radical Vaginal Hysterectomy and Trachelectomy in Early-Stage Cervical Cancer

Objective: To communicate a minimally invasive technique for surgical handling of early-stage cervical cancer and its results. Methods: 110 patients with cervical cancer in stages IA2 and IB1, all of them operated in a period of 5 years in both hospitals, are presented. Laparoscopic systemic pelvic lymphadenectomy with radical vaginal hysterectomy or radical vaginal trachelectomy was performed to patients, with the exception of those patients who had compromised nodes detected in contemporary biopsy. results: Between April 2008 and May 2013, 110 patients were submitted to this technique. 15 patients had their surgery aborted: 13 presented positive nodes for carcinoma in contemporary biopsy and 2 had extensive cervical compromise when performing radical vaginal hysterectomy or trachelectomy. Analysis of the remaining 95 cases shows an average age of 43.9 years (26 - 61), all of them had given birth before, 23 (21%) of them through C-section. BMI averaged 30.5 and 31 (28.2%) had cone surgery performed previously. Average duration of surgery was of 220 minutes. Postoperative hospitalization averaged 3.1 days. Bleeding volume was estimated at 125 cc and one patient required blood transfusion. En 25 patients’ uterine annexes were kept and all of them were suspended by means of laparoscopy. On average, 25.4 pelvic nodes were obtained. Complications 13.6% with eight patients suffered bladder injury, two had rectovaginal fistula, 3 patients requires surgical repair of ureteral obstruction, two patients present thromboembolic disease. The disease-free and overall survivals are consistent with reports in the literature. Conclusion: We believe that handling patients with this technique is possible and has the advantages of vaginal and laparoscopic surgery with minimal complications.

Grapes are one of the most economically and culturally important crops worldwide,and they have been bred for both winemaking and fresh consumption.Here we evaluate patterns of diversity across 33 phenotypes collected over a 17-year period from 580 table and wine grape accessions that belong to one of the world’s largest grape gene banks,the grape germplasm collection of the United States Department of Agriculture.We find that phenological events throughout the growing season are correlated,and quantify the marked difference in size between table and wine grapes.By pairing publicly available historical phenotype data with genome-wide polymorphism data,we identify large effect loci controlling traits that have been targeted during domestication and breeding,including hermaphroditism,lighter skin pigmentation and muscat aroma.Breeding for larger berries in table grapes was traditionally concentrated in geographic regions where Islam predominates and alcohol was prohibited,whereas wine grapes retained the ancestral smaller size that is more desirable for winemaking in predominantly Christian regions.We uncover a novel locus with a suggestive association with berry size that harbors a signature of positive selection for larger berries.Our results suggest that religious rules concerning alcohol consumption have had a marked impact on patterns of phenomic and genomic diversity in grapes.

Triple-modal imaging of stem-cells labeled with multimodal nanoparticles, applied in a stroke model

BACKGrOUND Mesenchymal stem cells(MSCs) have been widely tested for their therapeutic efficacy in the ischemic brain and have been shown to provide several benefits. A major obstacle to the clinical translation of these therapies has been the inability to noninvasively monitor the best route, cell doses, and collateral effects while ensuring the survival and effective biological functioning of the transplanted stem cells. Technological advances in multimodal imaging have allowed in vivo monitoring of the biodistribution and viability of transplanted stem cells due to a combination of imaging technologies associated with multimodal nanoparticles(MNPs) using new labels and covers to achieve low toxicity and longtime residence in cells.AIM To evaluate the sensitivity of triple-modal imaging of stem cells labeled with MNPs and applied in a stroke model.METHODS After the isolation and immunophenotypic characterization of human bonemarrow MSCs(hBM-MSCs), our team carried out lentiviral transduction of these cells for the evaluation of bioluminescent images(BLIs) in vitro and in vivo. In addition, MNPs that were previously characterized(regarding hydrodynamic size, zeta potential, and optical properties), and were used to label these cells,analyze cell viability via the 3-[4,5-dimethylthiazol-2-yl]-2,5 diphenyl tetrazolium bromide assay and BLI analysis, and quantify the internalization process and iron load in different concentrations of MNPs via magnetic resonance imaging(MrI),near-infrared fluorescence(NIrF), and inductively coupled plasma-mass spectrometry(ICP-MS). In in vivo analyses, the same labeled cells were implanted in a sham group and a stroke group at different times and under different MNP concentrations(after 4 h or 6 d of cell implantation) to evaluate the sensitivity of triple-modal images.rESULTS hBM-MSC collection and isolation after immunophenotypic characterization were demonstrated to be adequate in hBM samples. Optimal length of triple therapy for H pylori eradication in a population with high prevalence of infection in Chile

