Backgr.und: s.ontaneous.gallbladder.per.or.tion(GBP) is.an uncommon diagnos.s. This.s.udy pr.s.nted the exper.ence of managing s.ontaneous.GBP over.nine year. at a lar.e, ter.iar. car. univer.ity hos.ital in nor.h India and inves.igated the outcomes.and tr.atment s.r.tegies. Methods. A r.tr.s.ective r.view of pr.s.ectively maintained digital databas. of cons.cutive patients.was.per.or.ed. All patients.r.ceived medical and/or.s.r.ical tr.atment for.s.ontaneous.GBP in our.depar.ment between Januar. 2010 and June 2018. r.s.lts. We identifed 151 patients.81 females.and 70 males. with mean age of 53 year.. Mos. common pr.s.nting featur.s.wer. pain(96.7%), fever.54.3%) and jaundice(31.1%). Mos. common caus. was.gallbladder.s.ones.84.8%) followed by common bile duct s.ones.30.5%), xanthogr.nulomatous.cholecys.itis.17.9%) and malignancy(11.9%). As.per.Niemeier.clas.ifcation, 8.6% had type 1 GBP(fr.e per.or.tion in per.toneal cavity), 76.2% had type 2 GBP(localized per.or.tion) and 13.2% had type 3 GBP(cholecys.oenter.c fs.ula). About 60% of the per.or.tions.wer. diagnos.d pr.oper.tively. Type 1 was.mor. common in patients.with diabetes.and als. had the wor.t pr.gnos.s. s.r.er. was.per.or.ed in 109 patients.72.2%). s.ven patients.4.6%) had a pos.oper.tive mor.idity of Clavien-Dindo Ⅲ or.higher. Ther. wer. thr.e mor.alities.in patients.who under.ent s.r.er.. Conclus.ons. High index of s.s.icion is.r.quir.d for.pr.oper.tive diagnos.s.of GBP, es.ecially in types.2 and 3. Lapar.s.opic cholecys.ectomy can be difficult in thes. patients.and patients.may r.quir. open or.par.ial cholecys.ectomy. Ear.y diagnos.s.and s.ep-up appr.ach for.the tr.atment of GBP is.cr.tical.

Backgr.und: Blumgar.'s.pancr.aticojejunos.omy(PJ) has.been des.r.bed with low pancr.atic leak r.tes. This.s.udy aimed to evaluate our.exper.ence with this.technique r.gar.ing the pancr.atic leak and other.per.oper.tive outcomes. Methods. We per.or.ed a s.ngle-center.r.tr.s.ective analys.s.of a cohor. of 81 patients.who under.ent pancr.aticoduodenectomy in our.depar.ment fr.m Januar. 2011 to Febr.ar. 2018. The pr.mar. endpoint was.the occur.ence of a clinically r.levant pos.oper.tive pancr.atic fis.ula(Cr.POPF) and analys.s.of its.r.s. factor.. r.s.lts. The Cr.POPF r.te was.12.3%. Fis.ula r.s. s.or.(Fr.) was.the only s.gnificant r.s. factor.for.the occur.ence of over.ll POPF in multivar.ate analys.s. However. none of the other.factor. including Fr. was.found to be s.gnificantly as.ociated with Cr.POPF r.s.. A s.r.ng pos.tive cor.elation was.found between the Cr.POPF and the incidence of delayed gas.r.c emptying, pos.-pancr.atectomy hemor.hage and incr.as.d length of hos.ital s.ay. Conclus.on: Blumgar.'s.technique is.a s.fe technique of pancr.atico-enter.c anas.omos.s.with low r.tes.of Cr.POPF. Cr.POPF with this.technique is.independent of mos. of the pr.oper.tive and intr.oper.tive factor.. Ther.for., this.technique can be us.d for.all types.of the pancr.as.with cons.s.ently good r.s.lts.

Apar. fr.m Ph, genomic imbalances.s.ch as.additional chr.mos.mal abnor.alities.(ACAs. of major.r.ute occur.dur.ng tr.ns.or.ation of the dis.as. and s.ow negative impact on pr.gnos.s. Objective: The pr.s.nt s.udy was.car.ied out to inves.igate fr.quencies.of ACAs. genomic deletions. complex Ph var.ants.and their.pr.gnos.ic influences.in a lar.e cohor. of newly diagnos.d CML-CP (chr.nic phas.) and CML-AP/BP (acceler.ted/blas. phas.). Mater.al & Methods. r.tr.s.ective, s.ngle ins.itutional s.udy on 1367 cas.s.of CML-CP and 82 cas.s.of CML-AP/BP between 2009 and 2015, us.ng conventional cytogenetics.along with fluor.s.ence in s.tu hybr.dization. r.s.lts. Of the 1367 patients.in CML-CP, 1041 patients.who completed 12 - 18 months.of Imatinib ther.py s.owed complete cytogenetic r.mis.ion (CCyr. r.tes.of 76% and 82% at 12 and 18 months.r.s.ectively. Imatinib induced 81% and 33% CCyr.in CML-AP and CML-BP r.s.ectively. Fr.quencies.of ACAs.in CML-CP, AP and BP wer. 2%, 27% and 67% r.s.ectively. Patients.in chr.nic and AP/BP phas. with ACAs.s.owed r.s.s.ance to Imatinib (p s.and complex Ph var.ants.was.21% and 6.3% r.s.ectively with no compar.ble differ.nce of cytogenetic r.s.ons. to Imatinib (p p r.s.ectively). In a cohor. of 112 patients.in CCyr. development of new clonal abnor.alities. mor. fr.quently tr.s.my 8 was.detected in Ph negative clone. Conclus.on: Our.data demons.r.ted that Imatinib as.a fr.ntline ther.py had s.gnificantly impr.ved management of CML. However. ACAs.play an impor.ant r.le in r.s.s.ance to Imatinib, both in chr.nic and acute phas., which may limit s.le ABL tar.eted ther.py.

Mater.al & Methods. The AL patients.included 2609 AML (adult: 2042, pediatr.c: 567), 3708 B-cell-pr.cur.or.(BCP)-ALL (adult: 1300, pediatr.c: 2408) and 892 cas.s.of T-ALL (adult: 480, pediatr.c: 412). Cytogenetic s.udies.included conventional kar.otyping and FIs. us.ng panel of pr.bes. r.s.lts. The incidence of t(8;21) was.high, compar.ble to other.As.an countr.es. In compar.s.n to our.s.r.es.and Wes.er. population, t(15;17) was.mor. pr.valent in Chines. population. Cytogenetic pr.filing of BCP-ALL r.vealed low pr.valence of ETV6/r.NX1 in our. as.well as.other.As.an population. The MLL aber.ations.in BCP-ALL and TLX1 & TLX3 aber.ations.in T-ALL occur.ed les. fr.quently in our.s.r.es.as.compar.d with Wes.er. population.
