stuck up and fracture of coronary angioplasty hardware are unwonted complications of percutaneous coronary interventions (PCI) for which surgical retrieval and management is once in a while required. We present one case of a 59-year-old diabetic, a hypertensive gentleman who attended the emergency room (ER) with central chest pain for 2 hours. After evaluation of the patient by physical and noninvasive diagnostic tests, he was diagnosed as a case of acute myocardial infarction (AmI) with stable hemodynamic. For the aforementioned findings, the patient was treated first with a thrombolytic agent in ER and then shifted to Cardiology Department for monitoring and further coronary evaluation by coronary angiography (CAG). CAG revealed essentially single vessel disease (SVD) with complex left anterior descending (LAD) artery lesion, so PCI was attempted but failed with an unfortunate incidence of the broken delivery shaft and left in vivo. Immediate decision making and surgical management for retrieval of lost angioplasty device and correction coronary lesion with revascularization save the patient from grave complication. All the series of events and management approaches of this very complex coronary artery lesion are discussed in this article.

This study was conducted to identify a cheap and suitable culture medium for the mycelial growth and sporulation of sternphylium vesicarium and to determine the cultural and morphological variability of this pathogen. A total of 24 isolates of s. vesicarium collected from eight different onion growing areas were characterized in terms of cultural and morphological aspects. Front colony colors were greenish brown to dirty white, deep grey to whitish, light grey to whitish, deep greenish white, light grey and dirty white to greenish. Reverse colony colors were brown, deep brown and light brown. Colony shapes were circular and irregular with umbonate, raised and flat type colony elevation. Colony textures were cottony, fluffy and velvety with entire, undulate and filiform type colony margin. Among the culture media, V-7 juice agar found to be the most suitable culture media for mycclial growth of s. vesicarium. The sporulation of the fungus was remarkably influenced by V-7 juice mixed with potato dextrose agar （PDA） media, this media exhibited the highest sporulation （87.76-169.0/mm^2） of s. vesicariumin comparison with other media. The minimum days （28 d to 31 d） for conidial production were observed on V-7 juice agar medium. The length of conidia varied from 14.6 μm to 30.6 μm. The maximum mean length of conidia was 29.97 μm found in isolate DssA, while the minimum mean length 17.36 μm was found in isolate msmm 02. The breadth of conidia ranged from 4.7 μm to 15.7μm. The maximum mean breadth of conidia was 12.55 μm found in the isolate DssA, while the minimum mean breadth 9.760 μm was found in the isolate CCKH 02. The horizontal septation varied from l to 3 and the longitudinal septation varied from 0 to 4.

strong>Background: strong>Progressive insulin resistance (IR) is an important pathophysiologic mechanism of gestational diabetes mellitus (GDm). Homeostatic model assessment (HOmA) is commonly used as a parameter of the severity of insulin resistance. strong>Aims:strong> To determine indices of insulin resistance (IR) and m>βm>-cell function in gestational diabetes mellitus (GDm). strong>methods:strong> This cross sectional study was conducted from march 2017 to september 2018 at Department of Endocrinology, Bangabandhu sheikh mujib medical University (BsmmU), Dhaka, Bangladesh. The study was performed with 41 GDm and equal number of pregnant women with normal glucose tolerance (NGT) diagnosed on basis of WHO criterion-2013 during 24 - 40 weeks of gestation. serum glucose was measured by glucose oxidase method and fasting serum insulin was measured by chemiluminescent immunoassay. Equations of homeostatic model assessment (HOmA) were used to calculate insulin indices like-insulin resistance (HOmA-IR), m>βm>-cell function (HOmA-B) and insulin sensitivity (HOmA-%s). Data were analyzed and compared by statistical tests. strong>Results: strong>A total of eighty-two (82) subjects [41 women with GDm (age: 28.29 ± 3.79 years, BmI: 27.16 ± 4.13 kg/m2) and 41 women with NGT (age: 26.22 ± 5.13 years, BmI: 25.27 ± 3.01 kg/m2)] were included in this study. It was observed that GDm women were significantly older (p = 0.041) and had significantly higher BmI (p = 0.020) than pregnant women with NGT. Background: Cardio-renal syndrome is the leading cause of death globally. Acute kidney injury (AKI) is a major complication among patients admitted in Coronary Care Unit (CCU) with acute coronary syndrome (ACs). AKI in ACs patients is associated with higher morbidity, mortality and prolong hospital stay. Objective: To determine the incidence, risk factors and short term outcome of acute kidney injury (AKI) among the patients admitted in CCU with ACs. methods: This cross sectional study was conducted at Department of Nephrology, Bangabandhu sheikh mujib medical University (BsmmU), Dhaka, Bangladesh from January 2013 to December 2013. Following selection criteria a total of two hundred (200) patients with ACs were studied. AKI was defined according to the KDIGO guideline and further categorized by RIFLE criteria. Data of patients with AKI and those without AKI were analyzed and compared by statistical tests. Results: majority of the study patients were male (79.5%). AKI was developed in 40 of the 200 study patients (20.0%). The mean (±sD) age of the study patients was 60.1 ± 11.1 years in patients with AKI and 55.9 ± 10.1 years in patients without AKI. Our data analysis showed that older age (>65 years), diabetes mellitus, dyslipidemia and smoking were significantly correlated to the development of AKI. mortality was significantly high in AKI group (7.15%) compared to no AKI group (1.25%). Out of 27 patients who achieved renal recovery most (80.8%) belonged to risk class. severity of AKI showed significant effect on renal recovery and final outcome. Conclusion: This study demonstrated that, incidence of AKI among patients with ACs was 20%. Elderly patients, diabetes mellitus, dyslipidemia, and exposure to smoking were significantly associated with development of AKI in ACs.