AIM:To compare the efficacy of 7-d versus 14-d triple therapy for the treatment of H pylori infection in Chile,with a prevalence of 73% in general population. METHODS:H pylori-infected patients diagnosed by rapid urease test,with non-ulcer dyspepsia or peptic ulcer disease were randomized to receive omeprazole 20 mg bid,amoxicillin 1 g bid,and clarithromycin 500 mg bid for 7 (OAC7) or 14 (OAC14) d. Primary outcome was eradication rate 6 wk after the treatment. Subgroup analysis was carried out considering the eradication rate among patients with or without peptic ulcer disease and eradication rate among smokers or non-smokers. rESULTS:One hundred and thirty-one patients were randomized to OAC7 (n = 69) or OAC14 (n = 62). The overall eradication rate (intention-to-treat) was 78.3% in OAC7 and 85.5% in OAC14 groups,without a significant difference (P =0.37). No significant difference in the eradication rate was found among the patients with peptic ulcer disease (n = 31) between the OAC7 group (85.7%) and OAC14 group (87.5%). However,smokers had an obviously lower eradication rate compared to non-smokers,particularly in the OAC7 group (57.1% in smokers vs 83.6% in non-smokers; P = 0.06). Adverse effects rate were similar between both groups. CONCLUSION:Short-term efficacy of triple therapy with OAC for 7 d is comparable to 14 d in this high-prevalence population. Longer follow-up,and studies focused to some subgroups of patients (smokers and non-ulcerpatients) are necessary to support widespread use of 7-d instead of 10-14-d triple therapy in a developing country like Chile.

Biodiversity loss from disturbances caused by human activities means that species are disappearing at an ever increasing rate.The high number of species that have yet to be described have generated extreme crisis to the taxonomist.Therefore,more than in any other era,effective ways to discover and delimitate species are needed.This paper reviews the historically fore-most approaches used to delimit species in Ascomycota,the most speciose phylum of Fungi.These include morphological,biological,and phylogenetic species concepts.We argue that a single property to delineate species boundaries has various defects and each species concept comes with its own advantages and disadvantages.recently the rate of species discovery has increased because of the advancement of phylogenetic approaches.However,traditional phylogenetic methods with few gene regions lack species-level resolution,and do not allow unambiguous conclusions.We detail the processes that affect gene tree heterogeneity,which acts as barriers to delimiting species boundaries in classical low-rank phylogenies.So far,limited insights were given to the DNA-based methodologies to establish well-supported boundaries among fungal species.In addition to reviewing concepts and methodologies used to delimit species,we present a case study.We applied differ-ent species delimitation methods to understand species boundaries in the plant pathogenic and cryptic genus Phyllosticta(Dothideomycetes,Botryosphaeriales).Several DNA-based methods over-split the taxa while in some methods several taxa fall into a single species.These problems can be resolved by using multiple loci and coalescence-based methods.Further,we discuss integrative approaches that are crucial for understanding species boundaries within Ascomycota and provide several examples for ideal and pragmatic approaches of species delimitation.

role of hemostatic powders in the endoscopic management of gastrointestinal bleeding

Acute gastrointestinal bleeding(AGIB) is a prevalent condition with significant influence on healthcare costs. Endoscopy is essential for the management of AGIB with a pivotal role in diagnosis, risk stratification and management. recently, hemostatic powders have been added to our endoscopic armamentarium to treat gastrointestinal(GI) bleeding. These substances are intended to control active bleeding by delivering a powdered product over the bleeding site that forms a solid matrix with a tamponade function. Local activation of platelet aggregation and coagulation cascade may be also boosted. There are currently three powders commercially available: hemostatic agent TC-325(Hemospray), EndoClotTM polysaccharide hemostatic system, and Ankaferd Bloodstopper. Although the available evidence is based on short series of cases and there is no randomized controlled trial yet, these powders seem to be effective in controlling GI bleeding from a variety of origins with a very favorable side effects profile. They can be used either as a primary therapy or a secondline treatment, and they seem to be especially indi-cated in cases of cancer-related bleeding and lesions with difficult access. In this review, we will comment on the mechanism of action, efficacy, safety and technical challenges of the use of powders in several clinical scenarios and we will try to define the main current indications of use and propose new lines of research in this area.

Differences of Wood Elements of Prosopis laevigata from Two Areas of Northeast Mexico

Anatomical features of wood have a great variation among species as a result of genetic and environmental factors. The anatomical heartwood characteristics of Prosopis laevigata species from two areas with differences on temperature and rain precipitation on northeast Mexico were compared. Fibers length (μm), diameter of vessels (μm), and the area of the vessels (μm-2) were measured using light microscopy coupled with a digitized-image analysis system. The differences were statistically analyzed with analysis of variance. Statistical differences between fibers length (p 0.0001) and diameter of the vessels (p 0.001) from the two localities were found. The locality Linares, Nuevo León, Mexico, with higher precipitation and lower temperature, showed higher fiber length and higher diameter of the vessels than China, Nuevo León. Hard environmental conditions, where low precipitation values and high temperatures prevail, condition P. laevigata trees do reduce the risk of losing water.
