Statin use and cognitive function in middle-aged adults with type 1 diabetes

AIM to test associations between statin use and cognitive impairment in adults with childhood-onset type 1 diabetes(t1D).MEtHODS In 2010-13, n = 108 middle-aged participants from ongoing observational Pittsburgh Epidemiology of Diabetes Complications Study underwent neurocognitive assessment(mean age and t1 D duration of 49 and 41 years, respectively). All were diagnosed with childhoodonset(i.e., prior to age 18) t1 D between 1950 and 1980 and were seen within one year of diagnosis at Children's Hospital of Pittsburgh. Self-reported statin use(yes/no and if yes, name of statin) was collected biennially from parent study baseline(1986-1988) to time of neurocognitive testing. Logistic regression models tested associations between statin use groups and cognitive impairment(defined as having two or more cognitive test scores 1.5SD or worse than published norms) while linear regression models tested associations between statin use groups and cognitive domain z-scores(domains: Verbal IQ, memory, executive function, psychomotor speed, and visuo-construction). All models controlled for education and age. to address confounding by indication, models were repeated using a propensity score for statin *** Of the 108 participants, 51 reported never using statins. Median duration of statin use among the 57 ever users was 6 years. these 57 ever statin users were split to create two groups(≤ or > median years of statin use): 1-6 years(n = 25), and 7-12 years(n = 32). Compared with never users, using statins 1-6 years tripled the odds of cognitive impairment(OR = 3.16; 95%CI: 0.93-10.72; P = 0.06) and using statins 7-12 years almost quintupled the odds of cognitive impairment(OR = 4.84; 95%CI: 1.63-14.44; P = 0.005). Compared with never users, using statins 1-6 or 7-12 years was related to worse performance in the memory domain(β =-0.52; P = 0.003, and-0.39; P = 0.014, respectively). Evidence for colorectal sarcomatoid carcinoma arising from tubulovillous adenoma

Sarcomatoid carcinomas of the colorectum are rare tu- mors that display both malignant epithelial and stromal components. Clinically, they are aggressive tumors with early metastasis. Due to their infrequent occurrence, the pathogenesis is poorly understood. We report a case of a 52-year-old woman who presented with a rectal mass and intermittent hematochezia. Superficial biopsies during colonoscopy revealed a tubulovillous adenoma with high-grade dysplasia. Endoscopic ultra- sonography confirmed an invasive nature of the mass, and deeper biopsies revealed the presence of neoplasm with mixed histological components. the surgically- excised specimen demonstrated the presence of poorly differentiated spindle cells underneath the tubulovillous adenoma and an intermediate stage of invasive adeno- carcinoma. Based on the histological appearance and immunohistochemical studies, a diagnosis of sarcoma- toid carcinoma was made. Only nine cases of sarcoma- toid carcinomas of the colorectum have been reported to date. As a result, the terminology and pathogenesis of sarcomatoid carcinoma remain speculative. to the best of our knowledge, this is the first report of co- existence of sarcomatoid carcinoma and invasive ad- enocarcinoma with tubulovillous adenoma; all stages represented within the same tumor. this observation supports the "monoclonal theory" of pathogenesis with an adenoma-sarcoma progression with or without an intermediate stage of carcinoma.

Like mother, like daughter： heritability of female Richardson＇s ground squirrel Urocitellus richardsonii cortisol stress responses

Activation of the hypothalamic-pituitary-adrenal （HPA） axis liberates glucocorticoids, which provides an acute indication of an individual＇s response to stressors. the heritability of the stress response in wild mammals, however, remains poorly documented. We quantified the cortisol stress response of female Richardson＇s ground squirrels （RGSs） to handling and physical restraint, testing for：（1） the effects of individual age, time of day, and sample latency; （2） repeatability within individuals; （3） narrow-sense heritability; and （4） differences among individuals owing to potential genetic and/or environmental effects. We detected a positive linear relationship between baseline plasma cortisol （BL-cortisol） concentration and stress-induced plasma cortisol （Sl-cortisol） concentration that defined each individual＇s cortisol stress response. BL-cortisol, SI-cortisol, and stress response did not differ according to the time the sample was taken, or by subject age. Cortisol stress response was highly repeatable within individuals, had a mother-offspring heritability of h2 =0.40 ± 0.24 （mean ±SE）, full-sibling heritability of h2s = 0.37 ± 0.71, and half-sibling heritabil- ity of hHS^2 = 0.75 ± 1.41. Stress responses of sibling groups, immediate-family groups, and squir- rels within a given area did not differ, whereas those of individuals from more distantly related matrilines did. Our results highlight the natural variability in HPA axis reactivity among individuals by quantifying both BL- and SI-cortisol levels, demonstrate partial heritability of the stress response that is not attributable to environmental variation, and suggest that at least part of an indi- vidual＇s stress response can be accounted for by differences in matrilineal history.

