Background:Tension pneumothorax is one of the leading causes of preventable death on the *** prehospital diagnosis relies on a subjective clinical impression complemented by a manual thoracic and respiratory *** techniques are not fully applicable in field conditions and on the battlefield,where situational and environmental factors may impair clinical *** aimed to assemble a device able to sample,analyze,and classify the unique acoustic signatures of pneumothorax and ***:Acoustic data was obtained with simultaneous use of two sensitive digital stethoscopes from the chest wall of an ex-vivo porcine *** second samples of acoustic data were obtained from the in-house assembled digital stethoscope system during mechanical *** thoracic cavity was injected with increasing volumes of 200,400,600,800,and 1000 ml of air or saline to simulate pneumothorax and hemothorax,*** data was analyzed using a multi-objective genetic algorithm that was used to develop an optimal mathematical detector through the process of artificial evolution,a cutting-edge approach in the artificial intelligence ***:The in-house assembled dual digital stethoscope system and developed genetic algorithm achieved an accuracy,sensitivity and specificity ranging from 64%to 100%,63%to 100%,and 63%to 100%,respectively,in classifying acoustic signal as associated with pneumothorax or hemothorax at fluid injection levels of 400 ml or more,and regardless of background ***:We present a novel,objective device for rapid diagnosis of potentially lethal thoracic *** further optimization,such a device could provide real-time detection and monitoring of pneumothorax and hemothorax in battlefield conditions.

The efficacy of vitamin E in preventing arthrofibrosis after joint replacement

Background:Arthrofibrosis is a joint disorder characterized by excessive scar formation in the joint *** E is an antioxidant with potential anti-fibroblastic *** aim of this study was to establish an arthrofibrosis rat model after joint replacement and assess the effects of vitamin E supplementation on joint ***:We simulated knee replacement in 16 male Sprague–Dawley *** immobilized the surgical leg with a suture in full *** control groups were killed at 2 and 12 weeks(n=5 per group),and the test group was supplemented daily with vitamin E(0.2 mg/mL)in their drinking water for 12 weeks(n=6).We performed histological staining to investigate the presence and severity of *** staining andα2-macroglobulin(α2M)enzyme-linked immunosorbent assay(ELISA)were used to assess local and systemic *** weight bearing(total internal reflection)and range of motion(ROM)were collected for functional ***:The ROM and weight-bearing symmetry decreased after the procedure and recovered slowly with still significant deficit at the end of the study for both *** analysis confirmed fibrosis in both lateral and posterior periarticular *** E supplementation showed a moderate anti-inflammatory effect on the local and systemic *** vitamin E group exhibited significant improvement in ROM and weight-bearing symmetry at day 84 compared to the control ***:This model is viable for simulating arthrofibrosis after joint *** E may benefit postsurgical arthrofibrosis,and further studies are needed for dosing requirements.

Preliminary Validation of Transcutaneous CO<sub>2</sub>Monitoring in Patients Undergoing Cardiac Ablation Using Jet Ventilation

Objectives: There is no data in the current medical literature on efficacy or accuracy of transcutaneous (tcPCO2) monitoring during jet ventilation for cardiac ablation. The use of tcPCO2 during cardiac ablation procedures offers the opportunity to compare end-tidal and transcutaneous methods of CO2 measurement before and after the use of the jet ventilation. Comparison of these measurements with arterial blood gas CO2 levels allows evaluation of the accuracy of the tcPCO2 technique for use during jet ventilation. Design: Observational study;patients served as their own controls. Setting: Cardiac electrophysiology laboratory. Participants: 15 adult patients (9 M), ASA III-IV, aged 26 to 82 years (median 66 years) undergoing radiofrequency ablation for atrial fibrillation. Interventions: Jet ventilation (JV) versus conventional ventilation. Measurements and Main Results: Paired measurements of end-tidal CO2 (EtCO2) and transcutaneous CO2 (tcPCO2) were recorded during periods of conventional ventilation. Paired measurements of arterial blood CO2 (PaCO2) levels and tcPCO2 were recorded during JV. ABG samples were drawn at the anesthesiologist’s discretion to assess the patient’s respiratory status. The level of agreement between the three methods was compared using the Bland Altman plot. We found that tcPCO2 values consistently provided a close approximation to PaCO2 levels. The mean difference between tcPCO2 and EtCO2 values in baseline and post-JV was on the order of 3 - 5 mmHg, with standard deviation of 4 - 6 mmHg. This is well within the range of variability that is accepted in clinical practice. Conclusions: These preliminary results suggest that tcPCO2 provides an acceptable estimate of CO2 concentration in arterial blood during JV, as well as prior to and following JV.

