Effectiveness of patient education focusing on circadian pain rhythms:A case report and review of literature

BACKGROUND Neuropathic pain management should aim at improving quality of life and daily living activities of patients;therefore,emphasis should be placed on pain management including understanding the pain patterns during daily ***,lifestyle guidance should be based on a detailed understanding of pain;however,previous studies commonly evaluated pain intensity at a single point in *** report a case on patient education intervention based on the relationship between pain circadian rhythms and detailed physical activity during the *** SUMMARY A man in his 60s,who suffered a brachial plexus injury in a traffic accident,presented with neuropathic *** assessment of the importance of daily living activities to the patient,pain rhythmicity,and physical activity,was *** early assessments showed that the pain intensity was lower on days when more light-intensity physical activity(LIPA)was performed,than on days when less LIPA was ***,patient education focused on methods to decrease the pain intensity that tended to worsen in the afternoon,and encouraged behavioral changes by suggesting the patient to take walks,which could be used to maintain LIPA in the *** reassessment,the afternoon LIPA,which had been the focus of attention,had increased and a change was noted in the circadian rhythm of *** Patient education based on a composite assessment elicited positive results in relation to the pain circadian rhythm and physical activity.

Objective： To evaluate the treatment effect of acupuncture on patients with idiopathic trigeminal neuralgia（ITN） by case-control longitudinal blinded study. Methods： Sixty ITN patients and 30 healthy subjects were included. The ITN patients were randomly assigned to acupuncture group（15 cases）, sham-acupuncture group（15 cases） and carbamazepine group（30 cases）, respectively. Clinical orofacial evaluation（including pain intensity and medication doses）, research diagnostic criteria for temporomandibular disorders（RDC/TMD） and Helkimo indexes（for functional evaluation of the masticatory system）, and quantitative sensory testing for sensory thresholds（gustative, olfactory, cold, warm, touch, vibration and superficial and deep pain） were evaluated before treatment, immediately after treatment, and 6 months after treatment. Results： The mean pain intensity by the Visual Analogue Scale only decreased in the acupuncture group at the last evaluation（P=0.012）. Patients in the sham-acupuncture group had an increase in carbamazepine doses according to the prescriptions（P〈0.01）. There was a reduction in secondary myofascial pain and mandibular limitations at the acupuncture and sham-acupuncture groups, however only the acupuncture group kept the changes after 6 months（P〈0.01, P=0.023）. There was a decrease in mechanical thresholds in the acupuncture group（tactile, P〈0.01; vibration, P=0.027） and an increase in deep pain thresholds in both acupuncture and sham-acupuncture groups（P=0.013）. Conclusion： Acupuncture can be an option in the treatment of ITN due to its analgesic effect in both ITN and secondary myofascial pain associated with it.

Although the number of hepatocellular carcinoma(HCC)patients is decreasing due to the development of hepatic anti-virus therapy,it remains a leading cause of cancer-related death *** recent development of systemic or locoregional therapy for advanced or unresectable HCC has enabled advanced HCC to be downstaged,and reports of conversion therapy have increased;however,many clinical and scientific subjects rely on the therapeutic strategies of advanced *** is a need to assess the evidence and develop clinical guidelines for conversion therapy for HCC;therefore,the Chinese Expert Consensus on Conversion Therapy for Hepatocellular Carcinoma(2021 Edition),consisting of 16 statements,has been developed for clinical practice(1).This consensus addresses treatment strategies for both technically and oncologically unresectable *** applaud Chinese establishment of this comprehensive and cutting-edge consensus on the treatment of advanced HCC.

对于客观参量EDT,或者更确切地说,对于稳态声场衰变曲线的初始斜率与包括所有简正模式在内的平均阻尼常数之间关系的研究工作,已有一些研究者做过,如H.库特鲁夫就曾指出此参量与所测试环境的几何细节有密切关系.因此,文中选择它作为分析工具,以一种较为深入的方式,通过对一个歌剧院的研究,来检验这种关系.通过对脉冲响应在频域上进行分析,这在前人的工作中也采用过.文中对EDT从倍频程以及1/3倍频程加以研究,并利用在Matlab环境下开发的一些工具来探讨乐池的简正模式与从频带中心频率导出的最大的时间值之间的相关关系.研究表明,在所考察的环境中简正模式与衰变曲线的初始斜率之间有良好的相关关系,与自相关系数(ACF)也有良好的相关关系;研究还证实,用于分析的参量本身既是时间的函数,也是脉冲响应频率变化的函数.

FoxP3^(+)regulatory T cells(Tregs)play crucial roles in peripheral immune *** addition,Tregs that reside or accumulate in nonlymphoid tissues,called tissue Tregs,exhibit tissue-specific functions and contribute to the maintenance of tissue homeostasis and *** an experimental mouse model of crescentic glomerulonephritis induced by an anti-glomerular basement membrane antibody,Tregs started to accumulate in the kidney on day 10 of disease onset and remained at high levels(~30–35%of CD4^(+)T cells)during the late stage(days 21–90),which correlated with stable disease *** depletion on day 21 resulted in the relapse of renal dysfunction and an increase in Th1 cells,suggesting that Tregs are essential for disease control during the convalescence *** Tregs that accumulated in the kidney showed tissue Treg phenotypes,including high expression of GATA3,ST2(the IL33 receptor subunit),amphiregulin(Areg),and PPARγ.Although T-bet^(+) Tregs and RORγt^(+) Tregs were observed in the kidney,GATA3^(+) Tregs were predominant during the convalescence stage,and a PPARγagonist enhanced the accumulation of GATA3^(+) Tregs in the *** understand the function of specific genes in kidney Tregs,we developed a novel T cell transfer system to T cell-deficient *** experiment demonstrates that ST2,Areg,and CCR4 in Tregs play important roles in the accumulation of GATA3^(+) Tregs in the kidney and in the amelioration of renal *** data suggest that GATA3 is important for the recruitment of Tregs into the kidney,which is necessary for convalescence after renal tissue destruction.

In Japan,viral hepatitis is the main cause of chronic liver diseases,including liver *** to the availability of highly effective antiviral drugs in combination with antihepatitis measures,Japan has become one of the most successful countries in the world with regard to hepatitis virus *** Japan,there are many elderly patients who have been infected with hepatitis C virus for a long time,and antihepatitis measures have been in place since the ***,the medical situation in Japan is expected to reflect the future situation in other countries where hepatitis countermeasures have been recently initiated and the infected population is *** nationwide survey clarified the changes in the etiologies of liver cirrhosis in *** viral hepatitis remains a major cause of liver cirrhosis in Japan,nonviral cirrhosis is suggested to increasingly contribute to the etiologies of liver cirrhosis,as opposed to viral hepatitis‐related liver *** changing etiology of liver cirrhosis in Japan may help to predict future global trends in chronic liver diseases.
