Hepatitis B virus （HBV） infection is a global public health problem that concerns 350 million people worldwide. Individuals with chronic hepatitis B （CriB） are at increased risk of developing liver cirrhosis, hepatic de-compensation and hepatocellular carcinoma. To maintain undetectable viral load reduces chronic infection complications. There is no treatment that eradicates HBV infection. Current drugs are expensive, are associated with adverse events, and are of limited efficacy. Current guidelines try to standardize the clinical practice. nevertheless, controversy remains about management of asymptomatic patients with CriB who are hepatitis B e antigen （HBeAg）-positive with normal alanine aminotransferase, and what is the cut-off value of viral load to distinguish HBeAg- negative CriB patients and inactive carriers. We discuss in detail why DnA level alone is not sufficient to begin treatment of CriB.

Geographic Routing （GR） algorithms require nodes to periodically transmit HELLO messages to allow neighbors to know their positions （beaconing mechanism）. Beacon-less routing algorithms have recently been proposed to reduce the control overheads due to these messages. However, existing beacon-less algorithms have not considered realistic physical layers. Therefore, those algorithms cannot work properly in realistic scenarios. In this paper we present a new beacon-less routing protocol called BOSS. Its design is based on the conclusions of our open-field experiments using Tmote-sky sensors. BOSS is adapted to error-prone networks and incorporates a new mechanism to reduce collisions and duplicate messages produced during the selection of the next forwarder node. We compare BOSS with Beacon-Less Routing （BLR） and Contention-Based Forwarding （CBF） algorithms through extensive simulations. The results show that our scheme is able to *** almost perfect packet delivery ratio （like BLR） while having a low bandwidth consumption （even lower than CBF）. Additionally, we carried out an empirical evaluation in a real testbed that shows the correctness of our simulation results.

This study proposes a hypodermic needle(Hn)as a sorbent holder and an electrospray(ESI)emitter,thus combining extraction and analysis in a single device.A novel nylon 6-cellulose(n6-Cel)composite sorbent is proposed to extract methadone from oral fluid *** cellulosic substrate provides the composite with high porosity,permitting the flow-through of the sample,while the polyamide contributes to the extraction of the *** low price of the devices(considering the holder and the sorbent)contributes to the affordability of the method,and their small size allows easy transportation,opening the door to on-site *** the optimum conditions,the analyte can be determined by high-resolution ambient ionization mass spectrometry at a limit of detection(LOD)as low as 0.3 mg/L and precision(expressed as relative standard deviation,RSD)better than 9.3%.The trueness,expressed as relative recovery(RR),ranged from 90%to 109%.As high-resolution mass spectrometers are not available in many laboratories,the method was also adapted to low-resolution *** this sense,the direct infusion of the eluates in a triple quadrupole-mass spectrometry provided an LOD of 2.2 mg/*** RSD was better than 5.3%,and the RR ranged from 96%to 121%.

Over the last years, major improvements in the field of male infertility diagnosis have been achieved. The aim of this study was to determine the diagnostic usefulness of sperm DnA integrity and sperm vacuolisation for predicting outcome in infertile couples undergoing in vitroferti lisation （IVF） and intracytoplasmic sperm injection （ICSI） treatments. A cohort study from 152 infertile couples undergoing sperm DnA fragmentation and high-magnification tests prior to an assisted reproduction treatment was designed. We found that the most predictive cutoff for pregnancy was 25.5% of DnA fragmentation with a negative predictive value of 72.7% （P=0.02）. For the degree of vacuolisation, the best predictor of pregnancy was 73.5% of vacuolated sperm grades Ⅲ ＋ Ⅳ with a negative predictive value of 39.4% （P=0.09）, which was not statistically significant. In conclusion, sperm DnA fragmentation greater than 25.5% could be associated with higher probability of failure IVF treatment. Regarding the results of the sperm analysis at high magnification, they do not allow us to predict whether or not oatients will become pregnant.
