sevoflurane Preconditioning and Total Knee Arthroplasty Bleeding: Randomized Controlled Trial

Background: Total knee arthroplasty (TKA) is a highly complex and effective surgery even though its perioperative bleeding may increase the need for blood transfusion and its associated infection risk, cardiovascular overload, increased costs, and mortality. As the tourniquet reduces intraoperative bleeding, it may be associated with postoperative bleeding, venous thrombosis, and distal ischemia. The reperfusion may trigger a local and systemic inflammatory response. Anesthetic preconditioning (APC) with sevoflurane minimizes ischemia-reperfusion syndrome (IRs). This study evaluated the effects of APC with sevoflurane on perioperative bleeding in TKA. Methods: We allocated 30 patients into two groups: a sevo group (sevoflurane 2% for 15 minutes before the tourniquet) and a control group (propofol infusion). Laboratory tests were collected right before the tourniquet (LAB PRE, in the operating room) and after its release at four moments: LAB POsT (immediately after), LAB 2 (two hours after), LAB 12 (12 hours after), and LAB 24 (24 hours after). The volume of the suction drain was measured at one, two, 12, and 24 hours after the end of the surgery. Antifibrinolytics were not administered. Results: There was no statistically significant difference in bleeding-related variables, such as drained volume and hemoglobin and hematocrit measurements. Drainage volume was higher in the first two hours after the procedure, while hematocrit decreased pre- to postoperatively and between two and 12 hours post-procedure. Conclusion: sevoflurane as an anesthetic preconditioning did not reduce postoperative bleeding in TKA surgery.

We present major and trace element compositions of mineral concentrates comprising garnet xenocrysts,ilmenite,phlogopite,spinel,zircon,and uncommon minerals(titanite,calzirtite,anatase,baddeleyite and pyrochlore)of a newly discovered Late Cretaceous kimberlite(U-Pb zircon age 90.0±1.3 Ma;2σ)named Osvaldo Franca 1,located in the Alto Paranaíba Igneous Province(APIP),southeastern *** grains are lherzolitic(Lherz-1,Lherz-2 and Lherz-3),harzburgitic(Harz-3)and wehrlitic(Wehr-2).The pyrope xenocrysts cover a wide mantle column in the subcratonic lithosphere(66–143 km;20–43 kbar)at relatively low temperatures(811–875°C).The shallowest part of this mantle is represented by Lherz-1 pyropes(20–32 kbar),which have low-Cr(Cr_(2)O_(3)=1.74–6.89 wt.%)and fractionated middle to heavy rare earth elements(MREE-HREE)*** deepest samples are represented by Lherz-2,Lherz-3,Harz-3,and Wehr-2 pyropes(36–43 kbar).They contain high-Cr contents(Cr_(2)O_(3)=7.36–11.19 wt.%)and are characterized by sinusoidal(Lherz-2 and Wehr-2)and spoon-like(Lherz-3 and Harz-3)REE *** to their REE and trace elements,pyrope xenocrysts have enriched nature(e.g.,Ce and Yb ***_(2)O_(3)),indicating that the cratonic lithosphere has been affected by a silicate melt with subalkaline/tholeiite composition due to their low Zr,Ti and Y *** minerals with typical kimberlitic signatures,such as ilmenite and zircon,the exotic compositions of phlogopite and ulvöspinel suggest an enriched component in the magma *** formation of rare mineral phases with strong enrichment of light-REE(LREE)and high field strength elements(HFsE)is attributed to the late-stage kimberlitic *** propose a tectonic model where a thermal anomaly,represented by the low-velocity seismic anomaly observed in P-wave seismic tomography images,supplied heat to activate the alkaline magmatism from a metasomatized cratonic mantle source during the late-stages of Gondwana fragmentation and conse

Hepatic abscesses are an uncommon extra-intestinal manifestation of inflammatory bowel disease,the incidence of which has been estimated to be approximately 7 per 10,000 patients with inflammatory bowel *** is unclear whether patients with Crohn's disease or patients with ulcerative colitis(UC)are at higher risk of developing this *** on case reports,most cases are found in Crohn's disease;however,a recent cohort study showed an increased risk in UC *** abscesses in the pediatric population are rare,and there have been no reported cases of hepatic abscesses in a pediatric patient with *** describe herein a pediatric patient with UC who developed hepatic abscesses and portal vein *** patient also had an extended time in remission from his UC despite being off of immunosuppressive therapies,and we speculate on how his clinical course and treatment strategies may have contributed to this.

