Consultation-Liaison Psychiatry for Outpatients in a Great Metropolitan Hospital

Consultation-liaison psychiatry remains the guardian of holistic approach to the patient and consultation services in general hospital could therefore be referred to as“filters”for mental health care,in particular for patients with minor depressive or anxiety *** is worth noting that psychiatric disorders,even when sub-clinical,worsen outcome whereas their early detection and treatment reduce health care costs *** present study was transversal,exploratory,and descriptive in order to investigate the sample of outpatients referred from general practitioners(GP)to service of consultation-liaison psychiatry of the ASST Great Metropolitan Hospital Niguarda of Milan over a 7-months *** from all psychiatric consultations for outpatients were collected by a structured clinical report including socio-demographic features,features of referrals,and features of *** hundred and eleven consultations were *** were mainly female(58.7%)with an average age between 45 and 55 years(27.6%);31.1%of these patients was at the first contact with a mental health *** diagnoses formulated were distributed as follows:The most common diagnoses were affective disorders(44.6%),followed by neurotic disorders(34.1%),personality disorders(6.8%),and finally,psychosis(6.2%).The study showed an example of a consultation-liaison service situated in a Great Metropolitan Hospital offering to GP the opportunity of a diagnostic assessment and shared management of minor psychiatric *** approach could lead to several vantages,such as the management in a non-psychiatric setting of patients with reduction of stigma and reduced costs for specific psychiatric services where are referred more severe patients.

Impact of new treatment options for hepatitis c virus infection in liver transplantation

Liver transplant candidates and recipients with hepatitis C virus(HCV)-related liver disease greatly benefit from an effective antiviral therapy. The achievement of a sustained virological response before transplantation can prevent the recurrence of post-transplant HCV disease that occurs universally and correlates with enhanced progression to graft cirrhosis. Previous standard-of-care regimens(e.g.,pegylated-interferon plus ribavirin with or without first generation protease inhibitors,boceprevir and telaprevir) displayed suboptimal results and poor tolerance in liver transplant recipients. A new class of potent direct-acting antiviral agents(DAA) characterized by all-oral regimens with minimal side effects has been approved and included in the recent guidelines for the treatment of liver transplant recipients with recurrent HCV disease. Association of sofosbuvir with ribavirin and/or ledipasvir is recommended in liver transplant recipients and patients with decompensated cirrhosis. Other regimens include simeprevir,daclatasvir,and combination of other DAA. Possible interactions should be monitored,especially in coinfected human immunodeficiency virus/HCV patients receiving antiretrovirals.

The misfolding and aggregation of a-synuclein is the general hallmark of a group of devastating neurodegenerative pathologies referred to as synucleinopathies,such as Parkinson’s disease,dementia with Lewy bodies,and multiple system *** such conditions,a range of different misfolded aggregates,including oligomers,protofibrils,and fibrils,are present both in neurons and glial *** expe rimental evidence supports the proposition that solu ble oligomeric assemblies,formed during the early phases of the aggregation process,are the major culprits of neuronal toxicity;at the same time,fibrillar confo rmers appear to be the most efficient at propagating among interconnected neurons,thus contributing to the spreading ofα-synuclein ***,α-synuclein fibrils have been recently repo rted to release soluble and highly toxic oligomeric species,responsible for an immediate dysfunction in the recipient *** this review,we discuss the current knowledge about the plethora of mechanisms of cellular dysfunction caused byα-synuclein oligome rs and fibrils,both contributing to neurodegeneration in synucleinopathies.

TIME-TEMPERATURE-MISCIBILITY AND MORPHOLOGY OF POLYOLEFIN BLENDS

Miscibility and crystallization have been studied for polypropylene-polyethylene and polyethylene-polyethyleneblends. In the case of the polypropylene blends the composition of interest is 20% polypropylene. At this composition thepolypropylene has been found to be soluble in linear low density polyethylene but insoluble in high, low and very lowdensity polyethylenes. The miscibility has been concluded from the crystallization kinetics and polarised optical microscopywith a hot stage. Polyethylene-polyethylene blends have been formed from polymers with similar average branching contentbut where they have different melting temperatures. Important consequences are to introduce long branches into apolyethylene that only has short branches, and to modify the morphology of a polyethylenes so that haze, gloss and strainhardening are improved. Polyethylene blends must be developed after careful consideration of the branch content anddistribution within each of the constituents. It is not sufficient to simply blend polyethylenes, with the desired range ofproperties, without regard to the miscibility of the blend composition.

Wavelet Cleaning of Solar Dynamic Radio Spectrograms

By applying the state-of-the-art mathematical apparatus, the wavelet transformation, we explore the possibility of a dynamic cleaning of raw data ob- tained with the Chinese solar radio spectrographs over a wide wavelength range (from 0.7 to 7.6 GHz). We consider the problem of eliminating the interference caused by combination rates of data sampling (10-20 ins), and the low-frequency interference (4-30 s) caused by the receiving equipment changing its characteristics with time. It is shown that the best choice to reconstruct a signal suffering from amplitude, frequency and phase instabilities, is by means of wavelet transformation at both high and low frequencies. We analysed observational data which contained interferences of nonsolar origin such as instrumental effects and other man-made signals. A subsequent comparison of the reference data obtained with the acousto- optical receiver of the Siberian Solar Radio Telescope (SSRT) with the 'cleaned' spectra confirms the correctness of this approach.

An Encyclopedic Approach to the Conservation of 20^(th)-Century Architecture

The built heritage of the 20^(th) century concerns society in general: it is the background to everyday life and the stage of ‘past experience’. Therefore, recent past buildings should not be considered merely for their aesthetic features;they also have to do with physical well-being, social rituals and representations, as well as with associated concepts such as values and emotions. But conservation of 20^(th)-century heritage is not straightforward. It requires specific strategies as well as critical and operational tools, which are not part of the cultural background of the actors of the transformation of our cities (architects, engineers, heritage officials, etc.) and do not yet feature in current design teaching curricula in most universities. To fill these gaps, in 2008 four Swiss architectural schools (USI, EPFL, ETHZ, SUPSI) launched a research project titled ‘Critical Encyclopaedia for reuse and restoration of 20^(th)-century architecture’ (2009-2013). The article presents one of the outcomes of this research: the work developed by the section titled ‘Historical and Critical Tools for Conservation’, which will be published shortly. It provides readers with the cultural, theoretical and critical frame of reference required to understand recent architectural heritage in its widest sense: as a historical, symbolic and aesthetic resource, but also as being endowed with social, economic and ecological value. By examining theories and doctrines which have developed over time and thanks to the exemplary nature and variety of the selected cases, it provides a historical appraisal of how intervention in the field of recent heritage has evolved over the past 60 years.
