Metastatic stomach lymphoepithelioma-like carcinoma and immune checkpoint inhibitor therapy:A case report

BACKGROUND Pulmonary lymphoepithelioma-like carcinoma(PLELC)is a rare type of nonsmall-cell lung cancer.Stomach lymphoepithelioma-like carcinoma(LELC)metastasis secondary to PLELC has not been reported recently.CASE SUMMARY A 64-year-old female was admitted to our hospital for a regular gastroscopy examination with a 6-year history of surgical resection for left PLELC.Positron emission tomography/computed tomography suggested high accumulation of 18F-fludeoxyglucose in the gastric cardia region.Upper gastrointestinal endoscopy confirmed a large mass at the stomach fundus.Immunohistochemistry(IHC)of the biopsy suggested metastatic stomach LELC.Proximal gastrectomy showed that this 6.5 cm×5.0 cm mass was located in the stomach fundus near the cardia.Histopathological examination showed a poorly differentiated carcinoma with prominent lymphoplasmacytic infiltration.IHC demonstrated that the tumor was positive for CK(AE1/AE3),p63,p40,p53,Ki-67(70%),and EGFR(3+)and negative for CK7,CK20,Her2,and CD10.In situ hybridization analysis showed positive staining Epstein-Barr virus-encoded RNA.Tumor programmed cell death ligand 1(PD-L1)expression score was 98%,and the combined positive score was 100,with no evidence of microsatellite instability.Thus,the patient was unequivocally diagnosed with metastatic stomach LELC secondary to pulmonary LELC.After discharge,this patient underwent PD-1 inhibitor treatment(toripalimab,240 mg)every 3 wk for ten cycles,and she has had no tumor recurrence.CONCLUSION For gastric LELC metastasis,PD-1 inhibitor therapy could become a new therapeutic approach,though there is still no evidence from large data sets to support this.

Amorphous/nanocrystalline dual-phase structures have recently emerged as an effective way for over-coming the strength-ductility trade-offand breaking the limitation of the reverse Hall-Petch effect.Here,we proposed a new strategy to develop a hierarchical and interconnected amorphous-crystalline nanocomposite arising from the nanoscale elemental interdiffusion and oxygen adsorption behavior dur-ing thermal treatment processes.The nanocomposite consisted of a three-dimensional(3D)hierarchical network structure where the crystalline phase(Cr-Co-Ni-Al)was embedded into the Al-O-based amor-phous phase network with critical feature sizes encompassing three orders of magnitude(from microm-eter to nanometer scale).It can achieve ultrahigh compression yield strength of-3.6 GPa with large homogeneous deformation of over 50%strain.The massive interstitial atoms induced lattice distortion and hierarchical amorphous phase boundary contributed to the strength improvement.in situ Uniaxial compression inside a transmission electron microscope(TEM)revealed that the exceptional deformability of the nanocomposites resulted from the homogenous plastic flow of nano-sized amorphous phase and the plastic co-deformation behavior restricted by the nano-architected dual-phase interface.The proposed dual-phase synthesis approach can outperform conventional nanolaminates design strategies in terms of the mechanical properties achievable while providing a pathway to easily tune the microstructure of these nanolaminates.

Effects of transient temperature gradient on frost heave of saturated silty clay in an open system

Frost heave in seasonally frozen regions is a one-dimensional process that could severely damage infrastructure subgrades.Stress state,temperature and water migration are important factors for frost heave.This work investigated the effects of soil temperature and volumetric water content on the transient frost heave ratio during the freezing of saturated silty clay in an open system and analyzed the relationships between the transient frost heave ratio and freezing rate and between temperature gradient and frost heave rate.The results show that the frost heave ratio,frost heave rate,and freezing rate are positively correlated with the temperature gradient since the temperature gradient drives the water migration during freezing,indicating the transient temperature gradient could be used to evaluate the frost heave of saturated silty clay.The transient freezing rate and transient frost heave ratio are logarithmically related to the transient frost heave ratio and transient temperature gradient,respectively.The effects of transient temperature gradient on frost heave are the principal mechanism responsible for different frost heave characteristics and uneven frost heave along a subgrade of the same soil type.

Impact of an emergency department nursing intervention on continuity of care, self-care, and psychological symptoms

BACKGROUND The emergency department plays a crucial role in providing acute care to patients.Nursing interventions in this setting are essential for improving the continuity of care,enhancing patients’self-care abilities,and reducing psychological symp-toms.AIM To evaluate the effect of nursing interventions in the emergency department on these indicators in an emergency department.METHODS A retrospective analysis was conducted on 120 patients admitted to the emergency department between January 2022 and May 2023.The patients were divided into two groups:The control group(conventional nursing intervention)and the observation group(conventional nursing intervention+emergency department nursing intervention).The two groups were compared regarding continuity of care,self-care ability,psychological symptoms,and satisfaction with care.RESULTS The emergency department nursing interventions significantly improved the continuity of care,enhanced patients’self-care abilities,and reduced psycho-logical symptoms such as anxiety and depression.CONCLUSION Nursing interventions in the emergency department positively impact continuity of care,self-care,and psychological symptoms.However,it is important to acknowledge the limitations of this study,including the small number of studies,variable methodological quality,and the heterogeneity of the study population.Future research should address these limitations and further explore the effects of different types of nursing interventions in the emergency department.Additionally,efforts should be made to enhance the application and evaluation of these interventions in clinical practice.

