Proximal and distal esophageal sensitivity is decreased in patients with Barrett’s esophagus

AIM: To investigate sensations to multimodal pain stimulation in the metaplastic and normal parts of the esophagus in patients with Barrett’s esophagus (BE). METHODs: Fifteen patients with BE and 15 age-matched healthy volunteers were subjected to mechanical, thermal and electrical pain stimuli of the esophagus. Both the metaplastic part and the normal part (4 and 14 cm, respectively, above the esophago-gastric junction) were examined. At sensory thresholds the stimulation intensity, referred pain areas, and evoked brain potentials were recorded. REsULTs: Patients were hyposensitive to heat stimulation both in the metaplastic part [median stimulation time to reach the pain detection threshold: 15 (12-34) s vs 14 (6-23) s in controls; F = 4.5, P = 0.04] and the normal part of the esophagus [median 17 (6-32) s vs 13 (8-20) s in controls; F = 6.2, P = 0.02]. Furthermore, patients were hyposensitive in the metaplastic part of the esophagus to mechanical distension [median volume at moderate pain: 50 (20-50) mL vs 33 (13-50) mL in controls; F = 5.7, P = 0.02]. No indication of central nervous system abnormalities was present, as responses were comparable between groups to electrical pain stimuli in the metaplastic part [median current evoking moderate pain: 13 (6-26) mA vs 12 (9-24) mA in controls; F = 0.1, P = 0.7], and in the normal part of the esophagus [median current evoking moderate pain: 9 (6-16) mA, vs 11 (5-11) mA in controls; F = 3.4, P = 0.07]. Furthermore, no differences were seen for the referred pain areas (P-values all > 0.3) or latencies and amplitudes for the evoked brain potentials (P-values all > 0.1). CONCLUsION: Patients with BE are hyposensitive both in the metaplastic and normal part of esophagus likely as a result of abnormalities affecting peripheral nerve pathways.

In response to environmental variation, angiosperm trees bend their stems by forming tension wood, which consists of a cellulose-rich G （gelatinous）-Iayer in the walls of fiber cells and generates abnormal tensile stress in the secondary xylem. We produced transgenic poplar plants overexpressing several endoglycanases to reduce each specific polysaccharide in the cell wall, as the secondary xylem consists of primary and secondary wall layers. When placed horizontally, the basal regions of stems of transgenic poplars overexpressing xyloglucanase alone could not bend upward due to low strain in the tension side of the xylem. In the wild-type plants, xyloglucan was found in the inner surface of G-layers during multiple layering. In situ xyloglucan endotransglucosylase （XET） activity showed that the incorporation of whole xyloglucan, potentially for wall tightening, began at the inner surface layers s1 and s2 and was retained throughout G-layer development, while the incorporation of xyloglucan heptasaccharide （XXXG） for wall loosening occurred in the primary wall of the expanding zone. We propose that the xyloglucan network is reinforced by XET to form a further connection between wall-bound and secreted xyloglucans in order to withstand the tensile stress created within the cellulose G-layer micro fibrils.

Hepatic resection (HR) and radiofrequency ablation (RFA) are popular local therapies for early-stage hepatocellular carcinoma (HCC).Alpha-fetoprotein,Lens culinaris agglutininreactive fraction of alpha-fetoprotein,and des-c-carboxy prothrombin are well-known and useful tumor markers for *** positive number status of these tumor markers has recently been demonstrated as beneficial for predicting outcome for HCC patients treated with local *** the normal ranges reported have differed by institution,the positivity of tumor markers is consistent and can easily be *** and Wakayama's group clearly demonstrated the following:1) Regardless of the degree of tumor stage,a triple-positive tumor marker profile can predict poor outcome in HCC patients undergoing HR;2) For RFA alone,HCC patients with double-and triple-positive status,having less than three lesions and lesions ≤3 cm in diameter show comparably insufficient outcomes;3) For HCC patients with lesions ≤5 cm in Child-Pugh grade A,HR is preferred over RFA;4) Microvascular invasion rates increased even in the double-positive patients,while poorly differentiated HCC was frequently observed only in the triple-positive patients;and 5) RFA with chemoembolization,anatomical liver resection,and postoperative adjuvant chemoembolization or hepatic arterial chemotherapy might improve the outcome for patients with highly malignant HCC with multiple positive tumor ***,the impacts of these therapies still need to be evaluated in prospective comparative studies.

