Prevention of Postoperative Nausea and Vom.ting in Elec.ive Hysterec.om.: A Prospec.ive, Random.zed, Plac.bo c.ntrolled Outc.m.s Trial of Aprepitant NK-1-Rec.ptor Antagonist

Objec.ives: c.rrent prophylac.ic.interventions fail to c.m.letely elim.nate postoperative nausea and vom.ting (PONV) for a substantial num.er of patients. A new antiem.tic.(aprepitant) has been effec.ive in preventing c.em.therapy induc.d nausea and vom.ting (c.NV). We hypothesized that adding aprepitant to our c.rrent prophylac.ic.regim.n of dexam.thasone and ondansetron would reduc. the inc.denc. of PONV in our elec.ive hysterec.om. population. m.thods: 256 patients undergoing elec.ive hysterec.om. were enrolled in this prospec.ive, random.zed, double blinded, plac.bo c.ntrolled trial. Subjec.s rec.ived either oral aprepitant 40 m. or oral plac.bo 30 m.nutes prior to induc.ion of standardized anesthesia. The prim.ry outc.m. was vom.ting within the first 24 hours after surgery. Postoperative nausea, vom.ting, and use of resc.e antiem.tic. were doc.m.nted over a 24 h period. Additionally, adverse events, hospitalization days, and readm.ssions for PONV were c.m.ared. Results: There was a trend towards reduc.ion of postoperative nausea and vom.ting in the aprepitant group. Nausea and vom.ting were noted for 24% and 17% of wom.n in the aprepitant group versus 38% and 29% of wom.n in the Plac.bo group, respec.ively. Supplem.ntal antiem.tic.m.dic.tion was used by 42% of wom.n in the aprepitant group versus 60% of wom.n in the Plac.bo group. No adverse events were substantially m.re c.m.on in the aprepitant group than the Plac.bo group. c.nc.usions: Preem.tive use of aprepitant prior to elec.ive hysterec.om. m.y reduc. the inc.denc. of PONV and dim.nish the need for resc.e antiem.tic. postoperatively. Further studies with larger power are needed to c.nfirm.the trends observed in this study.

A 63-year-old m.n with diabetes and asym.tom.tic.c.ronary artery disease developed refrac.ory ventric.lar arrhythm.a at 20 hours at rest after his sec.nd c.VID-19 vac.ine. Despite signific.nt stenosis in the c.ronary arteries, there was no evidenc. of ac.te or old m.oc.rdial infarc.ion, heart failure, m.oc.rditis or struc.ural abnorm.lities on post-m.rtem.to ac.ount for the substrate for the fatal arrhythm.a. The refrac.ory and inc.ssant nature of the ventric.lar fibrillation and post-m.rtem.finding of a grossly elevated unexplained IgE level (in the absenc. of ac.te m.oc.rdial infarc.ion) suggested the possibility of Kounis Syndrom. or allergic.ac.te c.ronary syndrom..

Obtaining high prec.sion is an im.ortant c.nsideration for astrom.tric.studies using im.ges from.the Narrow Angle c.m.ra(NAc.of the c.ssini Im.ging Sc.enc. Subsystem.ISS).Selec.ing the best c.ntering algorithm.is key to enhanc.ng astrom.tric.*** this study,we c.m.ared the ac.urac. of five c.ntering algorithm.:Gaussian fitting,the m.dified m.m.nts m.thod,and three point-spread func.ion(PSF)fitting m.thods(effec.ive PSF(ePSF),PSFEx,and extended PSF(x PSF)from.the c.ssini Im.ging c.ntral Laboratory for Operations(c.c.OPS)).We assessed these algorithm. using 70 ISS NAc.star field im.ges taken with c.1 and c.2 filters ac.oss different stellar *** ePSF m.thod c.nsistently dem.nstrated the highest ac.urac.,ac.ieving prec.sion below 0.03 pixels for stars of m.gnitude *** to the previously c.nsidered best,the m.dified m.m.nts m.thod,the e PSF m.thod im.roved overall ac.urac. by about 10%and 21%in the sam.le and line direc.ions,***,the xPSF m.del provided by c.c.OPS had lower prec.sion than the ***,the ePSF exhibits an im.rovem.nt in m.asurem.nt prec.sion of 23%and 17%in the sam.le and line direc.ions,respec.ively,over the *** disc.epanc. m.ght be attributed to the xPSF foc.sing on photom.try rather than *** findings highlight the nec.ssity of c.nstruc.ing PSF m.dels spec.fic.lly tailored for astrom.tric.purposes in NAc.im.ges and provide guidanc. for enhanc.ng astrom.tric.m.asurem.nts using these ISS NAc.im.ges.

Bac.ground: Hem.pathies were rarely observed in m.jor sic.le c.ll disease patients som. thirty years ago, probably due to the high m.rtality rate am.ng the latter as a result of progressive c.m.lic.tions. Thanks to advanc.s in the m.nagem.nt of sic.le c.ll disease, patients' life expec.anc. has inc.eased c.nsiderably, exposing them.m.re frequently to neoplasia, inc.uding hem.tologic.l m.lignanc.es. The inc.eased risk of leukem.genesis is m.ltifac.orial and linked to the pathophysiologic.l m.c.anism. of the c.inic.l m.nifestations of sic.le c.ll disease. Study Setting: The c.inic.l haem.tology departm.nt of c.m.us teac.ing hospital and the paediatric.onc.-haem.tology unit of Sylvanus Olym.io teac.ing hospital in Lom. were used as study settings. Observations: Four hem.tologic.m.lignanc.es were c.llec.ed in a c.hort of 5847 m.jor sic.le c.ll syndrom.s. The m.dian age of the patients was 31.25 years (extrem.s: 14 and 58 years) and they were predom.nantly fem.le (sex ratio m.F = 0.25). Two were on bac.ground therapy with hydroxyurea. Am.ng the four patients, there were two c.ses of ac.te lym.hoc.tic.leukem.a, inc.uding ALL3 in a 58-year-old SS wom.n and T-ALL2 in a 12-year-old Sc. Then, a c.se of lym.hoc.tic.lym.hom. in a 20-year-old SS m.n was reported and finally a c.se of c.ronic.m.eloc.tic.leukem.a in a 33-year-old wom.n of Sβ+ thalassaem.a phenotype. c.nc.usion: To further report this c.existenc., it is therefore essential to system.tic.lly c.nsider hem.tologic.l m.lignanc.es during m.jor sic.le c.ll syndrom.s even if there are sim.larities in the sym.tom.tology of these two serious pathologic.l situations.
