H pylori (CagA) and Epstein-Barr virus infection in gastric carcinomas:Correlation with p53 mutation and c-Myc,Bcl-2 and Bax expression

AIM: To investigate the interrelationship between H pylori and Epstein-Barr virus (EBV) infection in the gastric carcinogenesis having in focus the p53 mutation and the c-Myc, Bcl-2 and Bax expression. METHODs: seventy-one gastric carcinoma tissues were assessed by polymerase chain reaction (PCR) for H pylori and in situ hybridization for EBV. c-Myc, Bcl-2 and Bax expression were detected by immunohistochemistry and single-stranded conformational polymorphism (ssCP) for p53 mutation. REsULTs: The positivity rates for H pylori and EBV were 94.4% and 8.45%, respectively. The majority of the cases displayed only the H pylori presence. All EBV positive cases were also H pylori positive. None infectious agent was observed in 5.55% of the cases. The intestinal type tumor was more frequent in the co-infected and non-infected groups. The female predominated in the non-infected group showing statistical significance (70.4% vs 29.6%, P=0.039). The Bcl-2 was only detected in the group exclusively infected by H pylori. However, c-Myc and Bax were detected in the three groups but with a low frequency in the co-infected group. Mutation of p53 was present in all groups, with the highest frequencies in the H pylori positive groups. CONCLUsION: The frequency of H pylori infection in gastric carcinomas was high. The presented data indicated that gastric carcinogenesis has different pathways depending of the presence of the two investigated infectious agents, suggesting a possible involvement of H pylori with apoptotic process. The low expression of c-Myc and Bax in the EBV-positive groups suggests that EBV may inhibit the expression of these proteins. Nevertheless, p53 mutation shows to be a relevant alteration, independent of both infectious agents.

Introduction: systemic Lupus Erythematosus (sLE) usually presents neuropsychiatric manifestations, such as psychosis, convulsion, mood disorder and chronic headache, with mood disorder being the most prevalent. It is also known that other psychiatric disorders and menstrual cycle disorders are frequent in sLE. The aim of this study was to describe for the first time the frequency of premenstrual dysphoric disorder in patients with sLE diagnosis, and its association with depression and anxiety. Material and Methods: An evaluation was made of 62 (sixty-two) patients with diagnosis of sLE, based on the American College of Rheumatology criteria, who received follow-up care at an ambulatory reference center. The patients were submitted to a sociodemographic evaluation questionnaire, upon which the Mini International Neuropsychiatric Interview (MINI PLUs), Brazilian Version 5.0.0, was used to evaluate the diagnosis. Results: Twenty (32%) patients were found to have premenstrual dysphoric disorder, and there was a statistically significant association with major depression (p = 0.010), but no statistical significance was found with dysthymia (p = 0.063) or anxiety (p = 0.223). Conclusions: The frequency of premenstrual dysphoric disorder (PMDD) in patients with sLE was high, as well as the frequency of major depression in this group. There was an association between these two disorders.

Organizational Trouble shooting through Integration between the Theory of the Restrictions Thinking Process and Lean Tools

The theory of constraints thinking process, created by Israeli physicist Eliyahu M. Goldratt, has emerged as a tool for achieving competitive advantage. Many researches also focused on the application of lean thinking developed by Toyota and proposed by Ohno. This philosophy has been proven to be effective in several production processes. This paper aims to propose a method of problem solving through the integration of theory of constraints thinking process and the principles of lean production. As a tool for problem identification, the method defends the use of current reality tree and, to solve problems, the lean thinking tools, proposed by Picchi. The developed method was implemented in a contractor. The research methodology was research-action. Among the results, there was a realistic diagnosis about the core problems in company. According to this, the core problem of the contractor is “the lack of commitment of manpower” that results in the main problem “the financial loss”. The principle of perfection was verbalized as a proposal to solve the problems and the tools to be implemented for solving problems were “commitment of senior management to employees” and “simplicity in communication”.

Four Cases of X-Linked Hypophosphatemic Rickets, Clinical Description and Genetic Testing

One of the major causes of congenital hypophosphatemic rickets is the X-linked hypophosphatemic rickets (XHR), due to a defect on PHEX gene. The XHR increases the renal elimination of phosphate, that condition leads a defective mineralization of bones and also affects the growth in children. Clinical diagnosis should be suspected in children with signs of rickets and hypophosphatemia with normal calcium levels. We describe clinical characteristics and genetic results of four patients diagnosed and treated in our Nephrology section. All patients have a “de novo” XHR as none familiars are affected. Early diagnosis should be suspected before the bone deformities have been submitted and the growth would have been impaired.

