A clinical dilemma:abdominal tuberculosis

AIM:To evaluate the clinical,radiological and microbiological properties of abdominal tuberculosis (TB) and to discuss methods needed to get the diagnosis. METHODS:Thirty-one patients diagnosed as abdominal TB between March 1998 and December 2001 at the Gastroenterology Department of Kartal State Hospital, Istanbul,Turkey were evaluated *** physical examination,medical and family history,blood count erythrocyte sedimentation rate,routine biochemical tests, Mantoux skin test,chest X-ray and abdominal ultrasonography (USG) were performed in all cases,whereas microbiological examination of ascites,upper gastrointestinal endoscopy,colonoscopy or barium enema,abdominal tomography,mediastinoscopy,laparoscopy or laparotomy were done when needed. RESULTS:The median age of patients (14 females,17 males) was 34.2 years (range 15-65 years).The most frequent symptoms were abdominal pain and weight loss. Eleven patients had active pulmonary *** most common abdominal USG findings were ascites and *** fluid analysis performed in 13 patients was found to be exudative and acid resistant bacilli were present in smear and cultured only in one patient with BacTec (3.2%).Upper gastrointestinal endoscopy yielded nonspecific findings in 16 *** performed in 20 patients showed ulcers in 9 (45%),nodules in 2 (10%) and,stricture,polypoid lesions,granulomatous findings in terminal ileum and rectal fistula each in one patient (5%). Laparoscopy on 4 patients showed dilated bowel loops, thickening in the mesentery,multiple ulcers and tubercles on the *** with abdominal TB were divided into three groups according to the type of involvement. Fifteen patients (48%) had intestinal TB,11 patients (35.2%) had tuberculous peritonitis and 5 (16.8%) tuberculous *** diagnosis of abdominal TB was confirmed microbiologically in 5 (16%) and histo- pathologically in 19 patients (60.8%).The remaining nine patients (28.8%) had

Endoscopic scoring of late gastrointestinal mucosal damage after adjuvant radiochemotherapy

AIM： To evaluate late effects of chemoradiation on gastrointestinal mucosa with an endoscopic scoring system and compare it to a clinical scoring system. METHODS： Twenty-four patients going to receive chemoradiation after gastric surgery underwent endoscopy four wk after surgery and one year after the chemoradiation finished. Upper gastrointestinal findings were recorded according to a system proposed by World Organisation for Digestive Endoscopy （OMED） and clinical scoring was done with RTOG-EORTC radiation morbidity scoring systems. RESULTS： There was no significant endoscopic difference in gastric and intestinal mucosa after chemoradiation （P 〉 0.05） and there was no association between endoscopic scores and clinical scores. Endoscopic changes were minimal. CONCLUSION： Late effects after chemoradiation in operated patients with gastric cancers can be evaluated with an endoscopic scoring system objectively and this system is superior to clinical scoring systems.

Encapsulating peritonitis and familial Mediterranean fever

AIM: To investigate the relationship between encapsulating peritonitis and familial Mediterranean fever (FMF). METHODS: The patient had a history of type 2 diabetes and Iaparoscopic cholecystectomy was performed one year ago for cholelithiasis. Eleven months after the operation she developed massive ascites. Biochemical evaluation revealed hyperglycemia, mild Fe deficiency anemia, hypoalbuminemia and a CA-125 level of 2 700 IU. Ascitic evaluation showed characteristics of exudation with a cell count of 580/mm^3. Abdominal CT showed omental thickening and massive ascites. At exploratory laparotomy there was generalized thickening of the peritoneum and a Iaparoscopic clip encapsulated by fibrous tissue was found adherent to the uterus. Biopsies were negative for malignancy and a prophilactic total abdominal hysterectomy and bilateral salpingooophorectomy were performed. RESULTS: The histopathological evaluation was compatible with chronic nonspecific findings and mild mesothelial proliferation and chronic inflammation at the uterine serosa and liver biopsy showed inactive cirrhosis. CONCLUSION: The patient was evaluated as sclerosing encapsulating peritonitis induced by the iaparoscopic clip acting as a foreign body. Due to the fact that the patient had FMF the immune response was probably exaggerated.

AIM： To prospectively investigate serum CA 19-9 levels in type 2 diabetic patients in comparison with age and gender-matched control subjects.METHODS： We recorded duration of diabetes and examined fasting glucose levels, HbAlc levels and serum CA 19-9 levels in 76 type 2 diabetic patients and 76 controls. Abdominal CT was performed in order to eliminate abdominal malignancy in the diabetic and control groups.RESULTS： The average CA 19-9 level was 46.0 ± 22.4 U/mL for diabetic patients whereas it was 9.97± 7.1 U/mL for the control group （P 〈 0.001 ）. Regression analysis showed a positive correlation between diabetes and CA 19-9 independent from age, gender, glucose level and HbAlc level （t = 8.8, P 〈 001 ）. Two of the diabetic patients were excluded from the study because of abdominal malignancy shown by CT at the initial evaluation. For all patients, abdominal CT showed no pancreatic abnormalities. CONCLUSION： CA 19-9 is a tumor-associated antigen, which is elevated in pancreatic, upper gastrointestinal tract, ovarian hepatocellular, and colorectal cancers, as well as in inflammatory conditions of the hepatobiliary system, biliary obstruction and in thyroid diseases. Diabetes has been claimed to be a risk factor for pancreatic cancer, which is increasing its incidence and has one of the lowest survival rates of all cancers. CA 19-9 is used in the diagnosis of pancreatic cancer but is also a marker of pancreatic tissue damage that might be caused by diabetes. We propose that a higher cutoff value of CA 19-9 should be used in diabetics to differentiate benign and malignant pancreatic disease, and subtle elevations of CA 19-9 in diabetics should be considered as the indication of exocrine pancreatic dysfunction.

