The healthcare industry is in dire need of rapid microbial identification techniques for treating microbial *** infections are a major healthcare issue worldwide,as these widespread diseases often develop into deadly *** studies have shown that an early appropriate antibiotic treatment significantly reduces the mortality of an infection,this effective treatment is difficult to *** main obstacle to early appropriate antibiotic treatments is the long turnaround time of the routine microbial identification,which includes time-consuming sample ***,we propose a microscopy-based framework that identifies the pathogen from single to few *** framework obtains and exploits the morphology of the limited sample by incorporating three-dimensional quantitative phase imaging and an artificial neural *** demonstrate the identification of 19 bacterial species that cause bloodstream infections,achieving an accuracy of 82.5%from an individual bacterial cell or *** performance,comparable to that of the gold standard mass spectroscopy under a sufficient amount of sample,underpins the effectiveness of our framework in clinical ***,our accuracy increases with multiple measurements,reaching 99.9%with seven different measurements of cells or *** believe that our framework can serve as a beneficial advisory tool for clinicians during the initial treatment of infections.

Stud welding for fixation of cryogenic insulation of membrane tanks in LNG ship building

The support plates including all of the inner hulls such as E grade steel, weld seams and SUS304L were considered to establish the optimal stud welding condition in LNG ship building. The stainless steel plate was especially applied to the bracket on the liquid dome. The polished and etched surfaces of the welded stud and support plate were examined if the bead was properly formed. With the micrographic examination, the fusion boundary of metal weld zone formed by stud welding was analyzed to optimize the welding parameters. And also the analysis of the unacceptable welding faults such as cold and hot weld were performed. The mechanical tests such as tensile tests and bending tests were carried out to reveal any abnormal variation in the relationship of process parameters and the strength.

Stone extraction balloon-guided repeat self-expanding metal stent placement

Self-expanding metal stent (SEMS) placement offers safe and effective palliation in patients with upper gastrointestinal obstruction due to a malignancy. Well described complications of SEMS placement include tumor growth, obstruction, and stent migration. SEMS occlusions are treated by SEMS redeployment, argon plasma coagulation application, balloon dilation, and surgical bypass. At our center, we usually place the second SEMS into the first SEMS if there is complete occlusion by the tumor. We discovered an unusual complication during SEMS redeployment. The guide-wire passed through the mesh of the first SEMS and caused the second SEMS to become entangled with the first SEMS. This led to the distortion and malfunction of the second SEMS, which worsened the gastric outlet obstruction. For lowering the risk of entanglement, we studied stone extraction balloon-guided repeat SEMS placement. This is the first report of a SEMS entangled by the mesh of the first SEMS and stone extraction balloon-guided repeat SEMS placement for lowering the risk of this complication.

Metastasis to the gallbladder:A single-center experience of 20 cases in South korea

AIM： To evaluate the clinicopathologic characteristics of patients with metastases to the gallbladder （MGBs）. METHODS： We performed a single-center retrospective study of 20 patients with MGBs diagnosed pathologically from 1999 to 2007. rESULTS： Among 417 gallbladder （GB） malignancies, 20 （4.8%） were MGBs. The primary malignancies originated from the stomach （n = 8）, colorectum （n = 3）, liver （n = 2）, kidney （n = 2）, skin （n = 2）, extrahepatic bile duct （n = 1）, uterine cervix （n = 1）, and appendix （n = 1）. Twelve patients were diagnosed metachronously, presenting with cholecystitis （n = 4）, abdominal pain （n = 2）, jaundice （n = 1）, weight loss （n = 1）, and serum CA 19-9 elevation （n = 1）; five patients were asymptomatic. The median survival after the diagnosis of MGB was 8.7 mo. On Cox regression analysis, r0 resection was the only factor associated with a prolonged survival [hazard ratio （Hr）： 0.01, P = 0.002]; presentation with cholecystitis was associated with poor survival （Hr： 463.27, P = 0.006）. CONCLUSION： MGBs accounted for 4.8% of all pathologically diagnosed GB malignancies. The most common origin was the stomach. The median survival of MGI3 was 8.7 mo.

Laparoscopy-assisted pylorus-preserving gastrectomy for early gastric cancer: A retrospective study of long-term functional outcomes and quality of life

BACkGrOUND Laparoscopy-assisted pylorus-preserving gastrectomy (LAPPG) was known to have benefits of function-preserving surgery compared to laparoscopy-assisted distal gastrectomy (LADG). However, in clinical settings, delayed gastric emptying and esophageal reflux following LAPPG can be serious issues, making surgeons reluctant to perform LAPPG. It is unclear that LAPPG had better longterm functional outcomes and quality of life compared to LADG. AIM To evaluate the long-term functional outcomes and patient-reported quality of life of LAPPG compared to those of LADG. METHODS We reviewed the clinicopathological data of 195 patients who underwent LADG with Billroth II anastomosis and 101 patients who underwent LAPPG for cT1N0 gastric cancer in the middle third of the stomach between 2012 and 2015. Postoperative complications, nutritional parameters, and survey results of the European Organization for research and Treatment of Cancer Questionnaire C30 and STO22 questionnaire were compared between the two groups. rESULTS The serum hemoglobin level was significantly higher in the LAPPG group than in the LADG group (P reflux was lower (P r, the incidence of residual food was higher in the LAPPG group than in the LADG group (P regarding the quality of life score, the LAPPG group had a better physical functioning score (86.7 vs 90.0, P = 0.032) but also greater pain and reflux when compared to the LADG group [8.3 vs 16.7 in pain, 11.1 (interquartile range, 0, 22.2) vs 11.1 (interquartile range, 11.1, 33.3) in reflux, P = 0.034 and 0.001, respectively]. CONCLUSION LAPPG is beneficial to recovery of anemia and to bile reflux, however, it might be unfavorable in terms of pain and reflux symptoms compared to LADG with Billroth II anastomosis.