Etiology of Fungemia in Neonates: Four Decades of Case Reports

Objectives: To compare groups of etiologic agents for neonatal fungemia based on previously published case reports with regard to case outcomes. Methods: secondary analysis of data from case reports published in scientific literature indexed in Medline, scopus and LILACs databases, and starting from the year, full texts were registered until the year 2010. Results: Thirty-four species of fungi were isolated in 252 cases reported over a 40-year period. The distribution of fungal groups according to the development level of a case’s region of origin was very uneven (p Aspergillus sp.) in more developed countries. However, 73% of the chi-squared distribution corresponded to differences observed in the frequency of Candida sp. and non-albicans Candida. While the frequency of candidemia by C. albicans was progressively declining in the period from 1966 to 2010, reports of non-albicans candidemia and other non-Candida fungi became significantly (p albicans Candida or non-Candida fungi were isolated. Conclusion: fungi of the genera Candida and Aspergillus were the etiologic agents most frequently identified in published cases of neonatal fungemia;the outcome of death was more often related to cases where non-albicans Candida or non-Candida fungi were isolated.

Ulnar variance related to biological and training characteristics, pain and handgrip strength in Portuguese skeletally immature male gymnasts

Purpose： This study was to investigate the association between ulnar variance（UV） and biological and training characteristics, handgrip, and wrist pain in a group of 23 Portuguese skeletally immature male gymnasts（aged 11.2 2.5 years）.Methods： Left and right UV was obtained using Hafner＇s procedure and skeletal age was determined by the TannereWhitehouse 3-method. A negative mean value for UV measures was observed（ 2.4 to 3.6 mm） without significant differences with increasing age-category（ p=0.09 to p=0.48）. significant low correlations were observed between some UV parameters and stature, fat percentage, years of training, and left handgrip ***： Ten gymnasts reported wrist pain with gradual onset and UV values were very similar between painless and painful ***： The findings of this study do not directly support the thesis that gymnastics training and biological variables or wrist pain are associated with UV.

Integration of Theory of Constraints Thinking Process and Lean Production for Troubleshooting on a Contractor

The thinking process of TOC （Theory of Constraints） was an important consolidated tool to the systematization of the companies＇ necessities because of its competitive advantage. The lean thinking proposed by Taiichi Ohno and developed by Toyota Company has been tested and found effective in several production processes. This paper aims to propose a method of problem solving through the integration of the principles of lean thinking and the TPTOC （Thinking Process of the Theory of Constraints）. The method was implemented on a civil contractor, using the research methodology and the action research. We concluded that, after the research, the core problem regarding the company is the technical failure of the administrative staff, which causes delay in the construction works and financial loss. We also concluded that the obstacles that could prevent the achievement of objectives are related to lack of a vision from senior management regarding financial return when investing in employee training. The goal was accomplished with the integration of two philosophies, TPTOC and lean. And only the tools proposed by Fl^vio Augusto Picchi were not enough for executing the action, so new lean actions were verbalized.

Virtual chromoendoscopy in small bowel capsule endoscopy: New light or a cast of shadow?

AIM: To evaluate whether virtual chromoendoscopy can improve the delineation of small bowel lesions previously detected by conventional white light small bowel capsule endoscopy(sBCE). METHODs: Retrospective single center study. One hundred lesions selected from forty-nine consecutive conventional white light sBCE(sBCE-WL) examinations were included. Lesions were reviewed at three Flexible spectral Imaging Color Enhancement(FICE) settings and Blue Filter(BF) by two gastroenterologists with ex-perience in sBCE, blinded to each other's findings, whoranked the quality of delineation as better, equivalent or worse than conventional sBCE-WL. Inter-observer percentage of agreement was determined and analyzed with Fleiss Kappa(k) coefficient. Lesions selected for the study included angioectasias(n = 39), ulcers/ero-sions(n = 49) and villous edema/atrophy(n = 12). REsULTs: Overall, the delineation of lesions was im-proved in 77% of cases with FICE 1, 74% with FICE 2, 41% with FICE 3 and 39% with the BF, with a percent-age of agreement between investigators of 89%(k = 0.833), 85%(k = 0.764), 66%(k = 0.486) and 79%(k = 0.593), respectively. FICE 1 improved the delineation of 97.4% of angioectasias, 63.3% of ulcers/erosions and 66.7% of villous edema/atrophy with a percentage of agreement of 97.4%(k = 0.910), 81.6%(k = 0.714) and 91.7%(k = 0.815), respectively. FICE 2 improved the delineation of 97.4% of angioectasias, 57.1% of ulcers/erosions and 66.7% of villous edema/atrophy, with a percentage of agreement of 89.7%(k = 0.802), 79,6%(k = 0.703) and 91.7%(k = 0.815), respectively. FICE 3 improved the delineation of 46.2% of angioecta-sias, 24.5% of ulcers/erosions and none of the cases of villous edema/atrophy, with a percentage of agreement of 53.8% [k = not available(NA)], 75.5%(k = NA) and 66.7%(k = 0.304), respectively.