Discipline Impact Factor: some of Its History, some of the Author's Experience of Its Application, the Continuing Reasons for Its Use and… Next Beyond

Purpose: This work aims to consider the role and some of the 42-year history of the discipline impact factor(DIF) in evaluation of serial publications. Also, the original "symmetric" indicator called the "discipline susceptibility factor" is to be presented. Design/methodology/approach: In accordance with the purpose of the work, the methods are analytical interpretation of the scientific literature related to this problem as well as speculative explanations. The information base of the research is bibliometric publications dealing with impact, impact factor, discipline impact factor, and discipline susceptibility ***: Examples of the DIF application and modification of the indicator are given. It is shown why research and university libraries need to use the DIF to evaluate serials in conditions of scarce funding for subscription to serial publications, even if open access is available. The role of the DIF for evaluating journals by authors of scientific papers when choosing a good and right journal for submitting a paper is also briefly discussed. An original indicator "symmetrical" to the DIF(the "discipline susceptibility factor") and its differences from the DIF in terms of content and purpose of evaluation are also briefly *** limitations: The selection of publications for the information base of the research did not include those in which the DIF was only mentioned, used partially or not for its original purpose. Restrictions on the length of the article to be submitted in this special issue of the JDIs also caused exclusion even a number of completely relevant publications. Consideration of the DIF is not placed in the context of describing other derivatives from the Garfield impact factor. Practical implications: An underrated bibliometric indicator, viz. the discipline impact factor is being promoted for the practical application. Isolation and characterization of marine biofilm forming bacteriafrom a ship＇s hull

Diverse aquatic microorganisms are capable of colonizing living and non-living surfaces leading to the formation of biofilms. Commonly visualized as a slimy layer, these biofilms are filled with hundreds of other microorganisms compared to free living planktonic cells. Microbial surface colonization and surface-associated metabolic activities also exert several macroscale deleterious effects, including biofouling, biocorrosion and the persistence and transmission of harmful or pathogenic microorganisms and virulence determinants. The present study deals with the isolation and screening of marine bacteria for biofilm formation. The screened isolates were characterized and identified as Pychrobacter celer, Pychrobacter alimentarius and Kocuria rhizophila by 16s rRNA sequencing. Biofilm forming bacteria were isolated by spread plate technique and subjected to screening by microtiter plate assay. The potent biofilm formers were identified by molecular characterization using 16s rRNA gene sequencing. Twelve bacterial isolates were obtained by pour plate technique and subjected to biofilm assay. Among the 12 isolates three isolates which showed maximum biofilm formation were subjected to molecular characterizationby 16s rRNA gene sequencing method. The isolates were identified as Pychrobacter celer, Pychrobacter alimentarius and Kocuria rhizophila. The EPs produced by the three biofilm forming bacteria was extracted and the protein and carbohydrate content determined. Among the isolates screened, isolate 8 (Kocuria rhizophila) produced maximum protein and carbohydrate which was also in accordance with the results of microtiter plate assay.

Distribution of the related weevil species sitophilus oryzae and s. zeamais in Brazil

The genus sitophilus （Coleoptera： Curculionidae） encompasses species of great economic importance as stored grain pests worldwide. Among these species, the maize and the rice weevils （sitophilus zeamais and sitophilus oryzae, respectively） are partic- ularly important in warmer climates. These two weevils exhibit closely morphological and ecological resemblance making difficult their proper identification and recognition of their distribution in grain-producing regions. Both species are recorded in south America and particularly in Brazil, but their respective distribution and prevalence were not yet assessed in the region. Therefore, several insect samples throughout Brazil were collected and subjected to morphological identification using male genitalia and also using molec- ular identification with species-specific primers designed for clear recognition of both the species. The primers were designed for the specific amplification of a gene fragment of the cytochrome oxidase subunit I, which exhibited high specificity during our prelimi- nary experiments with insects from six populations of known species （either s. zeamais or s. oryzae）. Both identification strategies provided the same results indicating preva- lence of the maize weevil s. zeamais throughout the country. Two hypotheses may explain such prevalence：（i） the likely host preference ofs. zeamais for maize because this is the most cultivated cereal in Brazil, and （ii） the prevalence ofs. zeamais in tropical regions as compared with s. oryzae, which is more disseminated in subtropical and temperate regions.

Defining and predicting deep remission in patients with perianal fistulizing Crohn's disease on anti-tumor necrosis factor therapy

Perianal fistulas can occur to up to one-third of patients with Crohn's disease(CD) leading to significant disabling disease and morbidity. Fistulising perianal CD treatment often necessitates a combined pharmacological and surgical approach. Anti-tumor necrosis factor(anti-TNF) therapy, particularly infliximab, has been shown to be very effective for both perianal and internal fistulising CD. Nevertheless, current data suggest that sustained remission and long-term complete fistula healing can be achieved in only 30% to 50% of patients. Moreover, these percentages refer mostly to clinical rather than deep remission, defined as endoscopic and radiologic remission, which is quickly emerging as the preferred goal of therapy. Unfortunately, the therapeutic options for perianal fistulising CD are still limited. As such, it would be of great value to be able to predict, and more importantly, prevent treatment failure in these patients by early and continued optimization of antiTNF therapy. similar to ulcerative colitis and luminal CD, recent data demonstrate that higher infliximab concentrations are associated with better clinical outcomes in patients with perianal fistulising CD. This suggests that therapeutic drug monitoring and a treatto-trough therapeutic approach may emerge as the new standard of care for optimizing anti-TNF therapy in patients with perianal fistulising CD.
