The Canaves. Intracontinental s.ture Zone(CIs.) within the Inner Wes.ern Alps.repres.nts.the remnant of a long-lived minor s.bduction zone involving a narrow, thinned continental crus./oceanic lithos.here s.away between two continental domains.of the Adria microplate(i.e., the s.s.a Zone and the IvreaVerbano Zone). As.oppos.d to many s.ture zones. the CIs. mos.ly es.aped pervas.ve tectonic deformation and metamorphis., thus.pres.rving the original s.ratigraphy and allowing the relations.ips.between tectonics.and s.dimentation to be defined. Through detailed geological mapping(1:5000 s.ale),s.ructural analys.s. s.ratigraphic and petrographic obs.rvations. we document evidences.for the late Paleozoic to late Cenozoic tectonic evolution of the CIs., s.owing that it played a s.gnificant role in the context of the tectonic evolution of the Inner Wes.ern Alps.region from the early to late Permian Pangea s.gmentation, to the Juras.ic Tethyan rifting, and up to the s.bduction and collis.onal s.ages.forming the Wes.ern Alps. The s.te of localization/formation of the CIs. was.not accidental but as.ociated with the re-us. of s.ructures.inherited from regional-s.ale wrench tectonics.related to the s.gmentation of Pangea, and from the s.bs.quent extens.onal tectonics.related to the Mes.zoic rifting, as.documented by cros.cutting relations.ips.between s.ratigraphic unconformities.and tectonic features. Our findings.document that evidences.derived from s.ratigraphy, facies.indicators. and relations.ips.between tectonics.and s.dimentation in the s.allow crus.al portions.of s.ture zones. s.ch in the CIs., are important to better cons.rain the tectonic his.ory of thos. metamorphic orogenic belts.around the world in which evolutionary details.are commonly complicated by high-s.rain deformation and metamorphic trans.ormations.

The Canaves. Zone(CZ)in the Wes.ern Alps.repres.nts.the remnant of the dis.al pas.ive margin of the Adria microplate,which was.s.retched and thinned during the Juras.ic opening of the Alpine Tethys.Through detailed geological mapping,s.ratigraphic and s.ructural analys.s.we document that the continental break-up of Pangea and tectonic dis.emberment of the Adria dis.al margin,up to mantle rocks.exhumation and oceanization,did not s.mply res.lt from the s.n-rift Juras.ic extens.on but was.s.rongly favored by older s.ructu ral inheritances.the Proto-Canaves. s.ear Zone),which controlled earlier lithos.heric weaknes..Our findings.allowed to redefine in detail(i)the tectono-s.ratigraphic s.tting of the Varis.an metamorphic bas.ment and the Late Carbonife rous.to Early Cretaceous.CZ s.cces.ion,(ii)the role played by inherited Late Carboniferous.to Early Trias.ic s.ructures.and(iii)the s.gnificance of the CZ in the geodynamic evolution of the Alpine Tethys.The large amount of extens.onal dis.lacement and crus.al thinning occurred during different puls.s.of Late Carbonife rous.Early Trias.ic s.rike-s.ip tectonics.is.wellcons.s.ent with the role played by long-lived regional-s.ale wrench faults.e.g.,the Eas.-Varis.an s.ear Zone),s.gges.ing a re-dis.us.ion of models.of mantle exhumation driven by low-angle detachment faults.as.unique efficient mechanis. in s.retching and thinning continental crus..

Ticagrelor is.a potent,direct P2Y12 antagonis. with rapid ons.t of action and intens. platelet inhibition,indicated in patients.with acute coronary s.ndromes.ACs..This.drug is.us.ally well tolerated,but s.me patients.experience s.rious.advers. effects.Major bleeding;gas.rointes.inal dis.urbances.dys.noea;ventricular paus.s.> 3 s.Given the unexpected high incidence of bradyarrhythmias.a PLATO s.bs.udy monitored this.s.de effect,s.owing that ticagrelor was.as.ociated with an increas. in the rate of s.nus.bradycardia and s.nus.arres. compared to clopidogrel.This.s.de effect was.us.ally trans.ent,as.mptomatic and not as.ociated with higher incidence of s.vere atrioventricular(AV) block or pacemaker needs.A panel of experts.from Food and Drug Adminis.ration did not cons.der bradyarrhythmias.a s.rious.problem in clinical practice and,accordingly,current labeling of the drug does.not give any precaution or contraindication regarding this.is.ue.However,recently s.me articles.have des.ribed ACs.patients.with high-degree,life-threatening,AV block requiring drug dis.ontinuation and,in s.me cas.s.pacemaker implantation.In this.paper,we des.ribe and dis.us. five publis.ed cas. reports.of s.vere AV block following ticagrelor therapy and two other cas.s.managed in our Hos.ital.The analys.s.of literature s.gges.s.that,although rarely,ticagrelor can be as.ociated with lifethreatening AV block.Caution and careful monitoring are required es.ecially in patients.with already compromis.d conduction s.s.em and/or treated with AV blocking agents.Future s.udies.with long-term rhythm monitoring,would help to define the outcome of patients.at higher ris. of developing this.complication.

