the association between the cumulative effects of ehALT and the risk of new-onset MAFLD.REsuLTS A total of 83.13%of participants with MAFLD had normal ALT levels.The incidence rate of MAFLD showed a linear increasing trend in the cumulative ehALT group.Compared with those in the low-normal ALT group,the multivariate adjusted hazard ratios of the equally and unequally weighted cumulative effects of ehALT were 1.651[95%confidence interval(CI):1.199-2.273]and 1.535(95%CI:1.119-2.106)in the third quartile and 1.616(95%CI:1.162-2.246)and 1.580(95%CI:1.155-2.162)in the fourth quartile,respectively.CONCLUSION Most participants with MAFLD had normal ALT levels.Long-term high-normal ALT levels were associated with a cumulative increased risk of new-onset MAFLD.

The long-term safety and effectiveness of once-daily tadalafil is crucial,but limited data are available in Chinese patients witherectile dysfunction(ED).In this post-marketing,multicenter,randomized,open-label trial with 2-year follow-up,635 ED caseswere randomized to receive daily oral tadalafil 2.5 mg or 5 mg for 3 months,of whom 580 continued once-daily tadalafil 5 mg for21 months.Treatment-emergent adverse events in the 12-month and 24-month period were similar,with the most common beingviral upper respiratory tract infection,upper respiratory tract infection,and headache.Significant improvement from baseline inthe International Index of Erectile Function-Erectile Function(IIEF-EF)score was detected at month 12(least squares mean[LSM]change:7.9,95%confidence interval[CI]:7.5–8.4,Psults showed improved erection function in 97.5%ofpatients and improved ability to engage in sexual activity in 95.9%of patients at month 12;these values were 96.1%and 95.0%at month 24,respectively.The quality of sexual life score based on the Sexual Life Quality Questionnaire(SLQQ)was increased by52.2%at month 12 and by 55.3%at month 24(both Psus 65.9±20.2 at month 24.Two-year daily application of tadalafil 5mg in Chinese men with ED showed a favorable safety profile and durable improvement in sexual performance and satisfaction.

BACKGROUND Laparoscopic radical gastrectomy is widely used,and perioperative complications have become a highly concerned issue.AIM To develop a predictive model for complications in laparoscopic radical gastrectomy for gastric cancer to better predict the likelihood of complications in gastric cancer patients within 30 days after surgery,guide perioperative treatment strategies for gastric cancer patients,and prevent serious complications.METHODS In total,998 patients who underwent laparoscopic radical gastrectomy for gastric cancer at 16 Chinese medical centers were included in the training group for the complication model,and 398 patients were included in the validation group.The clinicopathological data and 30-d postoperative complications of gastric cancer patients were collected.Three machine learning methods,lasso regression,random forest,and artificial neural networks,were used to construct postoperative complication prediction models for laparoscopic distal gastrectomy and laparoscopic total gastrectomy,and their prediction efficacy and accuracy were evaluated.REsuLTS The constructed complication model,particularly the random forest model,could better predict serious complications in gastric cancer patients undergoing laparoscopic radical gastrectomy.It exhibited stable performance in external validation and is worthy of further promotion in more centers.CONCLUSION Using the risk factors identified in multicenter datasets,highly sensitive risk prediction models for complications following laparoscopic radical gastrectomy were established.We hope to facilitate the diagnosis and treatment of preoperative and postoperative decision-making by using these models.

Cosmological Constraints on Neutrino Masses in Light of JWST Red and Massive Candidate Galaxies

The overabundance of the red and massive candidate galaxies observed by the James Webb Space Telescope(JWST)implies efficient structure formation or large star formation efficiency at high redshift z~10.In the scenario of a low or moderate star formation efficiency,because massive neutrinos tend to suppress the growth of structure of the universe,the JWST observation tightens the upper bound of the neutrino masses.Assuming A cold dark matter cosmology and a star formation efficiency∈[0.05,0.3](flat prior),we perform joint analyses of Planck+JWST and Planck+BAO+JWST,and obtain improved constraints∑m_(ν)sumptions,the inverted mass ordering,which implies ∑m_(ν)≥0.1 eV,is excluded by Planck+BAO+JWST at 92.7% confidence level.

