BACKGROUND Solid pseudopapillary neoplasms(SPN)of the pancreas represents approximately 2%of non-endocrine tumors of the *** is described in the literature as a rare and predominant tumor in young *** To report a case series with SPN and analyzing clinical,surgical,anatomopathological characteristics,as well as the prognosis and review of *** Retrospective analysis of patients undergoing surgery,with histological diagnosis of SPN between 1998 and 2018,using standardized and prospectively completed forms,performed at the Surgery Service of the Upper Digestive System at Hospital São Rafael/Rede D’Or in *** of literature through a database search in MEDLINE/PubMed of retrospective *** Fourteen female patients with the average age of 31.6 years(range min-max)were *** patients(85.7%)were asymptomatic,being an incidental diagnosis or due to screening for other *** patient had abdominal pain due to gastric compression and another patient had *** 14 patients were staged with computerized tomography or magnetic resonance *** had evidence of *** 8 patients(57.1%),the tumor was in the tail and *** average size was 6.7 cm(range min-18).The type of surgery was according to the anatomical location of the *** was no lymph node *** two cases,vascular resection with the use of a prosthesis was required for *** surgical margins were *** all cases,postoperative immunohistochemistry confirmed that it was a solid pseudo-papillary neoplasia of the *** has been no disease recurrence in any case so *** The tumors had a benign,indolent and histopathological behavior compatible with the *** surgery is recommended in all cases.

A significant portion of Landslide Early Warning Systems (LEWS) relies on the definition of operational thresholds and the monitoring of cumulative rainfall for alert issuance. These thresholds can be obtained in various ways, but most often they are based on previous landslide data. This approach introduces several limitations. For instance, there is a requirement for the location to have been previously monitored in some way to have this type of information recorded. Another significant limitation is the need for information regarding the location and timing of incidents. Despite the current ease of obtaining location information (GPS, drone images, etc.), the timing of the event remains challenging to ascertain for a considerable portion of landslide data. Concerning rainfall monitoring, there are multiple ways to consider it, for instance, examining accumulations over various intervals (1 h, 6 h, 24 h, 72 h), as well as in the calculation of effective rainfall, which represents the precipitation that actually infiltrates the soil. However, in the vast majority of cases, both the thresholds and the rain monitoring approach are defined manually and subjectively, relying on the operators’ experience. This makes the process labor-intensive and time-consuming, hindering the establishment of a truly standardized and rapidly scalable methodology on a large scale. In this work, we propose a Landslides Early Warning System (LEWS) based on the concept of rainfall half-life and the determination of thresholds using Cluster Analysis and data inversion. The system is designed to be applied in extensive monitoring networks, such as the one utilized by Cemaden, Brazil’s National Center for Monitoring and Early Warning of Natural Disasters.

Pain management for the Nuss procedure: comparison between erector spinae plane block, thoracic epidural, and control

Objective Pectus excavatum is a congenital deformity characterized by a caved-in chest *** requires *** less invasive Nuss procedure is very successful,but postoperative pain management is challenging and *** pain management techniques to reduce opioid reliance include the erector spinae plane(ESP)*** retrospectively examined opioid consumption after Nuss procedure comparing three pain management techniques:ESP block,thoracic epidural(TE),and patient-controlled analgesia(PCA).Methods This retrospective cohort study compared pain management outcomes of three patient ***-eight subjects aged 10–18 years underwent Nuss procedure at our institution between January 2014 and January *** primary outcome measure was opioid consumption measured in morphine milligram *** measures included pain ratings and length of stay(LOS).Pain was quantified using the Numeric Pain Rating *** of variance was performed on all outcome *** Average cumulative opioid use was significantly lower in the ESP block(67 mg)than the TE(117 mg)(p=0.0002)or the PCA group(172 mg)(p=0.0002).The ESP block and PCA groups both had a significantly shorter average LOS(3.3 and 3.7 days,respectively)than the TE group(4.7 days).ESP block performed best for reducing opioid consumption and *** opioid consumption is key for limiting side *** study supports use of ESP block as a superior choice when choosing among the three postoperative pain management options that were *** ESP resulted in reduced opioid consumption postoperatively and shorter LOS than TE or PCA for patients undergoing the Nuss procedure for surgical repair of pectus excavatum.

Introduction: At the beginning of the COVID-19 pandemic, a drop in the number of patients treated for cardiac emergencies raised concern about cardiovascular mortality in that period. An increase in care delay for patients with ST-segment elevation myocardial infarction (STEMI) may have affected clinical outcomes. Objectives: To analyze delay times and clinical outcomes of patients with STEMI undergoing primary percutaneous coronary intervention (PPCI), before and during the COVID-19 pandemic. Methods: Retrospective observational study that included patients with STEMI undergoing PPCI from December 2018 to July 2021. The COVID-19 pandemic cases were divided into two groups: pandemic I—from March to August 2020;and pandemic II—from September 2020 to July 2021. Patients were compared according to the period of hospitalization. Primary outcomes were delay times in assistance and clinical outcomes (acute kidney injury [AKI], post-procedural vascular complications and in-hospital mortality). Results: 108 patients were included, 39 (36.1%) in the pre-pandemic period, 13 (12.1%) in pandemic I and 56 (51.8%) in pandemic II. Time from onset of symptoms to arrival at the service and door-to-balloon time did not differ significantly among groups. Vascular complications were more frequent during the pandemic (I and II) than in the pre-pandemic period (2.5% pre-pandemic vs 15.4% pandemic vs 12.5% pandemic II;p = 0.03). AKI incidence was similar in all three periods. There was a non-significant increase in in-hospital mortality during the COVID-19 pandemic. Conclusion: In patients with STEMI, there was an increase in vascular complications and a trend toward increased mortality during the COVID-19 pandemic. Delay times to admission and reperfusion did not differ significantly between before and during the pandemic.

