One-year survival in recipients older than 50 bridged to heart transplant with Impella 5.5 via axillary approach

BACKGROUND Optimizing patients with advanced heart failure before orthotopic heart transplantation(OHT), especially in patients greater than 50 years old, is imperative to achieving successful post-transplant outcomes. Complications are well-described for patients bridged to transplant(BTT) with durable left ventricular assist device(LVAD) support. Given the lack of data available in older recipients after the recent increase in mechanical support use, we felt it crucial to report our center’s one-year outcomes in older recipients after heart transplantation with percutaneously placed Impella 5.5 as a *** Forty-nine OHT patients were supported with the Impella 5.5 intended as a bridge between December 2019 and October 2022 at mayo Clinic in Florida. Data were extracted from the electronic health record at baseline and during their transplant episode of care after Institutional Review Boards approval as exempt for retrospective data *** Thirty-eight patients aged 50 or older were supported with Impella 5.5 as BTT. Ten patients underwent heart and kidney transplantation within this cohort. The median age at OHT was 63(58–68) years, with 32 male(84%) and six female patients(16%). Etiology was divided into ischemic(63%) and non-ischemic cardiomyopathy(37%). The baseline median ejection fraction was 19%(15–24). most patients were in blood group O(60%), and 50% were diabetic. The average duration of support was 27 days(range 6–94). The median duration of follow-up is 488 days(185–693). For patients that have reached the 1-year follow-up timeframe(22 of 38, 58%), the 1-year post-transplant survival is 95%.CONCLUSION Our single-center data provides awareness for using the Impella 5.5 percutaneously placed axillary support device in older heart failure patients in cardiogenic shock as a bridge to transplantation. One-year survival outcomes after heart transplantation are excellent despite the older recipient’s age and prolonged pre-transplant support.

Cosmological parameters for spatially flat dust filled Universe in Brans-Dicke theory

We have investigated late time acceleration for a spatially fiat dust filled Universe in Brans- Dicke theory in the presence of a positive cosmological constant A. Expressions for Hubble＇s constant, luminosity distance and apparent magnitude have been obtained for our model. The theoretical results are compared with observed values of the latest 287 high redshift （0.3 ≤ z ≤1.4） Type Ia supernova data taken from the Union 2.1 compilation to estimate present values of matter and dark energy parame- ters, （Ωm）0 and （ΩA）0. We have also estimated the present value of Hubble＇s constant H0 in light of an updated sample of Hubble parameter measurements including 19 independent data points. The results are found to be in good agreement with recent astrophysical observations. We also calculated various physical parameters such as matter and dark energy densities, present age of the Universe and decelera- tion parameter. The value for Brans-Dicke-coupling constant ω is set to be 40 000 based on accuracy of solar system tests and recent experimental evidence.

Autoheated thermophilic aerobic sludge digestion and metal bioleaching in a two-stage reactor system

A two-stage process has been developed for stabilization of sludge and removal of heavy metals from the secondary activated sludge with high rate of energy and time conservation. The first stage of the process involves autoheated thermophilic aerobic digestion at 55-60℃ inoculated with less-acidophilic thermophilic sulfur-oxidizing microorganisms （ATAD）. The results show that it is possible to maintain the autoheated conditions （55-60℃） in the ATAD reactor up to 24 hr, leading to reduction of 21% total solids （TS）, 27% volatile solids （VS）, 27% suspended solids （SS） and 33% volatile suspended solids （VSS） from the sludge. The sludge pH also decreased from 7 to 4.6 due to the activity of less-acidophilic thermophilic microorganisms. In the second stage operation, the digested sludge （pH 4.6, TS 31.6 g/L） from stage one was subjected to bioleaching in a continuous stirred tank reactor, operated at mean hydraulic retention times （HRTs） of 12, 24 and 36 hr at 30℃. An HRT of 24 hr was found to be sufficient for removal of 70% Cu, 70% mn, 75% Ni, and 80% Zn from the sludge. In all, 39% VSS, 76% Cu, 78.2% mn, 79.5% Ni and 84.2% Zn were removed from the sludge in both the stages.

Effect of nitrogen doping on structural and optical properties of ZnO nanoparticles

Infl uence of nitrogen doping on structural and optical properties of ZnO nanoparticles has been studied. Undoped and N doped ZnO nanoparticles were synthesized via chemical precipitation approach. The prepared samples were characterized through X-ray diffraction(XRD),Transmission electron microscopy(TEm) equipped with Energy dispersive X-ray(EDAX) spectroscopy, UV–visible spectroscopy, Fourier transform infrared(FTIR) spectroscopy and micro-Raman spectroscopy(m RS). Wurtzite phase of undoped as well as 0.5–10% N doped ZnO nanoparticles was con fi rmed through characteristic XRD patterns. The particle size expansion due to N incorporation in ZnO was further revealed by TEm and EDAX analysis where 11 nm size undoped and 18–22 nm size 0.5–10% N doped ZnO(N:ZnO) nanoparticles without any impurity were ascertained. Slight blue-shift in band gap energy, as observed in our case, symbolized weak quantum con fi nement of the prepared nanoparticles. The alterations in vibrational modes of ZnO due to N incorporation, remarkably H substituting at O site and subsequently causing the passivation in N:ZnO nanoparticles, were detected through FTIR analysis. Finally, the effect of the nano-size of crystallite and gradual prominence of N into ZnO lattice due to increase of N doping concentration in prepared nanoparticles was meticulously expatiated though m RS analysis.