The main purpose of this paper is to introduce some approximation properties of a Kantorovich kind q-Bernstein operators related to B′ezier basis functions with shape parameterλ∈[−1,1].Firstly,we compute some basic results such as moments and central moments,and derive the Korovkin type approximation theorem for these ***,we estimate the order of convergence in terms of the usual modulus of continuity,for the functions belong to Lipschitz-type class and Peetre’s K-functional,***,with the aid of Maple software,we present the comparison of the convergence of these newly defined operators to the certain function with some graphical illustrations and error estimation table.

Seismic performance of buildings during 2011 Van earthquakes and rebuilding efforts

The October 23, 2011 M7.2 Tabanli - Van and November 9, 2011 M5.2 Edremit - Van earthquakes caused damage in a widespread area across the Van province in Turkey. The ground motions, the damage caused by these earthquakes and the recent progress related to recovery efforts are presented herein. First, the key properties of the recorded strong ground motions like spectral amplitudes and directionality are evaluated. The observed damage in the affected reinforced concrete and masonry structures are discussed. The set of common structural damage mechanisms （i.e., soft story failure, torsional response due to plan irregularity, short column failure, pull out failure, pounding） observed in the damaged buildings were identified. The relationship between the key structural properties and the extent of damage is investigated. The primary loss drivers across the region were identified to be the poor quality of workmanship and improper use of building materials. The results from the investigation suggest that a large portion of the loss could have been prevented if sufficient attention and care were given to the implementation of the design regulations and in particular to the construction practice. Lastly, the recent progress in the ongoing rebuildin~ activities is presented.

This study investigates the perturbed Chen–Lee–Liu model that represents the propagation of an optical pulse in plasma and optical *** generalized exponential rational function method is used for this *** a result,we obtain some non-trivial solutions such as the optical singular,periodic,hyperbolic,exponential,trigonometric soliton *** aim to express the pulse propagation of the generated solutions,by taking specific values for the free parameters existed in the obtained *** obtained results show that the generalized exponential rational function technique is applicable,simple and effective to get the solutions of nonlinear engineering and physical ***,the acquired solutions display rich dynamical evolutions that are important in practical applications.

Severe eosinophilia associated with hydroxychloroquine use in a patient with COVID-19

Hydroxychloroquine,a 4-aminoquinoline derivative,is indicated in systemic lupus erythematosus,rheumatoid arthritis and other inflammatory rheumatic diseases[1].Its main adverse drug reactions are retinopathy during long-term exposure with high doses,cardiac damage with arrhythmias,potentially severe skin damage,neuromyopathy,and hematological damage such as bone marrow aplasia and agranulocytosis[2].Eosinophilia,although very rare,is a known side effect of this drug[3].We came across a case of severe eosinophilia without skin manifestations during the treatment of COVID-19 with hydroxychloroquine.

N-acetylcysteine attenuates alcohol-induced oxidative stess in rats

AIM:To investigate free-radical scavenger effect of n- acetylcysteine in rats intragastrically fed with ethanol. METHODS:Twenty-four rats divided into three groups were fed with ethanol (6 g/kg/day,Group 1),ethanol and n- acetylcysteine (1 g/kg,Group 2),or isocaloric dextrose (control group,Group 3) for 4 *** animals were sacrificed under ether anesthesia,and intracardiac blood and liver tissues were *** were made in both serum and homogenized liver tissues. Malondialdehyde (MDA) level was measured by TBARS *** peroxidase (GSH-Px) and superoxide dismutase (SOD) levels were studied by commercial kits. Kruskal-Wallis test was used for statistical analysis. RESULTS:ALT and AST in Group 1 (154 U/L and 302 U/L, respectively) were higher than those in Group 2 (94 U/L and 155 U/L) and Group 3 (99 U/L and 168 U/L) (P=0.001 for both).Serum and tissue levels of MDA in Group 1 (1.84 nmol/mL and 96 nmol/100 mg-protein) were higher than that in Group 2 (0.91 nmol/mL and 64 nmol/100 mg protein) and Group 3 (0.94 nmol/ml and 49 nmol/100 mg-protein) (P<0.001 for both).On the other hand,serum GSH-Px level in Group 1 (8.21 U/g Hb) was lower than that in Group 2 (16 U/g Hb) and Group 3 (16 U/g-Hb) (P<0.001).Serum and liver tissue levels of SOD in Group 1 (11 U/mL and 26 U/100 rag-protein) were lower than that in Group 2 (18 U/ mL and 60 U/100 mg protein) and Group 3 (20 U/mL and 60 U/100 rag-protein) (P<0.001 for both). CONCLUSION:Ethanol-induced liver damage was associated with oxidative stress,and co-administration of n-acetylolsteine attenuates this damage effectively in rat model.
