Corneal biomechanical changes and intraocular pressure in patients with thyroid orbitopathy

AIM：To determine the relevance of the objective parameters addressing the altered biomechanical properties of cornea for glaucoma monitoring in patients with mild or moderate thyroid associated orbitopathy（TAO）,and in healthy individuals.· METHODS：Twenty-five patients with TAO（group 1）and 25 healthy adults（group 2） were included to the *** groups were of a similar age and the ratio women：*** each patient,the following parameters of both eyes were measured with ocular response analyzer（ORA）：corneal hysteresis（CH）,corneal resistance factor（CRF）,Goldmann correlated intraocular pressure（IOpg） and corneal compensated intraocular pressure（IOpcc）.In both groups participating in our study,all measurements were performed within minutes to reduce the diurnal effects.· RESULTS：The mean age in group 1 was 56±11y and76%were women,24%were *** mean age in group 2 was 64±11y and 68%were women,32%were *** correlated negatively with IOpg in group 1（r^2=0.10,p 〈0.05）.IOpg strongly correlated with IOpcc in both groups（group 1：r^2=0.79,p〈0.0001;group 2：r^2=0.85,p〈0.0001）.There was positive correlation between CRF and IOpg in group 1（r^2=0.12,p〈0.05） and in group 2（r^2=0.31,p〈0.0001）.Statistical analysis revealed no significant correlation between CRF and IOpcc in group 1（r^2=0.009,p 〉0.05） and also no significant correlation in group 2（r^2=0.04,p〉0.05）.CRF mean value in group 2（11.51±1.72 mm Hg） was higher than in group 1（10.85±1.45 mm Hg）（p〈0.05）.IOpg strongly correlated with IOpcc in both groups（group 1：r^2=0.79,p〈0.0001;group 2：r^2=0.85,p 〈0.0001）.There was also strong correlation between CRF and CH in both populations：group 1：（r^2=0.58,p〈0.0001）,group 2：（r^2=0.41,p 〈0.0001）.· CONCLUSION：Biomechanical parameters of cornea,as quantified by CH and CRF,and measured together with IOpcc,precisely reveal glaucoma staging in TAO and thus are reliable for diagnosing and follow-up in clinical practice. 〈/tr〉

MRI versus 64-row MDCT for diagnosis of hepatocellular carcinoma

AIM:To compare the diagnostic capability of multidetector computed tomography(MDCT) and magnetic resonance imaging(MRI) for the detection of hepatocellular carcinoma(HCC) tumour nodules and their effect on patient ***:A total of 28 patients(25 male,3 female,mean age 67±10.8 years) with biopsy-proven HCC were investigated with 64-row MDCT(slice 3 mm native,arterial and portal-venous phase,120 mL Iomeprol,4 mL/s,delay by bolus trigger) and MRI(T1fs fl2d TE/ TR 2.72/129 ms,T2tse TE/TR 102/4000 ms,5-phase dynamic contrast-enhanced T1fs fl3d TE/TR 1.56/4.6,Gadolinium-DTpA,slice 4 mm).Consensus reading of both modalities was used as *** nodules were analyzed with respect to number,size,and ***:In total,162 tumour nodules were detected by consensus *** detected signifi cantly more tumour nodules(159 vs 123,ppared to MDCT,with the best sensitivity for early arterial phase ***-negative CT findings included nodules ≤5 mm(n=5),≤10 mm(n=17),≤15 mm(n=12),≤20 mm(n=4),and 1 nodule >20 *** missed 2 nodules ≤10 mm and 1 nodule ≤15 *** MRI,nodule diameters were greater than on CT(29.2±25.1 mm,range 5-140 mm vs 24.1±22.7 mm,range 4-129 mm,ppatients,MDCT showed only unilobar tumour spread,whereas MRI revealed additional nodules in the contralateral *** of these nodules could have changed the therapeutic ***:Contrast-enhanced MRI is superior to 64-row MDCT for the detection of HCC *** should be allocated to interventional or operative treatment according to a dedicated MRI-protocol.

Intrahepatic cholangiocarcinoma: Introducing the preoperative prediction score based on preoperative imaging

Background: Intrahepatic cholangiocarcinoma(ICC) still has a poor long-term outcome, even after complete resection. We investigated different parameters gathered in preoperative imaging and analyzed their influence on resectability, recurrence, and survival. Methods: All patients who underwent exploration due to ICC between January 2008 and June 2018 were analyzed retrospectively. Kaplan-Meier model, log-rank test and Cox regression were used. Results: Out of 184 patients, 135(73.4%) underwent curative intended resection. Median overall survival(OS) was 22.2 months with a consecutive 1-, 3-and 5-year OS of 73%, 29%, and 17%. Median recurrencefree survival(RFS) was 9.3 months with a consecutive 1-, 3-and 5-year RFS of 36%, 15%, and 11%. Site of tumor, parenchymal localization, tumor configuration/dissemination, and estimated tumor volume had significant influence on resectability. Univariate analyses showed that site of tumor, tumor configuration/dissemination, number of nodules, and estimated tumor volume had predictive values for OS and RFS. Together with tumor size the preoperative prediction(pOp) score was created showing significance for OS and RFS(all ppOp score(HR=1.779;95% CI: 1.26 8-2.4 95;p=0.001), T stage(HR=1.255;95% CI: 1.040-1.514;p=0.018) and N stage(HR=1.334;95% CI: 1.081-1.645;p=0.007) were the independent predictors for OS. For RFS, pOp score(HR=1.733;95% CI: 1.30 0-2.311;pp=0.006) were the independent predictors. Conclusions: The pOp score showed to have a highly significant influence on OS and RFS. The score is easy to assess through preoperative imaging. For patients in the high risk group at least staging laparoscopy or preoperative chemotherapy should be evaluated, because they showed equal outcome compared to the irresectable group.

