Male gender and increased body mass index independently predicts clinically relevant morbidity after spleen-preserving distal pancreatectomy

AIM To identify risk factors for clinically relevant complications after spleen-preserving distal pancreatectomy(SPDP). No previous studies explored potential predictors of morbidity after *** The data of 41 patients who underwent a SPDP in a single surgical center between 2000 and 2015 were retrospectively reviewed from a prospectively maintained electronic database established in our Department of Surgery. The database included demographic, clinical, bioumoral, pathological, intraoperative and postoperative parameters. Uni-and multivariate ana-lyses were performed to assess potential predictors of clinically relevant morbidity. Postoperative morbidity was defined as in-hospital complications and mortality was assessed at 90 d. Clinically relevant morbidity was defined as complication ≥ grade 2 *** Overall morbidity rate was 34.1%(14 patients): grade Ⅰ(6 patients, 14.6%), grade Ⅱ(2 patients, 4.8%), grade Ⅲa(1 patient, 2.4%), and grade Ⅲb(5 patients, 12.2%). A number of 5 patients(12.2%) required re-laparotomy for postoperative complications. There was no postoperative mortality. Thus, at least one clinically relevant complication occurred in 8 patients(19.5%). Univariate analysis identified male gender(P = 0.034), increased body mass index(P = 0.002) and neuroendocrine pathology(P = 0.013) as statistically significant risk factors. Multivariate analysis identified male gender [odds ratio(Or): 1.29, 95%CI: 1.07-1.55, P = 0.005] and increased body mass index(Or: 23.18, 95%CI: 1.72-310.96, P = 0.018) as the only independent risk factors of clinically relevant morbidity after *** Male gender and increased body mass index are independently associated with increased risk of clinically relevant morbidity after SPDP. These findings may assist a surgeon in clinical decision-making to better select patients suitable for SPDP.

This study examines the international profile and career of 157 romanian researchers. The majority considers the insufficient funding of research in romania as playing a major role in their decision to work in the knowledge based economy. In addition, the survey reveals several other factors which motivate the emigration of romanian scientists. While abroad, the majority kept in touch with their former university/research institute, colleagues and/or the romanian scientific community, in general. More than half of the respondents prefer to work outside romania, after completing their current professional projects.

rapid internationalization of economic activities following the Second World War has generated an unprecedented increase in international trade and capital flows. For example, only during 1963-1993, exports rose from 157 billion to 3.774 billion dollars. Also the flows of foreign direct investment （FDI） have grown exponentially. Although FDI flows are concentrated on developing countries, 80% being targeted towards 10 countries, capital flows towards the Third World have quadrupled only between 1990 and 1993. This trend towards a truly international economy influences also on how companies do business. International business environment provides many opportunities, as well as threats. romanian companies need to align to this trend. In addition to identifying new opportunities to do business in foreign markets, they have to deal with specific risks, which they did not have to face when working internally. In this context, we consider necessary to launch an investigation, whose main objective would be analyzing the manner in which romanian companies manage risks specific to international economic affairs. As the complexity of these businesses is great, and the majority of romanian companies, in their attempt to internationalize, use classical forms of export-import, the analysis took into account only the export activity undertaken by romanian companies. We mention that we did not take into account the determination of the exposure to these risks of the companies concerned, but an analysis of the management of those risks. For this analysis, data were collected based on the administration of a questionnaire, and these data were used to develop an econometric model analysis. The specific methods of risk analysis of specific export transactions are then interpreted using the GLOBE model.

