Disaster Awareness and Preparedness Among Older Adults in Canada Regarding Floods, Wildfires, and Earthquakes

Older adults are significantly impacted by natural hazards and disasters that are exacerbated by climate change. Understanding their awareness and preparedness is essential for enhancing disaster resilience. This study investigated the attitudes, actions, and recommendations of older adults regarding natural hazards that pose risks in their geographic area—specifically floods, wildfires, and/or earthquakes in Canada. Methods for this study included survey and focus groups with older adults(n = 161 and n = 10, respectively) and other high-risk groups from across Canada, that are vulnerable to these natural hazards. The main findings from this study are that current awareness and preparedness among older adults is low, though stronger perceptions of risks are associated with risks specific to geographic locations where respondents live. Several barriers, such as hazard vulnerability misperceptions, cost-related reasons, and lack of hazard awareness have resulted in low awareness and preparedness among these populations. The two main recommendations arising from this research are:(1) improve awareness and preparedness with tailor-made emergency preparedness materials for older adults;and(2) adopt community-based approaches to disaster preparedness through existing community groups to strengthen social connections with a focus on locally specific hazards. The findings from this research can be applied to other hazards, including heatwaves and pandemics.

Improvement in Patient Mobility Following Short-Term Rehabilitation in Skilled Nursing Facilities

Background: Mobility in older adults can be impaired after acute illness or hospitalization, and the level of severity can be used as a predictor of one’s ability to return to independent living. Patients are often referred to skilled nursing facilities in hopes of improving their mobility. We wanted to prove that rehabilitative services at Skilled Nursing Facilities improve overall outcomes. Methods: We conducted a retrospective analysis of data from 4612 patients admitted for short-term rehabilitation in a large nursing home chain in California. Our aim was to determine whether patients’ mobility scores, as measured by rehabilitative staff, significantly improved by time of discharge compared to their scores at admission. Mobility scores were rated from 1 to 6, with 1 being the most dependent on aid and 6 being the most independent, over a variety of tasks at admission and compared to scores at discharge. Pearson’s correlations were performed to determine if there were significant relationships in the data: the Pearson’s correlation coefficient was used to describe the relationships between patient admission to a skilled nursing facility and medical improvement upon discharge. Results: The study demonstrated a statistically significant improvement in patients’ mobility scores upon discharge, with Medicare insured patients showing on average 57% improvement and Managed Care insured patients showing on average 59% improvement. Additionally, admission scores appeared to be predictive of the patient’s outcome at discharge. Conclusions: The values and consistency of improvement support the use of acute rehabilitative services in skilled nursing facilities. An equation can be formulated that evaluates patients’ estimated mobility statuses upon discharge from facilities based on their conditions on their arrivals. With this, new interventions can be studied and compared to the current standard of care by using these measurements. They can determine if further improvement

Background: Self-care tasks or activities of daily living (ADL)’s performance are often disrupted after an older adult sustains an injury or illness, which can be a determining factor for discharge placement after hospitalization, or it can determine the level of assistance required after discharge from a skilled nursing facility (SNF). We believe that comprehensive rehabilitation can improve a patient’s overall functioning during their short term stay in SNFs. The purpose of this study is to determine if an individual’s level of independence improved after admission to a skilled nursing facility and procurement of rehabilitation services. Methods: This study retrospectively reviewed 4612 nursing home patients in California who underwent rehabilitative services at skilled nursing facilities to determine how the patients responded to their environments, therapy interventions, and any other additional supportive measures. The records of patients who were admitted for short-term rehabilitation were reviewed in a blinded fashion, looking specifically at Section GG of the Minimum Data Set (MDS). Self-Care Section GG scores recorded by rehabilitative staff provide objective data and measure patients’ levels of assistance that were required. Each activity is scored from 1, being completely dependent, to 6, being entirely independent, with varying levels of assistance with scores in between. The admission scores versus the discharge scores can be compared to determine if a patient improved their level of functioning upon discharge. Results: Utilizing the Pearson’s correlation coefficient, a strong correlation of improvement in ADL performance on Section GG between the admission and discharge scores was identified, implying significant improvement in functional independence upon discharge. A machine learning model for regression of interrupted Surface Electromyography(sEMG)signals to future control-oriented signals(e.g.,robot’s joint angle and assistive torque)of an active biomechatronic device for high-level myoelectric-based hierarchical control is proposed.A Recurrent Neural Network(RNN)was trained using output data,initially obtained from offline optimization of the biomechatronic(human–robot)device and shifted by the prediction *** input of the RNN consisted of interrupted sEMG signals(to mimic signal disconnections)and previous kinematic signals of the assistive *** RNN with a 0.1-s prediction horizon could predict the control-oriented joint angle and assistive torque with 92%and 86.5%regression accuracy,respectively,for the test *** proposed approach permits a fast,predictive,and direct estimation of control-oriented signals instead of an iterative process that optimizes assistive torque in the inverse dynamic simulation of a multibody human–robot *** with these interrupted input signals significantly improves the regression accuracy in the case of sEMG signal *** Robust Predictive Control-oriented Machine Learning(Robust-MuscleNET)model can support volitional high-level myoelectric-based control of biomechatronic devices,such as exoskeletons,prostheses,and assistive/resistive *** work should study the application to prosthesis control as well as the repeatability of the high-level controller with electrode *** low-level hierarchical controller that manages the human–robot interaction,the assistance/resistance strategy,and the actuator coordination should also be studied.

Objective:To conduct a systematic literature review on urethral calculi in a contemporary cohort describing etiology,investigation,and management ***:A systematic search of MEDLINE and Cochrane Central Register of Controlled Trials(CENTRAL)databases was ***,including case reports and case series on urethral calculi published between January 2000 and December 2019,were ***-text manuscripts were reviewed for clinical parameters including symptomatology,etiology,medical history,investigations,treatment,and *** were collated and analyzed with univariate ***:Seventy-four publications met inclusion criteria,reporting on 95 *** symptoms(41.1%),pain(40.0%),and acute urinary retention(32.6%)were common presenting *** calculi were most often initially investigated using plain X-ray(63.2%),with almost all radio-opaque(98.3%).Urethral calculi were frequently associated with coexistent bladder or upper urinary tract calculi(16.8%)and underlying urethral pathology(53.7%)including diverticulum(33.7%)or stricture(13.7%).Urethral calculi were most commonly managed with external urethrolithotomy(31.6%),retrograde manipulation(22.1%),and endoscopic in situ lithotripsy(17.9%).Conclusion:This unique systematic review of urethral calculi provided a summary of clinical features and treatment trends with a suggested treatment *** in contemporary urological practice should be according to calculus size,shape,anatomical location,and presence of urethral pathology.
