The scientific community is continuously working to translate the novel biomedical techniques into effective medical treatments.CRISPR-Cas9 system(Clustered Regularly Interspaced Short Palindromic Repeats-9),commonly known as the“molecular scissor”,represents a recently developed biotechnology able to improve the quality and the efficacy of traditional treatments,related to several human diseases,such as chronic diseases,neurodegenerative pathologies and,interestingly,oral diseases.Of course,dental medicine has notably increased the use of biotechnologies to ensure modern and conservative approaches:in this landscape,the use of CRISPR-Cas9 system may speed and personalize the traditional therapies,ensuring a good predictability of clinical results.The aim of this critical overview is to provide evidence on CRISPR efficacy,taking into specific account its applications in oral medicine.

Computed tomography colonography(CTC) in colorectal cancer(CRC) screening has two roles:one present and the other potential.The present role is,without any further discussion,the integration into established screening programs as a replacement for barium enema in the case of incomplete colonoscopy.The potential role is the use of CTC as a first-line screening method together with Fecal Occult Blood Test,sigmoidoscopy and colonoscopy.However,despite the fact that CTC has been officially endorsed for CRC screening of average-risk individuals by different scientif ic societies including the American Cancer Society,the American College of Radiology,and the US Multisociety Task Force on Colorectal Cancer,other entities,such as the US Preventive Services Task Force,have considered the evidence insuff icient to justify its use as a mass screening method.Medicare has also recently denied reimbursement for CTC as a screening test.Nevertheless,multiple advantages exist for using CTC as a CRC screening test:high accuracy,full evaluation of the colon in virtually all patients,non-invasiveness,safety,patient comfort,detection of extracolonic findings and cost-effectiveness.The main potential drawback of a CTC screening is the exposure to ionizing radiation.However,this is not a major issue,since low-dose protocols are now routinely implemented,delivering a dose comparable or slightly superior to the annual radiation exposure of any individual.Indirect evidence exists that such a radiation exposure does not induce additional cancers.

Background Aging is a multifactorial process associated with an impairment of autonomic nervous system(ANS)function.Progressive ANS remodeling includes upregulation of expression of circulating catecholamines and depletion of cardiac autonomic nerve fibers,and it is responsible,in part,for the increased susceptibility to cardiac diseases observed in elderly subjects.Neurotrophic factors,such as brain-derived neurotrophic factor(BDNF)and nerve growth factor(NGF),are involved in synaptogenesis and neurite outgrowth processes,supporting neuronal cell differentiation and maturation.However,whether and how these factors and their downstream signaling are involved in cardiac aging remains unclear.Here,we tested whether,in the aged heart,the overall extent of autonomic fibers is reduced,owing to lower production of trophic factors such as BDNF and NGF.Methods In vivo,we used young(age:3 months;n=10)and old(age:24 months;n=11)male Fisher rats,whereas,we used human neuroblastoma(SH-SY5 Y)cells in vitro.Results Compared to the young rats,old rats displayed a marked reduction in the overall ANS fiber density,affecting both sympathetic and cholinergic compartments,as indicated by dopamineβ-hydroxylase(dβh)and vesicular acetylcholine transporter(Va Ch T)immunohistochemical staining.In addition,a marked downregulation of GAP-43 and BDNF protein was observed in the left ventricular lysates of old rats compared to those of young rats.Interestingly,we did not find any significant difference in cardiac NGF levels between the young and old groups.To further explore the impact of aging on ANS fibers,we treated SH-SY5 Y cells in vitro with serum obtained from young and old rats.Sera from both groups induced a remarkable increase in neuronal sprouting,as evidenced by a crystal violet assay.However,this effect was blunted in cells cultured with old rat serum and was accompanied by a marked reduction in GAP-43 and BDNF protein levels.Conclusions Our data indicate that physiological aging is associated with an impairment of ANS structure

BACKGROUND Documentation of disease activity in patients affected by Crohn’s disease(CD)is mandatory in order to manage patients properly.Magnetic resonance imaging(MRI)is considered the reference cross-sectional technique for the assessment of CD activity.Among MRI findings,layered pattern(LP)of contrast enhancement seems to be one of the most significant signs of severe disease activity;however,it has also been associated with chronic disease and mural fibrosis.AIM To systematically evaluate the accuracy of LP of contrast enhancement in the diagnosis of active inflammation in patients with CD.METHODS In February 2019,we searched the MEDLINE and Cochrane Central Register of Controlled Trials databases for studies evaluating the diagnostic accuracy of LP of contrast enhancement on MRI for the detection of active inflammation in patients with CD.To be included,studies had to use histopathologic analysis(endoscopy or surgery)as the reference standard.Risk of bias and applicability concerns of the included studies were evaluated by using items from the Quality Assessment for Diagnostic Accuracy Studies 2(QUADAS-2)tool.Pooled sensitivity and specificity were determined using a bivariate random-effect model.Heterogeneity was quantified by using the I2 statistic.Our meta-analysis received no funding,and the review protocol was not published or registered in advance.RESULTS Of the 1383 studies identified,five articles were finally selected for quantitative and qualitative synthesis(245 patients,238 of whom had histopathologically confirmed CD,144 with active inflammation and 94 with inactive disease).The meta-analysis showed a pooled sensitivity of 49.3%(95%CI:41%-57.8%;I2:90.7%)and specificity of 89.1%(95%CI:81.3%-94.4%;I^2:48.6%).Pooled PLR and NLR were 3.3(95%CI:1.9-5.7;I2:6.1%)and 0.6(95%CI:0.5-0.9;I2 70.5%),respectively.SDOR was 6.8(95%CI:2.6-17.6;I2:27.1%).The summary ROC curve showed an area under the curve(AUC)of 0.82(SE 0.06;Q*0.76).High risk of bias and applicability concerns were observed in the domains of pa

目的：研究糖尿病终末期肾病（ＥＳＲＤ）患者的透析治疗，选择血液透析（ＨＤ）好还是持续性非卧床腹膜透析（ＣＡＰＤ）好。方法：观察了６８例接受透析治疗的糖尿病ＥＳＲＤ患者，其中５２例进行ＨＤ治疗，１６例进行ＣＡＰＤ治疗。对比两组患者透析前后的血液生化指标、生存率、死亡原因、血糖的控制、透析后主要并发症。结果：６０岁以下ＨＤ或ＣＡＰＤ治疗患者３年生存率均达７５％以上。但透析前合并有高血压、心脏肥大、冠心病或年龄大于６０岁者，行ＣＡＰＤ治疗后出现并发症的机会较ＨＤ少。结论：透析前合并有高血压、心脏肥大、冠心病或年龄大于６０岁的糖尿病ＥＳＲＤ患者以选择ＣＡＰＤ治疗较佳。
