Animal model-based simulation training for three emergent and urgent operations of penetrating thoracic injuries

Purpose:To develop animal models of penetrating thoracic injuries and to observe the effects of the animal model-based training on improving the trainees’performance for emergent and urgent thoracic surgeries.Methods:With a homemade machine,animal models of lung injuries and penetrating heart injuries were produced in porcine and used for training of chest tube drainage,urgent sternotomy,and emergent thoracotomy.Coefficient of variation of abbreviated injury scale and blood loss was calculated to judge the reproducibility of animal models.Five operation teams from basic-level hospitals(group A)and five operation teams from level III hospitals(group B)were included to be trained and tested.Testing standards for the operations were established after thorough literature review,and expert questionnaires were employed to evaluate the scientificity and feasibility of the testing standards.Tests were carried out after the training.Pre-and post-training performances were compared.Post-training survey using 7-point Likert scale was taken to evaluate the feelings of the trainees to these training approaches.Results:Animal models of the three kinds of penetrating chest injuries were successfully established and the coefficient of variation of abbreviated injury scale and blood loss were all less than 25%.After literature review,testing standards were established,and expert questionnaire results showed that the scientific score was 7.30±1.49,and the feasibility score was 7.50±0.89.Post-training performance was significantly higher in both group A and group B than pre-training performance.Post-training survey showed that all the trainees felt confident in applying the operations and were generally agreed that the training procedure were very helpful in improving operation skills for thoracic penetrating injury.Conclusions:Animal model-based simulation training established in the current study could improve the trainees’performance for emergent and urgent thoracic surgeries,especially of the surgical teams from basic-level hos

Integral precession calibration method of PIGA on linear vibration table

Linear vibration table can provide harmonic accelerations to excite the nonlinear error terms of Pendulous Integrating Gyro Accelerometer(PIGA).Integral precession calibration method is proposed to calibrate PIGA on a linear vibration table in this paper.Based on the precise expressions of PIGA’s inputs,the error calibration model of PIGA is established.Precession angular velocity errors of PIGA are suppressed by integer periodic precession and the errors caused by non-integer periods vibrating are compensated.The complete calibration process,including planning,preparation,PIGA testing,and coefficient identification,is designed to optimize the test operations and evaluate the calibration results.The effect of the main errors on calibration uncertainty is analyzed and the relative sensitivity function is proposed to further optimize the test positions.Experimental and simulation results verify that the proposed 10-position calibration method can improve calibration uncertainties after compensating for the related errors.The order of calibration uncertainties of the second-and third-order coefficients are decreased to 10^(-8)(rad.s^(-1))/g^(2)and 10^(-8)(rad.s^(-1))/g3,respectively.Compared with the other two classical calibration methods,the calibration uncertainties of PIGA’s nonlinear error coefficients can be effectively reduced and the proportional residual errors are decreased less than 3×10-6(rad.s^(-1))/g by using the proposed calibration method.

Visual Semantic Segmentation Based on Few/Zero-Shot Learning:An Overview

Visual semantic segmentation aims at separating a visual sample into diverse blocks with specific semantic attributes and identifying the category for each block,and it plays a crucial role in environmental perception.Conventional learning-based visual semantic segmentation approaches count heavily on largescale training data with dense annotations and consistently fail to estimate accurate semantic labels for unseen categories.This obstruction spurs a craze for studying visual semantic segmentation with the assistance of few/zero-shot learning.The emergence and rapid progress of few/zero-shot visual semantic segmentation make it possible to learn unseen categories from a few labeled or even zero-labeled samples,which advances the extension to practical applications.Therefore,this paper focuses on the recently published few/zero-shot visual semantic segmentation methods varying from 2D to 3D space and explores the commonalities and discrepancies of technical settlements under different segmentation circumstances.Specifically,the preliminaries on few/zeroshot visual semantic segmentation,including the problem definitions,typical datasets,and technical remedies,are briefly reviewed and discussed.Moreover,three typical instantiations are involved to uncover the interactions of few/zero-shot learning with visual semantic segmentation,including image semantic segmentation,video object segmentation,and 3D segmentation.Finally,the future challenges of few/zero-shot visual semantic segmentation are discussed.