Diets enriched with finely ground wheat bran alter digesta passage rate and composition of the gut microbiome in sows

We investigated the effects of finely ground wheat bran on the nutrient digestibility, digesta passage rate,and gut microbiota structure in sows. A 3 × 3 Latin square design with 3 test periods and 3 experimental diets was used. Six non-pregnant sows(parity: 5 to 7) were randomly assigned to 3 experimental diets with 2 replicates per treatment in each period. Each period lasted 19 d(12 d for adaptation and 7 d for experiment). The experimental diets included(a) a basal corn and soybean meal diet(CON),(b) a basal diet with 20% coarse wheat bran(CWB;particle size: 605 μm), and(c) a basal diet with 20% fine wheat bran(FWB;particle size: 438 μm). The results demonstrated that the apparent total tract digestibility of neutral detergent fiber, acid detergent fiber and energy were reduced(P < 0.05) in the FWB and CWB groups compared with those in the CON group. Viscosity of digesta increased(P < 0.001) in FWB-fed sows. The passage rate of digesta from the mouth to the ileum decreased(P < 0.001) in FWB-fed sows. Peptide YY(PYY) concentration increased(P = 0.01) in FWB-fed sows after 30 min of feeding.In the FWB group, the relative abundance of Lactobacillaceae at the family level increased(P < 0.05) in the ileal digesta. At the class level, the relative abundance of Clostridia in feces decreased(P < 0.05) in FWB-fed sows. FWB enhanced the concentration of butyrate in feces compared with CON and CWB(P = 0.04). These results suggest that dietary supplementation with finely ground wheat bran reduces the passage rate of digesta, increases the abundance of beneficial microorganisms, and elevates the concentration of short-chain fatty acids and PYY in sows. These findings indicate that the addition of finelyground wheat bran to the diets of sows is more effective than using coarse wheat bran for improving their satiety and intestinal microbial composition.

Wheat bran inclusion level impacts its net energy by shaping gut microbiota and regulating heat production in gestating sows

An accurate estimation of net energy(NE)of wheat bran is essential for precision feeding of sows.However,the effects of inclusion level on NE of wheat bran have not been reported.Inclusion level was hypothesized to impact NE of wheat bran by regulating gut microbiota and partitioning of heat production.Therefore,twelve multiparous sows(Yorkshire×Landrace;2 to 4 parity)were assigned to a replicated 3×6 Youden square with 3 successive periods and 6 diets in each square.The experiment included a corn-soybean meal diet(WB0)and five diets including 9.8%(WB10),19.5%(WB20),29.2%(WB30),39.0%(WB40)and 48.7%wheat bran(WB50),respectively.Each period included 6 d of adaptation to diets followed by 6 d for heat production measurement using open-circuit respiration chambers.Compared with other groups,WB30,WB40,and WB50 enriched different fiber-degrading bacteria genera(P<0.05).Apparent total tract digestibility of neutral detergent fiber and acid detergent fiber of wheat bran were greater in WB30 and WB40(P<0.05).Physical activity(standing and sitting)decreased as inclusion level increased(P=0.04),which tended to decrease related heat production(P=0.07).Thermic effect of feeding(TEF)was higher in WB50 than other treatments(P<0.01).Metabolizable energy of wheat bran was similar among treatment groups(except for WB10).NE of wheat bran conformed to a quadratic regression equation with inclusion level(R^(2)=0.99,P<0.01)and peaked at an inclusion level of 35.3%.In conclusion,increasing inclusion level decreased energy expenditure of sows on physical activity and promoted growth of fiber-degrading bacteria,which improved energy utilization of fiber.Fermentation of wheat bran fiber by Prevotellaceae_UCG-003 and norank_f_Paludibacteraceae might increase TEF.Consequently,sows utilized energy in wheat bran most efficiently at an inclusion level of 35.3%.

Objective Viral encephalitis is an infectious disease severely affecting human health.It is caused by a wide variety of viral pathogens,including herpes viruses,flaviviruses,enteroviruses,and other viruses.The laboratory diagnosis of viral encephalitis is a worldwide challenge.Recently,high-throughput sequencing technology has provided new tools for diagnosing central nervous system infections.Thus,In this study,we established a multipathogen detection platform for viral encephalitis based on amplicon sequencing.Methods We designed nine pairs of specific polymerase chain reaction(PCR)primers for the 12 viruses by reviewing the relevant literature.The detection ability of the primers was verified by software simulation and the detection of known positive samples.Amplicon sequencing was used to validate the samples,and consistency was compared with Sanger sequencing.Results The results showed that the target sequences of various pathogens were obtained at a coverage depth level greater than 20×,and the sequence lengths were consistent with the sizes of the predicted amplicons.The sequences were verified using the National Center for Biotechnology Information BLAST,and all results were consistent with the results of Sanger sequencing.Conclusion Amplicon-based high-throughput sequencing technology is feasible as a supplementary method for the pathogenic detection of viral encephalitis.It is also a useful tool for the high-volume screening of clinical samples.

