Teleophthalmology: a strategy for timely diagnosis of sight-threatening diabetic retinopathy in primary care, Concepción, Chile

AIM: To estimate the prevalence of diabetic retinopathy(DR) in a diabetic population of the public health system. METHODS: This non-experimental, descriptive and crosssectional study, with no direct control over the variables analysed, was carried out by the Telemedicine Unit of the University of Concepción(TELMED) and the Family Health Centers(CESFAM) of the Health Service Concepción, Chile, among a population of 7382 diabetic patients of the public health system, from October 2014 to June 2015. Two digital images for each patient’s eyes were obtained and uploaded to the TELMED tele-ophthalmology platform. The images were remotely evaluated by a retina expert ophthalmologist for diagnosis. Treatment and a referral(if required) were recommended, and an individualised report for each patient was sent to the place of ***: The prevalence of DR in this study was 14.9%. Of these, 48.7% had mild non-proliferative DR(NPDR), 30.8% moderate NPDR, 15.9% severe NPDR, and 4.6% proliferative DR. Of the patients with DR, 17.8% were referred for fluorescein angiography, 4.3% for panretinal photocoagulation, 1.2% for vitrectomy, and 0.4% for cataract ***: The use of telemedicine allowed an increased screening coverage for DR in diabetic patients. The different types of DR were detected in a timely manner, decreasing waiting times due to the lack of specialists in ophthalmology in the public health system.

AIM： To assess if the included vascular endothelial growth factor （VEGF） polymorphisms rs3025035, rs3025021 and rs2010963 are associated to proliferative retinopathy in a Mexican population with type 2 diabetes mellitus （T2DM）. METHODS： A case-control study was conducted in adult individuals with T2DM associated to proliferative retinopathy or non-proliferative retinopathy from Oct. 2014 to Jun. 2015 from the Retina Department of the Asociation to Prevent Blindness in Mexico. The selected patients were adults with a diagnosis of T2DM ≥5y. All subjects had a comprehensive ocular examination and the classification of the retinopathy severity was made considering the Early Treatment Diabetic Retinopathy Study （ETDRS） standardization protocols. Genomic DNA was extracted from whole fresh blood. All samples were genotyped by qPCR for selected VEGF polymorphisms. Hardy-Weinberg equilibrium was calculated by comparing Chi-square values between the expected and the observed values for genotype ***： In total 142 individuals were enrolled, 71 individuals with T2DM and associated proliferative retinopathy and 71 individuals with non-proliferative retinopathy. One-sided Fisher’s exact test was performed for rs3025021 [OR （95% CI）=0.44（0.08-2.2）; P=0.25] and rs2010963 [OR （95% CI）=0.63（0.25-1.6）; P=0.23]. The minor allelic frequencies obtained were 26% for rs3025021, 10% for rs3025035 and 61% for rs2010963. AIM: To investigate efficacy and safety of second-line treatment with irinotecan-loaded drug-eluting beads(DEBIRI) and cetuximab(DEBIRITUX) of unresectable colorectal liver ***: Patients with the following characteristics were included in the study: unresectable hepatic metastases from colorectal carcinoma(CRC-LM), progression after first line chemotherapy(any type of chemotherapeutic drug and combination was allowed), second line treatment(mandatory), which included for each patient(unregarding the KRas status) two cycles of DEBIRI(using 100-300 μm beads loaded with irinotecan at a total dose 200 mg) followed by 12 cycles of cetuximab that was administered weekly at a first dose of 400 mg/m2 and then 250 mg/m2; good performance status(0-2) and liver functionality(alanine aminotransferase and gamma-glutamyl transferase not exceeding three times the upper limit of normal, total bilirubin not exceeding 2.5 mg/m L). Data were collected retrospectively and included: tumor response(evaluated monthly for 6 mo then every 3 mo), overall response rate(ORR), KRas status, type and intensity of adverse events(G according to the Common Terminology Criteria for Adverse Events v3.0, CTCAE), overall survival(OS) and progression free survival(PFS).RESULTS: Forty consecutive cases of CRC hepatic metastases were included in the study. Median duration of DEBIRITUX was 4.4 mo(range, 4.0-6.5). Sixteen patients(40%) received the planned 2 cycles of DEBIRI and an average of 10 cetuximab cycles. ORR of the whole sample was 50%, in particular 4 patients were complete responders(10%) and 16(40%) partial responders. The most observed side effects(G2) were: post-embolization syndrome(30%), diarrhea(25%), skin rushes(38%) and asthenia(35%). The retrospective evaluation of KRas status(24 wild type, 16 mutated) showed that the group of patients with wild type KRas had ORR significantly higher than mutant KRas. The paper discusses the results of a field study carried out in four cities in Mexico： Hermosillo, Mexicali, Merida and Colima, during the warmest seasons of 2006-2007. The survey is according to the adaptive approach of thermal comfort. The cities＇ climates are hot dry, hot sub-humid and hot humid. The respondents were inhabitants of low cost housings without air conditioning. The research was performed during warm seasons and according to ISO 10551. The measurements were processed by the common method of linear regression and also by alternative methods, useful for asymmetric climates. Individuals declared comfort at very high temperatures, either high or low humidity, therefore, the resulting neutral temperatures are higher than 30 ℃, except in Colima （28.8 ℃）. The upper limits of comfort ranges achieved temperatures up to 35 ℃. The results suggest how great is the capacity of humans to adapt to conditions as extreme as those measured in the study.