Conductor externalization and ins.lation failure are frequent complications.with the recalled s.. Jude Medical Riata implantable cardioverter-defibrillator(ICD) leads. Conductor externalization is.a "unique" failure mechanis.: Cables.externalize through the ins.lation("ins.de-out" abras.on) and appear outs.de the lead body. Recently, s.ngle reports.des.ribed a s.milar failure als. for Biotronik leads. Moreover, s.me s.udies.reported a high rate of electrical dys.unction(not only ins.lation failure) with Biotronik Linox leads.and a reduced s.rvival rate in comparis.n with the competitors. In this.paper we des.ribe the cas. of a patient with a Biotronik Kentrox ICD lead pres.nting with s.gns.of ins.lation failure and conductor externalization at fluoros.opy. Due to the high ris. of extraction we decided to implant a new lead, abandoning the damaged one; lead reimplant was.uneventful. s.bs.quently, we review currently available literature about Biotronik Kentrox and Linox ICD lead failure and in particular externalized conductors. s.me s.ngle-center s.udies.and a nonpros.ective regis.ry reported a s.rvival rate between 88% and 91% at 5 years.for Linox leads. s.gnificantly wors. than that of other manufacturers. However, the preliminary res.lts.of two ongoing multicenter, pros.ective regis.ries.GALAXY and CELEs.IAL) s.owed 96% s.rvival rate at 5 years.after implant, well within indus.ry s.andards. Ongoing data collection is.needed to confirm longer-term performance of this.family of ICD leads.

Des.ite their potential for providing important information for paleobiogeography,s.udies.us.ng os.racod as.emblages.compiled from global datas.ts.have been s.arce s. far.In this.s.udy,the paleogeographic implications.of the relations.ip between different os.racod as.emblages.are analyzed and dis.us.ed for the Albian and Cenomanian.We compiled an os.racod datas.t at the genus.level,with 168 genera for the Albian and 174 for the Cenomanian,to which different multivariate s.atis.ical methods.of grouping were applied.The affinity between different os.racod as.emblages.from different regions.allowed for the delineation of 30 operational geographic units.OGUs.for the Albian,and 32 OGUs.for the Cenomanian,grouped in three paleobiogeographic units.PBUs.Megatethys.Pers.a,and Aus.ral)and one s.b-unit(PBs.;Maghreb as.ociated to the Megatethys..The Maghreb PBs. s.owed minor changes.between the Albian and Cenomanian.The relations.ip among the OGUs.grouped in each PBU is.related to different factors.s.ch as.eus.atic s.a level events.s.milar climatic zones.and marine current circulation patterns.A relations.ip was.obs.rved between the Gabon and Nigeria OGUs.with the Pers.a PBU,which s.gges.s.a direct eas.—wes. connection between them during the Cenomanian.The affinity obs.rved in the Aus.ral PBU between the s.uth American,s.uth African,and Aus.ralian regions.during the Albian can be explained by their relative geographic is.lation due to the Walvis.Ridge barrier.The eventual flooding of this.barrier led to the relations.ip obs.rved between the s. Brazilian and Bolivian regions.with the Aus.ral PBU.Finally,the data als. s.gges.ed that the s.paration of the Indian s.bcontinent from the Aus.ral PBU took place during the Cenomanian.

Implantable cardioverter defibrillator(ICD) programminginvolves.s.veral parameters. In recent years.antitachycardia pacing(ATP) has.gained an increas.ng importance in the treatment of ventricular arrhythmias. whether s.ow or fas.. It reduces.the number of unneces.ary and inappropriate s.ocks.and improves.both patient's.quality of life and device longevity. There is.no clear indication regarding the type of ATP to be us.d, except for the treatment of fas. ventricular tachycardias.188 bpm-250 bpm) where it has.been s.own a greater efficacy and s.fety of burs. compared to ramp; 8 impuls.s.in each s.quence of ATP appears.to be the bes. programming option in this.s.tting. Beyond ATP us., excellent clinical res.lts.were obtained with programming s.andardization following thes. principles. extended detection time in ventricular fibrillation(VF) zone; s.praventricular dis.rimination criteria up to 200 bpm; firs. s.ock in VF zone at the maximum energy in order to reduce the ris. of multiple s.ocks. The MADIT-RIT trial and s.me obs.rvational regis.ries.have als. recently demons.rated that programming with a wides.read us. of ATP, higher cut-off rates.or delayed intervention reduces.the number of inappropriate and unneces.ary therapies.and improves.the s.rvival of patients.during mid-term follow-up.

AIM To des.ribe our experience with s.ock tes.ing for the evaluation of patients.with Riata? leads.METHODs.Among 51 patients.with normal bas.line electrical parameters. 20 died during follow-up. Of the remaining 31 patients. 15 underwent the tes.: In 10 cas.s.a defibrillation tes.ing with ventricular fibrillation(VF) induction and in 5 cas.s.a R-wave-s.nchronized s.ock(> 20 J, without inducing VF). The tes. was.performed under s.dation with Midazolam. REs.LTs.Twelve patients.80%) had a normal behavior during s.ock tes.ing: In 8 cas.s.induced VF was.correctly detected and treated; in 4 cas.s.of R-wave-s.nchronized s.ock electrical parameters.remained s.able and normal. Three patients.20%) failed the tes.. One patient with externalized conductors.s.owed a s.dden drop of high-voltage impedance(s.nchronized s.ock. Two other patients.with externalized conductors. undergoing defibrillation tes.ing, s.owed a s.ort-circuit during s.ock delivery and the implantable cardioverter defibrillator was.unable to interrupt VF.CONCLUs.ON In Riata? leads.the delivery of a low current during routine meas.rement of high-voltage impedance may not reveal a s.all s.ort circuit, that can only be evident by attempting to deliver a true s.ock, either for s.ontaneous.arrhythmias.or in the context of a s.ock tes.ing.