Detection of DC series arc in more electric aircraft power system based on optical spectrometry

series arcing faults have been recognised as a hazardous phenomenon in the power system of more electric aircraft(MEA),especially with the increasing voltage level and power *** of the electrical method,the optical method can be used as an approach to detect arc fault with the advantages of fast response and immunity to electromagnetic *** is the basis of optical arc detection to determine the distribution spectrum range of direct current(DC)arc *** at series arc optical detection in the MEA DC system,a DC series arc test platform was established to measure the series arc *** spectrum was disassembled though empirical mode decomposition to extract characteristic wavelengths of arc light and denoised by means of wavelet decomposition and reconstruction(wavelet transformation)to reduce equipment *** resolve the baseline drift,the baseline of the arc spectrum measured was calculated and corrected based on savitzky−*** experimental results indicate that there are several series arc characteristic spectra of the MEA DC system,including 309.3 and 324.5 nm in the ultraviolet ***,the arc spectrum with different metals,copper and aluminium,were contrasted,indicating the arc spectral behaviour is related to the melting/boiling point and chemical properties of the anode *** measurement of the series arc spectrum makes it more effective to apply optical detection in the MEA discharging faults diagnosis.

Elevated CO2 leads to a decrease in potential net photosynthesis in long-term experiments and thus to a reduction in potential growth. This process is known as photosynthetic downregulation. There is no agreement on the definition of which parameters are the most sensitive for detecting CO2 acclimation. In order to investigate the most sensitive photosynthetic and molecular markers of CO2 acclimation, the effects of elevated CO2, and associated elevated temperature were analyzed in alfalfa plants inoculated with different sinorhizobium meliloti strains. Plants （Medicago sativa L. cv. Aragon） were grown in summer or autumn in temperature gradient greenhouses （TGG）. At the end of the experiment, all plants showed acclimation in both seasons, especially under elevated summer temperatures. This was probably due to the lower nitrogen （N） availability caused by decreased N2-fixation under higher temperatures. Photosynthesis measured at growth CO2 concentration, rubisco in vitro activity and maximum rate of carboxylation were the most sensitive parameters for detecting downregulation. severe acclimation was also related with decreases in leaf nitrogen content associated with declines in rubisco content （large and small subunits） and activity that resulted in a drop in photosynthesis. Despite the sensitivity of rubisco content as a marker of acclimation, it was not coordinated with gene expression, possibly due to a lag between gene transcription and protein translation.

Zeta potential measurement on lithium lanthanum titanate nanoceramics

The zeta potential, isoelectric point, and agglomeration of ***3 （LLTO） nanoparticles dispersed in aqueous media at different ionic strengths have been studied. The zeta potential was determined from electrophoretic mobility measurements, according to smoluchowski＇s equation, for Li0.5La0.5TiO3 suspen- sions in NaCl and KCI electrolytes with ionic strengths of 1, 10, and 100 mmol/dm3. The isoelectric point （IEP）, zeta potential （ζ）, and the agglomeration were shown to strongly depend on the ionic strength of the Li0.5La0.5TiO3 aqueous colloidal suspension in both NaCI and KCI electrolytes, which allows the deter- mination of the effects of environmental conditions for ***3 manipulation in aqueous colloidal systems. The suspensions of Li0.5La0.5TiO3 nanoparticles reach the IEP in the pH range 0f3-5. The （ of Li0.5La0.5TiO3 nanoparticles varied from positive to negative values with a pH increase, which allows for the control of the surface charge depending on the purpose. The pH range of 7-g and an ionic strength ≤1 mmol/dm3 are recommended as the most suitable conditions for both the LLTO colloidal shaping techniques application and the LLTO-based nanocomposite formation.

A VsC-based Model for Power Flow Assessment of Multi-terminal VsC-HVDC Transmission systems

This paper puts forward a new practical voltage source converter(VsC)based AC-DC converter model suitable for conducting power flow assessment of multi-terminal VsCbased high-voltage direct current(VsC-MTDC)*** model uses an advanced method to handle the operational limits and control modes of VsCs into the power flow *** new model is incorporated into a unified framework encompassing AC and DC power grids and is solved by using the Newton-Raphson method to enable quadratically convergent iterative *** use of complementarity constraints,together with the Fischer-Burmeister function,is proposed to enable the seamless incorporation of operational control modes of VsC and automatic enforcement of any converter’s operational limits that become violated during the iterative solution ***,a dedicated process for checking limits is no longer ***,all existing relationships between the VsC control laws and their operational limits are considered directly during the solution of the power flow *** applicability of the new model is demonstrated with numerical examples using various multi-terminal AC-DC transmission networks,one of which is a utility-sized power system.