Wear-resistant CoCrNi multi-principal element alloy at cryogenic temperature

Traditional high strength engineering alloys suffer from serious surface brittleness and inferior wear performance when servicing under sliding contact at cryogenic temperature.Here,we report that the recently emerging CoCrNi multi-principal element alloy defies this trend and presents dramatically enhanced wear resistance when temperature decreases from 273 to 153 K,surpassing those of cryogenic austenitic steels.The temperature-dependent structure characteristics and deformation mechanisms influencing the cryogenic wear resistance of CoCrNi are clarified through microscopic observation and atomistic simulation.It is found that sliding-induced subsurface structures show distinct scenarios at different deformation temperatures.At cryogenic condition,significant grain refinement and a deep plastic zone give rise to an extended microstructural gradient below the surface,which can accommodate massive sliding deformation,in direct contrast to the strain localization and delamination at 273 K.Meanwhile,the temperature-dependent cryogenic deformation mechanisms(stacking fault networks and phase transformation)also provide additional strengthening and toughening of the subsurface material.These features make the CoCrNi alloy particularly wear resistant at cryogenic conditions and an excellent candidate for safety–critical applications.

Mediating role of social support in dysphoria,despondency,and quality of life in patients undergoing maintenance hemodialysis

BACKGROUND Dysphoria and despondency are prevalent psychological issues in patients undergoing Maintenance Hemodialysis(MHD)that significantly affect their quality of life(QOL).High levels of social support can significantly improve the physical and mental well-being of patients undergoing MHD.Currently,there is limited research on how social support mediates the relationship between dysphoria,despondency,and overall QOL in patients undergoing MHD.It is imperative to investigate this mediating effect to mitigate dysphoria and despondency in patients undergoing MHD,ultimately enhancing their overall QOL.AIM To investigate the mediating role of social support in relationships between dysphoria,despondency,and QOL among patients undergoing MHD.METHODS Participants comprised 289 patients undergoing MHD,who were selected using a random sampling approach.The Social Support Rating Scale,Self-Rating Anxiety Scale,Self-Rating Depression Scale,and QOL Scale were administered.Correlation analysis was performed to examine the associations between social support,dysphoria,despondency,and QOL in patients undergoing MHD.To assess the mediating impact of social support on dysphoria,despondency,and QOL in patients undergoing MHD,a bootstrap method was applied.RESULTS Significant correlations among social support,dysphoria,despondency,and quality in patients undergoing MHD were observed(all P<0.01).Dysphoria and despondency negatively correlated with social support and QOL(P<0.01).Dysphoria and despondency had negative predictive impacts on the QOL of patients undergoing MHD(P<0.05).The direct effect of dysphoria on QOL was statistically significant(P<0.05).Social support mediated the relationship between dysphoria and QOL,and this mediating effect was significant(P<0.05).Similarly,the direct effect of despondency on QOL was significant(P<0.05).Moreover,social support played a mediating role between despondency and QOL,with a significant mediating effect(P<0.05).CONCLUSION These findings suggest that social support plays a signifi

Mixed D-vine copula-based conditional quantile model for stochastic monthly streamflow simulation

Copula functions have been widely used in stochastic simulation and prediction of streamflow.However,existing models are usually limited to single two-dimensional or three-dimensional copulas with the same bivariate block for all months.To address this limitation,this study developed a mixed D-vine copula-based conditional quantile model that can capture temporal correlations.This model can generate streamflow by selecting different historical streamflow variables as the conditions for different months and by exploiting the conditional quantile functions of streamflows in different months with mixed D-vine copulas.The up-to-down sequential method,which couples the maximum weight approach with the Akaike information criteria and the maximum likelihood approach,was used to determine the structures of multivariate Dvine copulas.The developed model was used in a case study to synthesize the monthly streamflow at the Tangnaihai hydrological station,the inflow control station of the Longyangxia Reservoir in the Yellow River Basin.The results showed that the developed model outperformed the commonly used bivariate copula model in terms of the performance in simulating the seasonality and interannual variability of streamflow.This model provides useful information for water-related natural hazard risk assessment and integrated water resources management and utilization.

Nanoscopic tribological characteristics of a cryogenically cycled Zr-based metallic glass

Optimized macroscopic tribological behavior can be anticipated in metallic glasses(MGs)by cryogenic cycling treatment(CCT),which is attributed to enhanced plasticity.However,the intrinsic friction mechanisms of MGs induced by cryogenic rejuvenation are still poorly understood.In the present study,nanoscopic wear tests were conducted on the Zr-based MGs surface with different CCT cycles using atomic force microscopy(AFM).After CCT treatment with 100 cycles,the MG displays the highest adhesion and ploughing frictions,but significantly improved anti-wear properties.Adhesion tests and molecular dynamics simulations disclose that the increased adhesion is attributed to the dominance of liquid-like regions in the CCT-treated MGs,and the impact of reduced hardness and weak elastic recovery results in the deteriorated ploughing friction.The enhanced plasticity effectively dissipates the strain from the AFM tip through multiple shear bands and weakens the adhesion during deformation,giving rise to excellent wear resistance.This study elucidates the promoting effect of CCT on the outstanding antiwear performance of MGs,and is helpful for the development of novel alloys.

