Autoimmune enteropathy(AIE)is a rare immune mediated disorder primarily affecting children,characterized by chronic diarrhea,malabsorption,vomiting,weight loss and villous *** has also been observed in adults presenting diagnostic and treatment challenges due to its overlap with other gastrointestinal disorders such as celiac *** diagnostic criteria for AIE include small bowel villous atrophy,lack of response to dietary restrictions,presence of anti-enterocyte antibodies,and predisposition to autoimmunity without severe *** criteria emphasize characteristic histological findings and exclusion of other causes of villous *** is associated with various autoimmune disorders and can present with overlapping features with Celiac disease,including villous atrophy but without significant intraepithelial *** primarily involves immunosuppression using corticosteroids,calcineurin inhibitors,and anti-tumor necrosis factor therapy,alongside nutri-tional *** the challenges,understanding AIE’s diverse manifest-ations and improving diagnostic criteria are essential for effective management and improved patient *** research is needed to elucidate the pathogenesis,disease progression and long-term outcomes of AIE.

The joints connecting vertical and horizontal elements are the ＂weak link＂ in structural systems assembled from wood panels. If they are too weak, local failures may occur, resulting in performance that is significantly below expectations. If they are too resistant, the joints may be unable to dissipate energy during vibrations, thus possibly initiating a fast progressive failure. This paper re-processes and re-elaborates the results of shaking table tests previously carried out by the author and other co-workers. The goal is to assess the feasibility of a joint which is able to dissipate energy during vibration, without degrading the connection performance.

Objective:To assess the role of debulking surgery in extensive hepatic alveolar echinococcosis(HAE).Methods:Five cases of extensive HAE undergoing debulking surgery with their long-term follow-ups were ***,a systematic review was carried ***:The average age of patients in the study was(50.8±17.1)years(range 36 to 77 years).Three cases were *** median follow-up period was 61 months(range 12 to 84 months).Left liver resections were performed in 2 cases and right hemihepatectomies were undertaken in 2 *** patients(cases 2 and 4)developed early surgical complications,while one of them(case 4)who suffered from the recurrent sub-hepatic abscess died at 12 months ***:This study highlights the importance of debulking surgery for the management of complex symptomatic HAE lesions in patients with no curative option available.

In Italy,a large stock of public housing was built during the 1970s and 1980s with industrialized/prefabricated *** buildings have envelopes characterized by the presence of many thermal bridges and low transmittance *** addition,they feature inefficient single heating systems in residential units and no cooling/ventilation *** a result,these buildings require urgent energy retrofitting actions,and it is therefore necessary to define procedures that will guarantee effective *** possible interventions must be compatible with building construction techniques as well as be minimally invasive and *** are only a limited number of technical solutions,considering that residents should not have to move out during the *** most Italian climatic zones,current interventions are usually linked to external insulation and window replacement,leading to an improvement in energy performance and comfort only during *** comfort conditions tend to worsen in summer months because seasonal temperatures tend to increase by a few ***,solutions should be proposed that will improve both summer and winter *** work proposes an energy recovery procedure applied to a representative building from the abovementioned period located in the Florence area and constructed with an industrialized system named the“tunnel system”(great panels structure).The procedure used in this study provides for the redevelopment of the envelope and the application of a simple mechanical ventilation system to achieve substantial energy savings and improved indoor comfort conditions.

We read with great interest the article that retrospectively analyzed 814 patients with primary gastric cancer,who underwent minimally invasive R0 gastrectomy between 2009 and 2014 by grouping them in laparoscopic vs robotic *** results of the study highlighted that age,American Society of Anesthesiologists status,gastrectomy type and pathological T and N status were the main prognostic factors of minimally invasive gastrectomy and showed how the robotic approach may improve long-term outcomes of advanced gastric *** to most of the current literature,robotic surgery is associated with a statistically longer operating time when compared to open and laparoscopic surgery;however,looking at the adequacy of resection,defined by negative surgical margins and number of lymph nodes removed,it seems that robotic surgery gives better results in terms of the 5-year overall survival and recurrencefree *** robotic approach to gastric cancer surgery aims to overcome the difficulties and technical limitations of laparoscopy in major *** threedimensional vision,articulation of the instruments and good ergonomics for the surgeon allow for accurate and precise movements which facilitate the complex steps of surgery such as lymph node dissection,esophagus-jejunal anastomosis packaging and reproducing the technical accuracy of open *** the literature,as well as the analyzed study,offers us countless data regarding the short-term oncological results of robotic surgery in the treatment of gastric cancer,satisfactory data on long-term follow-up are lacking,so future studies are necessary.

BACKGROUND Solid organ transplantation(SOT)and hematopoietic stem cell transplantation(HSCT)revolutionized the survival and quality of life of patients with malignant diseases,various immunologic,and metabolic disorders or those associated with a significant impairment in a patient's quality of *** To investigate admission causes and treatment outcomes of patients after SOT or HSCT treated in a medical intensive care unit(ICU).METHODS We conducted a single-center,retrospective epidemiological study in the medical ICU at the University Hospital Centre Zagreb,Croatia covering the period from January 1,2018 to December 31,*** The study included 91 patients with either SOT[28 patients(30.8%)]or HSCT[63 patients(69.2%)].The median age was 56(43.2-64.7)years,and 60.4%of the patients were *** with SOT had more comorbidities than patients after HSCT[χ^(2)(5,n=141)=18.513,Pllowed by acute respiratory insufficiency in patients following *** rate significantly differed between SOT and HSCT[χ^(2)(1,n=91)=21.767,Pdid not influence patients'survival[χ^(2)(1,n=91)=0.376,P=0.54].In the subgroup of patients with infection,90%had septic shock,and the majority had positive microbiological samples,mostly Gram-negative *** ICU survival of patients with sepsis/septic shock cumulatively was 15%.The survival of SOT patients with sepsis/shock was 45%.CONCLUSION Patients with SOT or HSCT are frequently admitted to the ICU due to sepsis and septic *** advancements in critical care,the mortality rate of patients with refractory septic shock and multiorgan failure in this patient population is extremely hig
