polymorphisms of the TLR2 and TLR4 genes are associated with risk of gastric cancer in a Brazilian population

AIM： TO investigate toll-like receptor 2 （TLR2） -196 to -274 del, and TLR4 （＋896A/G rs4986790 and ＋1196C/ T rs4986791） polymorphisms at risk of chronic gastritis and gastric cancer in a Brazilian population and associ-ation of gastric lesions with risk factors such as smoking, alcohol intake and Helicobacter pylori infection.METHODS： In this casecontrol study, polymorphism at TLR2 -96 to -174 del was investigated by using the allele-specific polymerase chain reaction （pCR） method, while the pCR-restriction fragment length polymorphism technique was carried out to identify the TLR4 （rs4986790 and rs4986791） genotypes in 607 Brazilian individuals （208 with chronic gastritis-CG, 174 with gastric cancer-GC and 225 controls -C）.RESULTS： The single nucleotide polymorphisms TLR4＋1196ClT was not associated with risk of chronic gastritis or gastric cancer and the homozygous genotypes TLR4＋896GG and TLR4＋1196TF were absent in the studied population. However, the frequency of TLR2 -196 to -174 ins/del ＋ del/del and TLR4＋896AGgenotypes was significantly higher （p 〈 0.01 and p = 0.01, respectively） in the cancer group （33.4% and 11.5%, respectively） than in the control group （16.9% and 4.5%, respectively）. It was also observed that the G-C haplotype of the TLR4＋896A/G＋1196C/T （p = 0.02） and the combination of variant alleles of the TLR21TLR4＋896G （p = 0.02） are associated with susceptibility to gastric cancer. In addition, the multiple logistic regression showed that male gender [odds ratio （OR） = 2.70; 95% CI： 1.66-4.41; p 〈 0.01], alcohol intake （OR = 2.93; 95% CI： 1.76-4.87, p 〈 0.01）, TLR2 -196 to -174 del （OR = 2.64; 95% CI： 1.56-4.44; p 〈 0.01） and TLR4＋896G （OR = 3.19; 95% CI： 1.34- 7.61; p 〈 0.01） polymorphisms were associated with a higher susceptibility to developing this neoplasm.CONCLUSION： Our data indicate that T/R2 -196 to -174 del and TLR4＋896G may increase the risk of gastric cancer in a Brazilian population.

Infections and inequalities: anemia in AIDS, the disadvantages of poverty

Objective:To study anemia in AIDS patients and its relation with socioeconomic, employment status and educational ***:A total number of 442 patients who visited the Infectious Diseases University Hospital in Buenos Aires, Argentina were included in the study. patients were dividied into two groups,*** with anemia and the other without anemia. Anemia epidemiology and its relationship with educational level, housing, job situation, monthly income, total daily caloric intake and weekly intake of meat were ***:Anemia was found in 228 patients(54%). Comparing patients with or without anemia, a statistically significant difference was found(pprimary school, who lived in a precarious home, who had no stable job or were unable to work, whose income was less than 30 dollars per month, whose meat consumption was less than twice a week or received less than 8000 calories per ***:The high prevalence of anemia found in poor patients with AIDS suggests that poverty increases the risk to suffer from this hematological *** relationship between economic development policies and AIDS is complex. Our results seem to point to the fact that AIDS epidemic may affect economic development and in turn be affected by it. If we consider that AIDS affects the economically active adult population, despite recent medical progress it usually brings about fatal consequences, especially within the poorest sectors of society where the disease reduces the average life expectancy, increases health care demand and tends to exacerbate poverty and iniquity.

Corticosteroids for severe influenza pneumonia: A critical appraisal

Influenza pneumonia is associated with high number of severe cases requiring hospital and intensive care unit(ICU) admissions with high mortality. Systemic steroids are proposed as a valid therapeutic option even though its effects are still controversial. Heterogeneity of published data regarding study design, population demographics, severity of illness, dosing, type and timing of corticosteroids administered constitute an important limitation for drawing robust conclusions. However, it is reasonable to admit that, as it was not found any advantage of corticosteroid therapy in so diverse conditions, such beneficial effects do not exist at all. Its administration is likely to increase overall mortality and such trend is consistent regardless of the quality as well as the sample size of studies. Moreover it was shown that corticosteroids might be associated with higher incidence of hospital-acquired pneumonia and longer duration of mechanical ventilation and ICU stay. Finally, it is reasonable to conclude that corticosteroids failed to demonstrate any beneficial effects in the treatment of patients with severe influenza infection. Thus its current use in s evere influenza pneumonia should be restricted to very selected cases and in the setting of clinical trials.