Diffusion-weighted 7.0T Magnetic Resonance Imaging in Assessment of Intervertebral Disc Degeneration in Rats

Background： lntervertebral disc degeneration （IDD） is a major cause of disc protrusion, likely to be associated with decrease of water content. This research aimed to evaluate IDD by diffusion-weighted imaging （DWI） with a 7.0 Tesla iT） magnetic resonance imaging （M RI） machine. Methods： A total of 24 healthy Sprague-Dawley rats were randomly selected and divided into four groups （A, B, C, and D）, each consisting of 3 male and 3 female rats （28, 42, 56, and 70 days old, respectively）. All the rats were imaged with a 7.0T MRI, producing T2WI, T1WI, and functional DWI sequences. Data were collected and apparent diffusion coefficient （ADC） charts were constructed. Nucleus pulposus and annulus fibrosus regions were identified, several regions of interest were chosen, and their ADC values were obtained. After imaging, rats were sacrificed and their intervertebral discs （ L I-L6） were dissected, yielding a total of 144 discs. protein was extracted for the purpose of Western blotting. Comparison among multiple samples used one-way analysis of variance and least significant difference methods. Results： 7.0T MRI revealed evident decrease in signal intensity within intervertebral discs of Sprague-Dawley rats with age. lntervertebral disc A DC values significantly decreased from Group A （0.00154 ± 0.00008） to Group D （0.00107 ± 0.00007： p 〈 0.01 ）; nucleus pulposus ADC values significantly decreased from Group A （0.00164± 0.00005） to Group D （0.00140± 0.00007; p 〈 0.01 ） and annulus fibrosus ADC values significantly decreased from Group A （0.00129 ± 0.00014） to Group D （0.00082 ± 0.00012; p 〈 0.01）, Meanwhile, it also revealed evident decrease from high spinal level to low spinal level： nucleus pulposus ADC values in GroupA significantly decreased from L1/L2 （0.00163 ± 0.00006） to L6/S1 （0.00139 ± 0.00004, p 〈 0.01 ）. Studies of ultrasonic dehydration efficiency

The aim of this investigation was to define the effectiveness of non-contact drying using ultrasonic vibrations. Disk radiators were used for carrying out experiments, and a special drying chamber was designed to provide resonant amplification of ultrasonic vibrations (from 130 to 150 dB). Drying of ginseng and other vegetables demonstrated that the application of ultrasonic vibrations reduced power inputs by 20% in comparison with convective drying. It also led to a decrease of 6% in final moisture content, if the duration of drying was constant. The level of intensification of ultrasonic drying was high (up to 50 g for 1 kg of drying material), which helped to lower the temperature of the drying agent and improve the quality of the dried products.

Step-by-step external fixation of unstable pelvis with separate anterior and posterior modules

Objective: To evaluate the treatment outcomes of patients with pelvic ring injury by applying step-by-step external pelvic fixation and circular external fixation ***: A total of 28 patients suffering from disintegrated pelvic ring injury are involved in the study. Fourteen patients(the treatment group) underwent step-by-step external pelvic fixation by applying anterior(anti-shock) and posterior modules. For the rest 14 patients(the experimental group), the osteosynthesis were conducted by means of a circular external fixation device. The long-term outcomes were evaluated in a year after the ***: The residual deformity of 5(4–7) mm was observed in 10 patients(71.4%) from the experimental group. In the treatment group, the residual deformity was evident only in 4(28.6%) cases being 2.5(2–3) mm(p = 0.000 319) on the average. The functional result(according to the Majeed scale) was statistically better in the treatment group(p = 0.000 319). Nine(64.3%) and five(35.7%) patients in treatment group showed excellent and positive results, respectively. The excellent result was demonstrated by 3patients(21.4%) of the experimental group, the positive outcomes were observed in 6 cases(42.9%) and the unsatisfactory one was displayed by 1 patient(7.1%) of the same ***: The modular approach applied is the advantage of the transosseous osteosynthesis allowing for a separate anterior(anti-shock) fixation and final posterior reposition of the pelvic ring preceded by the stabilization of vital functions. The above mentioned method gives an opportunity to increase the amount of techniques applied for the pelvic external fixation in polytrauma cases.