Mobile mini-fluorimeter for antibiotic aptasensing based on surface-plasmonic effect of burlike nanogolds enhanced by digitized imaging diagnosis

Antibiotic abuse now poses a grave threat to global ecology and bestirs public concerns about the residue issue in daily necessities.The traceability measurements along supply chain or logistic circulation have become increasingly essential given the labile nature of diverse synthetic residuals on site.In an attempt to answer this urgency,here a miniaturized fluorometric aptasensor prototype was contrived that catered to the point-of-care screening norm for two typical additives:chloramphenicol and enrofloxacin.The key target-indicating module worked in vitro based on the competitive binding-induced fluorescence recovery of fluorescein-labeled aptamers,which were photobleached beforehand in the format of double helix on burlike nanogold carriers.The“prickly”geometry of the latter not just enriched the capture probes at preferentially substrate-accessible spires;but also contributed to a tip-enhanced surface plasmon effect,sensitizing the signal-on during the duplex dissociation even at nanomolar threshold of the analytes.On the other hand,to encompass a full portable,a set of optical devices were mounted within a 3D-printed cartridge(adaptor)to converge the light beam and route it towards the detector,for which the smartphone camera came up in handy with a home-developed App for calibrating the emissive brightness.Enlightened by the high-dynamic-range compression,an imaging diagnostic algorithm was built in to grid and digitize each slide in the album for augmented detection performance.Thus,a novel bio-to-silico integration was invented that capable of in situ rapid reporting on the antibiotic presence with high sensitivity and selectivity.Further field practices in spiked milk on sales proved the precision and rudimentary feasibility of the well-assembled model of appliance,thus holding nice prospects in nonexpert(e.g.,family and local community)utilities for foodborne antibiotic identification.

Predict the evolution of mechanical property of Al-Li alloys in a marine environment

The ocean is one of the essential fields of national defense in the future,and more and more attention is paid to the lightweight research of Marine equipment and materials.This study it is to develop a Machine learning(ML)-based prediction method to study the evolution of the mechanical properties of Al-Li alloys in the marine environment.We obtained the mechanical properties of Al-Li alloy samples under uniaxial tensile deformation at different exposure times through Marine exposure experiments.We obtained the strain evolution by digital image correlation(DIC).The strain field images are voxelized using 2D-Convolutional Neural Networks(CNN)autoencoders as input data for Long Short-Term Memory(LSTM)neural networks.Then,the output data of LSTM neural networks combined with corrosion features were input into the Back Propagation(BP)neural network to predict the mechanical properties of Al-Li alloys.The main conclusions are as follows:1.The variation law of mechanical properties of2297-T8 in the Marine atmosphere is revealed.With the increase in outdoor exposure test time,the tensile elastic model of 2297-T8 changes slowly,within 10%,and the tensile yield stress changes significantly,with a maximum attenuation of 23.6%.2.The prediction model can predict the strain evolution and mechanical response simultaneously with an error of less than 5%.3.This study shows that a CNN/LSTM system based on machine learning can be built to capture the corrosion characteristics of Marine exposure experiments.The results show that the relationship between corrosion characteristics and mechanical response can be predicted without considering the microstructure evolution of metal materials.

Efficacy of scleral buckling for the treatment of rhegmatogenous retinal detachment using a novel foldable capsular buckle

●AIM:To evaluate the effectiveness and safety of scleral buckling for the treatment of rhegmatogenous retinal detachment(RRD)using a novel foldable capsular buckle(FCB).●METHODS:This was a series of case observation studies.Eighteen patients(18 eyes)who visited our ophthalmology department between August 2020 and August 2022 and were treated for RRD with scleral buckling using FCB were included.The procedure was similar to conventional scleral buckling,while a balloon-like FCB was placed onto the retinal break with balanced salt solution filling for a broad,external indentation instead of the silicone buckle.The retinal reattachment rate,best corrected visual acuity(BCVA),intraocular pressure(IOP),refractive dioptre and astigmatism degree,and complications were evaluated and recorded.●REsuLTS:There were 7 males and 11 females aged 19-58y.The average time course of RRD was 12d,ranging from 7-20d.The retinal break was located in the superior quadrants in 8 eyes and in the inferior quadrants in 10 eyes,with macula-off detachments in 12 eyes.The patients were followed-up for at least 6mo.The final retinal reattachment rate was 100%.The BCVA was significantly improved compared with the baseline(P0.05).Three patients had transiently high IOPs within one week after surgery.Mild diplopia occurred in 5 patients after surgery and then disappeared after the balloon fluid was removed.●CONCLUSION:The success rate of FCB scleral buckling for RRD is satisfactory.This procedure can be expected to be applied in new,uncomplicated cases of RRD.

