OBJECTIVE:To evaluate the efficacy of intramedullary Kirschner wires for the treatment of unstable tibial shaft fractures in children.;METHODS:This prospective study was conducted at the Department of Orthopaedic Surgery in Maharishi Markandeshwar Medical College from June 2005 to June 2010. Sixty-six children having closed fracture of the tibial shaft with a mean age of 7.7 years (range, 2-14 years) were recruited from emergency and outpatient department. They were treated with percutaneous intramedullary Kirschner wires. The clinical results of our study were rated on the basis of the criteria of union, nonunion, delayed union or malunion. All children were followed for one year.;RESULTS:Children achieved union in a mean time of 8 weeks (range, 6-10 weeks). Postoperatively, three children (4.55%) had delayed union, one (1.52%) valgus deformity of lower leg, three (4.55%) post- operative knee pain and twelve (18.18%) skin irritation at pin site.Wires were removed after 8-22 weeks without any complications. No patient was lost to follow-up. The results were excellent in 95.45% and good in 4.55% children.;CONCLUSIONS:This technique is cost-effective, simple, quick to perform, safe and reliable and avoids pro- longed hospitalization with good results.

Background: Fracture shaft humerus is a major cause of morbidity in patients with upper extremity injuries. The aim of this study was to evaluate the outcome of interlocking nail in humeral shaft fractures.Methods: This study was conducted in the Department of Orthopedic Surgery in SMS and R Sharda University from January 2010 to November 2013. Seventy-eight patients were recruited from emergency and out-patient department having a close fracture of humerus shaft. All patients were operated under general anesthesia and closed reamed interlocking nailing was done. All patients were followed for 9 months.Results: Out of 78 patients, 69 patients underwent union in 90–150 days with a mean of 110.68 days. Complications found in four patients who had nonunion, and five patients had delayed union, which was treated with bone grafting. All the patients were assessed clinically and radiologically for fracture healing, joint movements and implant failure. The results were excellent in 88.46% and good in 6.41% patients. Complete subjective, functional, and clinical recovery had occurred in almost 100% of the patients.Conclusions: The results of the present study indicates that in the presence of proper indications, reamed antegrade intramedullary interlocked nailing appears to be a method of choice for internal fixation of osteoporotic and pathologic fractures.

Dear editor,With interest we read the recent article on methemoglobinemia by Chan et al.[1]Through this letter,we would like to add few additional comments regarding methemoglobinemia and its relevance in medicine practice with regards to food poisoning.While Chan et al[1]successfully managed their patient of methemoglobinemia and could pinpoint choy sum as the responsible agent,we wonder if there were any more cases of choy sum related to methemoglobinemia detected at the same time in the community?

We have quantitatively analyzed the tree species diversity with respect to soil nutrient status in three sites of a sacred forest ecosystem of Niyamgiri hill range,Eastern Ghats,India.Extensive field surveys and sampling were conducted in 3 sites of the hill range:Site 1 Pterocarpus dominated forest(PTF)(19°40'02.2" N and 83°21'23.1" E),Site 2 Mangifera dominated forest(MAF)(19°40'02.8" N and 83°21'40.8" E) and Site 3 Mixed forest(MIF)(19°36'47.1" N and 83°21'02.7" E).A total of 28 families,42 genera,46 tree species,and 286 individual trees were recorded on an area of 0.6 ha.Tree density varied between 470 and 490 individuals ha-1 and average basal area between 3.16 and 10.04 m2 ha-1.Shannon Index(H') ranged from 2.34 to 4.53,Simpson's Index ranged from 0.07 to 0.09,and equitability Index ranged from 0.7 to 1.34.The number of individuals was highest in the girth at breast height(GBH) class of 50-70 cm.The soil nutrient status of the three forest types was related to tree species diversity.The soil pH value of the three sites reflected the slightly acidic nature of the area.Species diversity was positively correlated with organic carbon and phosphorus and negatively with nitrogen,EC and pH.The results of the current study may be helpful to further develop a conservation planfor tree species in tropical sacred forest ecosystems.

Ayurveda has attached great importance to preventive medicine and has described several methods for disinfection.Today we are living in a world where we are constantly facing new diseases and surrounded by various types of pathogenic microorganisms in our daily lives,which make us their prey as soon as they get a chance.Some of these microorganisms are the ones that give rise to terrible types of infectious diseases,which have the potential to influence a large section of the population.The Epidemic is the occurrence of more cases of a disease than would be expected in a community or region during a time of period,a severe outbreak of a disease such as SARS.Epidemiological diseases have been described in Ayurveda regarding Janpadodhwansa.Vayu(Air),Desha(Land),Kala(Season),and Jala(Water)are the four factors that are contaminated during the epidemic,according to Ayurveda.Dhupana(fumigation)is the act by which considerable success can be achieved in the field of disinfecting contaminated air and surface.Therefore,because of the present circumstances,this article is an attempt by which to show the importance of Dhupana Karma(fumigation)for the process of disinfection.

Objective To investigate the pharmacological effect of Nyctanthes arbortristis (NAT) leaf extract in the prevention of lung injury induced by silica particles. Method Lung injury was induced in Swiss mice through inhalation exposure to silica particles (<5m) using a Flow Past Nose Only Inhalation Chamber at the rate of -10 mg/m3 respirable mass for 5 h. Lung bronchoalveolar lavage (BAL) fluid collected between 48 and 72 h was subjected to protein profiling by electrophoresis and cytokine evaluation by solid phase sandwich ELISA. Lung histopathology was performed to evaluate lung injury. Results Inhalation of silica increased the level of tumor necrosis factor-a (TNF-a), and of the 66 and 63 kDa peptides in the BAL fluid in comparison to sham-treated control. Pre-treatment of silica exposed mice with NAT leaf extract significantly prevented the accumulation of TNF-a in the BAL fluid, but the 66 and 63 kDa peptides remained unchanged. The extract was also effective in the prevention of silica-induced early fibrogenic reactions like congestion, edema and infiltration of nucleated cells in the interstitial alveolar spaces, and thickening of alveolar septa in mouse lung. Conclusion NAT leaf extract helps in bypassing silica induced initial lung injury in mice.