AIM:The prognosis of early gastric carcinoma (EGC) is generally excellent after surgery. The presence or absence of lymph node metastasis in EGC is an important prognostic factor. The survival and recurrence rates of node-negative EGC are much better than those of node-positive EGC. This study examined the factors related to lymph node metastasis in EGC to determine the appropriate treatment for ***: We investigated 748 patients with EGC who underwent surgery between January 1985 and December 1999 at the Division of Gastroenterologic Surgery, Department of Surgery, Chonnam National University Hospital. Several clinicopathologic factors were investigated to analyze their relationship to lymph node metastasis: age, sex, tumor location, tumor size, gross type, histologic type, depth of invasion, extent of lymph node dissection, type of operation,and DNA ***:Lymph node metastases were found in 75 patients (10.0%). Univariate analysis showed that male sex, tumor size larger than 2.0cm, submucosal invasion of tumor, histologic differentiation, and DNA ploidy pattern were risk factors for regional lymph node metastasis in EGC patients. However, a multivariate analysis showed that three risk factors were associated with lymph node metastasis:large tumor size, undifferentiated histologic type and submucosal invasion. No statistical relationship was found for age, sex, tumor location, gross type, or DNA ploidy in multivariate analysis. The 5-year survival rate was 94.2% for those without lymph node metastasis and 87.3% for those with lymph node metastasis, and the difference was significant (Prvival rate of patients with positive lymph nodes is significantly worse than that of patients with no lymph node metastasis. Therefore,a standard D2 lymphadenectomy should be performed in patients at high risk of lymph node metastasis: large tumor size, undifferentiated histologic type and submucosal invasion.

The changes of oligodendrocytes induced by anesthesia during brain development

With the advent of modern techniques, drugs, and monitoring, general anesthesia has come to be considered an unlikely cause of harm, particularly for healthy patients. While this is largely true, newly emerging clinical and laboratory studies have sug- gested that exposure to anesthetic agents during early childhood may have long-lasting adverse effects on cognitive function. This concern has been the focus of intense study in the field of anesthesia research. A recent high-profile review by rappaport et al. （2015） concluded that while many questions remain un- answered, there is strong evidence from laboratory studies that commonly used anesthetics interfere with brain development and that clinical studies suggest a correlation between early childhood exposure to these agents and subsequent effects on learning and cognition. The issue is of sufficient public health importance that a public-private partnership known as Smar- Tots （Strategies for Mitigating Anesthesia-related Neurotoxicity in Tots） was developed by the FDA to study pediatric anesthetic neurotoxicity. The mechanism of injury underlying this phe- nomenon has yet to be fully elucidated, and there is evidence to suggest that anesthetics may have direct cytotoxic effects on neurons leading to cell death or suppressed neurogenesis （Strat- mann et al., 2010） and that they may interfere with key pro- cesses in neuronal growth and development that underlie brain circuit development （Wagner et al., 2014）.

Gene expression analysis of primary normal human hepatocytes infected with human hepatitis B virus

AIM： To find the relationship between hepatitis B virus （HBV） and hepatocytes during the initial state of infection by cDNA microarray. METHODS： Primary normal human hepatocytes （PNHHs） were isolated and infected with HBV. From the PNHHs, rNA was isolated and inverted into complement DNA （cDNA） with Cy3- or Cy5- labeled dUTP for microarray analysis. The labeled cDNA was hybridized with microarray chip, including 4224 cDNAs. From the image of the microarray, expression profiles were produced and some of them were confirmed by rT-PCr, immunoblot analysis, and NF-κB luciferase reporter assay. rESULTS： From the cDNA microarray, we obtained 98 differentially regulated genes. Of the 98 genes, 53 were up regulated and 45 down regulated. Interestingly, in the up regulated genes, we found the TNF signaling pathway-related genes： LT-α, TrAF2, and NIk. By using rT-PCr, we confirmed the up-regulation of these genes in HepG2, HuhT, and Chang liver cells, which were transfected with pHBV1.2x, a plasmid encoding all HBV messages. Moreover, these three genes participated in HBV- mediated NF-κB activation. CONCLUSION： During the initial state of HBV infection, hepatocytes facilitate the activation of NF-κB through up regulation of LT-α, TrAF2, and NIk.

The epidermal growth factor receptor(EGFr)pathway plays an important role in the progression of colorectal cancer(CrC).Anti-EGFr drugs based on antibodies have been widely used for treating CrC through inducing the cell death ***,the majority of CrC patients will inevitably develop drug-resistance during anti-EGFr drug treatment,which is mainly caused by a point mutation in the krAS *** developed a nanoliposomal(NL)particle containing the Cas9 protein and a single-guide rNA(sgrNA)complex(Cas9-rNP),for genomic editing of the krAS *** NL particle is composed of bio-compatible lipid compounds that can effectively encapsulate *** modifying the NL particle to include the appropriate antibody,it can specifically recognize EGFr expressing CrC and effectively deliver the gene-editing *** conditions of NL treatment were optimized using a krAS mutated CrC in vivo mouse *** with krAS-mutated CrC showed drug resistance against cetuximab,a therapeutic antibody *** treating the mice with the krAS gene-editing NL particles,the implanted tumors showed a dramatic decrease in *** results demonstrated that this genomic editing complex has great potential as a therapeutic carrier system for the treatment of drug-resistant cancer caused by a point mutation.
