在过去的几十年里，无论在理论界还是实践中，人力资源管理都得到了长足的发展。人力资源管理活动对于组织绩效的贡献被广泛认同与重视。然而，人力资源管理在巴拉圭的发展却相对落后。本研究基于行为视角，对巴拉圭当前人力资源管理水平与情况进行了定位与勾勒，并对巴拉圭企业中的人力资源管理实践与工作态度的关系进行了实证研究。由于巴拉圭经济水平相对落后，该国企业以5人以下的微小型企业为主，相应的人力资源管理十分有限。因此，本文选取了人力资源管理实践与工作态度中最基本的项目进行实证检验。数据来源于服务业、制造业和广告业，共收回132份有效问卷。经过软件分析，结果显示即使企业的人力资源管理实践活动相当有限，巴拉圭的员工在一定程度上仍具有较积极的工作态度，值得进一步探讨。本文贡献在于为巴拉圭的人力资源管理实践的研究提供了理论与实证支持，并且探索了微小企业中的人力资源管理实践与工作态度的关系。

Through the creation and construction of a curvature sensor of accelerometer type, using the spectral curvature concept or curvature energy that measures curvature in Volts/m3, an autonomous and mobile censorship of curvature sensing with reliable data transmission/reception in real time and remote position is designed and constructed considering the spectra of curvature of the measured curvature energy during the advance of the prototype as the normed measure by with β , a constant rationalized parameter according with the required advance of the mobile device in the control scale of their velocity. Likewise, the sensed curvature data are digitalized through wireless interconnectivity using a HC-05 Module with a programmable device that includes logic blocks whose interconnection and functionality can be configured according to the sensor measure in situs. Also an application is planted to the obtaining of an energy plus due to the curvature that could be used in the displacement of a vehicle.

背景与目的:炎性肠病(IBD)患者具有一定结直肠癌发生风险。研究显示,IBD相关结直肠癌与非IBD相关结直肠癌在肿瘤发生机制和组织形态学上存在差异,提示其肿瘤行为和治疗反应也可能有所不同。我们旨在比较接受术后化疗IBD相关与非IBD相关的结直肠癌患者肿瘤复发及生存率的差异。方法:检索Cleve?land临床中心结直肠癌数据库,筛选出1994-2010年间的65例IBD相关结直肠癌且接受了术后辅助化疗的患者,排除21例临床资料不完整者。按照年龄、手术目的、肿瘤部位、肿瘤病理分级、AJCC分期和T分期,对IBD相关和非IBD相关的结直肠癌患者进行1:4的病例匹配分组。分别采用竞争风险模型和Cox回归模型来评估IBD对无病生存和总体生存的影响。结果:44例IBD相关和176例非IBD相关的结直肠癌患者完成匹配。在IBD组患者中,29例(66%)为溃疡性结肠炎,14例(19%)为克罗恩病,1例(2%)诊断为不确定型肠炎。IBD平均诊断年龄为(28.1±14.5)岁,IBD诊断至结直肠癌治疗的平均间期为(21.5±12.6)年。10例(23%)IBD组患者出现了肿瘤复发,而非IBD组则有34例(19%)复发(P=0.074)。IBD组与非IBD组患者无瘤生存率(HR=0.60;95%CI:0.35~1.05;P=0.074)和总体生存率(HR=0.87;95%CI:0.54~1.4;P=0.58)的差异均无统计学意义。结论:接受术后化疗的IBD相关结直肠癌患者,其复发率与生存率与非IBD相关结直肠癌患者相当。有必要行进一步的前瞻性研究来证实这些发现,以指导治疗决策。

Objective:The aims of this study were to determine the effects of length of procedure on endoscopic retrograde cholangiopancreatography(ERCP)outcomes and adverse ***:All ERCP procedures,performed by experienced advanced endoscopists,in patients without prior papillary intervention from 2006 to 2008 were *** were arbitrarily divided into two groups:shorter procedures(SP),with a duration shorter than the overall mean procedure length,and longer procedures(LP),with a duration longer than overall mean procedure *** of procedure was defined as the time from endoscope insertion to endoscope ***:Two hundred and ninety-five procedures were included in the *** procedure length was 45.630.1 *** hundred and seventy-seven procedures(60%)were SP and 118(40%)were *** were no differences between the groups with regard to patients’ages,genders,race,or trainee *** cases were more likely to be biliary vs pancreatic or bi-ductal evaluations(P=0.03).LP had significantly higher complexity scores(34%with>3 vs 13%;P=0.046)and were more likely to require pre-cut papillotomy(39%vs 15%;P<0.001).There was no significant difference between the groups in overall completion rates(91.5%LP vs 96%SP;P=0.10)or adverse events(10.2%LP vs 6.2%SP;P=0.21).However,LP cases were associated with higher rates of post-ERCP bleeding(4.2%vs 0.6%;P=0.029).Conclusion:There was no significant difference in outcomes or overall adverse events between shorter and longer ERCP ***,longer procedures were associated with higher procedure complexity,higher utilization of pre-cut technique,and increased risk of bleeding.

Assessment of Malignancy Risk in Thyroid Nodules Using a Practical Ultrasound Predictor Model: “Alpha Score”

Objective: The aim of this study was to develop a simple predictor model to diagnose malignancy by using ultrasound features of thyroid nodules and the association with cytopathological diagnosis obtained by fine needle aspiration. Materials and Methods: The likelihood of malignancy from ultrasound features was assessed in thyroid nodules obtained by fine-needle aspiration biopsy (FNAB) according to cytopathological findings reported using Bethesda System. A score was developed depending on the presence of each ultrasound feature evaluated. Results: 429 nodules were assessed, 103 (24%) were malignant. The following ultrasound features were associated with malignancy, according to the logistic regression analysis and were assigned a score of 0, +1, +2 depending on the presence or absence of each one: hypoechogenicity, solid appearance, irregular margins, microcalcifications, absence of a halo, diameter of ≥10 mm and intranodular vascular flow. The area under the curve of the proposed model was 0.900, demonstrating its predictive capacity. 4 risk categories were stablished based on the score obtained. Malignant nodules scored higher than the benign nodules (7.24 ±1.87 vs. 3.74 ±1.83). Conclusions: The proposed predictive model demonstrated to be useful and easy to apply when stratifying thyroid nodule risk of malignancy using presented US features and applying the proposed risk categories to increase the accuracy at selecting nodules that need to be studied with FNA.
