Clinical Use of the Intrauterine Morcellator: A Single Academic Center’s Experience

Objective: To reveal the breadth of experience for the intrauterine morcellator amongst gynecologists in a single US academic center. Design: retrospective Descriptive. Setting: University Hospital. Patients: In total, 33 hysteroscopic procedures, with intrauterine morcellation, were performed for 28 patients for benign endometrial disease at?Magee―Womens Hospital at the University of Pittsburgh Medical center between October 2006 and February 2012. Intervention: Operative Hysteroscopy with an intrauterine morcellator. Measurement and Main results: The major indication for hysteroscopic surgery was endometrial polyp (54.5%), followed by intrauterine fibroids (18.2%), filling defect on sonohysterogram (15.2%), abnormal uterine bleeding (9.1%), and uterine synechiae (3%). The mean greatest diameter for all intrauterine pathology resected was 1.14 ± 0.46 cm (range 0.6 - 1.9 cm), and, notably, the largest fibroid resected was 1.5 cm in greatest diameter. The average operative time was 39 ± 29 minutes (range 15 - 122 minutes), and average hysteroscopic fluid deficit was 286 ± 479.5 mL (range 30 - 2000 mL). There were only 2 patients for whom the deficit was greater than 1 L, one of whom underwent a myomectomy with total operative time of 26 minutes, while the other underwent a hysteroscopic adhesiolysis and had a total operative time of 122 minutes. The complication rate was 6.0%, and complications reported included uterine perforation (n = 1) and cervical injury (n = 1). Conclusion: The intrauterine morcellator is a useful tool for surgical treatment of intrauterine pathology that confers a low operative risk.

Pseudomyogenic Hemangioendothelioma of Bone Initially Managed as Slipped Capital Femoral Epiphysis: A Case report

Pseudomyogenic hemangioendothelioma is a rare, recently described neoplasm that usually presents as multifocal lesions in a single extremity. The disease has demonstrated a high propensity for infiltrative growth and local recurrence but limited metastatic potential. Variations of histological appearance and immunohistochemical signatures have been described, but typically involve spindle or polygonal cells with nuclear atypia and neutrophilic infiltration. Here we present a case report of an 8-year-old female who presented with hip pain that was initially diagnosed and managed as a slipped capital femoral epiphysis (SCFE). Subsequent evaluation led to the diagnosis of pseudomyogenic hemangioendothelioma of bone. Due to the degree of osseous destruction, described patterns of local recurrence, and metastatic potential of this neoplasm, a wide resection with endoprosthetic reconstruction of the proximal femur was performed. This case highlights the importance of due diligence in the diagnoses of SCFE and bone tumors in young patients with abnormalities of the proximal femur, including consideration of the need for biopsy.

有机茶是一种按照有机农业的操作规程进行生产加工的茶叶。在其生产过程中,完全不施用任何人工合成的化肥、农药、植物生长调节剂、化学食品添加剂等外来合成化学物质,并符合国际有机农业运动联合会（IFOAM）标准,经有机（天然）食品颁证组织发给证书的茶叶。目前中国有机茶市场增长较快,截至2015年底,中国认证有机茶总面积达到18万公顷,占茶园总面积的6.2%。尽管目前中国获得认证的有机茶正为中小企业提供大量的资金与技术支持,但是众多小型有机茶生产商仍然面临许多挑战,包括有机认证的程序、有机转换期间茶园管理问题、在普通耕种转为有机种植方式获得收益之前,产量与病虫害问题,以及长期有机茶园状态下的产量与效益问题。基于此,本论文立足中国有机茶生产现状,在全国范围内开展有机茶生产企业茶园管理现状调查,并对有机茶生产周期对茶园土壤微生物影响进行深入研究。研究表明:有机茶园常见的虫害与病害之间存在高度显著的相关性（r2=0.71）。病虫害的共存表明病虫害的入侵会破坏细胞壁的结构,为致病菌提供了侵染茶树提供了入口。反过来,某些病原体感染的植物组织已显示出比健康的植物组织对某些昆虫更具吸引力。通过有机认证前后茶场害虫损害程度的差异程度进行调查。21%的茶园认证前后虫害度没有差异,34%的茶园认证后虫害度增加,45%的茶园认为认证后虫害度减少了;这些数据表明,部分农场已成功转为有机农场,但其他许多农场面临巨大困难。随茶园分布的海拔降低,相关的病虫害为害趋势有小幅增长趋势,这支持了先前的数据,并表明低海拔农场可能需要更仔细地考虑茶树品种的选择和病虫害防治策略。我们的数据发现,害虫种类的分布随茶园中为害程度（虫害度）的变化而变化.一些物种,例如小绿叶蝉,并没有随虫害度的变化而变化,这可能说明,无论有害生物的危害程度如何,大多数茶场都普遍存在一部分小绿叶蝉.但随着虫害度的提高,诸如粉虱等其他害虫也增加了害虫总数的代表。可能是粉虱本身可能对茶树造成相当大的损害,以至于茶农认为,随着粉虱数量的增加,病虫害的危害程度也更大。在本论文的数据里面,茶园中的粉虱种群是作为茶园总体虫害的的生物指标。我们的数据表明,有机农场的种植面积与茶场的虫害损害程度呈负相关,即面积大的茶场虫害程度较低。农场规模与虫害度之间存在很小但明显的相关性,这表明较大的农场经营者可能由于获得更多的技术、资金来源从而在减轻有害生物侵害方面取得更好的效果。有机茶园对土壤微生物多样性的影响的结果表明,微生物丰度、均匀度和α多样性均与土壤pH呈显著正相关,而这些度量与SOM,TN和有机耕种的时间进程呈负相关。随着有机种植时间的延长（茶树的变老）,SOM和土壤TN的增加,土壤pH值和微生物α多样性降低。重要的是,有机农场的土壤酸化率可能会因管理实践（例如茶树种植密度、间作和有机改良剂的应用）而有很大差异。由于茶树的自动酸化倾向很高,如果管理不当,即使有机农场也可能遭受土壤酸化的破坏。我们的土壤微生物组分析表明,固氮细菌居住在茶园土壤中,例如Hyphomicrobiaceae,这与土壤中的固氮酶浓度显着相关（r=0.63）。但是固氮菌种群数量受到低pH条件的抑制。考虑到土壤氮对茶产量和质量的重要性,我们的数据表明,维持pH稳态对有机系统中土壤肥力和茶叶品质至关重要。长期有机茶园种植可减少N2O排放。在当前的数据中,笔者发现了一些潜在有效的N2O还原的细菌家族。细菌科Gemmatimonadaceae与土壤中N2O还原酶（nosZ）的浓度（r=0.76）显著相关,然而,在低pH值的土壤中该细菌家族被显着抑制,这也许可以解释酸性茶土如何显示出大量的一氧化二氮。我们的数据发现,土壤氨单氧化酶浓度与土壤有效磷显着相关。该数据表明,驱动氨氧化的细菌也可能会改善磷的利用率。相反,抑制这些细菌将同时减少硝化作用和土壤磷的利用率。将硝化作用降低到适度的程度有利于铵的土壤供应,这表明茶树可以把它根际轻度酸化,以控制硝化细菌的种类,这减少磷的利用率,但增加铵的利用率。茶叶植物对铵的强烈偏爱和对低磷供应的高度耐受性支持了这一想法。由于土壤酸化导致土壤微生物多样性的降低会损害当地土壤养分的微生物矿化速率。而在有机茶园管理模式条件下,可以通过减轻土壤酸化从而对土壤微生物组群产生良性影响,进而对茶树生理产生间接影响。关键在于保持pH的稳定,以便这些固氮、增磷、增钾等微生物类群能够生存和繁衍。综合本研究的的结果,当今的有机茶种植是一项高风险、高回报的产业。从普通茶园转化到有机茶园后,许多茶场都遇到了很大的棘手问题,当然有一些茶场似乎在转化之后生产率提高了。但是有机农业的成功绝非偶然,而是旨在维护生态系统健康的而进行的不断农场管理实践的产物。笔者认为,通过适当的茶园管理和措施来维持地面上和地下的生态平衡,特别是pH稳定,与当前基于化学的管理方式