Angiogenesis-the formation of new blood vessels from existing blood vessels-has drawn significant attention in medical research.New techniques have been developed to control proangiogenic factors to obtain desired ef-fects.Two important research areas are 1)understanding cellular mechanisms and signaling pathways involved in angiogenesis and 2)discovering new biomaterials and nanomaterials with proangiogenic effects.This paper reviews recent developments in controlling angiogenesis in the context of regenerative medicine and wound healing.We focus on novel proangiogenic materials that will advance the field of regenerative medicine.Specifically,we mainly focus on metal nanomaterials.We also discuss novel technologies developed to carry these proangio-genic inorganic molecules efficiently to target sites.We offer a comprehensive overview by combining existing knowledge regarding metal nanomaterials with novel developments that are still being refined to identify new nanomaterials.

Nodulin 26-like intrinsic protein Cs NIP2;2 is a silicon influx transporter in Cucumis sativus L.

Nodulin 26-like intrinsic proteins(NIPs) are a family of channel-forming transmembrane proteins that function in the transport of water and other small molecules.Some NIPs can mediate silicon transport across plasma membranes and lead to silicon accumulation in plants,which is beneficial for the growth and development of plants.Cucumber is one of the most widely consumed vegetables;however,the functions of NIPs in this crop are still largely unknown.Here,we report the functional characteristics of Cs NIP2;2.It was found that Cs NIP2;2 is a tandem repeat of Cs NIP2;1,which had been demonstrated to be a silicon influx transporter gene.Cs NIP2;2 has a selectivity filter composed of cysteine,serine,glycine and arginine(CSGR),which is different from all previously characterized silicon influx transporters in higher plants at the second helix position.Xenopus laevis oocytes injected with Cs NIP2;2 c RNA demonstrated a higher uptake of silicon than the control,and the uptake remained unchanged under low temperature.Cs NIP2;2 was found to be expressed in the root,stem,lamina and petiole,and exogenous silicon treatment decreased its expression in the stem but not in other tissues.Transient expression of Cs NIP2;2-e GFP fusion sequence in onion epidermal cells showed that Cs NIP2;2 was localized to the cell nucleus,plasma membrane and an unknown structure inside the cell.The results suggest that Cs NIP2;2 is a silicon influx transporter in cucumber,and its subcellular localization and the selectivity filter are different from those of the previously characterized silicon influx transporters in other plants.These findings may be helpful for understanding the functions of NIPs in cucumber plants.

基于现代化与新农村建设之现实语境,乡村美学建设逐渐成为乡村振兴的新兴话语与实践导向。在基层实践中,乡村美学建设的推进仍面临着主体困境、文化困境与关系困境,因而迫切需要重新对传统乡村建设的内在机理进行梳理,并建构审美追求扎根乡村、美学建设可持续发展的实践逻辑与方法。基于布迪厄的场域分析框架,建构乡村美学场域必须沿袭这一路径:政策革新、人文回归与乡村业态创新分别导致了社会资本倾斜、文化资本重构与经济资本的积累,乡民主体性的唤醒形塑了向美而行的新“惯习”,为乡村美学建设提供了内生动力;乡村美学场域长久发展的关键在于,以现代化的审美视野,进一步促进审美资源转化为审美资本,明确生态、经济、文化、科技各层面与乡村美学之间的动态关联,实现乡村人居美学、产业美学、文化美学、数字美学全面发展,为打造现代化新乡村提供新思路、开创新局面。
