Vitamin D and Parathyroid Hormone Profiles in Living Kidney Failure Patients in Côte d’Ivoire

Introduction: Abnormalities in mineral and bone metabolism, particularly phosphocalcic metabolism, are common in renal failure and are associated with a significant morbidity and mortality. the regulation of phosphocalcic metabolism is subject to a particularly precise and complex control of parathormone (PtH) and vitamin D. Assessment of vitamin D and parathyroid hormone concentrations would help to improve the medical management of patients with chronic kidney disease and ensure a better quality of life. Methods: the study population consisted of 138 individuals including 46 non- dialysis renal failure patients, 46 chronic hemodialysis patients and 46 non- renal failure volunteers to serve as controls. Serum Parathyroid hormone and Vitamin D concentrations were measured using the Vidas automated system. Results: 25-hydroxyvitamin D concentrations in controls (65 ± 2.41 nmol/L) and dialysis patients (70 ± 3.03 nmol/L) were significantly higher than those in CKD patients (48 ± 3.34 nmol/L). On the other hand, the mean values of Parathyroid hormone in dialysis patients (312 ± 36.22 pg/mL) and CKD patients (117 ± 10.68 pg/mL) were very high compared to that in controls (25 ± 2.34 pg/mL). Conclusion: Secondary hyperparathyroidism is common in renal failure. Parathyroid hormone and 25-hydroxyvitamin D assays would be adequate for better management of chronic renal failure.

AIM to assess the relationship between the presence of toll-like receptor 4(tLR4) polymorphisms and bacterial infections in cirrhotic patients with ascites. MEtHODS We prospectively included consecutive patients with cirrhosis and ascites hospitalized during a 6-year period. Patients with human immunodeficiency virus(HIV) infection or any other immunodeficiency, patients with advanced hepatocellular carcinoma(beyond Milan's criteria) or any other condition determining poor short-term prognosis, and patients with a permanent urinary catheter were excluded. the presence of D299 G and/or t399 I tLR4 polymorphisms was determined by sequencing and related to the incidence and probability of bacterial infections, other complications of cirrhosis, hepatocellular carcinoma, and mortality during follow-up. A multivariate analysis to identify predictive variables of mortality in the whole series was performed. RESULtS We included 258 patients: 28(10.8%) were carriers of D299G and/or t399I tLR4 polymorphisms(polymorphism group) and 230 patients were not(wildtype group). the probability of developing any bacterial infection at one-year follow-up was 78% in the polymorphism group and 69% in the wild-type group(P = 0.54). the one-year probability of presenting infections caused by gram-negative bacilli(51% vs 44%, P = 0.68), infections caused by gram-positive cocci(49% vs 40%, P = 0.53), and spontaneous bacterial peritonitis(29% vs 34%, respectively, P = 0.99) did not differ between the two groups. the oneyear probability of transplant-free survival was 55% in the polymorphism group and 66% in the wild-type group(P = 0.15). Multivariate analysis confirmed that age, Child-Pugh score, active alcohol intake, previous hepatic encephalopathy, hepatocellular carcinoma and serum creatinine were associated with a higher risk of death during follow-up. CONCLUSION Genetic polymorphisms D299 G and/or t399 I of tLR4 do not seem to play a relevant role in the predisposition of cirrhotic patie

Glucose and Insulin Levels are Increased in Obese and Overweight Mothers’ Breast-Milk

Objective: to analyze the relationships between mothers’ pre-pregnancy body mass index (BMI) and levels of glucose and insulin in breast-milk. Methods: Participants were 32 breast-feeding mothers who were assigned to either normal weight (group 1: n = 21, BMI between 18.5 and 24.9 kg/m2) or overweight/obese (group 2: n = 11, BMI ≥ 25 kg/m2) based on their pre-pregnancy BMI. Breast-milk samples were collected at six weeks postpartum. Glucose and insulin concentrations were measured in the breast-milk samples. to control for any extreme dietary influences on breast-milk glucose concentrations, mothers completed 24-hour food recalls. Results: Mean milk-glucose values of groups 1 and 2 were 32.6 ± 21.8 mg/dL and 51.9 ± 20.5 mg/dL respectively. Milk-borne insulin values were 4.5 ± 7.6 μIU/mL and 30.1 ± 56.3 μ IU/mL, respectively. Independent-samples t tests found that glucose levels of group 2 were greater than those of group 1, t(30) = 2.43, p = 0.02;and milk-borne insulin was greater in group 2 than in group 1, t(30) = 2.075, p = 0.05. Mothers’ pre-pregnancy BMI was found correlated with both breast-milk glucose (r = 0.483, p = 0.005) and insulin (r = 0.565, p = 0.001). Conclusion: this study found that in comparison with normal weight mothers, over-weight/ obese mothers have higher concentrations of glucose and insulin in their breast-milk. Further studies are needed to explain the underlying mechanisms, and possible consequences of infant exposure to higher levels of glucose and insulin in breast-milk.