ALDH Activity Correlates with Metastatic Potential in Primary Sarcomas of Bone

Osteosarcoma (OS), chondrosarcoma (CSA), and Ewings sarcoma (ES) are the most common primary malignancies of bone, and are rare diseases. As with all sarcomas, the prognosis of these diseases ultimately depends on the presence of metastatic disease. Survival is therefore closely linked with the biology and metastatic potential of a particular bone tumor’s cells. Here we describe a significant correlation of aldehyde dehydrogenase (ALDH) activity and the presence/absence of distant metastases in ten consecutive cases of human bone sarcomas. Additionally, cultured human CSA cells, which are historically chemo- and radio-resistant, may be sensitive to the ALDH inhibitor, disulfiram. While it is premature to draw broad conclusions from such a small series, the importance of ALDH activity and inhibition in the metastatic potential of primary bone sarcomas should be investigated further.

Metastatic rectal Adenocarcinoma of the Hip and Knee Joints: A Case Series

Two cases are presented of patients with a history of metastatic rectal adenocarcinoma presenting with a painful joint effusion. Both cases are potential examples of metastasis to periarticular bone with local infiltration to the synovium, which is one proposed mechanism of intrasynovial metastasis. While skeletal metastases are a relatively common occurrence in metastatic adenocarcinoma, intraarticular metastasis is extremely rare. These cases highlight the need to consider metastasis in the differential of joint swelling in the setting of a history of adenocarcinoma.

The COVID-19 pandemic that emerged in Wuhan China has generated substantial morbidity and mortality impact around the world during the last four *** daily trend in reported cases has been rapidly rising in Latin America since March 2020 with the great majority of the cases reported in Brazil followed by Peru as of April 15th,*** Peru implemented a range of social distancing measures soon after the confirmation of its first case on March 6th,2020,the daily number of new COVID-19 cases continues to accumulate in this *** assessed the early COVID-19 transmission dynamics and the effect of social distancing interventions in Lima,*** estimated the reproduction number,r,during the early transmission phase in Lima from the daily series of imported and autochthonous cases by the date of symptoms onset as of March 30th,*** also assessed the effect of social distancing interventions in Lima by generating short-term forecasts grounded on the early transmission dynamics before interventions were put in *** to the implementation of the social distancing measures in Lima,the local incidence curve by the date of symptoms onset displays near exponential growth dynamics with the mean scaling of growth parameter,p,estimated at 0.96(95%CI:0.87,1.0)and the reproduction number at 2.3(95%CI:2.0,2.5).Our analysis indicates that school closures and other social distancing interventions have helped slow down the spread of the novel coronavirus,with the nearly exponential growth trend shifting to an approximately linear growth trend soon after the broad scale social distancing interventions were put in place by the *** the interventions appear to have slowed the transmission rate in Lima,the number of new COVID-19 cases continue to accumulate,highlighting the need to strengthen social distancing and active case finding efforts to mitigate disease transmission in the region.