Mesenteric adipose tissue B lymphocytes promote intestinal injury in severe acute pancreatitis by mediating enteric pyroptosis

Background:Visceral adipose tissue(VAT)has been linked to the severe acute pancreatitis(SAP)prognosis,although the underlying mechanism remains unclear.It has been reported that pyroptosis worsens SAP.The present study aimed to verify whether mesenteric adipose tissue(MAT,a component of VAT)can cause secondary intestinal injury through the pyroptotic pathway.Methods:Thirty-six male Sprague Dawley(SD)rats were divided into six different groups.Twelve rats were randomly divided into the SAP and control groups.We monitored the changes of MAT and B lymphocytes infiltration in MAT of SAP rats.Twelve SAP rats were injected with MAT B lymphocytes or phosphate buffer solution(PBS).The remaining twelve SAP rats were first injected with MAT B lymphocytes,and then with MCC950(NLRP3 inhibitor)or PBS.We collected blood and tissue samples from pancreas,gut and MAT for analysis.Results:Compared to the control rats,the SAP group showed inflammation in MAT,including higher expression of tumor necrosis factor(TNF-α)and interleukin-6(IL-6),lower expression of IL-10,and histological changes.Flow cytometry analysis revealed B lymphocytes infiltration in MAT but not T lymphocytes and macrophages.The SAP rats also exhibited intestinal injury,characterized by lower expression of zonula occludens-1(ZO-1)and occludin,higher levels of lipopolysaccharide and diamine oxidase,and pathological changes.The expression of NLRP3 and n-GSDMD,which are responsible for pyroptosis,was increased in the intestine of SAP rats.The injection of MAT B lymphocytes into SAP rats exacerbated the inflammation in MAT.The upregulation of pyroptosis reduced tight junction in the intestine,which contributed to the SAP progression,including higher inflammatory indicators and worse histological changes.The administration of MCC950 to SAP+MAT B rats downregulated pyroptosis,which subsequently improved the intestinal barrier and ameliorated inflammatory response of SAP.Conclusions:In SAP,MAT B lymphocytes aggravated local inflammation,and promoted the injury to the int

An Innovative Coupled Common-Node Discrete Element Method-Smoothed Particle Hydrodynamics Model Developed with LS-DYNA and Its Applications

In this study,a common-node DEM-SPH coupling model based on the shared node method is proposed,and a fluid–structure coupling method using the common-node discrete element method-smoothed particle hydrodynamics(DS-SPH)method is developed using LS-DYNA software.The DEM and SPH are established on the same node to create common-node DEM-SPH particles,allowing for fluid–structure interactions.Numerical simulations of various scenarios,including water entry of a rigid sphere,dam-break propagation over wet beds,impact on an ice plate floating on water and ice accumulation on offshore structures,are conducted.The interaction between DS particles and SPH fluid and the crack generation mechanism and expansion characteristics of the ice plate under the interaction of structure and fluid are also studied.The results are compared with available data to verify the proposed coupling method.Notably,the simulation results demonstrated that controlling the cutoff pressure of internal SPH particles could effectively control particle splashing during ice crushing failure.

BACKGROUND Hepatic cystic and alveolar echinococcosis coinfections,particularly with concurrent abscesses and sinus tract formation,are extremely rare.This article presents a case of a patient diagnosed with this unique presentation,discussing the typical imaging manifestations of both echinococcosis types and detailing the diagnosis and surgical treatment experience thereof.CASE SUMMARY A 39-year-old Tibetan woman presented with concurrent hepatic cystic and alveolar echinococcosis,accompanied by abdominal wall abscesses and sinus tract formation.Initial conventional imaging examinations suggested only hepatic cystic echinococcosis,but intraoperative and postoperative pathological examination revealed the coinfection.Following radical resection of the lesions,the patient’s condition improved,and she was discharged soon thereafter.Subsequent outpatient follow-ups confirmed no recurrence of the hydatid lesion and normal surgical wound healing.Though mixed hepatic cystic and alveolar echinococcosis with abdominal wall abscesses and sinus tract formations are rare,the general treatment approach remains consistent with that of simpler infections of alveolar echinococcosis.CONCLUSION Lesions involving the abdominal wall and sinus tract formation,may require radical resection.Long-term prognosis includes albendazole and follow-up examinations.