Assessment of multi-modality evaluations of obscure gastrointestinal bleeding

AIMto determine the frequency of bleeding source detection in patients with obscure gastrointestinal bleeding (OGIB) who underwent double balloon enteroscopy (DBE) after pre-procedure imaging [multiphase computed tomography enterography (MPCtE), video capsule endoscopy (VCE), or both] and assess the impact of imaging on DBE diagnostic *** cohort study using a prospectively maintained database of all adult patients presenting with OGIB who underwent DBE from September 1st, 2002 to June 30th, 2013 at a single tertiary *** hundred and ninety five patients (52% females; median age 68 years) underwent DBE for OGIB. AVCE and/or MPCtE performed within 1 year prior to DBE (in 441 patients) increased the diagnostic yield of DBE (67.1% with preceding imaging vs 59.5% without). Using DBE as the gold standard, VCE and MPCtE had a diagnostic yield of 72.7% and 32.5% respectively. there were no increased odds of finding a bleeding site at DBE compared to VCE (OR = 1.3, P = 0.150). there were increased odds of finding a bleeding site at DBE compared to MPCtE (OR = 5.9, P tients with overt OGIB, diagnostic yield of DBE was not affected by preceding *** is a safe and well-tolerated procedure for the diagnosis and treatment of OGIB, with a diagnostic yield that may be increased after obtaining a preceding VCE or MPCtE. However, inpatients with active ongoing bleeding may benefit from proceeding directly to antegrade DBE.

Cardiac extracellular matrices(ECM)play crucial functional roles in cardiac *** studies have mainly focused on collagen,the major structural ECM in heart *** role of elastin in cardiac mechanics,however,is poorly *** this study,we investigated the spatial distribution and microstructural morphologies of cardiac elastin in porcine left *** demonstrated that the epicardial elastin network had location-and depth-dependency,and the overall epicardial elastin fiber mapping showed certain correlation with the helical heart muscle fiber *** compared to the epicardial layer,the endocardial layer was thicker and has a higher elastin-collagen ratio and a denser elastin fiber network;moreover,the endocardial elastin fibers were finer and more wavy than the epicardial elastin fibers,all suggesting various interface *** myocardial interstitial elastin fibers co-exist with the perimysial collagen to bind the cardiomyocyte bundles;some of the interstitial elastin fibers showed a locally aligned,hinge-like structure to connect the adjacent cardiomyocyte *** collagen-elastin combination reflects an optimal design in which the collagen provides mechanical strength and elastin fibers facilitate recoiling during ***,cardiac elastin fibers,along with collagen network,closely associated with the Purkinje cells,indicating that this ECM association could be essential in organizing cardiac Purkinje cells into“fibrous”and“branching”morphologies and serving as a protective feature when Purkinje fibers experience large deformations in *** short,our observations provide a structural basis for future in-depth biomechanical investigations and biomimicking of this long-overlooked cardiac ECM component.

Cancer patients are at increased risk for severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection and *** other viruses in the SARS family,SARS-CoV-2 employs two host proteins,angiotensin-converting enzyme 2(ACE2)and transmembrane serine protease 2(tMPRSS2),for viral entry.1 Recent studies showed that many of the host proteins identified as potential targets for developing COVID therapies are dysregulated in cancer,2 prompting us to investigate whether human cancer cells are susceptible to SARS-CoV-2 infection,and whether chemotherapy could modulate a cancer patient's risk for infection.
