Sir,Fingertip amputation injuries compromise the sensation,function,and appearance of the hand.They are often exquisitely painful and take a substantial amount of time to heal,causing significant functional disability.Conservative management of these injuries may result in decreased range of motion,hypo-or hyperesthesia,loss of pulp contour,scarring,and other long-term sequelae.[1,2]In addition,they may prolong patient discomfort with dressing changes,hindering adherence with wound therapy regimens,especially in the presence of multiple finger injuries.

Among the risk factors for late onset sporadic Alzheimer's disease(AD)is the expression ofε4 allele of apolipoprotein E(APOE4)gene(Mahley et al.,2006).Elevated amyloid processing and reduced degradation of Aβ,which lead to Ap plaque deposition,are evident in APOE4-positive AD patients and mice(Mahley et al.,2006).These features correlate with neuronal cell loss.Impaired mitochondrial activity and increased oxidative stress have long been recognized as additional hallmarks of AD pathology(Mahley et al.,2006).The effect of APOE4 expression on autophagy and mitochondrial dynamics and activity in astrocytes is discussed in this perspective and is summarized in Figure 1.

Objective: Unilateral mimicking bilateral benign paroxysmal positional vertigo (umb-BPPV) was attributed to inappropriate head positioning during testing of the posterior canal. Despite its inclusion in theDiagnostic criteria for the classification of vestibular disorders of the Barany Society, the clinical characteristics and treatment responsiveness of this BPPV subtype have not been intensively studied.Methods: Records of patients with BPPV seen at a single outpatient dizziness clinic during the years 2000e2020 were reviewed. Eighty seven patients with umb-BPPV and 86 random patients with posteriorcanal BPPV (p-BPPV) were retrieved. Their demographics and BPPV characteristics were analyzed.Results: Patients' and BPPV characteristics were similar in umb- and p-BPPV except for the prevalence ofmales in the umb-BPPV group. No differences were found between treatment responsiveness and recurrences in both groups. The recurrence rate of umb-BPPV was not influenced by age, gender, BPPV side,duration of symptoms or treatment responsiveness during the first attack.Conclusions: In accordance with our hypothesis about mixed canalo- and cupulolithiasis as the underlying mechanism of umb-BPPV, patients did not differ in characteristics and treatment responsivenessfrom p-BPPV patients. Recognition of umb-BPPV is important since inappropriate treatment can cause anunnecessary delay in therapy success.

Normal tissues are organized in a hierarchical model,whereas at the apex of these hierarchies reside stem cells (SCs) capable of self-renewal and of producing differentiated cellular progenies,leading to normal development and homeostasis.Alike,tumors are organized in a hierarchical manner,with cancer SCs residing at the apex,contributing to the development and nourishment of tumors .p53,the well-known 'guardian ofthe genome',possesses various roles in embryonic development as well as in adult SC life and serves as the ‘guardian of tissue hierarchy’.Moreover,p53 serves as a barrier for dedifferentiation and reprogramming by constraining the cells to a somatic state and preventing their conversion to SCs.On the contrary,the mutant forms of p53 that lost theirtumor suppressor activity and gain oncogenic functions serve as 'inducers oftissue anarchy' and promote cancer development.In this review,we discuss these two sides ofthe p53 token that sentence a tissue either to an ordered hierarchy and life or to anarchy and death.A better understanding of these processes may open new horizons for the development of new cancer therapies.

Antibodies against the toxin portion of recombinant immunotoxins （RIT） reduce their efficacy and pose a potential safety risk. To overcome this problem we mutated the very immunogenic immunotoxin SSIP to produce LMB-T20, a de-immunized RIT that has the eight human T-cell epitopes in SSIP modified or removed. To determine the effect of T-cell epitope removal in vivo we mapped the T-cell epitopes in immune-competent BALB/c mice and found that these mice recognize two epitopes. One corresponds to the human immunodominant T-cell epitope and the other to a human subdominant epitope; both were eliminated in LMB-T20. We found that mice immunized with LMB-T20 did not have T-cell activation and did not develop anti-drug antibodies （ADA）, whereas mice immunized with SSIP, showed T-cell activation, and developed ADA detected by both ELISA and drug neutralizing assays. The ability of the mice treated with LMB-T20 to respond to other antigens was not compromised. We conclude that elimination of T-cell epitopes is sufficient to prevent formation of antibodies to an immunogenic foreign protein.

糖皮质激素(glucocorticoid,GC)信号系统和激酶Kinome之间有着复杂的相互作用,其中,在GC处理后,由Kinome最终决定细胞的命运。当细胞表达足够水平的糖皮质激素受体(glucocorticoid receptor,GR)和Bim,以及有利于GSK3激活的Kinome时,就会继发细胞凋亡。通过防止Bim的上调和/或抑制GSK3的蛋白激酶,可在细胞内产生GC抵抗。通过抑制JNK、Src、PI3K、Akt或mTOR存活通路,可以克服T淋巴系统和B淋巴系统恶性肿瘤中的GC抵抗。最近已证明星状孢子素和雷帕霉素可以对GC诱导细胞凋亡相抵抗的恶性T淋巴细胞和B淋巴细胞加以有效致敏。这在理论上论证了通过改变细胞的Kinome提高GC疗效的可行性。

There is substantial public concern about the health risks of proximity to petrochemical industries.In the Haifa Bay Area(HBA),which contains Israel’s densest industrial area,these concerns have been strengthened by elevated cancer mortality rates since the late 1960s.We studied the association between adolescent exposure to industrial air pollution in the HBA and adult-onset cancer.This is a historical cohort study.The study population comprised 2,187,317 subjects,using the Israeli medical corps data linked to the Israel National Cancer Registry with follow-up of up to 45 years.Exposure assessments were estimated by a spatial kriging interpolation model of SO2,serving as a marker for the dispersion of air pollution emitted from the complex during the study period.We found increased crude(HR=1.23,95%CI=1.17 to 1.29)and adjusted(HR=1.16,95%CI=1.10 to 1.21)risk of cancer with increased exposure to air pollution in HBA.The associations remained robust in analyses stratified by decade and socio-economic status.We found evidence of monotonically increased risk in five of 13 cancer categories(leukemia,melanoma,female breast,central nervous system,and thyroid tumors).Our findings strengthen the hypothesis that this exposure posed a carcinogenic risk during the study period.