This paper examines the formation of the self in the context of emigration and the Indian individual in the Anglo-American space, in the novels: Fasting, Feasting by Anita Desai, The Inheritance of Loss by Kiran Desai, and The Namesake by Jhumpa Lahiri. The analysis focuses on exploring the way in which the hyphenated self is constructed under the influences of dialogical negotiation of desh and pardesh in the context of racism, global-local (glocal) specificity, becoming authenic under the presure of home memory, diaspora ethnicity and mimicry. Is reterritorialization and acculturation successful for the hybrid self? The answers is to be found by the characters of the novels and shown in instances of self expression and integration. Arun, the Indian guy, from Anita Desai’s novel left India for studying in America. During the summer, he is accommodated to an American family. The clash between the two cultures, traditions, and customs is felt as a huge breakage, which leads to desintegration and alienation. Biju and Harish-Harry from Kiran Desai’s novel go to America for a better living, and they succeeed in finding a job at a bakery. The impact of racism is higher in the lower strata of the society, thus their chances to integration are lost;Biju decides to return home, while Harish-Harry ends in a hyphenated self and a dissatisfied living. In The Namesake, Ashoke and Ashima, the Indian emigrants husband and wife, experience a different reshaping of identity. Ashoke works as a university assistant, while Ashima is a housewife. For him, America is a space for affirmation and self-verification, while for his wife, the space becomes a ghetto where she feels lonely and marginalised. Therefore, in the formation of the self there are many coordinates which should be taken into consideration and above this, their impact over the immigrant is different from case to case depending on many factors.

Molecular-based allergy diagnosis for the in vitro assessment of a patient immunoglobulin E(IgE) sensitization profile at the molecular level uses allergen molecules(also referred to as allergen components), which may be well-defined, highly purified, natural allergen components or recombinant allergens. Modern immunoassay methods used for the detection of specific Ig E against aeroallergen components are either singleplex(such as the fluorescence enzyme immunoassay with capsulated cellulose polymer solid-phase coupled allergens, the enzyme-enhanced chemiluminescence immunoassay and the reversed enzyme allergosorbent test, with liquid-phase allergens), multiparameter(such as the line blot immunoassay for defined partial allergen diagnostics with allergen components coating membrane strips) or multiplex(such as the microarraybased immunoassay on immuno solid-phase allergen chip, and the two new multiplex nanotechnology-based immunoassays: the patient-friendly allergen nanobead array, and the macroarray nanotechnology-based immunoassay used as a molecular allergy explorer). The precision medicine diagnostic work-up may be organized as an integrated "U-shape" approach, with a "top-down" approach(from symptoms to molecules) and a "bottomup" approach(from molecules to clinical implications), as needed in selected patients. The comprehensive and accurate Ig E sensitization molecular profiling, with identification of the relevant allergens, is indicated within the framework of a detailed patient's clinical history to distinguish genuine Ig E sensitization from sensitization due to cross-reactivity(especially in polysensitized patients), to assess unclear symptoms and unsatisfactory response to treatment, to reveal unexpected sensitizations, and to improve assessment of severity and risk aspects in some patients. Lactose intolerance is becoming a health state that requires the restriction of dairy products in the diet of people suffering from this condition. But milk and dairy products, due to a well-balanced composition in the main macro and micronutrients, cannot be missing from the diet of the consumer of any age. For these reasons, in recent years, in the milk processing industry, the production of low-lactose or lactose-free dairy products is explored. To reduce the lactose content of dairy raw materials, various industrial and biotechnological methods were used: enzymatic hydrolysis of lactose, baromembranous methods, bioconversion of lactose by lactic bacteria and others. The most widely used lactase enzymes in the industry are mesophilic enzymes from filamentous fungi (Aspergillus spp.) and yeasts (Kluyveromyces spp.). Therefore, the aim of this study was to evaluate the effect of the commercial enzyme β-galactosidase on the hydrolysis of cow’s milk at different enzyme concentrations, temperatures and pH. Two commercial enzymes β-galactosidase obtained from Bacillus licheniformis and β-galactosidase obtained from Kluyveromyces lactis, were used in this study, according to information provided by the manufacturer. The thermal stability of lactose, the effect of milk pH, the effect of temperature, duration of hydrolysis and the amount of enzymes on the lactose hydrolysis degree and the sweetness degree of milk were determined. research has identified the optimal parameters for obtaining a high degree of lactose hydrolysis in the use of these enzymes. Therefore, to ensure a high lactose hydrolysis degree (over 80%), the following lactose hydrolysis regimens were identified: temperature 4°C - 6°C, 0.3% Bacillus licheniformis
