Impact of a physician-supervised exercise-nutrition program with testosterone substitution in partial androgen-deficient middle-aged obese men

Andropause refers to a generalized decline of male hormones, including testosterone and dehydroepiandrosterone in middle-aged and aging men. This decline in hormones has been associated with changes such as depression, loss of libido, sexual dysfunction, and changes in body composition. Aging has been associated with an abundance of concomitant diseases, in particular cardiovascular diseases, and although andropause is correlated to aging, a causal relationship between reduction of androgens and the development of chronic diseases such as atherosclerosis and heart failure has not been convincingly established yet. On the other hand, increasing data has emerged that revealed the effects of low levels of androgens on cardiovascular disease progression. As an example, low levels of testosterone have been linked to a higher incidence of coronary artery disease. Whether hormone replacement therapy that is used for andropausal men to alleviate symptoms of ＂male menopause＂ can halt progression of cardiovascular disease, remains controversially discussed, primarily due to the lack of well-designed, randomized controlled trials. At least for symptom improvement, the use of androgen replacement therapy in andropausal men may be clinically indicated, and with the appropriate supervision and follow up may prove to be beneficial with regard to preservation of the integrity of cardiovascular health at higher ages.

A new approach is described for delivering small interfering RNA(siRNA)into cancer cells by noncovalently complexing unmodifi ed siRNA with pristine single-walled carbon nanotubes(SWCNTs).The complexes were prepared by simple sonication of pristine SWCNTs in a solution of siRNA,which then served both as the cargo and as the suspending agent for the *** complexes containing siRNA targeted to hypoxia-inducible factor 1 alpha(HIF-1)were added to cells growing in serum containing culture media,there was strong specific inhibition of cellular HIF-1 *** ability to obtain a biological response to SWCNT/siRNA complexes was seen in a wide variety of cancer cell ***,intratumoral administration of SWCNT-HIF-1 siRNA complexes in mice bearing MiaPaCa-2/HRE tumors signifi cantly inhibited the activity of tumor *** elevated levels of HIF-1 are found in many human cancers and are associated with resistance to therapy and decreased patient survival,these results imply that SWCNT/siRNA complexes may have value as therapeutic agents.

Vitamin d and Number of Falls in a Long-Term Care Facility

Falls represent a significant contribution to the morbidity and mortality of the elderly population. Because vitamin d is important in bone physiology, the use of vitamin d to restore deficient bone and ameliorate the effects of bone fractures due to falls has become a common practice in recent years. Following introduction of widespread use, reports began to emerge that vitamin d not only aided in repair of fall-induced bone fractures, but that it also reduced the occurrence of falls. Vitamin d now has become a routine intervention as a fall-prevention measure. Early analyses found evidence of prevention efficacy (reduced falls), but recent analyses are more equivocal. We retrospectively examined the records of 350 patients in a long-term care facility in which vitamin d administration and the number of falls were recorded as part of a comprehensive database of care. We found a dramatic rise in vitamin d use over the period covered (2006 – 2011) and a corresponding dramatic decrease in the number of falls. However, the number of falls continued to decline after 2008, despite a plateau in number of patients on vitamin d, particularly females. It appears that other factors contribute to the overall decline.

Entecavir for treatment of chronic hepatitis B: A clinical update for the treatment of patients with decompensated cirrhosis

The introduction of nucleos(t)ide analogues for the treatment of chronic hepatitis B virus (HBV) infection was transformative in reducing morbidity and mortality. Entecavir, a potent selective nucleoside analogue first approved in 2005 for treatment of chronic HBV, is associated with significant antiviral, biochemical, serologic, and histologic responses. Rapid reductions in HBV dNA levels, low risk of resistance development, and a favorable adverse event profile have contributed to its clinical usefulness. Re-cent developments in the use of entecavir have increased its utility in the management of difficult-to-treat patients with chronic HBV, including those patients with decompensated liver disease. Recent studies in this population have demonstrated that entecavir 1.0 mg/d given for up to 48 weeks had superior antiviral activity when compared with adefovir and was generally safe and well tolerated. Long-term outcomes of entecavir in difficult-to-treat populations are eagerly anticipated.

Embryonic Hypertension Following Exposure to Teratogenic doses of 5-Fluoro-2'-deoxyuridine

The teratogenicity of 5-fluoro-2'-deoxyuridine (FdU) is well established. Previously, we have demonstrated that teratogenic doses of FdU produce hematomas and suggested that those hematomas produced skeletal malformations in chicken embryos. In this study, the cardiovascular effects of teratogenic doses of FdU in chicken embryos were studied. A dose of either 0.026 μg FdU or 0.030 μg FdU was injected into the yolk sacs of fertile chicken eggs containing embryos at Hamburger and Hamilton stages 17-19 of development. The embryos were then returned to the incubator. Aortic systolic and diastolic blood pressure,blood velocity and heart rate were measured at stages 21, 24 or 27 using a servonull system and doppler ultrasound. In addition, mean arterial blood pressure, blood flow, and stroke volume were calculated from these data. Similar data were also recorded from uninjected and saline injected control embryos. Systolic and mean arterial blood pressures were significantly increased in FdU-treated embryos at stage 27. The other parameters measured or calculated were not significantly different from control embryos. Our study suggests that elevated systolic blood pressure in chicken embryos treated with FdU may lead to hematoma formation and subsequent birth defects