Inhibition roles of molybdate and borate on Q235 steel corrosion in resistance reducing agent

The effects of Na_(2)MoO_(4) and Na_(2)B_(4)O_(7) on corrosion behavior of Q235 steel in resistance reducing agent(RRA)containing sodium bentonite were studied by mass loss,scanning electron microscopy and electrochemical measurement.The results showed that both the independent and mixed additions of Na_(2)MoO_(4) and/or Na_(2)B_(4)O_(7),can reduce the corrosion rate of Q235 steel in RRA containing sodium bentonite.And the inhibition effect of Na_(2)MoO_(4) and/or Na_(2)B_(4)O_(7)increased with their dosage increase.With the same dosage,the inhibition efficiency of the mixed addition of Na_(2)MoO_(4) and Na_(2)B_(4)O_(7),was higher than that of their independent addition.The passivation effect of Q235 steel was easy to obtain in the RRA with the mixed addition of Na_(2)MoO_(4) and Na_(2)B_(4)O_(7).The optimized inhibitor for the RRA containing sodium bentonite was the mixture of Na_(2)B_(4)O_(7) and Na_(2)MoO_(4) with a total concentration of 1.5 wt.%.Furthermore,the increase in corrosion potential E_(corr) and the decrease in corrosion current density i_(cor) in carbon steel were one of the important criteria for the formation of passivation film.

Transcription and Secretion of Interleukin-1β and HMGB1 in Keratinocytes Exposed to Stimulations Mimicking Common Inflammatory Damages

Objective:Interleukin-1β(IL-1β)and high-mobility group box 1(HMGB1)are widely known damage-associated molecular patterns(DAMPs).However,their expression and secretion in different skin diseases,especially in inflammatory skin disorders,remain to be further elucidated.This study was performed to explore and compare the transcriptional and secretory levels of IL-1β and HMGB1 in keratinocytes under 3 types of stimulation:ultraviolet B(UVB)irradiation;co-stimulation by tumor necrosis factor-α(TNF-α)and interferon-γ(IFN-γ)(simulation of T helper 1 cell inflammatory challenge);and psoriasis-like stimulation by M5,a mixture of 5 proinflammatory cytokines.Methods:We used quantitative reverse-transcription polymerase chain reaction to determine the transcription levels of IL-1β and HMGB1.Western blotting and enzyme-linked immunosorbent assay were used to detect the secretion levels of IL-1β and HMGB1.The results were statistically analyzed by t test.Results:A rapid transcriptional and secretory response of IL-1β from keratinocytes occurred in all 3 types of stimulation mimicking common inflammatory environments(P<0.05).Transcription of HMGB1 was inhibited in all 3 types of stimulation(P<0.05),but secretion was increased after exposure to UVB irradiation and co-stimulation by TNF-α and IFN-γ(P<0.05).We observed no change in the secretion level of HMGB1 after treatment with M5(P=0.196).Conclusion:IL-1β is a critical cytokine for the immunomodulatory functions of keratinocytes in inflammatory responses.In this study,keratinocytes restrained transcription of HMGB1 when the secretion of HMGB1 was induced in certain stimulations(eg,by UVB exposure or stimulation by TNF-α and IFN-γ).

Efficacy and safety of remimazolam-based sedation for intensive care unit patients undergoing upper gastrointestinal endoscopy:a cohort study

BACKGROUND:Remimazolam is a novel ultra-short-acting sedative,but its safety and adverse events(AEs)in high-risk patients in the intensive care unit(ICU)setting remain unknown.METHODS:This was a single-center,retrospective study that compared remimazolam to propofol and midazolam in patients undergoing upper gastrointestinal endoscopy.The primary outcome was the incidence of treatment-related AEs.The secondary outcomes were the time to extubation,the length of ICU stay,and the average cost of sedative per case.RESULTS:Of the 88 patients analyzed,47 were treated with remimazolam(mean dose,7.90±4.84mg),and 41 were treated with propofol(21.19±17.98 mg)or midazolam(3.08±2.17 mg).There was no statistically significant difference in the average duration of the endoscopic procedure(35.89±13.37 min vs.44.51±21.68 min,P=0.133)or the time to extubation(15.00±9.75 h vs.20.59±18.71 h,P=0.211)in the remimazolam group(group I)compared to the propofol or midazolam group(group II).ICU stays(5.40±2.93 d vs.4.63±3.31 d,P=0.072)and treatment-related AEs(48.61%vs.51.38%,P=0.056)were similar between groups.The average cost of sedative per case was significantly lower in the group I than in the group II(RMB 16.07±10.58 yuan vs.RMB 24.37±15.46 yuan,P=0.016).CONCLUSION:Remimazolam-based sedation was noninferior to the classic sedatives and had lower average cost per case,indicating that it may be used as a promising sedative for high-risk patients during endoscopic procedures in the ICU setting.