Petit-Suisse Cheese Production with Addition of Probiotic <i>Lactobacillus</i><i>casei</i>

Growing concern to increase life expectancy has promoted several studies in the field of nutrition, especially those on food and their effects on the human body. Assays have been performed to improve their nutritional quality. In fact, there is considerable interest in encouraging development of new ingredients, with the innovation in food products and the establishment of new market niches for these ingredients. The global market for functional foods is growing and always envisages new products with functional technological and physiological characteristics. The petitsuisse cheese is a product of high nutritional value, rich in calcium, phosphorus and fat-soluble vitamins, with a high percentage of milk proteins, coupled to excellent digestion and assimilation by the human body. Current assay describes the production of a petit-suisse cheese to which was added the probiotic Lactobacillus casei BGP 93, and determines the physical, chemical and microbiological viability of lactobacilli in cheese during storage period. Levels of protein, fat in the dry extract, moisture and pH were analyzed for physical and chemical parameters;likewise, detection of coliforms, Staphylococcus, Salmonella, Listeria monocytogenes, and yeasts and molds was performed for microbiological parameters;monitoring of the viability of Lactobacillus casei BGP 93 during its life time was conducted. The petit-suisse cheese proved to be a good vehicle for the addition of probiotic microorganism. Lactobacillus casei remained viable during shelf life even with decreasing pH, and may be considered a functional product during 30 days of storage.

Hybrid combinations of Eucalyptus have increased due to expansion of plantations into unconventional areas and to the search for higher quality ***,most of these species have difficulties surviving in vitro *** chlorine and sealing systems are often used to reduce contamination and increase gas *** aim of the present study is to evaluate the establishment,multiplication,elongation and adventitious rooting of *** × *** clones(C1 and C2) and four active chlorine concentrations(0.000%,0.001%,0.003%,and 0.005%) were tested in the establishment and multiplication *** sealing forms(W/M,1/M and 3/M) and the same four active chlorine concentrations were applied to the elongation *** luminosities(dark and light)and three sealings(W/M,1/M and 3/M) were tested during adventitious *** chlorine concentration of0.005% led to the lowest fungal contamination rate and to the highest in vitro *** chlorine concentration of 0.003% resulted in the greatest length and highest number of shoots per explant in the multiplication *** were no phytotoxicity problems and the quality of plants grown in an environment with active chlorine was maintained in comparison with those grown in an *** increase in gas exchange in ventilation systems had a positive impact on the in vitro growth and development of plants.

AIM： To evaluate the classification and severity of Crohn＇s disease in different racial groups. METHODS： Patients with Crohn＇s disease from the outpatient clinic of the University Hospital Prof. Edgard Santos were enrolled in the study. This hospital is a reference centre for inflammatory bowel disease. Race was determined using self-identification. The Vienna＇s classification was applied for all subjects. The severity of Crohn＇s disease was determined according to the number of surgical procedures, hospital admissions in the last year and treatment with steroids and immunosuppressors. Statistical analysis was calculated using t test for means,χ^2 or F for proportions. A P value 〈 0.05 was considered to be significant. RESULTS： Sixty-five patients were enrolled. Non-white patients were more frequently diagnosed with Crohn＇s disease in the age less than 40 years than white patients. The behaviour of disease was similar in both groups with a high frequency of the penetrating form. There was a tendency for non-white patients to have a greater frequency of hospital admissions in the last year compared to white subjects. Non-whites also had a higher rate of colonic and upper gastrointestinal involvement, and were also more frequently on treatment with immunossupressors than white patients although this difference was not statistically significant. CONCLUSION： Non-white patients with Crohn＇s disease had an earlier diagnosis and appeared to have had a more severe disease Dresentation than white patients.

Hepatocellular carcinoma in patients without cirrhosis: relevance and clinical characteristics

Aim: The present study evaluated the frequency of hepatocellular carcinoma (HCC) in patients without cirrhosis. Methods: HCC patients were recruited from two reference centers for liver disease in Northeast Brazil from 2010 to 2016. The diagnosis of HCC and cirrhosis was based on international criteria. Results: A total of 169 patients were included, and 16% (27) of the patients did not have hepatocellular carcinoma in non-cirrhosis (HCC-NC). The mean age of HCC-NC was 64.4 ± 11.3 years, and 74.1% of the patients were male. The main risk factors were hepatitis C virus (HCV) in 29.6% (8), nonalcoholic steatohepatitis (NASH) in 14.8% (4) and hepatitis B virus (HBV) in 11.1% (3). Histological HCC diagnosis was performed in 81.5% (22) of the patients, and in 18.5% (5) of these patients, the diagnosis was performed by ultrasonography, computed tomography or nuclear magnetic resonance imaging methods. Single nodules were found in 56% of HCC-NC (14) when assessed by imaging methods. Conclusion: The frequency of HCC-NC was elevated and more common in males. HCV, NASH and HBV were the most frequent risk factors. These data contribute to discussion on future protocols and criteria for the early diagnosis and treatment of HCC in patients with chronic liver disease without cirrhosis.