Estimates of voluntary activation in individuals with anterior cruciate ligament reconstruction:Effects of type of stimulator,number of stimuli,and quantification technique

Background:Accurate quantification of voluntary activation is important for understanding the extent of quadriceps dysfunction in individuals with anterior cruciate ligament reconstruction(ACLR).Voluntary activation has been quantified using both percent activation derived from the interpolated twitch technique and central activation ratio(CAR)derived from the burst superimposition technique,as well as by using different types of electrical stimulators and pulse train ***,it is unclear how these parameters affect voluntary activation estimates in individuals with *** study was performed to fill this important knowledge gap in the anterior cruciate ligament ***:Quadriceps strength and voluntary activation were examined in 18 ACLR participants(12 quadriceps/patellartendon graft,6 hamstring tendon graft;time since ACLR:1.06±0.82 years,mean±SD)at 90°of knee flexion using 2 stimulators(Digitimer and Grass)and pulse train conditions(3-pulse and 10-pulse).Voluntary activation was quantified by calculating both CAR and percent ***:Results indicated that voluntary activation was significantly overestimated by CAR when compared with percent activation(ppulse train conditions when using percent activation;however,3-pulse stimuli resulted in greater overestimation than 10-pulse stimuli when using CAR(p=0.003).Voluntary activation did not differ between stimulators(p>0.05);however,the Digitimer evoked greater torque at rest than the Grass(ppercent activation derived from the interpolated twitch technique provides superior estimates of voluntary activation than CAR derived from burst superimposition and is less affected by pulse train conditions or stimulators in individuals with ACLR.

psidium guajava (guava) leaf is a phytotherapic used in folk medicine to treat gastrointestinal and respiratory dis-turbances and is used as anti-inflammatory medicine. In nuclear medicine, blood constituents (BC) are labelled with techne-tium-99m (99mTc) and used to image procedures. However, data have demonstrated that synthetic or natural drugs could modify the labelling of BC with 99mTc. The aim of this work was to evaluate the effects of aqueous extract of guava leaves on the labelling of BC with 99mTc. Blood samples of Wistar rats were incubated with different concentrations of guava extract and labelled with 99mTc after the percentage of incorporated radioactivity (%ATI) in BC was determined. The results suggest that aqueous guava extract could present antioxidant action and/or alters the membrane structures involved in ion transport into cells, thus decreasing the radiolabelling of BC with 99mTc. The data showed significant (p<0.05) alteration of ATI in BC from blood incubated with guava extract.

Migratory connectivity describes the linkage between breeding and nonbreeding sites,having major ecological implications in birds:1 season influence the success of an individual or a population in the following *** studies on migratory connectivity have used large-scale approaches,often considering regional populations,but fine-scale studies are also necessary to understand colony *** lesser kestrel Falco naumanni,an insectivorous migratory raptor which form colonies during the breeding period,was considered to have strong connectivity based on regional ***,no small-scale studies on migratory connectivity have been ***,we GpS(Global positioning System)-tracked 40 adult lesser kestrels from 15 different Spanish breeding colonies,estimating the overlap index between home ranges and the distance between their *** was found that lesser kestrels from the same breeding colony placed their nonbreeding areas at 347±281 km(mean±standard deviation)away from each other(range=23-990),and their home ranges over-lapped by 38.4±23.6%.No differences between intra-colony and inter-colony metrics were found,which suggests that lesser kestrels from the same breeding cluster do not overwinter together,but they spread out and mixed independently of the colony belonging throughout the nonbreeding range of the ***,this study highlights the importance of performing connectivity studies using fine-scale approaches.

High-resolution patterns of palaeoenvironmental changes during the Little Ice Age and the Medieval Climate Anomaly in the northwestern Iberian peninsula
