精神分裂症的社区治疗获益:基于长效注射用抗精神病药物

精神分裂症会长期影响患者的社会和职业功能,以社区为基础的精神卫生服务和管理模式通过提高可及性、可接受性、可负担性、实用性和可扩展性来促进患者心理健康。在社区管理模式中,长效注射用抗精神病药物是目前治疗精神分裂症的主要策略之一,但目前缺少相关的证据梳理。本文系统全面地探讨了长效注射用抗精神病药物对精神分裂症患者的获益情况,包括有效性和安全性、依从性、使用态度、社会功能恢复及社区康复等。本文研究结果表明在不同的社区管理模式下,长效注射用抗精神病药物在精神分裂症患者的社区康复中可发挥比口服药物更卓越的疗效,不仅体现在能降低疾病复发率和再住院率,也在患者社会功能恢复、降低肇事率等方面有优秀的表现。但我国依然存在长效注射用抗精神病药物整体处方率低的问题,主要原因包括医疗机构与基层社区信息共享不足,社区医疗服务治疗率较低,患者随访率低,社区精神康复开展较少等。本文可为我国精神分裂症患者的社区治疗提供一定的参考,以探索更多的社区治疗方案,提高精神分裂症患者治疗效果及生活质量。

Background Studies conducted outside of China have found that stigma can predict low quality of life, and research in China has shown that stigma has an impact on the quality of life of convalescent patients with schizophrenia. Nevertheless, there is no indepth research on the impact of stigma on the quality of life of *** To research the correlation of stigma and the subjective quality of life of persons with chronic schizophrenia in the *** We adopted a stratified sampling *** questionnaire, quality of life scale and stigma scale were given to 602 persons with chronic schizophrenia in the *** The mean(SD) value of the Subjective Quality of Life Scale total scores was 32.99(13.85). The mean(SD)value of the total stigma scores was 43.50(5.02). After correlation analysis and multivariate stepwise regression analysis, it was shown that the total scores for self-stigma and each factor were positively correlated with subjective quality of life(r=0.462, pcommunity, the lower the subjective quality of life is. This suggests that reduction of self-stigma should be considered in the improvement of the quality of life of persons with schizophrenia in the community.

目的汇总11个开展家庭医疗服务的专业姑息治疗团队效应。设计进行一项回顾性队列研究的汇总分析。地点加拿大，安大略省。参与者3109名在2009-2011年接受专业姑息治疗团队医疗服务的患者（暴露组）及3109名在此期间接受常规医疗服务的患者（非暴露组）。干预措施虽然这些姑息治疗团队所在地区和规模不同，但包含了相同的核心团队成员和角色分配，即能够为患者在其家中提供综合姑息治疗的姑息治疗专科医师、护士和家庭医生，团队任务包括全天候的管理症状、给予医疗信息和治疗措施以及协调服务。主要结局测量指标患者（a）在死亡前最后2周在医院度过；（b）死亡前最后2周有过急诊；（c）在医院内死亡。结果暴露组和非暴露组中癌症患者约占80％，且78％患者接受临终家庭医疗护理服务的平均时间相同。在所有姑息治疗团队中，暴露组中分别有970（31．2％）和896（28．9％）名患者在生命最后2周住院治疗和有急诊经历，而在非暴露组，分别为1219（39．3％）和1070（34．5％）名患者（P〈0．001）。暴露组与非暴露组患者在生命终期住院和经历急诊的汇总相对危险度分别为0．68[95％可信区间（CI，0．61～0．76）]和0．77[95％CI（0．69～0．86）]。与非暴露组相比，暴露组患者选择院内死亡的更少[503（16．2％）比887（28．6％），P〈0．01]，且在医院内濒死的汇总相对危险度为0．46（0．40-0．52）。结论尽管社区专业姑息治疗团队的组成和地区不同，但均能有效降低患者临终时急诊及选择在医院内死亡比例。