In an area of about 305 km2, seventeen stream sediment samples were taken from alluvium on the banks and floodplain of the Lokoundjé River and its tributaries. sand samples after being dried in an oven, are subjected to sedimentological analyses in laboratory. Amongst the analysis carried out;the particle size analysis helped to establish a precise textural description of the sediment and to understand the active mechanisms which occur during transport and particle deposition. The morphoscopy of quartz grains consists in determining the mode and duration of transport of the grains, thus allowing reconstructing the geological history of the latter. Heavy minerals are suitable to know the nature of the rock in which they come from and equal to specify their distributing province. The morphometry of pebble makes contributes for a better understanding to apprehension of the agents responsible for the transport of these materials. It appears that the alluvium of the Lokoundjé and its Bipindi tributaries are very fine to coarse. They are well segregated, classified, and homometric. It is the result of the actions of a hydrodynamic turbulent which is sometimes abrupt. From a morphoscopic point of view, using a binocular microscope, grains mainly belong to three categories based on their surface appearance: Grain unworn, the sub-dull and dull shiny. From the morphometric point of view, there is evidence of flattening and dissymmetry of pebbles which are slightly worn to form sub-spherical shapes. With regard to the study of heavy minerals, it has identified two mineralogical processions, which include a metamorphic process with the presence of sillimanite and Andalusite. In addition, there is the presence of minerals such as green’s Hornblende, Tourmaline, and reflecting magmatic distribution. However, there are specific minerals such as zircon, which can be economically viable where the contents sometimes exceed 10 kg/m3. In the same vein, it has been noted the presence of

This Laubry-Pezzi syndrome is rarely seen in patients with ventricular septal defect (VsD) combined with aortic regurgitation. A 20-year-old male patient presented to the Cardiothoracic and vascular surgery department with a large VsD with severe aortic regurgitation (AR). However, he was first symptomatic at 12 years of age and initially treated by medical management with the suggestion of surgical correction. Nevertheless, he was delayed getting surgical management due to his financial problem. This article reported on an adult patient with Laubry-Pezzi syndrome and his surgical correction and outcome. The association of congenital defects VsD and AR needs to be identified and corrected in early life for better outcomes.

Background: Impaired fasting glucose (IFG) is a prediabetic condition and is a high-risk state for developing diabetes and associated complications. The aim of this study was to explore the association of vitamin D with insulin secretory function among the IFG subjects. materials and methods: This was a prospective cross-sectional study conducted in the Department of Biochemistry and Cell Biology, Bangladesh University of Health sciences (BUHs), Dhaka, Bangladesh;from June 2016 to may 2017, on forty-six (46) IFG subjects. serum blood glucose was measured by glucose-oxidase method, Fasting serum lipid profile (Total cholesterol, TG and HDL-c);liver enzyme like alanine aminotransferase (ALT) was measured by enzymatic-colorimetric method;serum creatinine was measured by colorimetric kinetic method;serum Insulin and vitamin D [25(OH)D] were measured by ELIsA method;insulin secretory capacity (HOmA%B) and insulin sensitivity (HOmA%s) were calculated by Homeostasis model Assessment (HOmA) using HOmA-CIGmA software. Results: Among 46 IFG subjects, 22 (47.8%) were male and rest 24 (52.2%) were female and their mean age was 40 (±8) years. mean fasting serum glucose level of the study subjects was 6.33 (±0.23) mmol/l and mean postprandial serum glucose level was 7.23 (±0.41) mmol/l. mean serum vitamin D level of the study subjects was 26.54 (±8.83) ng/ml. mean HOmA%s, HOmA%B and HOmA%IR of the total study subjects were 48.34 (±16.70), 102.16 (±23.13) and 2.28 (±0.70) respectively. Insulin secretory capacity (HOmA%B) was significantly higher (p = 0.047) in the subjects having higher vitamin D [25(OH)D level ≥ 20 ng/ml] compared to the subjects having lower vitamin D [25(OH)D levels Conclusion: High prevalence of hypovitaminosis D exists among IFG subjects. Vitamin D is associated with insulin secretory function in IFG subjects. Population based prospective studies using larger sample size should be done to confirm the results.
