Effective trea.ment of sepsis not only dema.ds prompt a.ministra.ion of a.propria.e a.timicrobia.s but a.so requires precise dosing to enha.ce the likelihood of pa.ient surviva.. a.equa.e dosing refers to the a.ministra.ion of doses tha. yield thera.eutic drug concentra.ions a. the infection site. This ensures a.fa.ora.le clinica. a.d microbiologica. response while a.oiding a.tibiotic-rela.ed toxicity. Thera.eutic drug monitoring (TDM) is the recommended a.proa.h for a.ta.ning these goa.s. However, TDM is not universa.ly a.a.la.le in a.l intensive ca.e units (ICUs) a.d for a.l a.timicrobia. a.ents. In the a.sence of TDM, hea.thca.e pra.titioners need to rely on severa. fa.tors to ma.e informed dosing decisions. These include the pa.ient's clinica. condition, ca.sa.ive pa.hogen, impa.t of orga. dysfunction (requiring extra.orporea. thera.ies), a.d physicochemica. properties of the a.timicrobia.s. In this context, the pha.ma.okinetics of a.timicrobia.s va.y considera.ly between different critica.ly ill pa.ients a.d within the sa.e pa.ient over the course of ICU sta.. This va.ia.ility underscores the need for individua.ized dosing. This review a.med to describe the ma.n pa.hophysiologica. cha.ges observed in critica.ly ill pa.ients a.d their impa.t on a.timicrobia. drug dosing decisions. It a.so a.med to provide essentia. pra.tica. recommenda.ions tha. ma. a.d clinicia.s in optimizing a.timicrobia. thera.y a.ong critica.ly ill pa.ients.
