Purpose: In acute lym.hoblastic leukem.a (ALL), m.ltidrug resistance is often m.diated by AT- Pase Binding Cassette (ABC) proteins, which principally involve ABCC1 (m.ltidrug resistance protein 1, m.P1) and ABCB1 (m.ltidrug resistance 1, m.R1). However, direct com.arisons between the differential effects of ABCC1 and ABCB1 have been difficult, since identical cell lines with differential expression of these transporters have not been developed. Experim.ntal Design: In this study, we developed and com.ared the biological profiles of Jurkat cell lines that selectively over-expressed ABCC1 and ABCB1. Vincristine (VCR) plays an im.ortant role in the treatm.nt of T-lineage ALL (T-ALL), and is often the first drug given to newly-diagnosed patients. Because of its im.ortance in treatm.nt, we provide descalating, sub-lethal doses of VCR to Jurkat cells, and extended our observations to expression profiling of newly diagnosed patients with T-ALL. Results: We found that VCR-resistant cells over-expressed ABCC1 nearly 30-fold. The calcein Am.assay confirm.d that VCR-resistant cells actively extruded VCR, and that ABCC1-m.diated drug resistance conferred a different spectrum.of m.ltidrug resistance than other T-ALL induction agents. siRNA experim.nts that blocked ABCC1 export confirm.d that VCR resistance could be reversed in vitro. Analyses of T-lym.hoblasts obtained from.100 newly diagnosed T-ALL patients treated on Children’s Oncology Group Phase III studies 9404 and AALL0434 that induction failure could be could be partially explained by the over-expression of ABCC1 and ABCB1. Conclusions: Taken together, these results suggest that over-expression of ABC transporters plays a contributing role in m.diating treatm.nt failure in T-ALL, and underscore the need to em.loy alternate treatm.nt approaches in patients for whom.induction failed or for those with relapsed disease.

Degradation of Indigo Carm.ne Using Advanced Oxidation Processes: Synergy Effects and Toxicological Study

The physicochem.cal degradation of Indigo Carm.ne (IC) dye in aqueous solution was perform.d using single and com.ined Advanced Oxidation Processes (AOP’s). Photocatalysis (TiO2-UV), Ozonation (O3) and Sonolysis (SN) were tested either in a standalone m.thodology or by com.ination of two sim.ltaneous AOP’s. The dye conversion was followed by both m.asurem.nts: 1) color rem.val determ.ned by UV-VIS spectrom.try and 2) organic and inorganic load determ.ned by the chem.cal oxygen dem.nd (COD). A com.lete and quick color disappearance of m.del water waste has been obtained by using com.ination of non-irradiated AOP’s, nam.ly, O3/SN, which contrasts to the com.ination of irradiated photocatalysis with O3 or sonolysis. Color rem.val with sim.ltaneous TiO2-UV/SN reached 77% while TiO2-UV/O3 reached 96% at sim.lar reaction tim.. On the other hand, the standalone O3 yielded the highest color rem.val (94.4%) in 32 m.nutes whereas SN reached only 39.2% in 4 hours. The standalone light irradiated TiO2-UV reached 93.3% color re-m.val in one hour of reaction tim.. These results indicated that non-irradiated (SN and O3) enhance synergistic effects that provoke structural changes in dye m.lecule without reaching total degradation. This is evidenced from.FTIR of residuals from.reaction m.xture in which it has been observed the presence of organic m.lecules such as arom.tics, sulfonic and am.nes refractory com.ounds that are m.chanisti-cally possible to be found during IC degradation. Also, toxicity tests (m.croTox? Technique) were perform.d using com.ercially available bacteria culture before and after IC degradation for each AOP and their com.ination. Reduction of aqueous dye concentration decreased the level of toxicity of the treated water which is the m.in target of the AOP’s but the presence of the rem.ining recalcitrant com.ounds have also toxic effect.

We report a kindred with autosom.l recessive interleukin- 1 receptor- associated kinase 4 (IRAK- 4) deficiency in 3 fourthdegree relatives. A diagnosis of IRAK- 4 deficiency should be considered in fam.lies with invasive bacterial disease, even if the individuals affected are only distantly related, which falsely suggests m.ltigenic or dom.nant inheritance with low penetrance.

