Endoscopic treatment of biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants: lessons learned from single-balloon enteroscopy-assisted ERCP

Background: Endoscopic treatment of biliopancreatic pathology is challenging due to surgically altered anatomy after Whipple's pancreaticoduodenectomy. This study aimed to evaluate the feasibility and safety of single-balloon enteroscopy-assisted endoscopic retrograde cholangiopancreatography(sBE-ERCP) to treat biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants. Methods: We retrospectively analyzed 106 sBE-ERCP procedures in 46 patients with Whipple's variants. Technical and clinical success rates and adverse events were evaluated. Results: Biliary sBE-ERCP was performed in 34 patients and pancreatic sBE-ERCP in 17, including 5 with both indications. From a total of 106 sBE-ERCP procedures, 76 were biliary indication with technical success rate of 68/76(90%) procedures and clinical success rate of 30/34(88%) patients. Mild adverse event rate was 8/76(11%), without serious adverse events. From a total of 106 sBE-ERCP procedures, 30 were pancreatic indication with technical success rate of 24/30(80%) procedures( P = 0.194 vs. biliary sBEERCP) and clinical success rate of 11/17(65%) patients( P = 0.016 vs. biliary sBE-ERCP). Mild adverse event rate was 6/30(20%)( P = 0.194 vs. biliary sBE-ERCP), without serious adverse events. After sBE-ERCP failure, endoscopic ultrasound-guided drainage, percutaneous drainage and redo surgery were alternative therapeutic options. Conclusions: Biliopancreatic pathology after Whipple's pancreaticoduodenectomy variants can be treated using sBE-ERCP without serious adverse events. Technical and clinical success rates are high for biliary indications, whereas clinical success rate of pancreatic indications is significantly lower. sBE-ERCP can be considered as first-line treatment option in this patient group with surgically altered anatomy.

Directed x-rays produced in the interaction of sub-picosecond laser pulses of moderate relativistic intensity with plasma of near-critical density are investigated. synchrotron-like (betatron) radiation occurs in the process of direct laser acceleration (DlA) of electrons in a relativisticlaser channel when the electrons undergo transverse betatron oscillations in self-generated quasi-static electric and magnetic fields. In anexperiment at the PHElIX laser system, high-current directed beams of DlA electrons with a mean energy ten times higher than the ponderomotive potential and maximum energy up to 100 MeV were measured at 10^(19) W/cm^(2)laser intensity. The spectrum of directed x-raysin the range of 5–60 keV was evaluated using two sets of Ross filters placed at 0°and 10°to the laser pulse propagation axis. The differential x-ray absorption method allowed for absolute measurements of the angular-dependent photon fluence. We report 10^(13) photons/sr withenergies >5 keV measured at 0°to the laser axis and a brilliance of 10^(21) photons s^(−1) mm^(−2) mrad−2(0.1%BW)−1. The angular distributionof the emission has an FWHM of 14°–16°. Thanks to the ultra-high photon fluence, point-like radiation source, and ultra-short emissiontime, DlA-based keV backlighters are promising for various applications in high-energy-density research with kilojoule petawatt-class laserfacilities.

Highly enhanced UV absorption and light emission of monolayer Ws_(2)through hybridization with Ti_(2)n MXene quantum dots and g-C_(3)n_(4)quantum dots

Two-dimensional(2D)transition metal dichalcogenides(TMD)are atomically thin semiconductors with promising optoelectronic applications across the visible ***,their intrinsically weak light absorption and the low photoluminescence quantum yield(PlQY)restrict their performance and potential use,especially in ultraviolet(UV)wavelength light *** dots(QD)derived from 2D materials(2D/QD)provide efficient light absorption and emission of which energy can be tuned for desirable light *** this study,we greatly enhanced the photon absorption and PlQY of monolayer(1l)tungsten disulfide(Ws_(2))in the UV range via hybridization with 2D/QD,particularly titanium nitride MXene QD(Ti_(2)n MQD)and graphitic carbon nitride QD(GCnQD).With the hybridization of MQD or GCnQD,1lWs_(2)showed a maximum Pl enhancement by 15 times with 300 nm wavelength excitation,while no noticeable enhancement was observed when the excitation photon energy was less than the bandgap of the QD,indicating that UV absorption by the QD played a crucial role in enhancing the light emission of 1l-Ws_(2)in our 0D/2D hybrid *** findings present a convenient method for enhancing the photo-response of 1l-Ws_(2)to UV light and offer exciting possibilities for harvesting UV energy using 1l-TMD.

The relationship between DnA fragmentation and the intensity of morphologically abnormal human spermatozoa

Objective:To determine the relationship between teratozoospermia and sperm DnA fragmentation(sDF)in the human ***:This retrospective study included 100 normozoospermic men as a control cohort(abnormal forms>14%),210 patients with a high level of abnormal forms(≤4%)and 65 patients presenting with a moderate level of abnormal forms(>4%to≤14%)based on the World Health Organization *** morphology was assessed using bright field *** DnA fragmentation was assessed using the sperm chromatin dispersion ***-parametric analyses were conducted to examine the relationship between abnormal sperm morphology and sperm DnA fragmentation;receiver operating characteristic(ROC)analyses were conducted to assess sensitivity and specificity of this ***:A correlation analysis revealed that the higher the proportion of abnormal spermatozoa in the ejaculate,the higher the level of sDF(spearman's Rho=-0.230;Psignificant differences in the proportion of sDF were found when all cohorts were compared(Pse significant differences were also retained when the different cohorts were compared *** analysis showed a moderate but significant predictive value for sDF to differentiate patients with different levels of ***:Although analysis of a more continuous range of values for teratozoospermia would help further clarify any causal relationship with sDF,there is clearly a synergistic or coincident affiliation between these variables that needs to be acknowledged by the clinician when interpreting the spermiogram.

Prevalence of visual impairment and estimation of refractive errors among school children in Kakamega,Kenya

AIM:To investigate the prevalence of visual impairment(VI)and provide an estimation of uncorrected refractive errors in school-aged children,conducted by optometry students as a community ***:The study was *** 3343 participants were included in the *** initial examination involved assessing the uncorrected distance visual acuity(UDVA)and visual acuity(VA)while using a+2.00 D *** inclusion criteria for a subsequent comprehensive cycloplegic eye examination,performed by an optometrist,were as follows:a UDVAl(0.20 logMAR)and/or a VA with+2.00 D≥0.8 decimal(0.96 logMAR).REsUlTs:The sample had a mean age of 10.92±2.13y(range 4 to 17y),and 51.3%of the children were female(n=1715).The majority of the children(89.7%)fell within the age range of 8 to *** the ethnic groups,the highest representation was from the luhya group(60.6%)followed by luo(20.4%).Mean logMAR UDVA choosing the best eye for each student was 0.29±0.17(range 1.70 to 0.22).Out of the total,246 participants(7.4%)had a full eye *** estimated prevalence of myopia(defined as spherical equivalent≤-0.5 D)was found to be 1.45%of the total *** around 0.18%of the total sample had hyperopia value exceeding+1.75 *** astigmatism(cils found in 0.21%(7/3343)of the *** VI prevalence was 1.26%of the total *** our cases of VI,76.2%could be attributed to uncorrected refractive *** was detected in 0.66%(22/3343)of the screened *** was no statistically significant correlation observed between age or gender and refractive ***:The primary cause of VI is determined to be uncorrected refractive errors,with myopia being the most prevalent refractive error *** findings underscore the significance of early identification and correction of refractive errors in school-aged children as a means to alleviate the impact of VI.
