Timing of chemotherapy and survival in patients with resectable gastric adenocarcinoma

AIM: To evaluate the timing of chemotherapy in gastric cancer by comparing survival outcomes in treatment groups. METHODS: Patients with surgically resected gastric adenocarcinoma from 1988 to 2006 were identified from the Los Angeles County Cancer Surveillance Program. To evaluate the population most likely to receive and/or benefit from adjunct chemotherapy, inclusion criteria consisted of Stage Ⅱ or Ⅲ gastric cancer patients > 18 years of age who underwent curative-intentsurgical resection. Patients were categorized into three groups according to the receipt of chemotherapy:(1) no chemotherapy;(2) preoperative chemotherapy; or(3) postoperative chemotherapy. Clinical and pathologic characteristics were compared across the different treatment ***: Of 1518 patients with surgically resected gastric cancer, 327(21.5%) received perioperative chemotherapy. The majority of these 327 patients were male(68%) with a mean age of 61.5 years; and they were significantly younger than non-chemotherapy patients(mean age, 70.7; P < 0.001). Most patients had tumors frequently located in the distal stomach(34.5%). Preoperative chemotherapy was administered to 11.3% of patients(n = 37) and postoperative therapy to 88.7% of patients(n = 290). An overall survival benefit according to timing of chemotherapy was not observed on univariate or multivariate analysis. Similar results were observed with stagespecific survival analyses(5-year overall survival: Stage Ⅱ, 25% vs 30%, respectively; Stage Ⅲ, 14% vs 11%, respectively). Therefore, our results do not identify a survival advantage for specific timing of chemotherapy in locally advanced gastric cancer. CONCLUSION: This study supports the implementation of a randomized trial comparing the timing of perioperative therapy in patients with locally advanced gastric cancer.

目的　研究代谢酶基因NAT2多态性与大肠息肉及腺瘤复发的关系 ,探讨其与大肠癌遗传易感性。方法　在 1980年 ,建立了 40 76例大肠癌高危人群队列 ,这 40 76例均有大肠腺瘤或息肉史。从高危人群中随机抽取 5 2例息肉、腺瘤复发 2次以上者为复发组 ,另抽取 5 2例无复发者为对照 ,从抗凝外周血白细胞中提取DNA ,采用聚合酶链反应限制性内切酶片段长度多态性 (PCR RFLP)方法分析 ,比较NAT2的多态性分布差异。结果　NAT2野生型Wt/Wt频率在无复发组 (17/ 5 2 ,32 7% )明显高于复发组 (8/ 5 2 ,15 .4% ) ,杂合型在复发组频率为 40 / 5 2 (76 .9% ) ,与无复发组频率30 / 5 2 (5 7.7% )比较 ,差异均有显著意义 (P <0 .0 5 )。以Wt/Wt基因型的危险度 (OR)为 1,则Wt/M 基因型的OR为 2 .96 ,95 %CI为 1.0 91～ 8.0 0 9,M /M 基因型的OR为 2 .12 5 ,95 %CI为 0 .6 6 6～6 781。在复发与无复发组中 ,未发现NAT2快速与慢速酶的差异。结论　NAT2野生型 (Wt/Wt)可能是大肠腺瘤复发的保护因素。

Impact of medical and surgical intervention on survival in patients with cholangiocarcinoma

AIM:To examine surgical and medical outcomes for patients with cholangiocarcinoma using a populationbased cancer ***:Using the California Cancer Registry's Cancer Surveillance Program,patients with intrahepatic cholangiocarcinoma treated in Los Angeles County from 1988 to 2006 were identified and evaluated for clinical and pathologic factors and therapies received(surgery,radiation,and chemotherapy).The surgical cohort was further categorized into three treatment groups:patients who received adjuvant chemotherapy,adjuvant chemoradiation,or underwent surgery alone(no chemotherapy or radiation administered).Survival was assessed by Kaplan-Meier method;and Cox proportional hazard modeling was used in multivariate ***:Of 825 patients,60.2% received no *** the remaining 328 patients,18.5% chemotherapy only,7.4% chemoradiation,and 13.8% underwent *** male patients underwent surgical resection(P = 0.004).Surgical patients were younger than the patients receiving chemotherapy or chemoradiation(P < 0.001).Of the surgical cohort(n = 114),60.5% underwent surgery alone while 39.5% underwent surgery plus adjuvant therapy(chemotherapy n = 20;chemoradiation,n = 21)(P < 0.001).Median survival for all patients in the study was 6.6 *** survival was highest for patients who underwent surgery(23 mo),whereas both chemotherapy(9 mo) and chemoradiation(8 mo) alone were each less effective(P < 0.001).By multivariate analysis,extent of disease,receipt of surgery,and administration of chemotherapy(with/without surgery) were independent predictors of overall ***:This study demonstrates that surgery is a critical treatment *** treatment has yet to be standardized,but play a role in optimal therapy for cholangiocarcinoma.

Accuracy of computed tomography in nodal staging of colon cancer patients

AIM: To predict node-positive disease in colon cancer using computed tomography(CT).METHODS: American Joint Committee on Cancer stage Ⅰ-Ⅲ colon cancer patients who underwent curavtiveintent colectomy between 2007-2010 were identified at a single comprehensive cancer center. All patients had preoperative CT scans with original radiology reports from referring institutions. CT images underwent blinded secondary review by a surgeon and a dedicated abdominal radiologist at our institution to identify pericolonic lymph nodes(LNs). Comparison of outside CT reports to our independent imaging review was performed in order to highlight differences in detection in actual clinical practice. CT reviews were compared with final pathology. Results of the outside radiologist review, secondary radiologist review, and surgeon review were compared with the final pathologic exam to determine sensitivity, specificity, positive and negative predictive values, false positive and negative rates, and accuracy of each review. Exclusion criteria included evidenceof metastatic disease on CT, rectal or appendiceal involvement, or absence of accompanying imaging from referring ***: From 2007 to 2010, 64 stageⅠ-Ⅲ colon cancer patients met the eligibility criteria of our study. The mean age of the cohort was 68 years, and 26(41%) patients were male and 38(59%) patients were female. On final pathology, 26 of 64(40.6%) patients had nodepositive(LN+) disease and 38 of 64(59.4%) patients had node-negative(LN-) disease. Outside radiologic review demonstrated sensitivity of 54%(14 of 26 patients) and specificity of 66%(25 of 38 patients) in predicting LN+ disease, whereas secondary radiologist review demonstrated 88%(23 of 26) sensitivity and 58%(22 of 38) specificity. On surgeon review, sensitivity was 69%(18 of 26) with 66% specificity(25 of 38). Societal biosecurity–measures built into everyday society to minimize risks from pests and diseases–is an important aspect of managing epidemics and *** aimed to identify societal options for reducing the transmission and spread of respiratory *** used SARS-CoV-2(severe acute respiratory syndrome coronavirus 2)as a case study to meet the immediate need to manage the COVID-19 pandemic and eventually transition to more normal societal conditions,and to catalog options for managing similar pandemics in the *** used a‘solution scanning’*** read the literature;consulted psychology,public health,medical,and solution scanning experts;crowd-sourced options using social media;and collated comments on a ***,we present a list of 519 possible measures to reduce SARS-CoV-2 transmission and *** provide a long list of options for policymakers and businesses to consider when designing biosecurity plans to combat SARS-CoV-2 and similar pathogens in the *** also developed an online application to help with this *** encourage testing of actions,documentation of outcomes,revisions to the current list,and the addition of further options.
