Pancreatic ductal adenocarcinoma is a highly aggressive tumor with an overall 5-year survival rate of less than 5%. Prognosis and treatment depend on whether the tumor is resectable or not, which mostly depends on how quickly the diagnosis is made. Chemotherapy and radiotherapy can be both used in cases of nonresectable pancreatic cancer. In cases of pancreatic neoplasm that is locally advanced, non-resectable, but non-metastatic, it is possible to apply percutaneous treatments that are able to induce tumor cytoreduction. The aim of this article will be to describe the multiple currently available treatment techniques(radiofrequency ablation, microwave ablation, cryoablation, and irreversible electroporation), their results, and their possible complications, with the aid of a literature review.

AIM To describe magnetic resonance(MR) imaging features of pancreatic neuroendocrine neoplasms(Pan NENs) according to their grade and tumor-nodes-metastases stage by comparing them to histopathology and todetermine the accuracy of MR imaging features in predicting their biological behavior.METHODS This study was approved by our institutional review board; requirement for informed patient consent was waived due to the retrospective nature of the study. Preoperative MR examinations of 55 Pan NEN patients(29 men, 26 women; mean age of 57.6 years, range 21-83 years) performed between June 2013 and December 2015 were reviewed. Qualitative and quantitative features were compared between tumor grades and stages determined by histopathological analysis.RESULTS Ill defined margins were more common in G2-3 and stage Ⅲ-Ⅳ PanN ENs than in G1 and low-stage tumors(P < 0.001); this feature had high specificity in the identification of G2-3 and stage Ⅲ-Ⅳ tumors(90.3% and 96%, 95%CI: 73.1-97.5 and 77.7-99.8). The mean apparent diffusion coefficient value was significantly lower in G2-3 and stage Ⅲ-Ⅳ lesions compared to well differentiated and low-stage tumors(1.09 × 10-3 mm2/s vs 1.45 × 10-3 mm2/s and 1.10 × 10-3 mm2/s vs 1.53 × 10-3 mm2/s, P = 0.003 and 0.001). Receiving operator characteristic analysis determined optimal cutoffs of 1.21 and 1.28 × 10-3 mm2/s for the identification of G2-3 and stage Ⅲ-Ⅳ tumors, with sensitivity and specificity values of 70.8/80.7% and 64.5/64%(95%CI: 48.7-86.6/60-92.7 and 45.4-80.2/42.6-81.3).CONCLUSION MR features of PanN ENs vary according to their grade of differentiation and their stage at diagnosis and could predict the biological behavior of these tumors.

By modulating hepcidin production, an organism controls intestinal iron absorption, iron uptake and mobilization from stores to meet body iron need. In recent years there has been important advancement in our knowledge of hepcidin regulation that also has implications for understanding the physiopathology of some human disorders. Since the discovery of hepcidin and the demonstration of its pivotal role in iron homeostasis, there has been a substantial interest in developing a reliable assay of the hormone in biological fluids. Measurement of hepcidin in biological fuuids can improve our understanding of iron diseases and be a useful tool for diagnosis and clinical management of these disorders. We reviewed the literature and our own research on hepcidin to give an updated status of the situation in this rapidly evolving fi eld.

Implications of small-bowel transit time in the detection rate of capsule endoscopy: A multivariable multicenter study of patients with obscure gastrointestinal bleeding

AIM To define the role of small-bowel transit time in the detection rate of significant small-bowel lesions.METHODS Small-bowel capsule endoscopy records, prospectively collected from 30 participating centers in the Lombardy Registry from October 2011 to December 2013, were included in the study if the clinical indication was obscure gastrointestinal bleeding and the capsule reached the cecum. Based on capsule findings, we created two groups: P2(significant findings) and P0-1(normal/negligible findings). Groups were compared for age, gender, small-bowel transit time, type of instrument, modality of capsule performance(outpatients vs inpatients), bowel cleanliness, and center volume.RESULTS We retrieved and scrutinized 1,433 out of 2,295 capsule endoscopy records(62.4%) fulfilling the inclusion criteria. Patients were 67 ± 15 years old, and 815(57%) were males. In comparison with patients in the P0-1 group, those in the P2 group(n = 776, 54%) were older(P < 0.0001), had a longer small-bowel transit time(P = 0.0015), and were more frequently examined in low-volume centers(P < 0.001). Age and smallbowel transit time were correlated(P < 0.001), with age as the sole independent predictor on multivariable analysis. Findings of the P2 group were artero-venous malformations(54.5%), inflammatory(23.6%) and protruding(10.4%) lesions, and luminal blood(11.5%).CONCLUSION In this selected, prospectively collected cohort of small-bowel capsule endoscopy performed for obscure gastrointestinal bleeding, a longer small-bowel transit time was associated with a higher detection rate of significant lesions, along with age and a low center volume, with age serving as an independent predictor.

Laterally reversed(mirrored)fingerprints are difficult to detect by applying routine search procedures.One suggestion to avoid errors when dealing with probable reversals is to perform comparisons with both direct and reversed fingerprints.This simple procedure has been applied and led to the detection of two more cases of reversed fingerprint usage in fake documents.In one of the reported cases,experts found on the web the same fingerprints used by criminals in fake documents.This finding is important because it indicates that matched fingerprints do not necessarily link different criminal cases.

Multiple source provenance of sediments and submarine fan formation are herein investigated based on Quaternary sandy barriers of the Brazilian Southernmost Coastal Plain.LA-ICP-MS dating on 1625 detrital zircons from marine and aeolian facies sands of four successive lagoon-barrier systems are analyzed.The characterization of Archean to Pleistocene zircons into a younger Andean(22±4 Ma to 1±1 Ma,48 from 1625),Mesozoic to Paleozoic,and Mesoproterozoic(479 from 1622)populations suggest that the main feeding of sediments to the coastal plain occurred through the La Plata River drainage system.The significant contribution of sediments is transported from the mouth of La Plata River northward by longshore circulation(littoral drift).Minor contributions are also recognized as.a farther source associatedwith the Patagonia drainage and nearby source related to the Uruguay/Rio Grande do Sul Shield and the ParanáBasin,drained by the Camaquãand Jacuírivers.The latter one is recognized by the contribution fromNeoproterozoic to Early Paleozoic,and some Paleoproterozoic and Archean zircon grains.The definition of the sources of clastic sediments allows inferences about the origination of Rio Grande Fan where both the cold Falkland and the warm Brazil currents played a major role.

A characteristic of the fracture and cleavage experiments is that they are usually intrinsically destructive. Cracks do not completely heal in an unstressed system, even in crystals such as mica. Here, we used magnetic solids composed of magnetic strips for the non-destructive cleavage and brittle fracture experiments. Between the magnetic strips materials with different mechanical characteristics can be inserted, such as Teflon or foam strips, to change the mechanical properties of the solid. For the cleavage experiments, we developed an apparatus where parameters such as the main involved force can be measured easily. By inserting flaws, the magnetic solid can be used in dynamic fracture experiments, with the advantages of simulating macroscopically a non-destructive experiment in an easier way, that happen in real materials with much higher velocities. The apparatus and the used magnetic solid may be useful for demonstrations of fractures in classes.

Splenic rupture is a rare complication of diagnostic and therapeutic gastrointestinal endoscopy procedures.Herein,we report for the first time a case of splenic rupture following therapeutic retrograde double-balloon enteroscopy,which occurred in an 85-year-old man who was treated for recurrent mid-intestinal bleeding that resulted from ileal angioectasia.This patient promptly underwent an operation and eventually recovered.
