Cytomegalovirus is the most frequent agent of congenital viral infections,affecting approximately 0.2%-2.4%of live births in different countries[1-7].Universal screening for congenital cytomegalovirus infection in newborns is not a standard practice considering cost-benefit issues and viabil-ity of diagnostic ***,most newborns are asympto-matic and there is no effective treatment for this population[2-5].

目的论证指导妇女进行乳房自我检查(BSE)是否能有效降低乳腺癌患者的死亡率。方法自1989年10月起,上海26.6万名年龄在30～64岁的妇女,随机进入指导组(132979名)和对照组(133085名)。在指导组,最初先由医务人员做规范的BSE手法演示,随后的4年中,使用BSE录像带进行二次强化教育,并定期进行BSE指导和在医务人员监察下进行BSE操作。本研究进行5年,全部活动均被记录。队列人群随访到2000年7月,乳腺癌患者的生存状况随访到2001年12月。统计分析采用Kaplan-meier方法、Logrank检验以及Cox模型。结果指导组检出乳腺癌864例,其中死于乳腺癌133例;对照组检出乳腺癌896例,其中死于乳腺癌130例。两组乳腺癌病例的肿块大小、TNm分期和累积死亡率的差异均无统计学意义(P均>0.05)。但是指导组比对照组有较多和较小的纤维腺瘤被检出(P<0.01)。结论对人群开展BSE教育和指导,未能降低人群的乳腺癌死亡率;接受BSE教育的妇女比没有接受教育的妇女可检出较多的和较小的良性乳房肿块。

This paper aimed to evaluate the effects of different concentrations of cadmium on growth rates, photosynthetic pigments, photosynthetic performance, biochemical parameters and structure of chloroplasts in G. domingensis. To accomplish this, apical segments of G. domingensis were cultivated with different concentrations of cadmium, ranging from 100 to 300 μm, over a period of 16 days, and were processed for transmission electron microscopy analysis. The plants exposed to cadmium showed chloroplast alteration, especially degeneration of thylakoids and a decrease in the concentration of photosynthetic pigments, such as chlorophyll a and phycobiliproteins. However, the volume of plastoglobuli increased. As a defense mechanism, the plants treated with cadmium showed an increase in glutathione reductase activity. These results agree with the decreased photosynthetic performance and relative electron transport rate observed after exposure of algae to cadmium. Taken together, these findings strongly indicate that cadmium negatively affects the ultrastructure and metabolism of the agarophyte G. domingensis, thus posing a threat to the economic vitality of this red macroalga.

Randomized Trial Comparing Cyclophosphamide, methotrexate, and 5-Fluorouracil (CmF) Regimen with Rotational CmFEV Regimen (E=Epirubicin, V=Vincristine) as Adjuvant Chemotherapy in moderate Risk Operable Breast Carcinoma

Objectives: The CmFEV (cyclophosphamide, methotrexate, 5-fluorouracil, epirubicin, vincristine) regimen is an innovative schedule, designed by our Group, aimed at administering five partially or totally no cross-resistant cytotoxic agents in breast carcinoma. It was randomly compared to CmF (cyclophosphamide, methotrexate, 5-fluorouracil) as primary treatment in operable disease and demonstrated a short-term significant increase in clinical complete response rate and a long-term significant locoregional relapse-free survival in premenopausal patients. So, it seemed worth comparing this regimen with CmF as adjuvant chemotherapy in moderate risk operable breast carcinoma. methods: Four hundred and eighty-nine patients with stage I or II moderate risk breast carcinoma were randomized to receive CmF or CmFEV regimen for 6 cycles after surgery. main end points were overall survival (OS), invasive disease-free survival (IDFS) and recurrence-free interval (RFI), as estimated by Kaplan-meier analyses and log-rank tests. Results: At a median observation time of 7.3 years (range 5.4 months-10.3 years), no significant differences in OS and IDFS were observed between the two arms. Deaths from breast carcinoma were more frequent with CmF (58.5%) than with CmFEV regimen (41.7%) as well as recurrences from breast carcinoma (58.8% with CmF and 41.2% with CmFEV). These differences were not statistically significant. Conclusion: CmFEV appears more effective than CmF in preventing recurrences from primary disease in patients with moderate risk stage I-II breast carcinoma. The lack of statistical significance of the observed differences was probably due to the limited number of patients enrolled which rendered the study underpowdered.