【目的】本研究旨在系统探究环境因素对邻苯二甲酸单乙基己基酯水解酶(monoethylhexyl phthalate hydrolase,MehpH)活性的影响,模拟该酶的3D结构并分析活性中心氨基酸残基与底物之间的相互作用。【方法】根据标准的酶学实验验证了环境因素对酶活性的影响。该酶的基本结构分析由DNAMAN(2.1)分析所得,同时利用SWISS-MODEL对其3D模型进行预测,并通过PyMOL软件对相关结果作了可视化处理。Autodock工具,Swiss-Pdb以及Py MOL均用以酶与邻苯二甲酸单乙基己基酯(monoethylhexyl phthalate,MEHP)的相互作用分析。【结果】该酶的初级结构与已报道Gordonia sp.P8219菌株中的酯类水解酶相似,该酶的最适温度与p H(分别为40°C和8.0)和菌株P8219有所不同;该酶在有机溶剂、洗涤剂和金属离子环境中表现出良好的稳定性;然而,其酶活受2 mol/L的Ni^(2+)、Fe^(3+)、Cu^(2+)和Zn^(2+)离子,1 mol/L苯甲基磺酰氟(PMSF),0.5 mol/L的对氧磷,1 mol/L苯基醚(PGO),2 mol/L焦碳酸二乙酯(DEPC)和5 mol/L的毒扁豆碱所抑制。丝氨酸水解酶中高度保守五肽基序GXSXG与催化三联体HSD结构均存在于该酶的编码序列中。根据分子对接结果,Thr152与Ser230在水解酶与其模板之间也显得更为保守,且与MEHP联系较为紧密(分别为5.8?和3.6?),可能起着重要的催化作用。而MEHP与催化氨基酸残基Ser125、His291、Asp259并不十分靠近。【结论】YC-RL2中MehpH在有机溶剂、洗涤剂以及金属离子环境中具有稳定的生物活性,表明其具有良好的应用潜力。酶的结构及活性中心得到了初步的分析,对于催化机理及酶改造研究具有重要意义。

盖斯凯尔夫人的小说《露丝》以疫病流行为历史背景,通过对"堕落"女性露丝生存境遇的描写,展现社区共同体的人际关系、道德信念与人性道义等问题。其疫病书写与斑疹伤寒叙事成为传递道德判断的媒介与机制,在客观上推动了社会公共卫生向前迈进。围绕疫病传播与消除过程,小说描绘了一个通过追溯斑疹伤寒疫病的传播而构成的看不见的社区网络,它超越了可见的地区甚至国家的界限,超越了亲属关系、社会关系甚至政治关系的范畴。在交通日渐便捷、信息不断流动的向工业化转型的时代,一个人与人紧密联系、共生共存的社区共同体显露出来,并促成人们的共同体意识从无知到认知和确立。

《回家》是2005年诺贝尔文学奖得主、英国当代剧作家哈罗德·品特自己认为最好的作品。"艺术来源于生活",在伦敦哈克尼出生长大的品特对哈克尼有着爱恨纠结的"哈克尼情结",《回家》就是一朵盛开在品特"哈克尼情结"沃土上的"恶之花"。对于剧中妻子露丝怎么会愿意抛夫弃子、而丈夫泰迪又怎么能够袖手旁观历来颇有争议。笔者以为泰迪和露丝都是品特坚守"哈克尼兄弟情"的"牺牲品",品特对露丝淑女和妓女双重性的刻画投射了品特戏里戏外对女性作为妻子与情人、淑女与妓女合二为一的理想的矛盾与纠结;泰迪和露丝病态的夫妻情背后折射出的是泰迪对兄弟情的变态愚忠,露丝的"愿意"和泰迪的"能够"都是品特借戏剧创作来"卡塔西斯"他的个人"哈克尼情结",也因此成就了他荒诞的"威胁喜剧"对社会的批判。