Diabetic Renal-Retinal Syndrome and Inflammatory Markers of Acute Phase

Objective: To determine the association between the acute inflammatory markers and Diabetic Renal-Retinal Syndrome in our patients. Methods: A total of 44 patients were included in this transverse (cross sectional), observational, analytical, and comparative study. Two groups of patients were created. Results: In patients with chronic renal failure, stages 3, 4, or 5 (scale 1 to 5) were identified and divided into two groups: Group I (Patients with concomitant diabetic retinopathy stabilized with photocoagulation) 22 patients. Group II (Patients with progression of retinopathy, after photocoagulation, in the form of intraocular bleeding) 22 patients. The number of patients with chronic renal failure in stage 3 was: Group I;16. In Group II;12 patients (OR: 1.0). Stage 4: Group I;4 patients. Group II: 3 patients (OR: 1.0, CI 95%;0.3843 - 2.021). Stage 5: Group I;2 patients. Group II;7 patients (OR: 1.8148, CI 95%;1.0448 - 3.1523;Chi-squared (x2) 2.7450, p = 0.097). t-Test found statistical significance in Erythrocyte Sedimentation Rate (ESR): p < 0.0001 (CI 95%;7.8959 - 23.2515);R = 0.56 p = 0.001;and OR = 1.959 (CI 95%;1.193 - 3.217). By non-parametric tests, C-reactive protein with binomial test showed p < 0.001. ESR showed p < 0.001 with U of Mann Whitney. Conclusions: With this study revealed a possible association of inflammatory markers with Diabetic Renal-Retinal Syndrome.

The aim of neoadjuvant treatment in non-small cell lung cancer(NSCLC)is to eliminate micrometastatic disease to facilitate surgical *** chemotherapy(ChT)in localised NSCLC has numerous advantages over other therapeutic modalities and is considered standard treatment in resectable *** with immune checkpoint inhibitors(ICI)improves long-term survival in advanced disease and has a better toxicity profile than conventional *** immunotherapy agents(anti-PD1/PD-L1),administered with or without ChT,are currently being evaluated in the preoperative setting,with initial results showing better pathological response rates and more long-term ***,these drugs do not appear to increase the rate of severe adverse effects and/or postoperative ***,several questions still need to be resolved,including the identification of predictive biomarkers;comparative studies of immunotherapy alone vs combined treatment with ChT and/or radiotherapy;the optimal duration of treatment;the timing of surgery;the need for adjuvant treatment;appropriate radiologic evaluation and mediastinal staging;and the correlation between pathological response and survival *** we review the current evidence for immunotherapy from a multidisciplinary perspective and discuss current and future controversies.

Blueberry Extract Reduces Oxidative Stress in Patients with Metabolic Syndrome

Oxidative stress is a health condition that could potentially harm the patient, and it is defined as the lack of balance between the production of oxygen free radicals, which rises, and antioxidant defenses, which are in decrease. Metabolic syndrome implies a variety of risk factors that are based on abdominal obesity and insulin resistance. Moreover, the importance of metabolic syndrome is emphasized due to the fact that it presents an increase in oxidative stress, which is produced by the sum of alterations that each risk factor produces within the metabolic syndrome pathology. Reducing oxidative stress in these patients is currently one of the most interesting challenges of cardiovascular and metabolic therapeutics, because it is a molecular biology alteration that is not generally diagnosed and, therefore, not treated. The increasing incidence of overweight and obesity cause an increase in the incidence of metabolic syndrome, thus turning into a huge problem that keeps growing at alarming proportions. This syndrome＇s incidence oscillates between 20 and 40%, depending on the gender, age, ethnic group, and diagnostic criteria used for the definition of the disease. Prospective studies show that metabolic syndrome doubles the risk of cardiovascular disease and causes a five-fold increase in the risk of developing type 2 diabetes. With the aim of decreasing the oxidative stress caused by metabolic syndrome, we investigated the effect of antioxidant protection in DNA repair and cell membranes through the use of blueberry extract, which is the fruit with the highest antioxidant capacity, in patients with metabolic syndrome. Thirty patients were studied for a period of 6 months of intervention, and it could be demonstrated that they showed a highly significant decrease in the damage produced to the DNA, which was measured by the urinary excretion of 8-hydroxy-2＇-deoxyguanosine （8-OHdG） and the damage caused to the vascular endothelium and cell membranes, which was meas