Impact of Cluster Thinning on Quality of“malbec”Grapes in Encruzilhada do Sul-RS

The cultivar malbec has been present’s good performance and quality in the South Half of Rio Grande do Sul (RS). In this context, we aimed to evaluate the quality of “malbec” grapes in relation to the phytotechnical, physicochemical parameters and bioactive compounds under different intensities of cluster thinning in Encruzilhada do Sul-RS. The experiment was carried out in a commercial vineyard in the 2015/16 with the cultivar malbec (Vitis vinifera L.) under rootstock “1103 Paulsen”, with spacing of 2.2 m between rows and 1.1 m between plants. The experimental design was of randomized complete blocks with five replications. The treatment factor tested was the cluster thinning, with four levels (0%, 10%, 30% and 50%), performed in véraison. The evaluated phytotechnical variables were cluster length, cluster fresh weight, berries fresh weight, number of berries, rachis fresh weight. Also what were determined are the number of clusters, production and Ravaz index. The physicochemical variables verified were pH, soluble solids (SS), titratable acidity (TA) and SS/TA ratio. The bioactive compounds were total polyphenols index and total anthocyanins. The appropriate balance between source and sink, by the Ravaz index, occurs to zero and 10% of cluster thinning. The “malbec” grapes present an increase of the maturation index (SS/TA) at 50% of cluster thinning, due to the low titratable acidity at this level. The total polyphenols index has a larger increment when the plants are submitted to the cluster thinning of 10%, providing a larger quality to the fruits. At all the levels of cluster thinning tested, when compared to the control, there was a greater contribution of total anthocyanins in the berries.

The interest in non-invasive brain stimulation techniques is increasing in recent years. Among these techniques, transcranial direct current stimulation(t DCS) has been the subject of great interest among researchers because of its easiness to use, low cost, benign profile of side effects and encouraging results of research in the field. This interest has generated several studies and randomized clinical trials, particularly in psychiatry. In this review, we provide a summary of the development of the technique and its mechanism of action as well as a review of the methodological aspects of randomized clinical trials in psychiatry, including studies in affective disorders, schizophrenia, obsessive compulsive disorder, child psychiatry and substance use disorder. Finally,we provide an overview of t DCS use in cognitive enhancement as well as a discussion regarding its clinical use and regulatory and ethical issues. Although many promising results regarding t DCS efficacy were described, the total number of studies is still low, highlighting the need of further studies aiming to replicate these findings in larger samples as to provide a definite picture regarding t DCS efficacy in psychiatry.

Evaluation of Pulmonary Function in Adults with Grade Ⅲ Obesity

Pulmonary function studies in obese individuals have alerted the pathophysiological changes due to weight gain. However, these changes are not fully explained yet. Objective: To evaluate and analyze the pulmonary function of obese adults in the city of manaus (Am, Brazil) and to correlate the parameters of spirometry with weight and BmI. method: A descriptive cross-sectional study, in which pulmonary function of 22 adults with grade III obesity was evaluated pre and post bronchodilator administration, performed at the Adriano Jorge Hospital Foundation—FHAJ. Results: Of the 22 patients evaluated, 6 were male and 16 were female;mean age was 37.1 ± 7.4 years, with a weight of 134.2 ± 28.6 kg and a BmI of 51.1 ± 9.4 kg/m2, corresponding to grade III obesity. Spirometry showed a reduction in values obtained from Forced Vital Capacity (FVC), Forced Expiratory Volume in the first second (FEV1) and Peak Expiratory Flow (PEF) in relation to those predicted, with 23% of restrictive disorders. There were negative correlations between BmI and PEF, weight and FVC, weight and PEF. The P value for correlation of weight and FEV1 is not statistically significant. Conclusion: Restrictive and obstructive disorders are prevalent in the population with grade III obesity. Correlations between pulmonary function and BmI/weight were negative for this specific population.

Surgical Preparation of a Permanent Carotid Transposition in Sheep

In large animal research, when frequent sampling of arterial blood is needed, the carotid artery transposition is the most used technique. The objective of this paper is to describe a new technique for carotid artery transposition in sheep and evaluate its effectiveness using the echo-Doppler ultrasound. The animals enrolled in this study had their carotid surgically elevated to the subcutaneous level, by suturing underneath muscles sternocephalic and brachiocephalic. None of the animals that underwent the procedure in this study had trans- or postoperative complications. Six months after the suture removal, all animals presented patent ca-rotids by echo-Doppler ultrasound. This is an easy and safe procedure for carrying out experiments that re-quire the collection of arterial blood.