A new numerical approach based on a multiblock, multigrid, local refinement method has been developed. the multiblock structure makes grid generation for complex geometries easier,multigrid techniques significantly accelerate the rate of convergence and the local refinement method provides high spatial resolution of boundary lay6r and separated vortical flows with much reduced computer memory and CPU time. Sample three-dimensional flow computations for complex geometries are presented to illustrate the advantages in term of memory savings and reductions of CPU time in obtaining high spatial resolution solutions. High spatial resolution in the inner viscous layer is achieved by using the values of y+ for the first grid centers rom the wall in the order of 1 to 7. the required solution resolution in the dominantly viscous flow region (boundary layer, vortex core, or three-dimensional separated zone) dictates the acceptable spacing of grid cells in that region. It is very desirable to be able to reduce grid spacing locally in the critical region without changing the overall grid. the local refinement technique doubles grid numbers in all three directions for every level of refinement. Multiple levels of local grid refinement with moderate grid stretching can be used to provide fine grid spacing in rapidly changing flow regions such as near the wall, vortex core and separated shear layer. thus, the method provides the required fine spatial resolution in the dominantly viscous region which is relatively small compared to the entire computational domain. this also avoids the used of excessively large aspect ratios of the grid cells near the rapidly changing now regions thus reduces truncation error and improves robustness. Furthermore, the results of numerical experimentation indicate that the present multigrid local refinement technique provides very effective numerical communications across the fine and coarse grid interfaces because the finer mesh is embedde

We report the preparation of poly(3,4-ethylene dioxythiophene)(PEDOt)-modified polyvinylidene fluoride electrospun fibers and their use as a novel adsorbent material for the removal of the anionic dye Methyl Orange(MO)from aqueous *** novel adsorbent material can be used to selectively remove MO on a wide p H range(3.0-10.0),with a maximum capacity of 143.8 mg/g at p H *** used in a recirculating filtration system,the maximum absorption capacity was reached in a shorter time(20 min)than that observed for batch mode experiments(360 min).Based on the analyses of the kinetics and adsorption isotherm data,one can conclude that the predominant mechanism of interaction between the membrane and the dissolved dye molecules is ***,considering the estimated values for the Gibbs energy,and entropy and enthalpy changes,it was established that the adsorption process is spontaneous and occurs in an endothermic *** good mechanical and environmental stability of these membranes allowed their use in at least 20 consecutive adsorption/desorption cycles,without significant loss of their *** suggest that the physical-chemical characteristics of PEDOt make these hybrid mats a promising adsorbent material for use in water remediation protocols and effluent treatment systems.

the soils of Benin in general and those of the department of Zou, in particular, are highly degraded. this study aimed to evaluate the effectiveness of sustainable land management practices on soil erodibility in two villages in the Plateau of Abomey. Soil samples were collected on plots under Sustainable Land Management (SLM) measures (direct seeding, maize residue management and soybean-cereal rotation) and on their adjacent control. the soil samples were prepared and analyzed in laboratory to determine variables such as soil permeability, organic matter content, and particle size. Soil erodibility was determined as proposed by Wischmeier & Smith. the effect of SLM practices was significant (0.02) on soil permeability. On plots under SLM measurements, soil permeability is higher with an average of 93.97 mm/h at Folly and 82.43 mm/h at Hanagbo. SLM measurements significantly (0.04) added organic matter to the soil. the average organic matter of the plots under SLM measures in Folly varies from 0.73% to 1.39% while it varies from 0.49% to 0.73% in the control plots. In Hanagbo, the average organic matter of the plots under SLM measures varies from 1.86% to 2.48% against 1.41% to 1.66% for the control plots. Regarding soil erodibility, it was found that the influence of SLM measures is significant in both villages. In villages, direct seeding and maize residue management significantly (0.008) reduced soil erodibility compared to their adjacent controls, while the soybean-cereal rotation measure increased soil erodibility compared to plot witnesses. the average soil erodibility of plots under SLM measures varies by 0.21 t⋅h/Mj⋅mm at 0.38 t⋅h/Mj⋅mm in the village of Hanagbo and 0.25 t⋅h/Mj⋅mm at 0.38 t⋅h/Mj⋅mm in the village of Folly. It varies from 0.24 t⋅h/Mj⋅mm at 0.28 t⋅h/Mj⋅mm for the control plots at Hanagbo and 0.31 t⋅h/Mj⋅mm at 0.37 t⋅h/Mj⋅mm in Folly. these practices can therefore

Surveillance of Surgical Site Infections: A Public Health Emergency in a Regional Hospital of Northern Benin. A Prospective Observational Pilot Study

Background: Surgical site infections (SSIs) are considered as result of the healthcare quality in hospitals. Objective: to study SSI at Saint Jean de Dieu Hospital tanguieta (SJDHt), prior to the implementation of a permanent monitoring system. Method: transversal, and descriptive study with prospective data collection was performed from 1 July to 31 janvier 2017 in the department of general surgery of SJDHt. the hospital lacks in a microbiology unit. All patients who underwent surgery during this period were included and the monitoring lasted one month. SSIs diagnostic was carried out according to WHO criteria as described in the Practical Guide for the Prevention of Nosocomial Infections published in 2002. Statistical tests (χ-square and Student’s t-test) were applied and p 0.05 were statistically significant. Results: Of 343 patients recorded, 105 (30.6%) had SSI. their age averaged 40.3 years and the sex-ratio (men/women) was 2.8. the emergency surgery resulted in a 50.0% rate of SSI (p = 0.00). the SSI rate for clean and clean-contaminated surgery was 6.3% against 94.6% for infected surgery (p = 0.00). the SSI rates were 100% and 66.7% for NNISS = 2 and NNISS = 1 (p = 0.00), respectively. Superficial SSI rate was 13.3%, while deep SSI and organ/space SSI were 46.7% and 40%, respectively. the hospital stay of patients with SSI was three times longer than the length of patients without SSI (p = 0.00). Conclusion: SSIs are real burden at SJDHt. Appropriate measures must be adopted to reduce its prevalence.