Purpose: To com.are outcom.s of m.croincision cataract surgery (m.CS) with coaxial phacoem.lsification. Design: Prospective random.zed consecutive case series. Participants: One hundred eyes of 50 patients with nuclear or corticonuclear cataract (grades 2+to 4) with Lens Opacities Classification System.III. m.thods: One hundred eyes (50 patients) were random.y operated through clear corneal incisions using 2 techniques: coaxial phacoem.lsification (50 eyes) and m.croincision cataract surgery (50 eyes). m.in Outcom. m.asures: m.an phacoem.lsification tim., total phacoem.lsification percent, effective phacoem.lsification tim. (EPT) (calculated by m.ltiplying total phacoem.lsification tim. in seconds by the average power percent used), intraoperative total balanced salt solution (BSS) volum., visual outcom., vectorial astigm.tic changes, corneal thickness, endothelial cell count, and anterior cham.er flare and cells preoperatively and at 1 day, 1 m.nth, and 3m.nths. Results: Statistically significant differences were found between m.CS and coaxial phacoem.lsification regarding m.an incision size, m.an total phacoem.lsification percent, and EPT. There were no significant differences between the techniques regarding the m.an percent of endothelial cell loss, anterior cham.er cell count and flare, m.an phacoem.lsification tim., pachym.tric m.asures or total BSS volum. utilized, or visual outcom.. The vectorial astigm.tic changes in the m.CS group showed a change of ≤0.25 diopters (D) in 35%of the eyes, 0.25 to 0.5 D in 50%of the eyes, and 0.5 to 1.0 D in 15%of the eyes. These changes were induced by the surgery. Vectorial astigm.tic changes of >1 D were not observed. In the coaxial phacoem.lsification group, vectorial astigm.tic changes of 1.0 D. m.an vectorial astigm.tic changes were 0.36±0.23

Age is a risk factor in acute lower gastrointestinal hem.rrhages (LGIH). The objectives here were to analyze: (1) diagnostic and therapeutic handling, (2) related m.rbidity and m.rtality, (3) the indications for surgery, and (4) the evolution of acute LGIH in patients ≥80 years. Forty-three patients ≥80 years with acute LGIH were reviewed retrospectively. In 86%(n = 37) related co-m.rbidities were found, in 9%(n = 4) there had been prior colorectal surgery, 19%(n = 8) were antiaggregated, and 7%(n = 3) were anticoagulated. One hundred thirty-two cases of acute LGIH in patients m.rgency ward 11 cases (26%) had hem.dynam.c instability and 8 of these were stabilized using conservative m.asures. In 39 cases an endoscopy was perform.d, allowing for an etiological diagnosis in 59%(n = 23) of cases, above all in those carried out in an urgent or sem.urgent way. The arteriography perm.tted an etiological diagnosis in two of the four cases in which it was carried out. In seven patients (16%) urgent surgery was indicated: three were hem.rrhoidectom.es, three were subtotal colectom.es, and one was a resection of the sm.ll intestine. The m.rbidity rate was 10%(n = 4) in the patients who were not treated and 14%(n = 1) in those treated, with a m.rtality rate of 8%(n = 3) and 14%(n = 1), respectively. The rate of relapse of bleeding after discharge from.hospital was 42%(n = 18), with nine of these needing to be readm.tted into hospital. In com.arison with the control group, they present a different bleeding etiology (diverticulosis as opposed to the benign anal-rectal and sm.ll intestinal pathology in the younger population; P = 0.017), surgery is indicated with less frequency (9 versus 33%; P = 0.007), and there is a higher relapse rate (42 versus 26%; P = 0.045). Background/Purpose: Severe gastrointestinal dysm.tility (GID) im.airs patients’ quality of life and is alm.st uniform.y fatal after com.lications of parenteral nutrition. Intestinal and m.ltivisceral transplants have been used as alternative treatm.nt of these disorders. We studied patients with GID treated with transplantation in our center, and reviewed their outcom. to determ.ne the therapeutic efficacy of m.ltivisceral transplants. m.thods: The transplant database was searched for patients with GID from.1994 to 2001. We excluded patients with Hirschsprung disease, scleroderm., and diabetic enteropathy. We reviewed explanted organs, histochem.stry, and im.unohistochem.stry and classified cases by etiology. Results: We selected 12 children with GID from.124 patients transplanted. Nine presented before 1 year and 3 started with sym.tom. between 2 and 8 years. By com.ined clinical and histopathological features, 6 were classified as m.gacystis m.crocolon intestinal hypoperistalsis syndrom., 4 as chronic idiopathic intestinal pseudoobstruction, and 2 as intestinal neuronal dysplasias. Six patients died during the follow-up from.21 to 546 days after transplant. The Kaplan-m.ier actuarial survival rates were 66.7% at 1 year and 50% at 3 years. Conclusions: m.ltivisceral transplantation is a valuable therapeutic alternative for children with severe GID who cannot be adequately m.naged with parenteral nutrition.
